RESUMO

Persistent infection with high-risk human papillomavirus remains the primary factor associated with the progression of cervical squamous intraepithelial lesions and the development of cervical cancer. Nevertheless, a combination of factors, including genetic predisposition, immune response, hormonal influences, and nutritional status, contribute synergistically to the development of cervical cancer. Among the various factors involved in the pathogenesis and therapy of cervical cancer, retinoids have gained considerable attention due to their multifaceted roles in different cellular processes. This review investigates defects within the vitamin A metabolism pathway and their correlation with cervical cancer. Additionally, it integrates epidemiological and experimental findings to discuss the potential utility of retinoid-based therapies, either alone or combined with other therapies, as agents against premalignant lesions and cervical cancer.

AIM: To assess the value of a prehabilitation program adapted to the current COVID-19 pandemic using a teleprehabilitation modality in a public Latin American hospital. METHODS: The medical records of candidates for elective colorectal cancer surgery (CRC) and who completed a teleprehabiltation program were analyzed. Sociodemographic, clinical, and functional variables were analyzed, such as cardiorespiratory capacity with the sit-to-stand test (STST), independence in activities of daily living with the Barthel index, balance with the five-times STST (FSTST) and fatigue with Brief Inventory Fatigue (BFI). The feasibility of the program was analyzed in terms of recruitment, retention, user satisfaction, and reporting of adverse events. RESULTS: Of 107 people recruited, 57 completed the program (54%, 68.78 ± 12.36 years). There was a significant difference in the BFI, FSTST, and STS 1-min scores after the intervention (p < .01), with an effect size (Cliff's delta) that varied between -.13 and .21. There were no differences in the Barthel index score. In relation to the viability of the program, 99% of patients referred for surgery could be recruited into the program, with 53% retention. Regarding user satisfaction with the program, seven items (77.7%) were rated as "very satisfied," and two items (22.3%) as "satisfied." No adverse events were recorded. CONCLUSION: The structured prehabilitation program adapted to teleprehabilitation for CRC candidates for surgery was effective in optimizing functional results prior to surgery and was feasible to implement in a public hospital with limited resources during the COVID-19 pandemic.

Brachybacterium conglomeratum, traditionally considered an environmental bacterium, has recently garnered attention for its potential involvement in human health. While prior research hinted at its pathogenic role in humans, our study aims to determine its prevalence and associations in diverse clinical contexts. We examined vaginal swabs from three distinct patient groups: patients with low-grade squamous intraepithelial lesions (LSIL), patients with cervicovaginal infections, and patients with a history of precancerous lesions undergoing follow-up. B. conglomeratum was present in all three patient groups, with the highest prevalence observed in the LSIL group. Statistically significant associations were primarily identified in the LSIL group, where B. conglomeratum was present in 60% of cases. Notably, the LSIL group exhibited coinfections with multiple high-risk oncogenotypes of human papillomavirus (HPV), suggesting potential synergistic effects, and understanding these microbial relationships and their influence on viral persistence, particularly with HPV, holds promise for mitigating HPV-related carcinogenesis. Furthermore, Gardnerella vaginalis and Atopobium vaginae were frequently detected in this group, along with Ureaplasma parvum as the predominant sexually transmitted bacterium. In all cases, B. conglomeratum was found in association with these microorganisms rather than as a sole pathogen. This coexistence underscores the intricate microbial interactions within cervicovaginal infections and precancerous lesions. This study marks the first report of B. conglomeratum prevalence in women with these clinical conditions.

Background: Acute appendicitis (AA) is the most common surgical emergency in children. Assessment scales improve the timely detection of cases. Objective: To evaluate the association of the Pediatric Appendicitis Scale (PAS) with hospital stay (HS) and postoperative complications of appendectomy. Material and methods: Observational, analytical, longitudinal and retrolective study. Patients from 2 months to 15 years of age, without comorbidities, who underwent appendectomy for complicated (CAA) and uncomplicated (UCAA) acute appendicitis were included. PAS score prior to an appendectomy was obtained, and the time of HS and postoperative complications were evaluated. Results: 64 patients were evaluated, age 8 (IQR 8-12) years, PAS score 6 (IQR 4-8). The time from the onset of symptoms to the request for care in the emergency department was shorter (p < 0.0001) and the total neutrophil count was higher (p = 0.001) in the CAA group. No difference was shown in the PAS score between patients with CAA and UCAA (6 [4-7] vs. 7 [4-8], p = 0.087]. A PAS score ≥ 7 was associated with an increased risk of prolonged HE (p = 0.007), but was not associated with postoperative complications. The PAS score alongside the time elapsed from the onset of symptoms until the assistance in the Pediatric Emergency Department, were associated with an increased risk of prolonged HE (R2 = 0.2246, p = 0.003). Conclusions: A PAS score ≥ 7 is associated with prolonged HS, likewise, the PAS score alongside the time elapsed between the onset of symptoms and assistance in emergency care is associated with prolonged HS.


