RESUMO

OBJECTIVE: To date, no secondary prevention program is in place for patients carrying an increased risk for developing head and neck cancer (HNSCC). In terms of successful, long-term curative therapy and increased quality of life, it would be useful to detect such diseases at an early stage. PATIENTS AND METHODS: A total of 370 patients with at least one risk factor such as "smoking", "alcohol", or "reflux disease" and without any symptoms were examined during a 1-year period using standard HNO methods (e. g. endoscopy) for suspicious alterations of the mucosa of the upper aerodigestive tract. RESULTS: In 13 (3.5%) of all 370 cases a biopsy was taken for further diagnosis. Squamous cell carcinoma was found in eight cases, while one further patient was suffering from non-Hodgkin lymphoma. CONCLUSIONS: It is simple and safe to examine patients at risk of developing HNSCC by standard HNO methods. The rate of detected carcinomas is much higher than in former investigations, likely because our survey focused only on patients with specific risk factors.

RESUMO

The Austrian Alzheimer Society developed evidence-based guidelines based on a systematic literature search and criteria-guided assessment with subsequent transparent determination of grades of clinical recommendation. The authors evaluated currently available therapeutic approaches for the most common forms of dementia and focused on diagnosis and pharmacological intervention, taking into consideration the situation in Austria. The purpose of these guidelines is the rational and cost-effective use of diagnostic and therapeutic measures in dementing illnesses. Users are physicians and all other providers of care for patients with dementia in Austria.

OBJECTIVE: The goal of this study was to investigate pharmacological treatment strategies used by residential primary care providers for patients with dementia. METHODS: A postal questionnaire survey was sent to all residential primary care providers, internists, neurologists and psychiatrists (n = 689) in the western region of Austria. RESULTS: The response rate (53 %) was similar in all four physician groups. Acetylcholinesterase inhibitors are considered to have a higher efficacy (p < 0.0005) compared to nootropic drugs. However, the vast majority of primary care providers (95 %) prescribe nootropic drugs. Two thirds (64 %) of the primary care providers prescribe acetylcholinesterase inhibitors. The dementia subtype influences the prescription frequency of acetylcholinesterase inhibitors, but not the specific choice of nootropic compound. Half of the primary care providers (52 %) combine antidementia drugs. Nearly two-thirds (62 %) of all primary care providers frequently prescribe antidepressants. Specific serotonin reuptake inhibitors are applied by the majority of primary care providers (96 %). About one-third (39 %) of primary care providers and internists (29 %) prescribe tricyclic antidepressants. Antipsychotics are applied frequently by around a quarter (29 %) of all physicians. More than half of primary care providers (62 %) and internists (58 %) treat patients with typical antipsychotics. Psychiatrists and neurologists are significantly more reluctant to prescribe tricyclic antidepressants and typical antipsychotics. CONCLUSIONS: Despite the lack of scientific evidence, residential primary care providers combine antidementia drugs very frequently. Therefore, controlled studies on combination therapies are urgently needed; in contrast to neurologists and psychiatrists, primary care providers and internists frequently prescribe tricyclic antidepressants and typical antipsychotics. The reasons for this should be clarified in further studies.

The atypical antipsychotic zotepine was studied in an open, multicentre uncontrolled, post-marketing surveillance study in 108 schizophrenic patients hospitalized in 12 trial centres in Austria. Within the dosage range of 50-450 mg (mean at the end of the study, 207 +/- 125 mg/day), a significant reduction of positive as well as negative symptoms was noted. There was no increase in extrapyramidal side-effects during the study and a significant decrease in akathisia scores. The medication was well tolerated during the 42-day observation period. Zotepine improved both positive and negative symptoms and was not accompanied by extrapyramidal side-effects, justifying its classification as an atypical antipsychotic.

About one third of all elderly people suffer from psychosocial disturbances reducing their well-being, ability or independence. Those disturbances can be mitigated more often than expected by medical treatment, psychotherapy or other therapies. In this field it is above all the general practitioner who has to be engaged as gate keeper and case manager.

37 of 43 patients who had been hospitalised because of schizophrenia for the first time in 1954 and subjected to electroconvulsive treatment in the same year, were reexamined 30 years later in respect of course of the disease, social situation, psychopathological pattern and late complications. Whereas in 1954 the direct treatment result was assessed as full remission in 11 cases, as part remission in 16 and as not improved in 10 patients, a similar pattern was obtained in the assessment of the final stages according to the criteria of Bleuler or Ciompi, respectively, namely, a high proportion of socially inconspicuous patients and of severe residual states or permanently hospitalised cases. This result is in agreement with wellknown long-term follow-up studies and is clearly opposed to the assumption that the course of the disease is favourably influenced by drastic shock treatment.

RESUMO

Microorganisms that are pathogenic to insects provide a wealth of biological material that can be exploited by humans to control insect pests. Innovative applications of a few such entomopathogens are found throughout the world, but widespread commercial production of microbial insecticides awaits further studies of the biology, ecology, and pathogenicity of the agents. Genetic engineering techniques may be used to increase the virulence of these microorganisms, as well as to make them more tolerant of physical and chemical conditions and perhaps to broaden their host ranges. The use of microbial insecticides could decrease our dependence on chemical pesticides.

Under closed laboratory conditions, at non-limiting nutrient levels, the biomass of Anabaena variabilis, Anacystis nidulans, Chlorella pyrenoidosa, and Selanastrum capricornutum increased with increasing levels of dissolved organic matter (DOM) as a result of bacterially produced carbon dioxide (CO2) and (or) cofactors. Oxygen (O2) produced as a result of algal photosynthesis was sufficient to supply the najority of O2 required by the bacterial community. The percentage of DOM utilized by bacteria which was subsequently incorporated into algal biomass varied with individual species indicating that the association between individual algal species and the bacterial microbiota varied. Under natural conditions bacteria could provide CO2 and (or) cofactors for algal photosynthesis which in turn supplies O2 for bacterial respiration. This mutualistic association in aquatic environments could result in an increase in planktonic and epiphytic algal biomass if other nutrients are available.

Survival of lyophilized vegetative Bacillus popilliae in dry soil and in soil kept in atmospheres with different relative humidities (RH) was determined. Cells survived for at least 1 year when the RH was 22% or lower, but an RH of 33% or higher reduced viability. At 33% RH, no bacteria were recovered after 5 months of storage; at 42% or above, no bacteria were recovered after 1 month of storage.

The per cent viability of Bacillus popilliae after lyophilization of liquid nitrogen frozen cells was determined. Lyophilization of 9- to 12-hr cells which had been suspended in 5% sodium glutamate plus 0.5% gum tragacanth, frozen in liquid nitrogen vapor, and dried 4 to 5 hr with the ampoules exposed to room temperature resulted in survival of 64.6% of the original cells. After storage of these lyophilized preparations for 6 months at room temperature, 10.5% of the original cells were still viable.
