Background The aim of the present study was to investigate the correlation between the triglyceride/glucose index (TyG index) and homeostasis model assessment of insulin resistance (HOMA-IR). Additionally, we compared the ability of the TyG index and triglycerides/high-density lipoprotein cholesterol (TG/HDL-c) index and the combination of these two indices (TyG index plus TG/HDL-c) to predict insulin resistance (IR) in South American overweight and obese children and adolescents. Methods A cross-sectional study was carried out in 345 overweight adolescents aged 10-18 years, from both the sexes. The TyG index was calculated as Ln (fasting triglycerides [mg/dL] × fasting glucose [mg/dL])/2, while the TG/HDL-c index was calculated by the division of TG (mg/dL) by HDL-c (mg/dL). HOMA-IR was calculated with the formula: fasting insulin (FI) (U/mL) × fasting glucose (mmol/L)/22.5. The cut-off point used to determine the presence of IR was HOMA-IR ≥ 3.16. Results The TyG index showed a positive correlation with HOMA-IR. The area under the receiver operating characteristic (ROC) curve of the TyG index was 0.74, indicating good sensitivity (75.7%) and specificity (67.4%). Furthermore, the TyG index cut-off point of >4.44 was established for IR prediction in this population. Conclusions The TyG index is a simple and cost-effective surrogate marker of IR in South American overweight children and adolescents. Moreover, due to its good accessibility, it can be used in large epidemiological studies.

PURPOSE: The aim of the present study was to verify the effects of a 16-week multidisciplinary obesity treatment program (MOTP) on health-related quality of life (HRQoL) in adolescents with weight excess. In addition, we verified a possible association between changes on HRQoL and anthropometric, body composition, and cardiorespiratory fitness (CRF) parameters. METHODS: Two hundred four adolescents aged from 15 to 18 years were distributed in control group (CG) and intervention group (IG). They underwent a 16-week MOTP composed by nutritional, psychological, and health-related physical activity group sessions. Moreover, they performed physical exercise sessions three times per week during the whole 16-week program. Data on HRQoL, body composition, CRF and anthropometry were collected using standard protocols and validated questionnaires. RESULTS: The MOTP promoted significant enhancements in all HRQoL domains, except for the school domain in IG. Furthermore, positive correlations between HRQoL and body weight (BW), body fat (BF), waist circumference (WC), and body mass index (BMI) z-score were verified. Higher scores of HRQoL might be achieved by improving these variables. CONCLUSIONS: The 16-week MOTP was effective to improve HRQoL in adolescents with weight excess. This improvement has a positive correlation with enhancements in BW, BMI z-score, WC, and BF. Nevertheless, these findings have not reached a consensus on literature and still need to be further enlightened.

OBJECTIVE: The aim of this study was to verify the prevalence of metabolically healthy obese (MHO) phenotype and associated factors in South American adolescents who are overweight. METHODS: A cross-sectional study was carried out with 340 overweight adolescent boys and girls between 10 and 18 y of age. The participants were classified as MHO according to two definitions: absence of any metabolic syndrome component and absence of insulin resistance (IR). The MHO phenotype-associated factors analyzed were age, sex, nutritional status, waist circumference (WC), body composition, metabolic profile, and cardiorespiratory fitness. Multivariable logistic regression was used to determine predictors of MHO using odds ratios with 95% confidence intervals. RESULTS: The prevalence of MHO in South American overweight adolescents was 49.4% and 55.9% according to MS and IR criteria, respectively. Sex and WC were predictors of the MHO phenotype, considering MS classification criterion. For the IR criterion, age, WC, and triacylglycerol levels were independent predictors of MHO in adolescents. Cardiorespiratory fitness did not predict MHO phenotype in any of the criteria used. CONCLUSIONS: The prevalence of MHO in South American overweight adolescents was high and varied according to the definition used. Age, sex, WC, and triacylglycerolslevel were independent predictors of the MHO phenotype in this population.

