RESUMO

Botulism is a rare cause of flaccid paralysis resulting from the neurotoxin produced by the bacterium Clostridium botulinum. It is clinically characterized by symmetric proximal-distal paralysis, diplopia, dysarthria, dysphonia, and dysphagia. Definitive diagnosis requires laboratory confirmation through the detection of the toxin in blood, vomit, or stool samples. Treatment with antitoxin should be promptly initiated upon clinical suspicion and in the presence of epidemiological support due to evidence of reduced mortality. Physical and rehabilitation medicine plays a fundamental role in the recovery of deficits and prevention of complications. In this report, the authors describe a 19-year-old patient with botulism with dysphagia, dysphonia, and facial paresis in the rehabilitation ward.

Mucopolysaccharidoses (MPSs) are rare inherited lysosomal storage diseases (LSDs) caused by deficient activity in one of the enzymes responsible for glycosaminoglycans lysosomal degradation. MPS II is caused by pathogenic mutations in the IDS gene, leading to deficient activity of the enzyme iduronate-2-sulfatase, which causes dermatan and heparan sulfate storage in the lysosomes. In MPS VI, there is dermatan sulfate lysosomal accumulation due to pathogenic mutations in the ARSB gene, leading to arylsulfatase B deficiency. Alterations in the immune system of MPS mouse models have already been described, but data concerning MPSs patients is still scarce. Herein, we study different leukocyte populations in MPS II and VI disease patients. MPS VI, but not MPS II patients, have a decrease percentage of natural killer (NK) cells and monocytes when compared with controls. No alterations were identified in the percentage of T, invariant NKT, and B cells in both groups of MPS disease patients. However, we discovered alterations in the naïve versus memory status of both helper and cytotoxic T cells in MPS VI disease patients compared to control group. Indeed, MPS VI disease patients have a higher frequency of naïve T cells and, consequently, lower memory T cell frequency than control subjects. Altogether, these results reveal MPS VI disease-specific alterations in some leukocyte populations, suggesting that the type of substrate accumulated and/or enzyme deficiency in the lysosome may have a particular effect on the normal cellular composition of the immune system.

Resumo As tecnologias da informação e comunicação têm influência cada vez maior na área da saúde, sendo o aumento significativo do recurso à teleconsulta um reflexo disso. Os benefícios que essa modalidade de prestação de serviços de saúde ocasiona são inquestionáveis, a começar pelo facto de auxiliarem a enfrentar os desafios contemporâneos que assolam os sistemas de saúde em todo o mundo. Todavia, a sua crescente utilização vem sublinhar a importância de salvaguardar questões éticas relacionadas com a autonomia, equidade, privacidade e qualidade da relação entre o utente e o profissional de saúde. Este artigo pretende estimular uma reflexão acerca dos desafios éticos que se colocam antes, durante e após o uso da teleconsulta, com o intuito de otimizar e modelar a sua utilização.


Abstract Information and communication Technologies are increasingly present in health care, as exemplified by the recourse to remote consultation. Such care delivery modality brings unquestionable benefits, such as helping to face the contemporary challenges plaguing health systems worldwide. But its ever-increasing use underlines the importance of safeguarding ethical issues related to autonomy, equity, privacy, and the quality of the user-professional relations. This paper reflects on the ethical challenges presented before, during, and after remote consultation as to optimize and shape its use.


Resumen Las tecnologías de la información y la comunicación tienen una influencia cada vez mayor en el área de la salud, y esto lleva a un aumento significativo en el uso de la teleconsulta. Muchos son los beneficios de esta modalidad de prestación de servicios sanitarios, comenzando por el hecho de que ayudan a abordar los desafíos contemporáneos que afectan a los sistemas de salud en todo el mundo. Sin embargo, su uso creciente destaca la importancia de salvaguardar las cuestiones éticas relacionadas con la autonomía, la equidad, la privacidad y la calidad de la relación entre el paciente y el profesional de la salud. Este artículo pretende incitar a una reflexión sobre los desafíos éticos que surgen antes, durante y después del uso de la teleconsulta, con el fin de optimizarlo y modelarlo.

