Are we good at thromboembolic disease prophylaxis - an audit of the use of risk assessment forms in emergency medical admissions.

Venous thromboembolism (VTE) is a major cause of morbidity and mortality in hospitalised patients. Thromboprophylaxis is an effective strategy for VTE prevention in high-risk patients. An initial audit in our district general hospital trust showed poor adherence to the thromboembolic risk factors consensus group recommendations and so a risk assessment form (RAF) was devised. We present repeated audits to assess the RAF uptake and its effects on VTE thromboprophylaxis. We also present data analysing perceptions among doctors of the RAF and reasons for its poor completion. We provide compelling evidence that the RAF is an invaluable tool in the assessment of VTE thromboprophylaxis.

Scalp pain and hyperlipidaemia.

Acute pancreatitis and eruptive xanthomata are the only recognised direct complications of severe hypertriglyceridaemia, although peripheral neuropathy has been described in patients with hyperlipidaemia. We describe a patient with mixed hyperlipidaemia presenting with severe scalp pain and eruptive xanthomata. Both resolved with treatment. We suspect that high triglyceride concentration can affect the function of sensory nerve fibres.

Total cardiac denervation in diabetic autonomic neuropathy.

Total loss of the autonomic regulation of heart rate is described in a 28-year-old diabetic with extensive autonomic neuropathy. The patient had an almost fixed heart rate that barely responded to any of the tests that stimulate or inhibit the autonomic nerves. Its behavior was similar to that of the transplanted heart.

Defective innervation of heart in diabetic autonomic neuropathy.

Heart rate responses to autonomic stimulation and inhibition were studied in 13 diabetic autonomic neuropathy. Parasympathetic function was impaired in all patients and sympathetic function in most. One patient's heart appeared to be totally denervated. The consequences of cardiac denervation include tachycardia, a fixed heart rate, and a possible tendency to cardiac dysrhythmias, which caused spontaneous cardiac arrests in three patients.

Modification by propranolol of cardiovascular effects of induced hypoglycaemia.

The cardiovascular effects of hypoglycaemia, with and without beta-blockade, were compared in fourteen healthy men. Eight received insulin alone, and eight, including two of the original insulin-only group, were given propranolol and insulin. In the insulin-group the period of hypoglycaemia was associated with an increase in heart-rate and a fall in diastolic blood-pressure. In the propranolol-insulin group there was a significant fall in heart-rate in most subjects and an increase in diastolic pressure. Typical S-T/T changes occurred in the insulin-group but in none of the propranolol-insulin group. Hypertension in diabetics prone to hypoglycaemia attacks should not be treated with beta-blockers because these drugs may cause a sharp rise in blood-pressure in such patients.

"Ischaemic" colitis in young adults.

Four cases of transient haemorrhagic colitis seen in young adults resemble 10 others in the literature. The 14 cases are distinguished from the classical transient ischaemic colitis syndrome by their youth, the low incidence of stricture formation, and the prevalence of right-sided lesions.
