RESUMO

In 2010, the Russian Federation (RF) registered palivizumab--innovative drug, based on monoclonal antibodies for passive immunization of seasonal respiratory syncytial virus (RSV) infection in children of disease severe progress risk group, which include primarily premature infants, children with bronchopulmonary dysplasia and hemodynamically significant congenital heart disease. Currently, palivizumab is included in the list of recommended medicines and medical care standards of different countries, including Russia. In the review the results of Russian research on the progress of RSV infection, its epidemiology and immunization experience gained over the 2010-2014 period are summarized in relation to the foreign data. During the four epidemic seasons palivizumab immunization covered more than 3,200 children of severe RSV infection risk group with a progressive annual increase in the number of patients who received the drug. Geography of palivizumab immunization is also greatly expanded in our country during this time. If during the first two seasons measures of immunization were taken mainly in Moscow and St. Petersburg, at the present time, thirty one territorial entities of the Russian Federation have the experience in the drug application. Analysis of the results of RSV infection immunization (made in several regions) confirms the high clinical efficacy and palivizumab safety already demonstrated in international studies. In addition, the analysis presents the potential to improve the efficiency of the integrated RSV infection immunization programs, realizing in the establishment of high-risk child group register, adequate counseling for parents, as well as the development of the routing of patients and coordination of interaction between different health institutions during the immunization.

RESUMO

The article presents 42-year experiences with surgical treatment of atresia of the esophagus in 814 neonatals in the St. Petersburg Center of surgery in neonatals. Under analysis are the consecutive periods of work in this field, development and introduction into practice of the methods of surgical interventions, the original Bairov's operation--double esophagostomy--included. Factors responsible for results of the treatment of esophageal atresia are described. Improvement of the methods of treatment has resulted in a possibility mainly to make anastomoses of the esophagus and reduce lethality of neonatals with esophageal atresia from 81 to 7.7%.

RESUMO

The article generalizes an experience with the treatment of 91 newborns with diaphragmatic hernias for the period from 1955 to 1985. The diagnostics has improved and greater amount of newborns were admitted to the clinic within the first day after birth. The frequency and character of postoperative complications were analyzed as well as causes of their appearance and effective measures for their prevention and structure of lethality. The tactics of resuscitation both before and after operation has been changed: the artificial ventilation of the lungs was performed without a positive pressure at the end of the expiration with the use of Tubarin and infusion of Dopamine. Lethality was 2 times reduced.

RESUMO

Results of examination of partial functions of the kidneys in 113 newborns have shown the reaction of the kidneys on a surgical aggression to be dependent on the age and stage of the postoperative period. The reaction manifests itself by renal economy of water and electrolytes on the 1-3 days after operation followed later by the osmotic dilution of urine and lower reabsorption of sodium and potassium. The maximum amount of sodium admissible for administration in the postoperative period has been established with special reference to age.

RESUMO

An effective use of continuous infusion of Dopmin in dose of 2-3 mkg/(kg/min) is described. Dopmin was used in 13 newborns with severe respiratory insufficiency due to postasphyxial syndrome, disease of hyaline membranes and congenital diaphragmal hernia. Continuous infusion of microdoses of Dopmin with artificial ventilation of lungs against the background of a decreased respiratory insufficiency according to clinico-laboratory evidence resulted in decreased value of bloodflow shunt from right to left and oxygen concentration in the inhaled mixture to 45%.

RESUMO

Results of the treatment of 334 newborns with anorectal defects are analyzed. Radical operations were performed on 181 children, praeternatural anus was made in 64 children, 37 children were operated upon for other associated defects. Conservative treatment was used in 52 children.