RESUMO

Persistent Homology (PH) analysis is a powerful tool for understanding many relevant topological features from a given dataset. PH allows finding clusters, noise, and relevant connections in the dataset. Therefore, it can provide a better view of the problem and a way of perceiving if a given dataset is equal to another, if a given sample is relevant, and how the samples occupy the feature space. However, PH involves reducing the problem to its simplicial complex space, which is computationally expensive and implementing PH in such Resource-Scarce Embedded Systems (RSES) is an essential add-on for them. However, due to its complexity, implementing PH in such tiny devices is considerably complicated due to the lack of memory and processing power. The following paper shows the implementation of 0-Dimensional Persistent Homology Analysis in a set of well-known RSES, using a technique that reduces the memory footprint and processing power needs of the 0-Dimensional PH algorithm. The results are positive and show that RSES can be equipped with this real-time data analysis tool.

PURPOSE: To analyze the visual and refractive results after secondary IOL implantation using different surgical techniques - iris-claw aphakic IOL through a corneal incision or scleral tunnel, and 3-piece IOL into the ciliary sulcus. PATIENTS AND METHODS: Retrospective study including patients that were submitted to secondary IOL implantation from January 2017 to December 2019 at the Department of Ophthalmology of Hospital de Braga, Portugal. We collected demographic data (age, surgical indication, comorbidities, surgical technique, IOL implanted, and intra and postoperative complications) and visual and refractive data [preoperative and 3-month, 6-month, and 12-month postoperative corrected-distance visual acuity (CDVA), spherical equivalent (SE), manifest cylinder, and intraocular pressure]. RESULTS: 128 eyes from 123 patients were included. The most frequent surgical indications were IOL subluxation/luxation (62.5%) and intraoperative posterior capsular rupture (23.4%). CDVA improved from 1.26 ± 0.51 to 0.47 ± 0.49 logMar (p < .001). CDVA was significantly better in the 3-piece into the ciliary sulcus IOL group than both other groups. The final SE was -0.68 ± 0.94 diopters. The mean manifest refractive cylinder remained stable until the 6th month after the surgery (p = .454) and improved in the last 6 months of follow-up (p = .015). In the postoperative period, the cylinder was higher in the corneal incision iris-claw aphakic IOL group and lower in the 3-piece IOL into the ciliary sulcus group (p < 0,05). The corneal incision iris-claw aphakic IOL group presented the most postoperative complications. CONCLUSION: All techniques showed satisfying results. The 3-piece IOL into the ciliary sulcus group showed the best visual and refractive performance, followed by the scleral tunnel iris-claw IOL group and the corneal incision iris-claw IOL group.

