RESUMO

We probed the psychological influence exerted on traumatic stress endured by healthcare workers (HCWs) and the coping behaviors adopted during the first wave of COVID-19 in Taiwan, which occurred one year later than in other countries. Clinical HCWs from two branches of a hospital network in Taichung, Taiwan, were recruited for this cross-sectional study. The participants were administered a questionnaire on sociodemographic and work-related characteristics, perceived influence exerted by COVID-19, coping behaviors in relation to COVID-19, and Impact of Event Scale-Revised scores. We obtained 769 valid questionnaires. A chi-square test, generalized linear modeling, and multivariate stepwise regression analyses were performed. Although the first wave of COVID-19 occurred one year later in Taiwan than in other countries, the traumatic stress experienced by Taiwanese HCWs was noted to be comparable to that of those in other countries. Factors for increased traumatic stress included caring for more patients with COVID-19, fair or poor self-rated mental health, higher perceived influence of COVID-19, vulnerable household income, and more negative coping behaviors. Positive coping behaviors such as exposure reduction and protection measures decreased traumatic stress. Accordingly, managers should strengthen protective measures, enhance COVID-19-related training, and provide psychological support and counseling for high-risk employees.

OBJECTIVES: To explore the risks and rates of readmission and their predictors 14 days, one year, and five years after discharge for the psychiatric population in Taiwan. METHODS: This was a prospective study based on claims from 44,237 first-time hospitalized psychiatric patients discharged in 2000, who were followed for up to five years after discharge. The cumulative incidence and incidence density of readmission were calculated for various follow-up periods after discharge, and Cox proportional hazard models were generated to identify the significant predictors for psychiatric readmission. RESULTS: The less than 14-day, one-year, and five-year cumulative incidences were estimated at 6.1%, 22.3%, and 37.8%, respectively. The corresponding figures for incidence density were 4.58, 1.04, and 0.69 per 1,000 person-days, respectively. Certain factors were significantly associated with increased risk of readmission irrespective of the length of follow-up, including male gender, length of hospital stay >15 days, economic poverty, a leading discharge diagnosis of schizophrenia/affective disorders, and residence in less-urbanized regions. OBJECTIVES: Few studies have reported on the relative ratio between the depths to which acupuncture needles are inserted in therapy (therapeutic depth) and safety depth (the distance from the surface of the skin of the acupoint to the transverse fascia). This ratio may be of clinical importance because it may have an impact on the safety and the therapeutic effectiveness of acupuncture. DESIGN: We retrospectively studied children aged 7 to 15 years who underwent abdominal computed tomography (CT) at our hospital during the period January 1997-March 2006. Twelve (12) abdominal acupuncture points (CV-3, CV-4, CV-6, CV-10, CV-12, CV-14, KI-12, ST-24, ST-25, SP-15, LV-13, and LV-4) were measured directly on the CT images. The ratio between the therapeutic depth and safe depth of the 12 abdominal acupoints with factors sex, body weight, age, and waist girths were analyzed by one-way analysis of variance and multiple linear regression analysis. RESULTS: A total of 204 patients were included in this study. There was no significant difference in the therapeutic depth over safety depth ratio (T/S ratio) between genders. The T/S ratio of these 12 acupuncture points ranged from 0.67 to 0.88 and increased significantly with body weight, age, and waist girth (p < 0.001). CONCLUSIONS: The therapeutic depth of abdominal acupoints is closer to the safe depth in overweight and in older children aged 7 to 15 years old. There was no significant difference between genders.
