RESUMO

Comparative assessment of methods aimed at amelioration of the immunohemostatic processes in patients with open-angle glaucoma suffering from chronic vascular diseases showed that the most remarkable improvement of the visual functions and decrease of the level of circulating immune complexes in the blood were attained in the patients treated by heparin inhalations combined with intravenous laser exposure of the blood as against patients treated by one of these methods alone or traditionally.

RESUMO

Computer methods for examining the visual functions are compared: computer-aided campymetry from the time of the sensorimotor reaction, computer-aided color campymetry, visocontrastometry, and threshold static perimetry (program 24-2) with Humphrey's visual field analyzer. These methods were used in examinations of 65 patients (105 eyes) with primary open-angle glaucoma at various stages of the disease. A complex of sufficiently sensitive psychophysical methods for examination of the visual functions has been defined, including computer-aided campymetry and visocontrastometry, which may be used for early diagnosis of glaucoma.

Courses of magnetotherapy (MT) using ATOS device with 33 mT magnetic field induction were administered to 31 patients (43 eyes) with primary open-angle glaucoma with compensated intraocular pressure. The operation mode was intermittent, with 1.0 to 1.5 Hz field rotation frequency by 6 radii. The procedure is administered to a patient in a sitting posture with magnetic inductor held before the eye. The duration of a session is 10 min, a course consists of 10 sessions. Untreated eyes (n = 15) of the same patients were examined for control. The patients were examined before and 4 to 5 months after MT course. Vision acuity improved by 0.16 diopters, on an average, in 29 eyes (96.7%) out of 30 with vision acuity below 1.0 before treatment. Visocontrastometry was carried out using Visokontrastometer-DT device with spatial frequency range from 0.4 to 19 cycle/degree (12 frequencies) and 125 x 125 monitor. The orientation of lattices was horizontal and vertical. The contrasts ranged from 0.03 to 0.9 (12 levels). MT brought about an improvement of spatial contrast sensitivity by at least 7 values of 12 levels in 22 (84.6%) out of 26 eyes and was unchanged in 4 eyes. Visual field was examined using Humphry automated analyzer. A 120-point threshold test was used. After a course of MT, visual field deficit decreased by at least 10% in 31 (72%) out of 43 eyes, increased in 3, and was unchanged in 9 eyes; on an average, visual field deficit decreased by 22.4% vs. the initial value. After 4 to 5 months the changes in the vision acuity and visual field deficit were negligible. In controls these parameters did not appreciably change over the entire follow-up period.

RESUMO

Examinations of 203 patients (100 ones with occlusion abnormalities of the extracranial segments of the carotid arteries and 103 with primary open-angle glaucoma) by means of ophthalmologic and dopplerographic techniques have revealed no relationship between occlusion abnormalities of the carotid arteries and glaucoma development. This fact is explained by collateral circulation development via the orbital anastomosis and anterior connective artery that provide ocular circulation compensation in occlusion of the internal carotid artery. Noteworthy that the incidence of occlusion involvement of the carotid arteries in glaucoma (8.7 percent of cases) corresponds to the incidence of this abnormality in a random sample of an age-matched population. These results suggest an independent mechanism of circulation autoregulation in the eye and optic nerve.

Compliance of the optic disc with the visual field has been examined with the Peritest in 137 eyes (90 patients) with initial open-angle glaucoma. Coincidence of the two states has been observed in 54% of cases, no coincidence in the rest. The authors' findings evidence that changes in the visual field have anticipated the optic disc changes in 48% of cases, whereas optic disc shifts have developed before the visual field defects in 52% of cases. An important factor contributing to the compliance of the optic disc with the visual field is, besides the excavation area, the neuroglial ring status, that has been changed in 58% of cases. The authors come to the conclusion that combined changes in the visual field and optic disc are a more reliable criterion confirming the diagnosis of glaucoma.