Introducción: la apendicitis aguda (AA) es la patología quirúrgica de urgencia más común en niños. Las escalas de valoración mejoran la detección oportuna de casos. Objetivo: evaluar la asociación de la Escala pediátrica de apendicitis (PAS) con la estancia hospitalaria (EH) y las complicaciones postoperatorias de apendicectomía. Material y métodos: estudio observacional, analítico, longitudinal y retrolectivo. Se incluyeron pacientes de 2 meses a 15 años, sin comorbilidades, intervenidos por apendicectomía por apendicitis aguda complicada (AAC) y no complicada (AANC). Se obtuvo el puntaje PAS previo a apendicectomía y se evaluó la EH y complicaciones postoperatorias. Resultados: se evaluaron 64 pacientes, edad 8 (RIC 8-12) años, puntaje de PAS 6 (RIC 4-8). El tiempo desde el inicio de los síntomas hasta la atención en urgencias fue menor (p < 0.0001) y la cuenta de neutrófilos totales fue mayor (p = 0.001) en el Grupo de AAC. No hubo diferencia en el puntaje PAS entre pacientes con AAC y AANC (6 [4-7] frente a 7 [4-8], p = 0.087). El puntaje PAS ≥ 7 se asoció con incremento del riesgo de EH prolongada (p = 0.007), pero no con complicaciones postoperatorias. El puntaje PAS en conjunto con el tiempo transcurrido desde el inicio de los síntomas hasta la atención en Urgencias Pediátricas se asoció a incremento del riesgo de EH prolongada (R2 = 0.2246, p = 0.003). Conclusiones: el puntaje PAS ≥ 7 se asocia con EH prolongada, y el puntaje PAS en conjunto con el tiempo transcurrido entre el inicio de síntomas y la atención en urgencias se asocia a EH prolongada.

Resumen La peritonitis por neumococo comprende un pequeno subconjunto de pacientes con enfermedad invasiva (ENI). Durante 15 años (2005-2020) de vigilancia de ENI en un hospital de pediatría, se detectaron 5 casos de peritonitis primaria. Los pacientes, 3 ninas y 2ninos con una media de edad de 5 anos, experimentaron signos y síntomas peritoneales; 3 de ellos presentaban síndrome nefrótico. En coincidencia con los perfiles locales, todos los aislamientos fueron sensibles a betalactámicos, una cepa expresó resistencia a tetraciclina y cotrimoxazol y otra solo a cotrimoxazol. Los serotipos encontrados en 4/5 cepas (una resultó no viable) fueron 1, 19F, 15C y 23A. Los ninos fueron tratados con cefalosporinas de tercera generación o con ampicilina, gentamicina y metronidazol; todos evolucionaron favorablemente. Se destaca la importancia del hallazgo de Streptococcus pneumoniae en peritonitis primarias en niños. Este trabajo contribuye al conocimiento de esta enfermedad en particular y al de la epidemiología local de la ENI.

Salmonellosis is a common infectious disease in humans caused by Salmonella spp., which in recent years has shown an increase in its incidence, with products of avian origin being a common source of transmission. To present a successful infective cycle, there are molecular mechanisms such as virulence factors that provide characteristics that facilitate survival, colonization, and damage to the host. According to this, the study aims to characterize the virulence factors of Salmonella spp. strains isolated from broilers (n = 39) and humans (n = 10). The presence of 24 virulence genes was evaluated using end-point PCR. All the strains of Salmonella spp. isolated from broiler chickens revealed presence of 7/24 (29, 16%) virulence genes (lpfA, csgA, sitC, sipB, sopB, sopE, and sivH). Regarding the strains isolated from cases of gastroenteritis in humans, all strains contained (14/24, 58, 33%) virulence genes (lpfA, csgA, pagC, msgA, spiA, sitC, iroN, sipB, orgA, hilA, sopB, sifA, avrA, and sivH). In summary, the presence of virulence genes in different strains of Salmonella isolated from broilers and humans could be described as bacteria with potential pathogenicity due to the type and number of virulence genes detected. These findings are beneficial for the pathogenic monitoring of Salmonella in Colombia.