Abstract A systematic review of the prevalence of Active Play in Brazilian children and adolescents was performed. Only fully available scientific papers that measured Active Play or leisure time physical activity independently from other types of physical activity in Brazilian children and/or adolescents were considered for inclusion. The search for potential articles was performed on the following electronic databases: Pubmed/Medline, Web of Science, Bireme, Scielo and Scopus. Initially, 63 papers met the eligibility criteria. However, after deeply analyzed, seven studies remained and were included in the present review. The overall prevalence of Active Play was 36%, in which varied from 27.2% to 79.3%. Boys presented a prevalence of 47%, ranging from 39,1% to 79.9%, while the prevalence in girls reached 26%, varying from 13.9% to 78.7%. Although the prevalence of Active Play in Brazil in not satisfactory, the potential to present a reliable data regarding this theme should be highlighted. In agreement with some other studies, boys presented a higher prevalence of physical activity than girls. Also, any socioeconomic pattern regarding the prevalence of Active Play in children and adolescents was observed when the results from this study were compared to other studies developed in other countries from several socioeconomic status. The Active Play should be further investigated individually, as a pivotal component of physical activity, as well as its impact on physical inactivity-related comorbidities.


Resumo Foi realizada Revisão sistemática acerca da prevalência de brincadeiras ativas em crianças e adolescentes brasileiros. Apenas trabalhos científicos totalmente disponíveis que mediram a atividade física praticada como brincadeira ou lazer em crianças e/ou adolescentes brasileiros foram considerados para inclusão. A busca por artigos foi realizada nas seguintes bases de dados eletrônicas: PubMed/Medline, Web of Science, Bireme, Scielo e Scopus. Inicialmente, 63 artigos preencheram os critérios de elegibilidade, e após análise minuciosa, sete estudos foram incluídos na presente revisão. A prevalência geral de brincadeiras ativas foi de 36%, variando de 27,2% a 79,3%. Os meninos apresentaram prevalência de 47%, variando de 39,1% a 79,9%, enquanto a prevalência nas meninas chegou a 26%, variando de 13,9% a78,7%. Embora no Brasil essa prevalência não seja satisfatória, destaca-se o potencial de apresentar dados relevantes sobre esta temática. Corroborando com outros estudos, os meninos apresentaram maior prevalência de atividade física como brincadeira ou lazer que as meninas. Além disso, não foi encontrado um padrão socioeconômico em relação à prevalência de brincadeiras ativas em crianças e adolescentes quando os resultados deste estudo foram comparados com outros estudos desenvolvidos em outros países de diferentes condições socioeconômicas. Portanto, brincadeiras ativas devem ser investigadas individualmente, como um componente essencial da atividade física, bem como seu impacto nas comorbidades relacionadas à inatividade física.

Abstract The prevalence of obesity is rapidly increasing worldwide, no mattering age groups and socioeconomic status. In Brazil, it is still unclear the prevalence of obesity in children and adolescents, since most Brazilian studies have only verified regional prevalence of obesity. Therefore, the aim of the present study was to analyze the scientific production regarding the prevalence of weight excess in Brazilian children and adolescents. A search in the relevant electronic databases Medline/Pubmed, Web of Science, Lilacs, Scielo and BVS was performed. After analyzing 61 studies, the overall prevalence was 25.5%. When sample was stratified weight excess degree, a prevalence of 17% and 11.6% for overweight and obesity were observed, respectively. Analyzing differences by sex, boys presented higher prevalence of overall weight excess (e.g., 26.4% vs 23.5%), overweight (17% vs 16%) and obesity (11.9% vs 9.1%) than girls. With respect to Brazilian regions and its differences, individuals from southern presented the highest prevalence of overall weight excess (33.2%) and overweight (20.1%). The southeastern region showed the highest prevalence of obesity (18.2%). The results obtained in the current study indicate that Brazil presents a scenario of crescent increasing on the prevalence of weight excess. These results are in accordance with studies from other countries, and reinforce the increase of the overall weight excess prevalence in Brazilian children and adolescents, highlighting the increasing of obesity rates, since it is a more concerning condition than overweight. Therefore, preventive measures to reduce weight excess increase, as well as treatment programs aiming to tackle obesity in childhood should be public health system top priority.