BACKGROUND: The clinical presentation of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection can range from mild or moderate disease (80% of the cases) to severe disease (15%) requiring oxygen support, and critical disease (5%), associated with acute respiratory distress syndrome and admission to the intensive care unit (ICU). In critically ill patients, prone positioning can be used to optimize oxygenation. Although there is a favourable response to this strategy, being a life-saving measure, additional associated complications may appear, including compressive neuropathies. Despite respiratory affection being more common, SARS-CoV-2 infection can also attack other systems and can, under certain conditions, affect the central or peripheral nervous system. It has been described that neurological manifestations can result from the neuroinvasive properties of the SARS-CoV-2 or as an indirect consequence of multiorgan dysfunction. AIMS: We intend to report the patients who presented with neurological complications associated with coronavirus disease 2019 (COVID-19) and/or complications of its treatment, followed in our physical and rehabilitation medicine (PRM) service. MATERIALS AND METHODS: A retrospective analysis of patients admitted to the PRM ward with outpatient consultation in the context of post-COVID-19 status between April 2020 and November 2021 (the period of the highest prevalence of infection) was carried out. Patients with neurological complications after SARS-CoV-2 infection and consequently a decline in previous functionality were identified. RESULTS: Thirteen patients (23.6%) admitted to the PRM ward had peripheral neurological complications, documented by electroneuromyography, including Guillain-Barré syndrome, sensory-motor polyneuropathy, peroneal nerve injury, femoral nerve injury, and lumbar plexus injury. The neurological complications of the patients followed in a post-COVID-19 consultation were also evaluated. Eight patients (20%) reported neurological sequelae. Five patients presented peripheral nerve damage (peroneal, accessory, ulnar, and recurrent laryngeal) of undefined aetiology, diagnosed after the acute phase of hospitalization. Two patients had COVID-19 infection followed by ischemic stroke (vertebrobasilar and middle cerebral artery), requiring hospitalization in the acute phase. One patient had COVID-19 infection followed by longitudinal myelitis, with positive anti-myelin oligodendrocyte glycoprotein (MOG). All patients required follow-up by the rehabilitation team with partial recovery of deficits. CONCLUSIONS:  All patients admitted to the PRM ward with neurological manifestations had critical disease and symptoms compatible with peripheral nervous system involvement. Patients admitted to the PRM consultation had different levels of viral disease severity and had sequelae related to peripheral and central nervous system disorders. Identifying the aetiology of these injuries is essential for us to act on their prevention, particularly with regard to indirect complications, such as compressive neuropathies. It will be necessary to maintain the follow-up of these patients to understand the evolution of the neurological consequences associated with COVID-19.

Endothelial cells (ECs) are an important component of the tumor microenvironment, playing key roles in tumor development and progression that span from angiogenesis to immune regulation and drug resistance. Heterotypic tumor spheroids are one of the most widely used in vitro tumor microenvironment models, presenting improved recapitulation of tumor microenvironments compared to 2D cultures, in a simple and low-cost setup. Heterotypic tumor spheroid models incorporating endothelial cells have been proposed but present multiple limitations, such as the short culture duration typically obtained, the use of animal-derived matrices, and poor reproducibility; the diversity of culture conditions employed hinders comparison between studies and standardization of relevant culture parameters. Herein, we developed long-term cultures of triple heterotypic spheroids composed of the HCC1954 tumor cell line, human fibroblasts, and ECs. We explored culture parameters potentially relevant for EC maintenance, such as tumor cell line, seeding cell number, cell ratio, and agitation vs. static culture. In HCC1954-based spheroids, we observed maintenance of viable EC for up to 1 month of culture in agitation, with retention of the identity markers CD31 and von Willebrand factor. At the optimized tumor cell:fibroblast:EC ratio of 1:3:10, HCC1954-based spheroids had a higher EC area/total spheroid area at 1 month of culture than the other cell ratios tested. EC maintenance was tumor cell line-dependent, and in HCC1954-based spheroids it was also dependent on the presence of fibroblasts and agitation. Moreover, vascular endothelial growth factor (VEGF) supplementation was not required for maintenance of EC, as the factor was endogenously produced. ECs co-localized with fibroblasts, which accumulated preferentially in the core of the spheroids and secreted EC-relevant extracellular matrix proteins, such as collagen I and IV. This simple model setup does not rely on artificial or animal-derived scaffolds and can serve as a useful tool to explore the culture parameters influencing heterotypic spheroids, contributing to model standardization, as well as to explore molecular cross talk of ECs within the tumor microenvironment, and potentially its effects on drug response.

The immune microenvironment of breast cancer (BC) is composed by high macrophage infiltrates, correlated with the most aggressive subtypes. Tumour-associated macrophages (TAM) within the BC microenvironment are key regulators of immune suppression and BC progression. Nevertheless, several key questions regarding TAM polarisation by BC are still not fully understood. Recently, the modulation of the immune microenvironment has been described via the recognition of abnormal glycosylation patterns at BC cell surface. These patterns rise as a resource to identify potential targets on TAM in the BC context, leading to the development of novel immunotherapies. Herein, we will summarize recent studies describing advances in identifying altered glycan structures in BC cells. We will focus on BC-specific glycosylation patterns known to modulate the phenotype and function of macrophages recruited to the tumour site, such as structures with sialylated or N-acetylgalactosamine epitopes. Moreover, the lectins present at the surface of macrophages reported to bind to such antigens, inducing tumour-prone TAM phenotypes, will also be highlighted. Finally, we will discuss and give our view on the potential and current challenges of targeting these glycan-lectin interactions to reshape the immunosuppressive landscape of BC.