La estética de la superficie de la nariz y especialmente de la punta nasal se crea mediante ciertas líneas, sombras y reflejos, con proporciones y puntos de ruptura específicos. La evaluación de la estética de la superficie nasal se logra utilizando el concepto de polígonos geométricos como subunidades estéticas, tanto para definir la deformidad existente como los objetivos estéticos. Los principios de los polígonos geométricos permiten al cirujano analizar las deformidades de la nariz, definir un plan operatorio para lograr objetivos específicos y seleccionar la técnica operatoria adecuada


The aesthetics of the surface of the nose and specially of the nasal tip is created through certain lines, shadows and reflections with proportions and specific breaking points. The evaluation of the aesthetics of the nasal surface is achieved using the concept of geometric polygons like aesthetics subunits to define the existing deformity as for the aesthetics objectives. The principles of the geometric polygons allow the surgeon to analyze the deformities of the nose, to define an operating plan to achieve specific objectives and to select the most accurate operating technique

Persistent infection with Helicobacter pylori (H. pylori) is an important factor in gastric diseases. The vacA and cagA virulence factors of H. pylori contribute to the development of these diseases. Triple therapy containing clarithromycin has been used to eradicate this infection. Unfortunately, resistance to this antibiotic is the primary cause of treatment failure. This study aimed to determine the prevalence of clarithromycin resistance-associated mutations and to assess the relationship between virulence factors and Mexican patients infected with H. pylori. The cagA and vacA genotypes were determined by multiplex PCR. Furthermore, a qPCR was used to identify mutations of the 23S rRNA gene. This study reported a prevalence of 84.3% of H. pylori among patients with gastric diseases, and the vacA s1m1/cagA+ genotype was the most frequent (44.8%) in antrum and corpus. Analysis of the 23S rRNA gene revealed a 19.8% prevalence of clarithromycin resistance-associated mutations. The most prevalent mutations were A2143G (56%) and A2142C (25%). A significant association (p < 0.05) between the A2142G and the vacA s1m1/cagA+ genotype was detected. In conclusion, we report a high prevalence (>15%) of clarithromycin resistance-associated mutations, and we found an association between the genotypes of virulence factors and a mutation in the 23S rRNA gene.

Resumen Objetivo: Determinar la prevalencia y factores de riesgo cardiovascular de los pacientes con infarto agudo de miocardio sin evidencia de lesiones coronarias obstructivas en la arteriografía coronaria, en una institución de salud con servicios de mediana y alta complejidad en la ciudad de Valledupar (Cesar), Colombia. Materiales y métodos: Estudio descriptivo, de corte transversal, único centro, en el que se incluyeron pacientes mayores de 18 años, atendidos en un centro hospitalario en la ciudad de Valledupar con síndrome coronario agudo, a quienes se les realizó arteriografía coronaria y cumplían con los criterios para MINOCA según la cuarta definición universal de infarto de miocardio, desde enero de 2016 hasta diciembre de 2019. Se calculó la prevalencia de MINOCA y descripción por sexo, edad, factores de riesgo cardiovascular y tipo de presentación del SCA. Resultados: De un total de 3.022 pacientes de la población estudiada con diagnóstico de infarto agudo de miocardio, 215 no tenían lesiones coronarias obstructivas, lo cual arrojó una prevalencia del 7.11% (IC 95%: 6.20-8.03%). La mayoría fueron mujeres (53.02%) con una edad promedio de 56,6 años. El 75.8% de los pacientes tenía al menos un factor de riesgo cardiovascular, con predominio de hipertensión arterial (67.4 %). La mayoría presentó síndrome coronario agudo sin elevación del segmento ST (93.5%). Conclusión: La prevalencia de MINOCA en nuestra institución se encuentra en el rango descrito en la literatura médica y tiene características clínicas similares en reportes publicados. Se plantea la necesidad de realizar estudios posteriores para la determinación de la causa en este tipo de pacientes.