Resumo A prevalência da obesidade tem aumentado em todo o mundo em todas as faixas etárias e níveis socioeconômicos. No Brasil, ainda é incerto a prevalência de obesidade em crianças e adolescentes devido a maioria dos estudos serem de características regionais.Portanto,o propósito do presente estudo foi analisar a produção científica referente à prevalência de excesso de peso em crianças e adolescentes brasileiros. Realizou-se uma busca nas bases de dados MEDLINE/PubMed, WEB of Science, LILACS, Scielo e BVS.Foram analisados 61 estudos, sendo identificado 17% de sobrepeso e 11,6% de obesidade.As prevalências de excesso de peso (26,4% vs 23,5%), sobrepeso (17% vs 16%) e obesidade (11,9% vs 9,1%) foram maiores entre os meninos comparados as meninas, respectivamente. As crianças e os adolescentes da Região Sul apresentaram maior prevalência de excesso de peso (33,2%) e sobrepeso (20,1%), e as do Sudeste maior prevalência de obesidade (18,2%). Nossa análise indicou que o Brasil apresenta um quadro de prevalência crescente de excesso de peso, similar ao de outros países.Os resultados reforçam o aumento da prevalência de excesso de peso em crianças e adolescentes no Brasil, especialmente pelo aumento da obesidade, que é considerada uma condição mais grave que o sobrepeso. Portanto, medidas preventivas para diminuir esta tendência de aumento do excesso de peso, bem como programas de tratamento da obesidade na infância e adolescência deve ser prioridade do Sistema único de Saúde (SUS).

ABSTRACT Background: Bariatric surgery is an alternative to the obesity treatment. Aim: To compare anthropometric variables such as body composition and physical fitness of those who performed Roux-en-Y gastric bypass. Methods: Were evaluated 108 women. They were subdivided in three groups: those who performed the bariatric surgery by private health insurance (SAS, n=36); by the public health care (SUS, n=36), and an equivalent group which did not perform the surgery (NO, n=36). Were performed physical fitness, anthropometric and body composition tests. Was evaluated the level of physical activity during the leisure period. Results: Statistically significant differences were observed between the groups sedentary operated (n=28) and sedentary non-operated (n=13) on anthropometry and fat percentage, being the highest indexes in the group operated. Conclusion: The level of physical activity showed a positive influence related to anthropometric variables, body composition of the individuals who performed the bariatric surgery when compared to the ones non-operated.


RESUMO Racional: Observa-se o aumento da realização da cirurgia bariátrica como alternativa de tratamento para obesidade. Objetivo: Comparar a antropometria, composição corporal e aptidão física de pacientes submetidos ao bypass gástrico em Y-de-Roux. Métodos: Foram avaliadas 108 mulheres subdivididas em: operadas por convênios de saúde particulares (SAS, n=36), Sistema Único de Saúde (SUS, n=36) e um grupo equivalente não operado (NO, n=36). Foram realizados testes de aptidão física, antropometria e composição corporal, e avaliado nível de atividade física no lazer. Resultados: Não houve diferença estatisticamente significativa nas variáveis estudadas entre os grupos SAS, SUS e NO. Observaram-se diferenças estatisticamente significativas entre os grupos operados sedentários (n=28) e não operados sedentários (n=13) na antropometria e percentual de gordura, sendo ao grupo operado os índices mais altos. Conclusões: O nível de atividade física apresentou influência positiva sobre as variáveis antropométricas, de composição corporal de pacientes submetidos à cirurgia bariátrica quando comparados aos não operados.