Caffeic acid (CA) and its phenethyl ester (CAPE) are naturally occurring hydroxycinnamic acids with an interesting array of biological activities; e.g., antioxidant, anti-inflammatory, antimicrobial and cytostatic. More recently, several synthetic analogs have also shown similar properties, and some with the advantage of added stability. The actions of these compounds on the cardiovascular system have not been thoroughly explored despite presenting an interesting potential. Indeed the mechanisms underlying the vascular effects of these compounds particularly need clarifying. The aim of this paper is to provide a comprehensive and up-to-date review on current knowledge about CA and its derivatives in the cardiovascular system. Caffeic acid, CAPE and the synthetic caffeic acid phenethyl amide (CAPA) exhibit vasorelaxant activity by acting on the endothelial and vascular smooth muscle cells. Vasorelaxant mechanisms include the increased endothelial NO secretion, modulation of calcium and potassium channels, and modulation of adrenergic receptors. Together with a negative chronotropic effect, vasorelaxant activity contributes to lower blood pressure, as several preclinical studies show. Their antioxidant, anti-inflammatory and anti-angiogenic properties contribute to an important anti-atherosclerotic effect, and protect tissues against ischemia/reperfusion injuries and the cellular dysfunction caused by different physico-chemical agents. There is an obvious shortage of in vivo studies to further explore these compounds' potential in vascular physiology. Nevertheless, their favorable pharmacokinetic profile and overall lack of toxicity make these compounds suitable for clinical studies.

The progress of next-generation sequencing has lead to the availability of massive data sets used by a wide range of applications in biology and medicine. This has sparked significant interest in using modern Big Data technologies to process this large amount of information in distributed memory clusters of commodity hardware. Several approaches based on solutions such as Apache Hadoop or Apache Spark, have been proposed. These solutions allow developers to focus on the problem while the need to deal with low level details, such as data distribution schemes or communication patterns among processing nodes, can be ignored. However, performance and scalability are also of high importance when dealing with increasing problems sizes, making in this way the usage of High Performance Computing (HPC) technologies such as the message passing interface (MPI) a promising alternative. Recently, MetaCacheSpark, an Apache Spark based software for detection and quantification of species composition in food samples has been proposed. This tool can be used to analyze high throughput sequencing data sets of metagenomic DNA and allows for dealing with large-scale collections of complex eukaryotic and bacterial reference genome. In this work, we propose MetaCache-MPI, a fast and memory efficient solution for computing clusters which is based on MPI instead of Apache Spark. In order to evaluate its performance a comparison is performed between the original single CPU version of MetaCache, the Spark version and the MPI version we are introducing. Results show that for 32 processes, MetaCache-MPI is 1.65× faster while consuming 48.12% of the RAM memory used by Spark for building a metagenomics database. For querying this database, also with 32 processes, the MPI version is 3.11× faster, while using 55.56% of the memory used by Spark. We conclude that the new MetaCache-MPI version is faster in both building and querying the database and uses less RAM memory, when compared with MetaCacheSpark, while keeping the accuracy of the original implementation.