Abstract Objective: To determine the prevalence and cardiovascular risk factors in patients with acute myocardial infarction with non-obstructive coronary lesions in coronary arteriography in a health institution with intermediate and specialized care in the city of Valledupar (Cesar), Colombia. Materials and methods: This was a descriptive, cross-sectional, single-center study. All patients included were over 18 years of age. They presented an acute coronary syndrome and were treated in a health center in Valledupar, Colombia. Coronary arteriography was completed, and patients met the criteria for MINOCA according to the fourth universal definition of myocardial infarction. The prevalence of MINOCA and description by sex, age, cardiovascular risk factors, and type of ACS presentation were calculated. The study was carried out between January 2016 and December 2019. Results: Of the 3.022 patients diagnosed with acute myocardial infarction, 215 did not have non-obstructive coronary lesions, with a prevalence of 7.11% (CI 95%: 6.20-8.03%). Most subjects were women (53.02 %), and the mean age was 56.6 years. 75.8% of participants had at least one cardiovascular risk factor, predominantly arterial hypertension (67.4%). Most patients had non-ST-segment elevation acute coronary syndrome (93.5%). Conclusion: The prevalence of MINOCA in our institution is within the range described in the medical literature. The clinical characteristics found were similar to those reported in the literature. Further studies need to be conducted to determine the cause in this type of patient.

El colgajo antebraquial anterior es una excelente área dadora para transferir tejido. A lo largo de los años el colgajo antebraquial libre ha tomado fuerza dado que proporciona la cantidad de tejido a donar, su localización y, por sobre todo, la facilidad de disección, que se necesita para cubrir diferentes defectos en todo el cuerpo. Además, debe optimizar los resultados funcionales y estéticos. El sitio receptor debe cumplir en la medida de lo posible, utilizando colgajos con características similares, restablecer la integridad estructural o la función. Material y métodos. Se han realizado desde marzo 2021 a marzo del corriente año un total de 26 colgajos antebraquial libre. Los pacientes de esta serie fueron 80% hombres y 20% mujeres con un rango de edad de 25 a 73 años. Se utilizó tanto para patologías oncológicas, traumáticas e incluso colgajo de elección en pacientes trans para la creación del falo. Se tuvo en cuenta la utilización de la mano no hábil del paciente para así disminuir los riesgos de secuelas funcionales. Este colgajo aporta buen volumen de isla de piel, su pedículo es constante y confiable. La disección del colgajo, en manos entrenadas, acorta los tiempos quirúrgicos y demostró ser útil en muchísimas zonas receptoras del cuerpo. Resultados. Se logró el aporte de tejido necesario en todas las patologías a tratar, utilizando como zonas receptoras diferentes tipos de pedículos. También se logró un buen resultado tanto estético como funcional, volviendo a dar al paciente una calidad de vida aceptable. Conclusiones. El colgajo antebraquial libre ha demostrado ser útil en diversas patologías a tratar. Buen tegumento, su disección es fácil y accesible para cirujanos reconstructivos que recién comienzan su formación así como para cirujanos ya entrenados. Consta de un pedículo confiable con buen diámetro y longitud, lo cual lo hace aún más verídico para contar como primera opción cuando se lo necesita como colgajo libre.

The COVID-19 pandemic has been a main concern over the last two years and has become one of the most important crises in the history of human health. Today, there is still a need for affordable and reliable diagnostic tests for massive disease monitoring. Previously, a set of highly specific DNA-aptamers (C7/C9) binding to the SARS-CoV-2 Spike (S) protein were isolated but its performance in clinical samples remained to be tested. Here, 242 samples were collected through three different methods and subjected to florescence-linked aptamer assays (FLAA) based on C7/C9 aptamers through two readout protocols. Then, a step-by-step statistical approach which included agreement tests, proportion comparisons and binomial and multinomial logistic regressions was used to predict optimal conditions for the novel C7/C9 FLAA test. RTqPCR threshold cycles, symptoms onset and processing time were influential factors on FLAA test results. Naturally occurring mutations on S were also detected and analyzed. Aminoacidic substitutions D614G and T732A appeared relevant for aptamer recognition although further studies are necessary. The methodology presented here is the first step to determine the performance and diagnosis across a range of clinical contexts and it might serve as a base for a complete analysis applicable to other designs of new diagnostic tests.