RESUMO Introdução: O risco cardiometabólico está associado a diversos fatores, entre os quais, o estado nutricional tem papel destacado. Objetivos: Este estudo verificou a influência do estado nutricional sobre o Índice de Adiposidade Visceral (VAI, do inglês) e seus componentes em crianças e adolescentes. Métodos: Estudo descritivo-associativo, de caráter transversal, realizado com 290 sujeitos de 10 a 18 anos, de ambos os sexos. Foram avaliados estatura, massa corporal, IMC, HDL-c, TG e VAI. Realizaram-se os testes Shapiro-Wilk, teste t de Student independente, U de Mann-Whitney, ANOVA One-Way, Kruskal-Wallis, Post Hoc de Bonferroni, Correlação de Pearson e cálculo da razão de chances (odds ratio). A significância adotada foi de p < 0,05. Resultados: Os testes demonstraram haver diferenças significativas entre os sexos para a idade, HDL-c e VAI. Nas variáveis HDL-c, TG e VAI foram constatadas diferenças significativas entre os adolescentes eutróficos e as demais classificações do estado nutricional. Verificou-se também uma piora das variáveis utilizadas para o cálculo do VAI para os adolescentes com classificação do estado nutricional alterado (não eutróficos), indicando, portanto, maior risco cardiometabólico associado ao excesso de peso. Verificou-se forte correlação entre o VAI e o TG (0,865) e correlação moderada negativa entre VAI e HDL-c (-0,405). Após o odds ratio, constatou-se que os participantes com excesso de peso e CC alterada, respectivamente, apresentaram 2,3 e 1,5 vezes mais chance de risco cardiometabólico (VAI alterado). Conclusões: O estado nutricional é capaz de influenciar as variáveis de risco cardiometabólico, VAI, CC, TG e HDL-c em crianças e adolescentes. Nesta população, independente do sexo, à medida que o valor do IMC se eleva, há progressão de alterações metabólicas. Em complemento, o VAI apresentou uma correlação significativa com TG, HDL-c, IMC e CC das crianças e adolescentes do estudo, podendo assim ser visto como alternativa de predição do risco cardiometabólico.


ABSTRACT Introduction: The cardiometabolic risk is associated with several factors, among which nutritional status plays a prominent role. Objectives: This study verified the influence of nutritional status on the Visceral Adiposity Index (VAI) and its components in children and adolescents. Methods: Descriptive, associative, cross-sectional study performed with a sample of 290 individuals from 10 to 18 years of age of both sexes. We assessed height, body mass, BMI, HDL-c, TG, and VAI. The following tests were used: Shapiro-Wilk, independent Student t test, Mann-Whitney U-test, ANOVA One-Way, Kruskal-Wallis, Bonferroni post hoc test, Pearson Correlation and odds ratio. The significance level was established at p<0.05. Results: The tests showed significant differences between the sexes for age, HDL-c, and VAI. Concerning the variables HDL-c, TG and VAI, significant differences were observed among the eutrophic adolescents and the other classifications of nutritional status. There was also a worsening of the variables used to calculate VAI for adolescents with altered nutritional status (non-eutrophic), indicating a higher cardiometabolic risk associated with overweight There was a strong correlation between VAI and TG (0.865) and moderate negative correlation between VAI and HDL-c (-0.405). After the odds ratio calculation, it was found that participants with overweight and altered WC, respectively presented 2.3 and 1.5 times more chance of cardiometabolic risk (altered VAI). Conclusions: The nutritional status is capable of influence the variables of cardiometabolic VAI, WC, TG, and HDL-c in children and adolescents. In this population, regardless of sex, as the BMI increases, there is a progression of metabolic changes. In addition, VAI index showed a significant correlation with TG, HDL-c, BMI, and WC of the children and adolescents in the study, which could be seen as an alternative to predict metabolic risk.


