BACKGROUND: Bacterial microleakage is an important cause of apical periodontitis and endodontic treatment failure. This study aimed to assess the bacterial microleakage of nano-mineral trioxide aggregate (nano-MTA) as a sealer, Endoseal MTA, and GuttaFlow Bioseal sealers in atmospheric pressure, and simulated underwater diving and aviation conditions. METHODS: In this in vitro, experimental study, 180 extracted single-rooted teeth were cleaned and shaped, and were then randomly divided into three groups for single-cone obturation using Endoseal MTA, GuttaFlow Bioseal, or nano-MTA as a sealer. Each group was then randomly divided into three subgroups, and subjected to ambient atmospheric pressure, 2 atm pressure (to simulate underwater diving), and 0.5 atm pressure (to simulate aviation) using a custom-made pressure chamber. The teeth then underwent microbial leakage test using Streptococcus mutans (S. mutans), and the percentage of samples showing microleakage was recorded for up to 1 month, and analyzed using the Chi-square test. RESULTS: The three sealer groups were significantly different regarding bacterial microleakage (P < 0.05). The nano-MTA group showed significantly higher microleakage after 15 days than the other two groups (P = 0.006). The effect of pressure on bacterial microleakage was not significant in any sealer group (P > 0.05). CONCLUSION: Within the limitations of this in vitro study, it may be concluded that single-cone obturation technique using nano-MTA as a sealer results in lower resistance to bacterial microleakage compared with the use of GuttaFlow Bioseal, and Endoseal MTA. Pressure changes in simulated underwater diving and aviation conditions had no significant effect on bacterial microleakage. Trial Registration Number This is not a human subject research.

OBJECTIVES: The aim of this study was to evaluate the immediate cytotoxic effects of orthodontic molar bands, on HGF-1 cell line, after multiple times of sterilization following size selection procedure. MATERIAL AND METHODS: 48 stainless steel orthodontic molar bands were divided into 4 groups according to times of sterilization (1, 2, 4 and 8 times). A liquid extract containing the ions released from each band was prepared and the HGF-1 cell line was exposed to the extracts. 2 control groups (positive and negative) were designated. An MTT assay was performed, and the absorbance was read at 492nm in a microplate reader (Antos 2020, Austria). RESULTS: There was no significant difference in pure optical density (OD) among the 4 groups (P=0.749) however a statistically significant difference was seen between the positive control group and other 4 groups (P<0.001). CONCLUSION: The stainless-steel orthodontic bands used in this study were inert as manufactured and even multiple times of sterilization did not decrease the biocompatibility of these bands for clinical use. The present study shows that clinicians can sterilize the tried-in molar bands for at least 8 times without any risk of cytotoxicity for patients.

OBJECTIVE: White spot lesion (WSL) is one of the most important complications of fixed orthodontic treatment. Many methods have been studied to prevent this problem. This study is aimed to investigate the effect of high concentration nano-hydroxyapatite (nHAP) on shear bond strength (SBS) of metal brackets and Adhesive Remnant Index (ARI) score in different preparation methods. MATERIAL AND METHODS: Sixty human premolars, which were extracted for orthodontic reasons were included in this in vitro study. The teeth were randomly divided into 4 groups of 15 each: one control group and three nHAP groups. After applying nHAP for 2 to 3 minutes daily for 10 days, the groups 2 to 4 underwent sandblasting using 50µm aluminium oxide and 30 seconds etching, 60 and 30 seconds etching, respectively, and the brackets were then bonded to the teeth. After debonding, SBS and ARI scores were recorded. Data were analysed using the ANOVA test and posthoc test for pairwise comparisons. RESULTS: No significant difference was observed in SBS between the control group and the nHAP groups. The sandblasted teeth showed significantly higher SBS than the 30 seconds etching after nHAP application (P=0.02). The teeth etched for 60 seconds showed a significantly higher ARI score than the 30 seconds etched teeth with (P=0.003) or without (P<0.001) nHAP application. CONCLUSIONS: The use of nHAP before bracket bonding can be considered as a caries preventive method since it did not compromise the SBS. Sixty seconds etching is recommended for less likely damage to the enamel after debonding.