Gestational diabetes (GD), pre-gestational diabetes (PD), and pre-eclampsia (PE) are morbidities affecting gestational health which have been associated with dysbiosis of the mother's gut microbiota. This study aimed to assess the extent of change in the gut microbiota diversity, short-chain fatty acids (SCFA) production, and fecal metabolites profile in a sample of Mexican women affected by these disorders. Fecal samples were collected from women with GD, PD, or PE in the third trimester of pregnancy, along with clinical and biochemical data. Gut microbiota was characterized by high-throughput DNA sequencing of V3-16S rRNA gene libraries; SCFA and metabolites were measured by High-Pressure Liquid Chromatography (HPLC) and (Fourier Transform Ion Cyclotron Mass Spectrometry (FT-ICR MS), respectively, in extracts prepared from feces. Although the results for fecal microbiota did not show statistically significant differences in alfa diversity for GD, PD, and PE concerning controls, there was a difference in beta diversity for GD versus CO, and a high abundance of Proteobacteria, followed by Firmicutes and Bacteroidota among gestational health conditions. DESeq2 analysis revealed bacterial genera associated with each health condition; the Spearman's correlation analyses showed selected anthropometric, biochemical, dietary, and SCFA metadata associated with specific bacterial abundances, and although the HPLC did not show relevant differences in SCFA content among the studied groups, FT-ICR MS disclosed the presence of interesting metabolites of complex phenolic, valeric, arachidic, and caprylic acid nature. The major conclusion of our work is that GD, PD, and PE are associated with fecal bacterial microbiota profiles, with distinct predictive metagenomes.

The COVID-19 pandemic, caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), has been one of the most catastrophic diseases observed in recent years. It has reported nearly 550 million cases worldwide, with more than 6.35 million deaths. In Mexico, an increased incidence and mortality of this disease were observed, where the immune response has been involved in the magnitude and severity. A critical version of the disease is accompanied by hyperinflammatory responses, with cytokine and defective cellular responses. A detailed understanding of the role of molecules and cells in the immune response during COVID-19 disease may help to generate effective protection mechanisms, improving those we already have. Here we analyzed blood samples obtained from patients at the Hospital Regional de Alta Especialidad de Ixtapaluca (HRAEI), Mexico, which were classified according to living guidance for clinical management of COVID-19 by the World Health Organization: asymptomatic, mild, severe, and critical disease. We observed increased interleukin (IL)-6 levels and a T-CD8+ and T-CD4+ cell reduction correlated with the critical disease version. Importantly, here, we described a significant reduction of CD11b+CD45highCD14low monocytes during severe disease, which displayed a non-classical profile, expressing IL-10, transforming growth factor (TGF)-ß, and indoleamine 2,3-dioxygenase (IDO)1 molecule. Moreover, CD11b+CD45highCD14low monocytes obtained from infected one-dose vaccinated patients (Pfizer® vaccine) who suffered minimal symptoms showed simultaneously a dual classical and no-classical profile expressing pro- and anti-inflammatory cytokines. These results suggest that blood monocytes expressing a dual pro- and anti-inflammatory profile might be a predictive marker for protection in the Mexican population during COVID-19 disease. KEY POINTS : • Exacerbated immune response is associated with COVID-19 severe disease. • Dual monocyte activation profile is crucial for predicting protection during COVID-19. • Vaccination is crucial to induce the dual activation profile in monocytes.

La presente investigación se fundamentó en las teorías que exploran la resiliencia en el ámbito de la Psicología del Deporte, se planteó como objetivo analizar la manifestación de la resiliencia psicológica en atletas cubanos de alto rendimiento, específicamente en la exdeportista corredora de 800 metros Ana Fidelia Quiroz Moret y en deportistas de la categoría Juvenil y Mayores del equipo nacional de tenis de Mesa de Cuba. Se utilizaron métodos de investigación con predominio de la metodología cualitativa analítica como estudios de caso individual y de grupo, análisis-síntesis, entrevista, análisis de documentos y videos, observación, el cuestionario de Afrontamiento de estrés de Sandín y Chorot y la escala de Resiliencia de Wagnild y Young. Se utilizó el porcentaje como método matemático. Los resultados principales permitieron constatar la presencia de nivel alto de resiliencia en la exdeportista estudiada y niveles medio-alto y alto en los tenimesistas estudiados, se identificaron los factores protectores que influyen en el afrontamiento de eventos vitales y situaciones estresantes. Las conclusiones principales destacaron la necesidad de valorar en los deportistas de alto rendimiento estudiados, las manifestaciones de la resiliencia psicológica de manera particular: los factores protectores de la personalidad, la influencia de las redes de apoyo social así como las estrategias y estilos de afrontamiento al estrés que afloran ante situaciones adversas que se presentan en la vida deportiva, dificultades propias del entrenamiento y la competencia a las que se enfrentan con regularidad, que pueden constituir modelos a tener en cuenta para otros deportistas que se inician.


