RESUMO

Vein of Galen aneurysmal malformation (VGAM) is a rare congenital malformation characterised by arteriovenous fistulas between primitive choroidal arteries and the median prosencephalic vein, the embryonic precursor to the vein of Galen. Endovascular techniques have changed the management of these patients with improved prognosis. An eight-month-old with VGAM managed by endovascular embolisation using ethylene vinyl alcohol copolymer (EVOH) developed a chemical abscess - a rare complication. It was managed conservatively and showed promising clinical outcome. Contribution: Chemical abscesses following EVOH embolisation are scarce - with imaging differentials, which include brain abscess and onyx granuloma. Knowledge and successful identification of this entity are essential as its management as prognoses differ. Chemical abscess is managed conservatively and has a good prognosis.

Purpose Hypospadias is an anomaly wherein the urethral opening is ectopically located on the ventral aspect of the penis. The most common complications after hypospadias repair are urethrocutaneous fistula (UCF) and meatal stenosis. Long Chain Cyanoacrylate (LCCA) tissue adhesive promises safety, feasibility, and durability due to its tensile strength and bacteriostatic and hemostatic properties. We conducted this study to ascertain whether LCCA tissue adhesive can prove a more effective adjunct to traditional suturing techniques. Methods Patients were divided into two groups. Group A underwent surgery with conventional reconstruction of the neourethral tube along with the buttressing layer using Buck's fascia or Tunica Vaginalis. In addition to the traditional procedure of Group A, Group B patients were administered a layer of LCCA tissue adhesive as an adjunct between the neourethral suture line and the buttressing layer. Patients were followed up for six months and were evaluated for complications like UCF, meatal stenosis, hematoma, skin infection, glans dehiscence, and flap necrosis. Results Thirty-eight children in the age group 1-6 years were studied, of which 20 were in Group A and 18 in Group B. Among patients of Group A seven (35%) developed complications. In contrast, only four (22.2%) patients developed complications in Group B. The statistical significance in the complication rates between the two groups could not be achieved due to the modest sample size. However, the numerical and proportional reduction in the number of complications was noted. Conclusion LCCA adhesive as an adjunct numerically reduces the number of complications compared to traditional suturing alone in patients undergoing surgery for hypospadias and UCF.

Objectives: Prolonged exposure to hypoxia at high altitudes can affect the eyes through various mechanisms, culminating in reduced vision. Multiple studies on this subject have focused on the ocular effects of high altitude on intraocular pressure (IOP), corneal thickness, and the retina. However, there is a lack of literature on the effects of 'moderate high altitude (2000-3000 m)' on the eyes. Hence, we designed our study to determine the impact of moderate high altitude on the eyes. Methods: We recruited two age and gender-matched groups of 250 research participants, each at moderate high altitude (Group A) and plains (Group B), respectively, to determine the effects of moderate high altitude on ocular health, if any. Results: We found a statistically significant increase in dry eye in Group A. We also found four cases of central serous retinopathy and one case of central retinal vein occlusion in Group A. There was no statistically significant change in visual acuity, IOP, or central corneal thickness. Conclusion: There is a vast lacuna in the literature on this particular altitude range, and further studies are required to confirm our findings. It is also recommended that lubricant eye drops be advised as a standard prophylactic measure for all individuals traveling and staying at moderate high altitude.

Background: Carcinoma breast is the commonest cancer among women. Various authors have studied breast cancer with Contrast-Enhanced Ultrasound (CEUS) with promising results. Despite promising results, the additional cost of post-processing software limits its availability. In this study, we evaluated the utility of CEUS in differentiating malignant from benign breast lesions on regular ultrasound equipment without the use of dedicated software. Methods: We performed CEUS in 121 women with 121 breast lesions. CEUS was done by creating a custom preset on existing ultrasound equipment with the help of an application specialist authorized by the vendor. Lesions were evaluated qualitatively without the use of any commercial software. The pattern of enhancement i.e. homogenous, heterogeneous, peripheral, or no enhancement, and the number of penetrating vessels i.e., few or multiple were recorded. Results were compared with histopathological diagnosis. Results: There were a total of 121 breast lesions. The study showed sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) of 86.67 %, 54.10 %, 65 %, and 80.49% respectively for differentiating benign vs malignant lesions on the basis of the pattern of contrast enhancement. Using penetrating vessels for differentiating malignant lesions from benign lesions, the sensitivity, specificity, PPV, and NPV were found to be 64%, 67.86%, 78.05%, and 51.35% respectively. Conclusion: CEUS is useful in differentiating malignant from benign breast lesions. It can be easily performed by creating a custom preset on standard ultrasound equipment without the use of expensive software.

We conducted the study to find the prevalence of ENT symptoms amongst mild Covid-19 patients from the hilly region of North India and attempted to propose a solution to curb the spread of Covid-19 through early identification, isolation, and treatment. A retrospective, cross-sectional study at a secondary healthcare center in a hilly region of North India covered 423 mildly symptomatic Covid-19 patients from April 2020 to March 2022. These patients were telephonically contacted or called in person at the outpatient department to answer a preset questionnaire with various parameters such as age, gender, ENT symptoms, and time to recover. The data obtained were statistically analyzed. 207 out of 423 mild Covid-19 patients complained of different ENT symptoms. Cough was the most common ENT symptom and was reported by 162 patients. Dizziness was the least common ENT symptom and was reported by 9 patients. Recovery time for tinnitus was maximum (persistent till six months in 5 patients). A high index of suspicion for Covid-19 disease in patients with ENT symptoms must be practiced. As the Covid-19 restrictions are gradually relaxed, widespread community education for strict adherence to Covid-19 appropriate behaviour and sensitization of General Practitioners a well as Otorhinolaryngologists regarding the importance of ENT symptoms in mild Covid19 disease will play a pivotal role in the early identification, isolation, and treatment of mild Covid-19 disease, which eventually may curb the future waves.