AIM: The aim of the current study was to evaluate the correlation of skeletal age based on cervical vertebrae maturation (CVM) stage and mid-palatal suture (MPS) maturation in an Iranian population. MATERIAL AND METHODS: This was a cross-sectional analytic study. A total number of 93 samples were included. Samples were taken from patients who were in CS3 to CS6 stages of CVM who had cone-beam computed tomography and lateral cephalometry based on inclusion criteria. The maturation of MPS was assessed based on the cone-beam computed tomography images. In the classification of maturation of MPS, there are five stages (A-E) and the suture fusion occurs in stage D. In stage E, the suture is fused completely. The CVM stage (CS1-6) was also assessed based on the lateral cephalograms. Data were analyzed using Spearman correlation with a significance level of 0.05. RESULTS: A total of 51 female individuals with a mean age of 14.98 ± 4.806 and 42 male individuals with a mean age of 15.79 ± 5.135 participated in this study. The correlation coefficient between the CVM stage and MPS maturation was 0.691 in female and 0.754 in male individuals (P < 0.001). Stage D was correlated with CS4. CONCLUSION: The results demonstrated that CVM stages had a significant but moderate positive correlation with the maturation of MPS. Until CS3, the MPS has not been fused and in CS6 the MPS is fused definitely.

Introduction: Mineral trioxide aggregate (MTA) is extensively used in endodontics. However, it has limitations such as long setting time, low compressive strength and poor handling properties. Our study aimed to compare the setting time and surface microhardness of MTA and fluoride-doped MTA (FMTA) using gel-like polymer (GLP) or distilled water (DW) as liquid. Methods and Materials: An MTA-like cement was prepared by mixing Portland cement, bismuth oxide and gypsum (75%, 20% and 5%, respectively). FMTA (1% and 5%) was made by substituting 1% and 5% of MTA powder with fluoride. GLP, composed of methyl cellulose (MC) and propylene glycol (PG), was used as the hydrating liquid and compared with distilled water. Six experimental groups (n=10) were examined for each test. The samples were subjected to Vickers surface microhardness test after 4 and 28 days. Setting time was measured using ANSI/ADA standards. Data was analyzed using two-way and repeated measured ANOVA and the Tukey HSD tests. Results: The MTA-like cement hydrated with GLP showed a significantly reduced setting time (P<0.05); 1% FMTA, mixed with GLP, had the shortest initial and final setting times. The microhardness values of all samples increased at different rates during 28 days (P<0.00001). The microhardness significantly decreased by addition of fluoride to MTA (P<0.001). Conclusions: The results of the present in vitro study suggested that using GLP as the hydrating liquid enhances the setting time with no adverse effect on the surface microhardness. However, adding fluoride can negatively affect the microhardness of MTA.

RESUMO

A simple overview of daily orthodontic practice involves use of brackets, wires and elastomeric modules. However, investigating the underlying effect of orthodontic forces shows various molecular and cellular changes. Also, orthodontics is in close relation with dentofacial orthopedics which involves bone regeneration. In this review current and future applications of stem cells (SCs) in orthodontics and dentofacial orthopedics have been discussed. For craniofacial anomalies, SCs have been applied to regenerate hard tissue (such as treatment of alveolar cleft) and soft tissue (such as treatment of hemifacial macrosomia). Several attempts have been done to reconstruct impaired temporomandibular joint. Also, SCs with or without bone scaffolds and growth factors have been used to regenerate bone following distraction osteogenesis of mandibular bone or maxillary expansion. Current evidence shows that SCs also have potential to be used to regenerate infrabony alveolar defects and move the teeth into regenerated areas. Future application of SCs in orthodontics could involve accelerating tooth movement, regenerating resorbed roots and expanding tooth movement limitations. However, evidence supporting these roles is weak and further studies are required to evaluate the possibility of these ideas.