The urban heat island effect creates warmer and drier conditions in urban areas than in their surrounding rural areas. This effect is predicted to be exacerbated in the future, under a climate change scenario. One way to mitigate this effect is to use the urban green infrastructure as a way to promote the cooling island effect. In this study we aimed to model, with a high spatial resolution, how Mediterranean urban parks can be maximized to be used as cooling islands, by answering the following questions: i) which factors influence the cooling effect and when?; ii) what type of green spaces contributes the most to the cooling effect?; iii) what is the cooling distance of influence? To answer these questions we established a sampling design where temperature and relative humidity were measured in different seasons, in locations with contrasting characteristics of green and grey cover. We were able to model the effect of green and grey spaces in the cooling island effect and build high spatial resolution predicting maps for temperature and relative humidity. Our study showed that even green spaces with reduced areas can regulate microclimate, alleviating temperature by 1-3 °C and increasing moisture by 2-8%, on average. Green spaces with a higher density of trees were more efficient in delivering the cooling effect. The morphology, aspect and level of exposure of grey surfaces to the solar radiation were also important features included in the models. Green spaces influenced temperature and relative humidity up to 60 m away from the parks' limits, whereas grey areas influenced in a much lesser range, from 5 m up to 10 m. These models can now be used by citizens and stakeholders for green spaces management and human well-being impact assessment.

Bone marrow contains endothelial progenitor cells (EPCs) that, upon pro-angiogenic stimuli, migrate and differentiate into endothelial cells (ECs) and contribute to re-endothelialization and neo-vascularization. There are currently no reliable markers to characterize EPCs, leading to their inaccurate identification. In the past, we showed that, in a panel of tumors, some cells on the vessel wall co-expressed CD14 (monocytic marker) and CD31 (EC marker), indicating a putative differentiation route of monocytes into ECs. Herein, we disclosed monocytes as potential EPCs, using in vitro and in vivo models, and also addressed the cancer context. Monocytes acquired the capacity to express ECs markers and were able to be incorporated into blood vessels, contributing to cancer progression, by being incorporated in tumor neo-vasculature. Reactive oxygen species (ROS) push monocytes to EC differentiation, and this phenotype is reverted by cysteine (a scavenger and precursor of glutathione), which indicates that angiogenesis is controlled by the interplay between the oxidative stress and the scavenging capacity of the tumor microenvironment.

PURPOSE: The aim of this study is to analyze the lamina cribrosa depth and to study the optic nerve in patients with spontaneous intracranial hypotension. METHODS: A total of 10 eyes of the patients with spontaneous intracranial hypotension with unknown etiology (study group) and 10 eyes of healthy patients without any ophthalmological or neurological pathologies (control group) were included. The subjects were submitted to ophthalmological examination with the evaluation of visual acuity, spherical equivalent, applanation tonometry, pachymetry, axial length, retinography, computerized static perimetry (Humphrey 30-2 SITA-Standard), and OCT Spectralis with enhanced depth image, to calculate the depth of the anterior surface of the lamina cribrosa and to measure the nerve fiber layer thickness of the optic nerve. All of these parameters were compared between the two groups. RESULTS: Mean anterior surface of the lamina cribrosa depth was 447.96 ± 118.51 (313.30-632.0) µm for the spontaneous intracranial hypotension group and 292.56 ± 40.71 (247-387) µm for the control group, with a statistically significant difference between them (p = 0.001). The thickness of the nerve fiber layer did not differ significantly in the temporal (p = 0.94), nasal (p = 0.173), superior (p = 0.41), central (p = 0.36) or inferior (p = 0.5) sectors. Four eyes of patients with spontaneous intracranial hypotension showed a marked reduction in the temporal nerve fiber layer. Pachymetry (p = 0.16), axial length (p = 0.71), and intraocular pressure (p = 0.6) did not differ significantly between groups, whereas spherical equivalent (p = 0.03) was significantly different. Visual fields were normal in both groups. CONCLUSION: The translaminar gradient is determinant in the structure of the lamina cribrosa. Low intracranial pressure associated with a high translaminar gradient leads to an increased lamina cribrosa depth similar to that observed in glaucoma patients.

