RESUMO

The basal-cell carcinoma is the most frequent malignant tumor in human, usually involving the healthy sun-exposed skin of head and neck area, but thoracic and members localisations are possible. Macroscopic appearance is variable excepting the pathognomonic pearl. Periungual lesions are in this context very rare and we report the case of a 64-year-old man presenting with a basal-cell carcinoma on the thumb. A literature review was also performed and our purpose is to warn our colleagues about this quite often unrecognized pathology which may need a more aggressive treatment in case of delayed diagnosis, and to remind them that all chronic lesions impose a biopsy.

RESUMO

AIM: The internal breast pedicle flap is not a well-known technique yet. Anyway, it is effective for some of breast reconstructions and partial thoracic defects. This technique is applied to patients presenting breast hypertrophy. In this article, we present the experience of our department in this technique, considering its indications, surgery details, complications that may occur and subsisting interrogations about this surgical approach. MATERIAL AND METHODS: Two patients were treated with internal breast pedicle flap, one for controlateral breast reconstruction and one for coverage of partial thoracic defect. RESULTS: Patients were followed; flap survival and aesthetic results were analyzed during consultations. Results were good and similar to other well-spread reconstruction techniques. CONCLUSION: The internal breast pedicle flap is a relevant, reproducible and easy technique. Overall results are satisfying and allow the possibility of applying other techniques in case of failure. You have to keep in mind the risk of making a transfer of a potentially carcinogenic tissue for a patient having already presented a breast cancer.

RESUMO

We present the case of a 51-year-old woman who suffered an avulsion injury of the right leg in a car accident. In a first time, the simple suture with tiny debridement of the flaps is a failure and lead to a nearly complete necrosis. In a second time, a total avulsion of the devitalized skin is realised and a circumferential VAC system is placed on the wound. Four cycles of vacuum therapy and twelve days later, a split-thickness skin mesh-graft is applied on the leg. This one has a very good take and allows the patient to stand up one month after the initial accident. This example underlines the role of cleaning and pro-budding of the negative therapy after the salvage of a leg avulsion.

RESUMO

The correction of type II sequelae of breast conservative surgery is a difficult problem to solve. The authors present the case of a 39 years old woman, in which the repair has been carried out in two steps. A latissimus dorsi muscular flap is first realised, then a breast implant and a double-opposing Z plasty on the areola finish the reconstruction. The final result is very satisfactory in a cosmetic point of view, with a little cost in terms of scars. This difficult case presents the muscular variant of the latissimus dorsi flap, as a technique of choice in the post-quadrantectomy sequelae.

RESUMO

The abdomino-cruroplasty is the result of a combination of an abdominoplasty (or "mini-lift" as described by French authors) and a reduction plasty of the thighs. Our prime objective is the one-stage correction of the cutaneous and fatty excess of both the abdomen and the thighs with a single scar hidden in the skin folds; we also obtain an equilibration of "up" and "down" forces to avoid deformation of the pubic region. The technical points are explained.

Treatment of basal cell epitheliomas of the labial region is essentially surgical excision. Curietherapy appears to be indicated for sclerodermiform epitheliomas, extensive lesions, and when surgery is contra-indicated.