Self-realization or the path which involves the elaborate process of knowing oneself leads people to a meaningful and fulfilling life. The present study aims to examine how the COVID-19-led lockdown proved to be an opportunity for emerging adults to reflect on and eventually discover their true selves. This study particularly explores the factors that facilitated the self-realization process during the lockdown. An online survey was conducted on 1280 Indian university students. The age of participants ranged between 18 and 29 years. An eight-item scale was developed to measure self-realization during the lockdown, which was thereafter administered along with other study variables. The results showed that factors including family bonding, social comparison, self-image, and a positive orientation toward life help young adults in the process of self-realization. Furthermore, the participants who employed adaptive ways to cope with the COVID-19 stress reported higher levels of self-realization. The present study showed that the interaction of young adults with their significant others, solving their daily problems, and having a positive outlook toward themselves and the future, helped them during the COVID-19 imposed lockdown and led them on the path of self-realization. RESUMO

The present study examined the fear of death among young adults in the wake of the COVID-19. It explored the association between neuroticism and death anxiety during this pandemic crisis, and it also examined the mediating role of perceived stress in this relationship. An online survey of 200 participants, including both students and employed young adults, aged between 18 and 30 years, was conducted. Results showed that neuroticism was positively correlated to death anxiety; however, the relationship was completely mediated by perceived stress. The results and implications are discussed in light of the COVID-19 pandemic.

Research on construal level theory shows the influence of abstract and concrete mindsets on self-control. These studies indicate that in the abstract mindset people have more self-control than in the concrete mindset. Though some studies have been carried out on behavioral control as well, however, the influence of mindsets on eye movement control has not been explored. Aiming to explore this relationship, two experiments were conducted on university students using an eye-tracker. The mindsets of participants in both the experiments were manipulated by the temporal distancing technique. To test eye movement control, experiment 1 used an anti-saccade task, in which the participants were asked to look away from the target, while experiment 2 used a visual search task where participants were asked to make goal-directed eye movements and avoid any distraction. The results of both the experiments showed that participants tended to have more control over their eye movement during the abstract mindset than the concrete mindset. This shows that mindsets not only help in controlling thoughts and general behaviors but also influence eye movements and govern what to look and where to look. Results are discussed in the light of self-control and construal level theory.

Research indicates that deliberate thinking and eye movements are related. However, how various thought processes influence eye movements is not explored well. The present research examined the role of convergent and divergent thinking in eye movement. Divergent thinking involves novel alternatives, multiple uses, and unconventional answers which may require scanning visual surroundings precisely and extensively. Hence, it was hypothesized that divergent thinking would involve more scattered and detailed visual scanning than convergent thinking. Two experiments were conducted using repeated measures design: both involved a visual remote association task for convergent thinking and an adapted visual task of the alternate uses task for divergent thinking. Experiment 1 tracked participants' eye movement while solving the task when images of objects were on the screen; whereas, in Experiment 2, the blank screen paradigm was used where the images of items were removed and the eyes were tracked while participants solved the task. Data analysis showed that the eye movements were more object focused during convergent thinking than divergent thinking. The results confirmed that participants in divergent thinking had more detailed and dispersed visual scanning than in convergent thinking.

RESUMO

Partial or complete failure of obliteration of the processus vaginalis in the female results in the formation of a potential space known as the canal of Nuck, into which various organs and/or collections can herniate. A 4-month-old female presented with a left labial mass related to herniation of the uterus, ovaries and fallopian tubes through the canal of Nuck. Early diagnosis is important as there is a high risk of ovarian torsion and incarceration.

RESUMO

A case of endosalpingiosis of the urinary bladder is presented with imaging features on sonography and CT. Patient presented with right flank pain, dysuria and haematuria. She had h/o right renal calculus and abdominal hysterectomy 15 years ago. On sonography a polypoidal filling defect was noted and possibility of a bladder neoplasia was suggested. On cystoscopy and removal of the lesion and subsequent histo-pathological analysis revealed the diagnosis of endosalphingiosis. This report emphasizes the need for evaluation of all clinical inputs while considering the differential diagnosis of an intraluminal bladder lesion. Imaging appearance and aetio-pathology of the rare intra vesical lesion is highlighted.

The present research examines the strength of terror management theory in an indigenous Indian context of religious fair called Magh Mela. It explores how elderly Hindu people deal with death anxiety through practicing Kalpvas in Magh Mela. The research explores the role of social detachment and self-esteem in coping with terror of death. Study 1, a field experiment on 150 Kalpvasis (practitioners of Kalpvas) confirms the significant role of social detachment as an adaptive strategy for coping with death terror. The role of self-esteem did not emerge in the study. Study 2, another field experiment on 62 Kalpvasis confirms results of study 1. Significant role of years of Kalpvas on fear of death shows importance of the religious practices in managing terror related to death. The relation of terror management theory and death anxiety thus follows a different explanation for more indigenous contexts.
