RESUMO

Despite the prevalence of substance use during pregnancy, studies focusing exclusively on Neonatal Intensive Care Units (NICU) admissions remain limited. This study investigates the impact of maternal use of tobacco, alcohol, and/or crack, on neonatal outcomes among infants admitted to three Brazilian NICUs. Additionally, the investigation explores the impact of substance use on DNA damage in newborns. Over a one-year period, data from 254 newborns were collected through medical records, accompanied by blood samples. Findings revealed that 16.1% of newborns had mothers reporting substance use during pregnancy. Significant associations were found between maternal substance use and adverse neonatal outcomes, including low birth weight, preterm birth, and sexually transmitted infections. Maternal variables linked to substance use encompassed non-white skin color, low education, non-masonry housing, lower income, diseases in other children, and fewer prenatal consultations. Notably, neonatal DNA damage showed no significant association with substance use. Our results underscore the substantial impact of maternal substance use on NICU-admitted infants, emphasizing the necessity for targeted interventions that address both neonatal health and maternal well-being, thereby underscoring the crucial role of comprehensive care in NICU settings.

INTRODUCTION: Marginal ulcers are the most prevalent endoscopic abnormality after RYGB. The etiology is still poorly understood; however, an increase in acid secretion has been strongly implicated as a causal agent. Although gastrin is the greatest stimulant of acid secretion, to date, the presence of gastrin producing G cells retained in the gastric pouch, related to the occurrence of marginal ulcers, has not been evaluated. OBJECTIVE: Evaluate the density of G cells and parietal cells in the gastric pouch of RYGB patients with a diagnosis of marginal ulcer on the post-op EGD. METHOD: We retrospectively evaluated 1104 gastric bypasses performed between 2010 and 2020. Patients with marginal ulcer who met the inclusion criteria and controls were selected from this same population. Endoscopic gastric pouch biopsies were evaluated using immunohistochemical study and HE staining to assess G cell and parietal cell density. RESULTS: In total, 572 (51.8%) of the patients performed endoscopic follow-up after RYGB. The incidence of marginal ulcer was 23/572 (4%), and 3 patients required revision surgery due to a recalcitrant ulcer. The mean time for ulcer identification was 24.3 months (2-62). G cell count per high-power field (× 400) was statistically higher in the ulcer group (p < 0.05). There was no statistical difference in parietal cell density between groups (p 0.251). CONCLUSION: Patients with a marginal ulcer after gastric bypass present a higher density of gastrin-producing G cells retained in the gastric pouch.

Brain glucose hypometabolism and neuroinflammation are early pathogenic manifestations in neurological disorders. Neuroinflammation may also disrupt leptin signaling, an adipokine that centrally regulates appetite and energy balance by acting on the hypothalamus and exerting neuroprotection in the hippocampus. The Goto-Kakizaki (GK) rat is a non-obese type 2 diabetes mellitus (T2DM) animal model used to investigate diabetes-associated molecular mechanisms without obesity jeopardizing effects. Wistar and GK rats received the maintenance adult rodent diet. Also, an additional control group of Wistar rats received a high-fat and high-sugar diet (HFHS) provided by free consumption of condensed milk. All diets and water were provided ad libitum for eight weeks. Brain glucose uptake was evaluated by 2-deoxy-2-[fluorine-18] fluoro-D-glucose under basal (saline administration) or stimulated (CL316,243, a selective ß3-AR agonist) conditions. The animals were fasted for 10-12 h, anesthetized, and euthanized. The brain was quickly dissected, and the hippocampal area was sectioned and stored at -80°C in different tubes for protein and RNA analyses on the same animal. GK rats exhibited attenuated brain glucose uptake compared to Wistar animals and the HFHS group under basal conditions. Also, the hippocampus of GK rats displayed upregulated leptin receptor, IL-1ß, and IL-6 gene expression and IL-1ß and the subunit of the transcription factor NF-κB (p-p65) protein expression. No significant alterations were detected in the hippocampus of HFHS rats. Our data indicated that a genetic predisposition to T2DM has significant brain deteriorating features, including brain glucose hypometabolism, neuroinflammation, and leptin signaling disruption in the hippocampal area.

Physical exercise is a non-pharmacological therapeutic approach to reduce adipose tissue mass, improving peripheral IR, glycemic control, and quality of life in obese animals or humans with T2DM. In this narrative review, we selected and analyzed the published literature on the effects of physical exercise on the metabolic features associated with non-obese-T2DM. Only randomized controlled trials with regular physical exercise training, freely executed physical activity, or skeletal muscle stimulation protocols in GK rats published after 2008 were included. The results indicated that exercise reduces plasma insulin levels, increases skeletal muscle glycogen content, improves exercise tolerance, protects renal and myocardial function, and enhances blood oxygen flow in GK rats.