O objetivo desta pesquisa foi analisar a manifestação da resiliência psicológica em atletas cubanos de alto rendimento, especificamente na ex-corredora Ana Fidelia Quiroz Moret, de 800m, e em atletas da seleção nacional cubana de tênis de mesa, nas categorias jovem e sênior. Foram utilizados métodos de pesquisa com predominância de metodologia analítica qualitativa, como estudos de casos individuais e de grupo, análise-síntese, entrevista, análise de documentos e vídeos, observação, o questionário de Cópia de Estresse de Sandin e Chorot e a escala de Resiliência de Wagnild e Young. A porcentagem foi utilizada como um método matemático. Os principais resultados mostraram a presença de um alto nível de resiliência na ex-esportista estudada e níveis médios-altos e altos nos tenistas estudados. Os fatores de proteção que influenciam o enfrentamento de eventos da vida e situações estressantes foram identificados. As principais conclusões destacaram a necessidade de avaliar nos atletas de alto rendimento estudados, as manifestações de resiliência psicológica em particular: os fatores protetores da personalidade, a influência das redes de apoio social, assim como as estratégias e estilos de lidar com o estresse que surgem em situações adversas que surgem na vida esportiva, as dificuldades de treinamento e competição que enfrentam regularmente, que podem constituir modelos a serem levados em conta para outros atletas que estão começando.


The present research was based on the theories that explore resilience in the field of Sport Psychology. The objective was to analyze the manifestation of psychological resilience in Cuban high-performance athletes, specifically in the former 800-meter runner Ana Fidelia Quiroz Moret and in athletes of the Cuban national Table Tennis team in the Juvenile and Senior categories. Research methods were used with a predominance of analytical qualitative methodology such as individual and group case studies, analysis-synthesis, interview, analysis of documents and videos, observation, the Stress Coping questionnaire of Sandin and Chorot and the Resilience scale of Wagnild and Young. The percentage was used as a mathematical method. The main results showed the presence of a high level of resilience in the ex-sportswoman studied and medium-high and high levels in the tennis players studied. The main conclusions highlighted the need to assess in the high performance athletes studied, the manifestations of psychological resilience in particular: the protective factors of the personality, the influence of social support networks as well as the strategies and styles of coping with stress that emerge in adverse situations that arise in sports life, difficulties of training and competition that they face regularly, which may constitute models to be taken into account for other athletes who are starting out.

As a result of the COVID-19 pandemic, various joint efforts have been made to support the creation of vaccines. Different projects have been under development, of which some are in the clinical evaluation stage and others in are in phase III with positive results. The aim of this paper was to describe the current situation of the development and production of vaccines available to the population to facilitate future research and continue developing and proposing ideas for the benefit of the population. So, we carried out a systematic review using databases such as PubMed, ScienceDirect, SciELO, and MEDLINE, including keywords such as "vaccines," "COVID-19," and "SARS-CoV-2". We reviewed the development and production of the anti-COVID vaccine and its different platforms, the background leading to the massive development of these substances, and the most basic immune aspects for a better understanding of their physiological activity and the immune response in those who receive the vaccine. We also analyzed immunization effects in populations with any medical or physiological conditions (such as immunosuppression, people with comorbidities, and pregnancy), as well as the response to immunization with heterologous vaccines and the hybrid immunity (the combination of natural immunity to SARS-CoV-2 with immunity generated by the vaccine). Likewise, we address the current situation in Mexico and its role in managing the vaccination process against SARS-CoV-2 at the national and international levels. There are still many clinical and molecular aspects to be described, such as the duration of active immunity and the development of immunological memory, to mention some of the most important ones. However, due to the short time since the global vaccination roll-out and that it has been progressive (not counting children and people with medical conditions), it is premature to say whether a second vaccination schedule will be necessary for the near future. Thus, it is essential to continue with health measures.

Background: Reported data suggest that 4.2 million deaths will occur within 30 days of surgery worldwide each year, half of which are in low- and middle-income countries. Postoperative complications are a leading cause of long-term morbidity and mortality. Patients who survive and leave the hospital after surgical complications regularly experience reductions in long-term survival and functional independence, resulting in increased costs. With a high volume of surgery performed, there is a growing perception of the substantial impact of even minor enhancements in perioperative care. The Latin American Surgical Outcomes Study (LASOS) is an international, multicentre, prospective cohort study of adults submitted to in-patient surgery in Latin America aiming to provide detailed data describing postoperative complications and surgical mortality. Methods: LASOS is a 7 day cohort study of adults undergoing surgery in Latin America. Details of preoperative risk factors, intraoperative care, and postoperative outcomes will be collected. The primary outcome will be in-hospital postoperative complications of any cause. Secondary outcomes include in-hospital all-cause mortality, duration of hospital stay after surgery, and admission to a critical care unit within 30 days after surgery during the index hospitalisation. Results: The LASOS results will be published in peer-reviewed journals, reported and presented at international meetings, and widely disseminated to patients and public in participating countries via mainstream and social media. Conclusions: The LASOS may augment our understanding of postoperative complications and surgial mortality in Latin America. Clinical trial registration: NCT05169164.