Abstract Purpose: To validate the comparative measurements of intraocular pressure performed with IcarePRO tonometer (IPT) in relation to Goldmann applanation tonometer (GAT) and non-contact tonometer (NCT), as well as to evaluate the influence of central corneal thickness (CCT) on these values. Methods: This was a prospective and comparative study conducted during 6 months in the Department of Ophthalmology, Hospital de Braga, Portugal. The study population comprised two groups: healthy adults and adults with primary open angle glaucoma (POAG). The IOP values were obtained by the three tonometers and the order of execution was randomly selected. CCT values were obtained by ultrasonic pachymetry. Results: A total of 168 eyes (74 with diagnosis of POAG and 94 healthy) of 84 patients were included in this study. The mean IOP values obtained by IPT, GAT and NCT were 17.36±3.78 mmHg; 14.59±3.32 mmHg; and 17.04±4.01 mmHg, respectively. The comparison between IPT and NCT did not reveal statistically significant differences in the two groups studied. However, statistically significant differences were obtained between IPT and GAT values (p <0.001). The IOP values, recorded by all the tonometers, were significantly and positivily correlated with the CCT (p <0.001). Conclusion: IOP readings measured by IPT are comparable with those obtained by NCT, but higher than those obtained by the GAT. CCT values are correlated with IOP measurements with IPT, as it does in GAT and NCT, and should be always taken into consideration.


Resumo Objetivo: Validar os valores de pressão intraocular (IOP) realizados com o tonômetro IcarePRO (IPT) em relação ao tonômetro de aplanação de Goldmann (GAT) e tonômetro de sopro (NCP), assim como avaliar a influência da espessura central da córnea (CCT) nesses valores. Métodos: Estudo prospectivo e comparativo realizado durante 6 meses consecutivos no Departamento de Oftalmologia do Hospital de Braga, Portugal. A população estudada compreendeu dois grupos: adultos saudáveis e adultos com glaucoma primário de ângulo aberto (POAG). Os valores de IOP foram obtidos pelos três tonômetros e a ordem de execução foi selecionada aleatoriamente. Os valores de CCT foram obtidos por paquimetria ultrassônica. Resultados: Um total de 168 olhos (74 com diagnóstico de POAG e 94 saudáveis) de 84 pacientes foram incluídos neste estudo. Os valores médios de IOP obtidos pelo IPT, GAT e NCT foram de 17.36±3.78 mmHg; 14.59±3.32 mmHg; e 17.04±4.01 mmHg, respectivamente. A comparação entre IPT e NCT não revelou as diferenças estatisticamente significativas nos dois grupos estudados. No entanto, foram obtidas diferenças estatisticamente significativas entre os valores registados pelo IPT em comparação ao GAT (p <0.001). Os valores de IOP, medidos por todos os tonômetros, foram significativamente correlacionados com a CCT (p <0.001). Conclusão: As leituras de PIO medidas pelo IPT são comparáveis às obtidas pelo NCT, mas superiores às obtidas pelo GAT. Os valores de CCT estão correlacionados com medidas de IOP, tal como acontece com o GAT e NCT, pelo que devem ser levados em considerados.

The growing human population concentrated in urban areas lead to the increase of road traffic and artificial areas, consequently enhancing air pollution and urban heat island effects, among others. These environmental changes affect citizen's health, causing a high number of premature deaths, with considerable social and economic costs. Nature-based solutions are essential to ameliorate those impacts in urban areas. While the mere presence of urban green spaces is pointed as an overarching solution, the relative importance of specific vegetation structure, composition and management to improve the ecosystem services of air purification and climate regulation are overlooked. This avoids the establishment of optimized planning and management procedures for urban green spaces with high spatial resolution and detail. Our aim was to understand the relative contribution of vegetation structure, composition and management for the provision of ecosystem services of air purification and climate regulation in urban green spaces, in particular the case of urban parks. This work was done in a large urban park with different types of vegetation surrounded by urban areas. As indicators of microclimatic effects and of air pollution levels we selected different metrics: lichen diversity and pollutants accumulation in lichens. Among lichen diversity, functional traits related to nutrient and water requirements were used as surrogates of the capacity of vegetation to filter air pollution and to regulate climate, and provide air purification and climate regulation ecosystem services, respectively. This was also obtained with very high spatial resolution which allows detailed spatial planning for optimization of ecosystem services. We found that vegetation type characterized by a more complex structure (trees, shrubs and herbaceous layers) and by the absence of management (pruning, irrigation and fertilization) had a higher capacity to provide the ecosystems services of air purification and climate regulation. By contrast, lawns, which have a less complex structure and are highly managed, were associated to a lower capacity to provide these services. Tree plantations showed an intermediate effect between the other two types of vegetation. Thus, vegetation structure, composition and management are important to optimize green spaces capacity to purify air and regulate climate. Taking this into account green spaces can be managed at high spatial resolutions to optimize these ecosystem services in urban areas and contribute to improve human well-being.