RESUMEN Introducción: El riesgo cardiometabólico se asocia con varios factores, incluyendo el estado nutricional, que tiene un papel importante. Objetivos: Este estudio examinó la influencia del estado nutricional sobre el Índice de Adiposi-dad Visceral (VAI, del inglés) y sus componentes en niños y adolescentes. Métodos: Estudio descriptivo-asociativo, de carácter transversal, realizado con 290 sujetos de 10 a 18 años, de ambos sexos. Se evaluó la altura, la masa corporal, IMC, HDL-c, TG y VAI. Fueron realizadas las pruebas Shapiro-Wilk, prueba t de Student independiente, prueba U de Mann-Whitney, ANOVA One-Way, Kruskal-Wallis, prueba post hoc de Bonferroni, Correlación de Pearson y cálculo de la razón de posibilidades (odds ratio). La significación adoptada fue de p < 0,05. Resultados: Las pruebas demostra-ron diferencias significativas entre los sexos para la edad, HDL-c y VAI. En las variables HDL-c, TG y VAI se constataron diferencias significativas entre los adolescentes eutróficos y las otras clasificaciones del estado nutricional. También hubo un empeoramiento de las variables utilizadas para el cálculo del VAI para los adolescentes con clasificación de estado nutricional alterado (no eutróficos), lo que indica un mayor riesgo cardiometabólico asociado con el exceso de peso. Hubo una fuerte correlación entre VAI e TG (0,865) y correlación negativa moderada entre VAI y HDL-c (-0,405). Después del cálculo de la razón de posibilidades se encontró que los participantes con sobrepeso y CC alterada, respectivamente, presentaron 2,3 y 1,5 veces más probabilidades de riesgo cardiometabólico (VAI alterado). Conclusiones: El estado nutricional puede influir en las variables de riesgo cardiometabólico, VAI, CC, TG y HDL-c en niños y adolescentes. En esta población, independientemente del sexo, cuando el valor del IMC se eleva, hay progresión de los cambios metabólicos. Además, el VAI mostró una correlación significativa con TG, HDL-c, IMC y CC de los niños y adolescentes del estudio y, por lo tanto, se puede considerar como un predictor alternativo de riesgos cardiometabólicos.

To compare water- versus land-based exercise programs, as part of a multidisciplinary intervention, on physical fitness and health-related quality of life in overweight and obese adolescents. A total of 151 overweight and obese adolescents (age: 13.2±1.8years) were divided into: 1) a water-based intervention (WB; n=28); 2) a land-based intervention (LB; n=57); and 3) a control group (CG; n=66). Anthropometric measures, body composition, flexibility, abdominal strength/endurance, cardiorespiratory fitness and health-related quality of life were assessed prior and by the end of a 16-week intervention. Both the water- and land-based interventions were part of a multidisciplinary program focusing on behavioral changes. Fat Mass (%) was significantly reduced in both intervention groups and a higher progression was observed in the WB group (-9.8±7.2% versus -6.1±6.2%; p<0.05 for pre-to-post changes within-groups). However, LB increased more abdominal strength than WB (69.6±165.8% versus 53.4±83.4%; p<0.05) and significantly increased social (19.7±88.8%), psychosocial (5.1±15.1%) and total score (6.3±14.8%), which was observed in neither WB nor CG. The intervention groups did not change body weight (WB: -1.7±5.3%; LB: 0.1±3.4%; p>0.05 for both groups) and increased (p<0.05) fat-free mass (WB: 4.6±4.0%; LB: 4.4±4.1%), VO2max (WB: 9.3±7.8%; LB: 11.0±13.4%), and physical dimension of quality of life (WB: 9.2±17.2%; LB: 8.9±18.0%) differently of what was observed in CG. The physical dimension improvement observed in the land-based and water-based samples was associated with decreased percentage fat mass (r=0.282 and 0.229; p<0.05). We suggest water- and land-based physical exercises within a multidisciplinary program promote similar improvements in body composition, physical fitness and health-related quality of life in overweight and obese adolescents.

