INTRODUCTION: Outcome measures of breast reconstruction have used panel assessment of photographs. This provides limited information to the assessor as these images are static.. The aim of this study was to assess whether the use of digital video was a valid assessment tool and to compare its use against photography. METHODS: 35 patients post-reconstruction underwent photography, digital video capture and completed Breast Cancer Treatment Outcomes Scale (BCTOS) questionnaires. The photographs/video clips were randomised and shown to a 21 member panel. Opinions on aesthetic aspects of the reconstruction were assessed using the BCTOS and Harris scale. Panel inter-rater agreement and patient-panel correlation was assessed using Kendall's Coefficient of Concordance and Spearman's rank correlation tests respectively. RESULTS: There was a "moderate" degree of inter-rater agreement amongst panel members in all categories. Greater agreement occurred using video footage to assess overall cosmesis (0.548 vs 0.507) and shape (0.505 vs 0.486). Video showed a greater degree of correlation with patient self-assessment scores in comparison to photography (0.311 vs 0.281). CONCLUSION: Video footage coupled with panel assessment is a valid method of assessing post-operative outcomes of breast reconstruction and appears superior to still photographs in terms of inter-rater agreement and correlation with patient self-assessment.

Despite being more expensive than conventional tissue expanders, Becker expanders offer the advantage of single stage breast reconstruction. However, the large series in published literature which report good outcomes of Becker expanders in breast reconstruction have a mean follow up period of less than three years. This does not allow for definitive conclusions as to whether the Becker expander truly meets its design goal of a lasting single stage breast reconstruction. This study is a retrospective case note review of all patients who underwent breast reconstruction using a Becker expander at our unit from 1993 to 1998, with a mean follow up of 12.5 years. Sixty-eight Becker-only breast reconstructions were carried out following oncological and risk-reducing mastectomies, and for congenital hypoplasias. There was a high premature overall explantation rate with 68% of expanders removed by 5 years due to complications which included poor aesthetics, capsular contracture and infection. The mean time to explantation for these patients was only 23 months, and time to 50% overall expander removal ('half life') was just 30 months. On subgroup analysis, patients in the congenital hypoplasias group had a significantly better rate of expander retention with 67% remaining in situ at 10 years. In comparison, patients in the oncological and risk-reducing mastectomy groups had implant retention rates of 2% and 7% respectively. The Becker expander does not appear to meet its design purpose of lasting single stage breast reconstruction in post-mastectomy cases. In contrast, it appears to have significantly better longevity when used for congenital hypoplasias.

Following endorsement of serial halving by the Faculty of Pre-Hospital Care of the Royal College of Surgeons of Edinburgh this study aimed to determine whether the technique was comparable to the rule of nines in making initial assessments of body surface area burned. Ten 'casualties' were made up to represent burn victims (range 6-61%). An external panel of six consultants and one specialist registrar in plastic surgery were invited to assess the simulated casualties. They gave individual and a consensus estimate of the burned areas. One hundred and twenty-five members of local emergency services and military paramedical staff were given a brief video and slide presentation describing either the rule of nines or serial halving method of burn area assessment. These techniques were then used to assess the 10 simulated casualties, giving 1250 estimates of burn surface area. The understanding of both techniques appeared adequate in both test groups. Estimates from serial halving and rule of nines groups differed from the assessments of the external panel. No statistical difference was demonstrated between serial halving and the rule of nines as an initial assessment tool when determining disposal. Serial halving has an inherent weakness when assessing certain sizes of burn. The rule of nines requires that the assessor knows and understands the proportionate areas of the body. The mathematics of percentages and fractions appeared to confuse some assessors.

The construction of a new breast after mastectomy involves fashioning the breast mound and creating a projecting nipple and a coloured areola. This should involve three episodes for a patient, but is this the experience of patients embarking on breast reconstruction? We identified 177 patients who had undergone breast reconstruction between 1 September 1997 and 31 March 1999. The clinical records for 164 of these patients were found and the data summarised. The techniques, complications and other ancillary procedures experienced by this group of patients are presented. Multiple procedures are likely to be required to complete breast reconstruction, and the patient should be so counselled from the outset. Particular problems may be encountered with each technique and this should be borne in mind when selecting a procedure for each patient, especially in the context of immediate reconstruction where avoiding any delay to adjuvant treatment is a consideration.

RESUMO

Complications of breast augmentation using silicone implants have been the subject of much discussion. We report a single case of a silicone granuloma, which has exhibited unusual behaviour in that it has grown rapidly and significantly. Whilst silicone granulomata have been reported on many occasions in the past, to our knowledge this is the first report of a rapidly growing example.

Tissue glues are in widespread use in Accident & Emergency departments, and are simple to use. Results of closure of uncomplicated wounds using glue are comparable to traditional methods such as suture, 'sterile strips'. There are certain times in the field situation where evacuation of a casualty with a simple laceration could be impossible. These circumstances could be suitable for an individual suitably trained to effect wound closure using a tissue glue. This study simply set out to establish whether it was possible to train an infantry soldier how and when to close a wound using a tissue glue. A short period of instruction was followed by a practical session using wound simulators. It was found that the theoretical concepts and practical skills required to successfully glue a simulated wound were readily acquired by infantry soldiers, irrespective of their previous level of first aid training. The authors suggest that this concept should be pursued with the aim of field trials of cyanoacrylate tissue adhesives being carried out.

RESUMO

Uroflowmetry has been performed on 140 occasions in 90 patients with hypospadias before and after reconstructive surgery. Children with hypospadias tend to have a lower maximum urinary flow rate for volume voided than the general population, lying around the population 5th centile. This is observed before any surgery is performed, and appears from our cross-sectional data to be unaffected by surgery. Uroflowmetry has proved a useful and economical means of gaining an objective assessment of urinary function in this group of patients. Further longitudinal studies to confirm this cross-sectional study would be useful.