Urban areas' population has grown during the last century and it is expected that over 60% of the world population will live in cities by 2050. Urban parks provide several ecosystem services that are valuable to the well-being of city-dwellers and they are also considered a nature-based solution to tackle multiple environmental problems in cities. However, the type and amount of ecosystem services provided will vary with each park vegetation type, even within same the park. Our main goal was to quantify the trade-offs in ecosystem services associated to different vegetation types, using a spatially detailed approach. Rather than relying solely on general vegetation typologies, we took a more ecologically oriented approach, by explicitly considering different units of vegetation structure and composition. This was demonstrated in a large park (44ha) located in the city of Almada (Lisbon metropolitan area, Portugal), where six vegetation units were mapped in detail and six ecosystem services were evaluated: carbon sequestration, seed dispersal, erosion prevention, water purification, air purification and habitat quality. The results showed that, when looking at the park in detail, some ecosystem services varied greatly with vegetation type. Carbon sequestration was positively influenced by tree density, independently of species composition. Seed dispersal potential was higher in lawns, and mixed forest provided the highest amount of habitat quality. Air purification service was slightly higher in mixed forest, but was high in all vegetation types, probably due to low background pollution, and both water purification and erosion prevention were high in all vegetation types. Knowing the type, location, and amount of ecosystem services provided by each vegetation type can help to improve management options based on ecosystem services trade-offs and looking for win-win situations. The trade-offs are, for example, very clear for carbon: tree planting will boost carbon sequestration regardless of species, but may not be enough to increase habitat quality. Moreover, it may also negatively influence seed dispersal service. Informed practitioners can use this ecological knowledge to promote the role of urban parks as a nature-based solution to provide multiple ecosystem services, and ultimately improve the design and management of the green infrastructure. This will also improve the science of Ecosystem Services, acknowledging that the type of vegetation matters for the provision of ecosystem services and trade-offs analysis.

Este relatório resulta da aprendizagem no Estágio de Enfermagem de Saúde da Criança e do Jovem II e no Mestrado em Enfermagem de Saúde da Criança e do Jovem. O objetivo é fundamentar as competências desenvolvidas no sentido de uma enfermagem avançada, através da prática baseada na evidência com recurso à Revisão Sistemática de Literatura, subordinada ao tema da disciplina no exercício da parentalidade. Inclui o enquadramento teórico sobre as áreas da parentalidade, a disciplina, o impacto no desenvolvimento das crianças em idade pré-escolar e a avaliação das competências parentais. A reflexão sobre as aprendizagens e atividades desenvolvidas. A Revisão Sistemática de Literatura, que questiona "Quais as competências parentais na área da disciplina, nos pais de crianças em idade pré-escolar?" com as evidências encontradas e a ligação à prática dos cuidados de enfermagem. Conclui-se que os enfermeiros têm um papel importante no apoio aos pais no exercício da parentalidade durante o desenvolvimento da criança, com consequente impacto no seu futuro e que se traduz em benefícios no comportamento das crianças e na relação pais-filhos. A avaliação das competências parentais contribui para a melhoria dos cuidados, permitindo desenvolver intervenções baseadas nas necessidades dos pais. Esta avaliação permite atestar o resultado das intervenções efetuadas e demonstrar ganhos em saúde sensíveis aos cuidados de enfermagem