OBJECTIVE: To compare body composition, hemodynamic parameters, health-related physical fitness, and health-related quality of life of adolescents with anthropometric diagnosis of overweight, obesity, and severe obesity. METHODS: 220 adolescents with excess body weight were enrolled. They were beginners in a intervention program that included patients based on age, availability, presence of excess body weight, place of residence, and agreement to participate in the study. This study collected anthropometric and hemodynamic variables, health-related physical fitness, and health-related quality of life of the adolescents. To compare the three groups according to nutritional status, parametric and non-parametric tests were applied. Significance level was set at p<0.05. RESULTS: There was no significant difference in resting heart rate, health-related physical fitness, relative body fat, absolute and relative lean mass, and health-related quality of life between overweight, obese, and severely obese adolescents (p>0.05). Body weight, body mass index, waist and hip circumference, and systolic blood pressure increased as degree of excess weight increased (p<0.05). Dyastolic blood pressure of the severe obesity group was higher than the other groups (p<0.05). There was an association between the degree of excess weight and the prevalence of altered blood pressure (overweight: 12.1%; obesity: 28.1%; severe obesity: 45.5%; p<0.001). The results were similar when genders were analyzed separately. CONCLUSION: Results suggest that overweight adolescents presented similar results compared to obese and severely obese adolescents in most of the parameters analyzed.

RESUMO

OBJECTIVE: To compare body composition, hemodynamic parameters, health-related physical fitness, and health-related quality of life of adolescents with anthropometric diagnosis of overweight, obesity, and severe obesity. METHODS: 220 adolescents with excess body weight were enrolled. They were beginners in a intervention program that included patients based on age, availability, presence of excess body weight, place of residence, and agreement to participate in the study . This study collected anthropometric and hemodynamic variables, health-related physical fitness, and health-related quality of life of the adolescents. To compare the three groups according to nutritional status, parametric and non-parametric tests were applied. Significance level was set at p<0.05. RESULTS: There was no significant difference in resting heart rate, health-related physical fitness, relative body fat, absolute and relative lean mass, and health-related quality of life between overweight, obese, and severely obese adolescents (p>0.05). OBJETIVO: Comparar composição corporal, parâmetros hemodinâmicos, aptidão física relacionada à saúde e qualidade de vida relacionada à saúde em adolescentes com o diagnóstico antropométrico de sobrepeso, obesidade e obesidade grave. MÉTODOS: Foram selecionados 220 adolescentes com excesso de peso, ingressantes em um programa de intervenção segundo os critérios de inclusão neste programa, baseados na idade, disponibilidade, presença de excesso de peso, local de residência e concordância na participação do estudo. Foram coletadas variáveis antropométricas, hemodinâmicas, aptidão física relacionada à saúde e qualidade de vida relacionada à saúde dos adolescentes. Para a análise de comparação entre os três grupos, foram utilizados testes paramétricos e não paramétricos quando apropriado. A significância foi pré-estabelecido em p<0,05. RESULTADOS: Não houve diferença significativa para a frequência cardíaca de repouso, aptidão física relacionada à saúde, gordura relativa, massa magra (relativa e absoluta) e qualidade de vida relacionada à saúde entre adolescentes com sobrepeso, obesos e obesos graves (p>0,05). O peso, índice de massa corporal, circunferência de cintura e de quadril e pressão arterial sistólica aumentaram conforme aumentou o grau de excesso de peso (p<0,05). A pressão arterial diastólica do grupo com obesidade grave foi maior que a dos demais grupos (p<0,05). Observou-se associação entre o aumento grau de excesso de peso e a prevalência de pressão arterial alterada (sobrepeso: 12,1%; obesidade: 28,1%; obesidade grave: 45,5%; p<0,001).