We previously reported preliminary characterization of adipose tissue (AT) dysfunction through the adiponectin/leptin ratio (ALR) and fasting/postprandial (F/P) gene expression in subcutaneous (SQ) adipose tissue (AT) biopsies obtained from participants in the GEMM study, a precision medicine research project. Here we present integrative data replication of previous findings from an increased number of GEMM symptom-free (SF) adults (N = 124) to improve characterization of early biomarkers for cardiovascular (CV)/immunometabolic risk in SF adults with AT dysfunction. We achieved this goal by taking advantage of the rich set of GEMM F/P 5 h time course data and three tissue samples collected at the same time and frequency on each adult participant (F/P blood, biopsies of SQAT and skeletal muscle (SKM)). We classified them with the presence/absence of AT dysfunction: low (<1) or high (>1) ALR. We also examined the presence of metabolically healthy (MH)/unhealthy (MUH) individuals through low-grade chronic subclinical inflammation (high sensitivity C-reactive protein (hsCRP)), whole body insulin sensitivity (Matsuda Index) and Metabolic Syndrome criteria in people with/without AT dysfunction. Molecular data directly measured from three tissues in a subset of participants allowed fine-scale multi-OMIC profiling of individual postprandial responses (RNA-seq in SKM and SQAT, miRNA from plasma exosomes and shotgun lipidomics in blood). Dynamic postprandial immunometabolic molecular endophenotypes were obtained to move towards a personalized, patient-defined medicine. This study offers an example of integrative translational research, which applies bench-to-bedside research to clinical medicine. Our F/P study design has the potential to characterize CV/immunometabolic early risk detection in support of precision medicine and discovery in SF individuals.

Resumen En la región latinoamericana son escasos los modelos de predicción que permitan conocer cuáles factores de riesgo son más prevalentes y los pocos estudios disponibles no funcionan bien en esta población. Objetivo: Determinar la probabilidad y predicción que tiene los factores de riesgo de sexo, edad, presión arterial sistólica (PAS), índice de masa corporal (IMC), diabetes mellitus y tabaquismo sobre la aparición de un evento cardiovascular adverso (ECA) de una población salvadoreña. Metodología: Estudio de casos y controles de tipo retrospectivo y predictivo. Participaron 527 personas adultas que acudieron a consulta externa de cardiología sin eventos cardiovasculares previos registrados. Se utilizó la prueba de la ji al cuadrado (X2) y V de Cramer para hacer la comparación de frecuencias y la regresión logística binaria para determinar la predicción y las probabilidades estimadas de presentar un ECA en un periodo de cinco años. Resultados: Se encontró una correlación significativamente baja entre los ECA y el sexo (rΦ= -.164; p < .001), diabetes (rΦ=-.244; p < .001) y con el fumando (rΦ= -.159; p < .001). Además, un efecto moderado con la estratificación de riesgo (V de Cramer= .359; p < .001). Existe una predicción significativa (X2= 82.1; p < .001), la cual responde el 32,6 % del modelo. Conclusiones: Las variables de sexo, la edad, la presencia de diabetes mellitus y el fumado predicen la aparición de un ECA en un periodo de cinco años, mientras que el IMC y PAS no influyen significativamente en la probabilidad de padecer de un ECA.


Abstract Influence of risk factors in the prediction of an adverse cardiovascular event in the adult population of primary prevention in El Salvador In the Latin American region, there are few prediction models that allow us to know which risk factors are more prevalent and the few available studies do not work well in this population. Purpose: Determine the probability and prediction that the risk factors of sex, age, systolic blood pressure (SBP), body mass index (BMI), diabetes mellitus and smoking have on the appearance of an adverse cardiovascular event (ACE) in a Salvadoran population. Methods: Retrospective and predictive case-control study. A total of 527 adults participated in the outpatient cardiology consultation with no prior recorded cardiovascular events. Chi Square (X2) and Cramer's V were used to compare frequencies and binary logistic regression to determine the prediction and estimated probabilities of presenting an ACE in a period of 5 years. Results: A significantly low correlation was found between ACEs and sex (rΦ= -.164; p <.001), diabetes (rΦ= -.244; p <.001) and with smoking (rΦ= -.159; p <.001). Also, a moderate effect with risk stratification (Cramer's V = .359; p <.001). There is a significant prediction (X2 = 82.1; p <.001), which responds to 32.6% of the model. Conclusion: The variables of sex, age, the presence of diabetes mellitus and smoking predict the appearance of an RCT in a period of 5 years, while BMI and SBP do not significantly influence the probability of suffering from an ACE.

