RESUMO

AIM: To study acid-base balance (ABB) of the organism, gastric secretion of hydrocarbonate (HCO3-) in ulcer patients and to examine their relationships. MATERIAL AND METHODS: ABB in capillary or venous blood before and after losec and famotidine treatment and gastric secretion of HCO3- were studied in 72 patients with ulcer. RESULTS: It was found that patients with gastroduodenal ulcer have metabolic, less frequently hypercapnic acidosis with suppressed gastric secretion of HCO3-. This entails abnormal neutralization of the hydrochloric acid imitating high acidity of the gastric juice. HCO3- secretion is inversely proportional to pH. H2-blockers normalize ABB in ulcer patients while losec either stimulates acidosis or transforms one kind of acidosis into another. CONCLUSION: ABB and gastric secretion of HCO3- are related. Acidosis in ulcer patients leads to reduction of HCO3- production and intensification of hydrochloric acid secretion.

RESUMO

Acid-base balance was studied in 60 ulcer patients in exacerbation and after treatment with losek and blockers of H2-histamine receptors. In exacerbation, all the patients had acidosis of metabolic or respiratory type. Losek changed blood pH insignificantly, and one acidosis type changed for the other. A single ranitidin dose shifted pH to alkalosis in healthy subjects while ulcer patients given famotidine restored acid-base balance. The latter is shown to correlate with gastric secretion of bicarbonates. Acidosis may contribute to exacerbation of ulcer.

AIM: The study of alkaline gastric secretion in health and disease and its changes in response to blockers of H2-histamine receptors. MATERIALS AND METHODS: The trial enrolled 74 patients with duodenal ulcer (DU), 28 patients with chronic gastric (CG) and 16 healthy controls. The secretion was studied initially, in administration of ranitidine, in stimulation with hydrochloric acid. RESULTS: DU patients demonstrated a significant reduction of gastric secretion of bicarbonates in the basal and stimulated phases and a 3-fold decrease in the proportion alkaline/acid secretion. There was also a trend to acidosis. Administration of H2-histamine receptors provided recovery of acid-alkaline balance. CONCLUSION: Bicarbonate deficiency is an important element of duodenal ulcer pathogenesis.

RESUMO

At the division of general therapy N 1 of the Kirov Military Medical Academy a version of automated training system was elaborated for the IBM AT/386 users of MS DOS operational system. The knowledge text control was carried out during the lessons at the display classroom equipped with Wyse-85 terminals ("Wyse Technology Company"), DEC VT 220 regimen, and connected up to PC IBM AT/386 which works in multiterminal function. Application of text control has shown that the volume of knowledge had increased up to 82-90%, but it was right only for the groups where the methodics of classes with application of the results of text controls was strictly adhered. The progress in other groups was less than 68%.

RESUMO

Studies into the content of nonspecific defense factors, secretory and serum immunoglobulins in blood and biological fluids (saliva, gastric juice, duodenal contents) in patients suffering from peptic ulcer have demonstrated significant changes in local and general immunity. Exacerbation of the disease was marked by a decrease of the titer of complement, C3-component of complement, and lysozyme level. At the same time there was an increase of the concentration of secretory immunoglobulins and immunoglobulins in blood serum and external secretions. The data obtained attest to the tension of humoral immunity and nonspecific defense in peptic ulcer patients, giving rise to the consideration of the changes in the humoral component as one of potential mechanisms by which ulcer is formed.

RESUMO

The effect of cholinolytics in combination with antacids, belomet, solcoseryl and enterodes on humoral immunity was evaluated in 134 ulcer patients. Elevated levels of salivary secretory IgA and blood IgG, IgA, lowered blood IgD exhibited in ulcer exacerbation. Antibodies to oligopeptides isolated from the serum of ulcer patients occurred in 80.6% of the patients. Whatever the drug used, after-treatment levels of secretory salivary IgA and blood IgA dropped, blood IgD rose. Belomet treatment gave rise to higher titres of antibodies to oligopeptides. Solcoseryl, cholinolytics with antacids led to increased blood levels of IgM and reduced levels of IgG. Recovery of the indices occurred in patients on enterodes. It is stated that changes in immunity depend both on ulcer healing and direct action of antiulcer drugs.

RESUMO

Serum levels of secretory IgA were measured in 66 patients with aggravation of peptic ulcer versus 52 healthy subjects. IgA concentrations in the patients were found elevated, the rise being dependent on the ulcer site, standing and severity of course. Treatment-induced decline of the ulcer exacerbation was accompanied by a fall in IgA levels. It is hold that serum IgA can serve a marker of gastroduodenal ulcer exacerbation.

RESUMO

An important role in the protection of the gastric and duodenal mucosa is played by bicarbonates. Gastric secretion of bicarbonates was examined in 42 patients with gastroduodenal pathology. Basal secretion of bicarbonates by the stomach was approximately the same (0.84 +/- 0.07 mmol/h) in patients with chronic gastritis, gastroduodenitis and peptic ulcer. After stimulation of gastric glands with a 5% alcoholic solution gastric secretion of bicarbonates increased in patients with chronic gastritis whereas in patients with peptic ulcer and gastroduodenitis, it significantly reduced. A inverse relationship was discovered between bicarbonate production and hydrochloric acid secretion. In patients with peptic ulcer, the reduction of bicarbonate and mucus output by the stomach is viewed as one of the mechanisms of ulcerogenesis.

The paper considers common phase-specific trends in hypothalamic-hypophyseal-gonadal function observed in duodenal ulcer. Progesterone and estradiol levels as well as testosterone/estradiol relationship correlated with frequency of duodenal ulcer recurrences.

RESUMO

Factors of unspecific defense and immunological state were evaluated in 520 patients with ulcer disease. It was established that these patients revealed disorders of excessive release into the internal medium of the body of food antigens, in particular partially split proteins. The latter induce the immune response. An increase occurs of the production of secretion of IgA in the secretions, IgG in the blood, the T-system of immunity is inhibited, the complement consumption is increased. On the basis of obtained data the author proposes a hypothesis of ulcer formations, the essence of which is that food antigens entering the body causes sensibilization of the organism with a development of an immune-complex reaction of the Arthus phenomenon type in the gastroduodenal zone.