This report is the result of the apprenticeship in the Child and Youth Health Nursing Internship II and the Master in Child and Youth Health Nursing. The objective is to base the competences developed towards an advanced nursing, through the practice based on evidence using the Systematic Review of Literature, subordinated to the subject of discipline in the exercise of parenting. It includes the theoretical framework on the areas of parenting, discipline, the impact on the development of pre-school children and the evaluation of parental competences. The reflection on the learning and activities developed. The Systematic Review of Literature, which questions "What are the parental competences in the area of the discipline, in the parents of preschool children?" with the evidence found and the connection to the practice of nursing care. It is concluded that nurses have an important role in supporting parents in the exercise of parenting during the development of the child, with a consequent impact on their future and that translates into benefits in children's behavior and parent-child relationship. The evaluation of parental competences contributes to the improvement of the care, allowing to develop interventions based on the needs of the parents. This evaluation allows to certify the results of the interventions carried out and to demonstrate health gains sensitive to nursing care

BACKGROUND: In the UK 15.8% of people aged 85 years and over live in a care home or long-stay hospital setting. With the projection of an ageing population it is realistic to expect that the number of people both living and dying in all care homes will increase. This article describes the implementation of an end-of-life care programme to empower staff to meet their resident's end-of-life care needs. METHODS: To implement an end-of-life care programme, namely the 'Steps to Success' programme, in residential care homes. Measurable outcomes were collected through audit. RESULTS: Over four years audit of all deceased residents' records in the participating homes was collected. This shows an increase of home deaths in 2011/12 to 2014/15 from 44% (n=8/18) within four residential care homes to 64% (n=74/115) in 23 residential care homes with corresponding increase in advance care plan discussions and completion of 'do not attempt cardiopulmonary resuscitation' forms. CONCLUSION: Achieving change is any organisation let alone sustaining such change is not easy. Six factors enabled this to occur and these should be considered when implementing other such initiatives in residential care homes.

Forested areas within cities host a large number of species, responsible for many ecosystem services in urban areas. The biodiversity in these areas is influenced by human disturbances such as atmospheric pollution and urban heat island effect. To ameliorate the effects of these factors, an increase in urban green areas is often considered sufficient. However, this approach assumes that all types of green cover have the same importance for species. Our aim was to show that not all forested green areas are equal in importance for species, but that based on a multi-taxa and functional diversity approach it is possible to value green infrastructure in urban environments. After evaluating the diversity of lichens, butterflies and other-arthropods, birds and mammals in 31 Mediterranean urban forests in south-west Europe (Almada, Portugal), bird and lichen functional groups responsive to urbanization were found. A community shift (tolerant species replacing sensitive ones) along the urbanization gradient was found, and this must be considered when using these groups as indicators of the effect of urbanization. Bird and lichen functional groups were then analyzed together with the characteristics of the forests and their surroundings. Our results showed that, contrary to previous assumptions, vegetation density and more importantly the amount of urban areas around the forest (matrix), are more important for biodiversity than forest quantity alone. This indicated that not all types of forested green areas have the same importance for biodiversity. An index of forest functional diversity was then calculated for all sampled forests of the area. This could help decision-makers to improve the management of urban green infrastructures with the goal of increasing functionality and ultimately ecosystem services in urban areas.

In this paper, an automatic histogram threshold approach based on a fuzziness measure is presented. This work is an improvement of an existing method. Using fuzzy logic concepts, the problems involved in finding the minimum of a criterion function are avoided. Similarity between gray levels is the key to find an optimal threshold. Two initial regions of gray levels, located at the boundaries of the histogram, are defined. Then, using an index of fuzziness, a similarity process is started to find the threshold point. A significant contrast between objects and background is assumed. Previous histogram equalization is used in small contrast images. No prior knowledge of the image is required.