RESUMEN El presente trabajo se corresponde con la investigación realizada sobre la resiliencia psicológica en atletas del equipo nacional de tenis de mesa y con la temática de Deporte de Alto Rendimiento. Tuvo como objetivo: elaborar una propuesta de programa de entrenamiento psicológico para estimular la resiliencia en atletas de las categorías Sub-18 y Sub-21 del Equipo Nacional de tenis de mesa. La necesidad de realizar estudios que exploren la resiliencia en el ámbito de la Psicología del Deporte revela la importancia y actualidad del tema, por la contribución de esta capacidad al rendimiento de los atletas durante el proceso de entrenamiento y competencia. Para cumplir con el objetivo de investigación, se utilizaron métodos del nivel teórico como el análisis-síntesis y del nivel empírico: la observación, la entrevista, así como la escala de resiliencia y el cuestionario de afrontamiento al estrés. Los resultados principales permitieron constatar la presencia de niveles medio y alto de resiliencia psicológica en los atletas estudiados, se identificaron los factores protectores y de riesgo que influyen en el afrontamiento de eventos vitales y situaciones estresantes. Además, posibilitaron la elaboración de la propuesta del programa de entrenamiento psicológico, en correspondencia con la constatación realizada. La propuesta del programa estuvo constituida por 16 sesiones, dirigidas a estimular la resiliencia psicológica en el equipo nacional de Tenis de mesa; fue sometida a criterio de 11 especialistas para la evaluación en su diseño, viabilidad y utilidad, donde se obtuvo una alta valoración por parte de estos.

O seu objetivo era elaborar uma proposta de programa de treino psicológico para estimular a resiliência dos atletas das categorias sub-18 e sub-21 da equipa nacional de Ténis de Mesa. A necessidade de realizar estudos que explorem a resiliência na área da Psicologia do Desporto revela a importância e atualidade do tema, devido à contribuição desta capacidade para o desempenho dos atletas durante o processo de treino e competição. A fim de cumprir o objetivo da investigação, utilizámos métodos do nível teórico como a análise-síntese e do nível empírico: observação, entrevista, bem como a escala de resiliência e o questionário de resposta ao stress. Os principais resultados permitiram-nos confirmar a presença de níveis médios e elevados de resiliência psicológica nos atletas estudados, e identificar os fatores de proteção e de risco que influenciam o enfrentamento dos eventos da vida e das situações de stress. Além disso, permitiram a elaboração da proposta do programa de formação psicológica, em correspondência com os resultados. A proposta de programa consistiu em 16 sessões, destinadas a estimular a resiliência psicológica na equipa nacional de Ténis de Mesa; foi submetida aos critérios de 11 especialistas para a avaliação da sua concepção, viabilidade e utilidade, onde foi altamente valorizada por eles.


ABSTRACT The present work corresponds to the research carried out on psychological resilience in athletes of the national Table Tennis team and with the theme of High Performance Sports. Its objective was to elaborate a proposal for a psychological training program to stimulate resilience in athletes of the U-18 and U-21 categories of the national table tennis team. The need to conduct studies that explore resilience in the field of Sport Psychology reveals the importance and timeliness of the topic, due to the contribution of this capacity to the performance of athletes during the training and competition process. In order to fulfill the research objective, methods from the theoretical level such as analysis and synthesis and from the empirical level, observation, interview, as well as the Resilience Scale and the Stress Coping Questionnaire were used. The main results made it possible to confirm the presence of medium and high levels of psychological resilience in the athletes studied, it was identified the protective and risk factors that influence the coping with life events and stressful situations. In addition, they made possible the elaboration of the psychological training program proposal, in correspondence with the findings. The program proposal consisted of 16 sessions, aimed at stimulating psychological resilience in the national table tennis team; it was submitted to the criteria of 11 specialists for the evaluation of its design, feasibility and usefulness, where it was highly valued by them.