Liver biopsy is the gold standard method to diagnose nonalcoholic fatty liver disease (NAFLD). However, ultrasound is widely recommended as the first-line imaging test for individuals with suspected NAFLD. This study aimed to estimate the accuracy of ultrasound as a screening test for NAFLD compared to liver biopsy in a cohort of patients with class II and III obesity undergoing bariatric surgery. This retrospective study included patients undergoing Roux-en-Y gastric bypass in southern Brazil between 2010 and 2019 who were screened for NAFLD with both ultrasound and liver biopsy. All samples were collected by a core biopsy needle and were analyzed by the same pathologist. Sensitivity, specificity, and positive and negative predictive values of ultrasound were estimated. The final database included 227 patients, mostly female (84%) and white (83.6%), with a mean age of 42.5 ± 10.2 years and a mean preoperative body mass index of 49.5 ± 8.4 kg/m2. A total of 153 subjects (67.4%) were diagnosed with NAFLD through liver biopsies: 41 (18%) had fatty liver and 112 (49.3%) had nonalcoholic steatohepatitis. Ultrasound sensitivity was 88.9% and specificity was 44.6%. Positive and negative predictive values were 76.8% and 66.0%, respectively. Positive likelihood ratio was 1.6 (95% CI 1.30-1.98), and negative likelihood ratio was 0.25 (95% CI 0.15-0.42). Therefore, approximately three every four subjects with an ultrasound suggesting NAFLD were true positives. Ultrasound showed a good sensitivity in detecting NAFLD in patients with class II and III obesity.

BACKGROUND: Uncoupling protein 2 (UCP2) plays an important role in energy expenditure regulation. Previous studies have associated the common -866G/A (rs659366) and Ins/Del polymorphisms in the UCP2 gene with metabolic and obesity-related phenotypes. However, it is still unclear whether these polymorphisms influence weight loss after bariatric surgery. OBJECTIVES: To investigate whether UCP2 -866G/A and Ins/Del polymorphisms are associated with weight loss outcomes after bariatric surgery. SETTING: Longitudinal study in a university hospital. METHODS: We retrospectively evaluated 186 patients who underwent Roux-en-Y gastric bypass (RYGB) surgery for clinical and laboratory characteristics in the preoperative period, 6, 12, and 18 months after RYGB. The -866G/A (rs659366) polymorphism was genotyped using real-time PCR, while the Ins/Del polymorphism was genotyped by direct separation of PCR products in 2.5% agarose gels. RESULTS: Patients with the -866A/A genotype showed higher body mass index (BMI) after 6, 12, and 18 months of surgery and excess body weight after 6 and 12 months compared with G/G patients. They also showed lower excess weight loss (EWL%) after 6 and 12 months of surgery. Ins allele carriers (Ins/Ins + Ins/Del) had lower delta (Δ) BMI 12 months after surgery compared with Del/Del patients. Accordingly, patients carrying haplotypes with ≥2 risk alleles of these polymorphisms had higher BMI and excess weight and lower EWL% during follow-up. CONCLUSION: UCP2 -866A/A genotype is associated with higher BMI and excess weight and lower EWL% during an 18-month follow-up of patients who underwent RYGB, while the Ins allele seems to be associated with lower ΔBMI 12 months after surgery. Further studies are needed to confirm the associations of the -866G/A and Ins/Del polymorphisms with weight loss after bariatric surgery.

The aim of this study was to compare the expression of UCP2, NLRP3, IL1B, IL18, and miR-133a-3p in subcutaneous adipose tissue (SAT) of 61 patients divided according to BMI: Group 1 (n = 8; BMI<25.0 kg/m2), Group 2 (n = 24; BMI 30.0-39.9 kg/m2), and Group 3 (n = 29; BMI≥40.0 kg/m2). SAT biopsies were obtained from individuals who underwent bariatric surgery or elective abdominal surgery. Gene expressions were quantified using qPCR. Bioinformatics analyses were employed to investigate target genes and pathways related to miR-133a-3p. UCP2 and miR-133a-3p expressions were decreased in SAT of Groups 2 and 3 while IL18 was increased compared to Group 1. NLRP3 and IL1B expressions did not differ between groups; however, NLRP3 was positively correlated with waist circumference and excess weight. Bioinformatics analysis demonstrated that UCP2 and NLRP3 are targets of miR-133a-3p. In conclusion, UCP2 and miR-133a-3p expressions are downregulated in patients with obesity, while IL18 is upregulated. NRLP3 is correlated with waist circumference and weight excess.

Context: Overall, giant prolactinomas are rare tumors (4%), especially those larger than 60 mm (1%). Despite the predominance of macroadenoma documented in multiple endocrine neoplasia type 1 (MEN1)-related prolactinoma, only three giant prolactinoma cases were described so far (size > 40 mm and prolactin > 1,000 ng/mL). None of them was larger than 60 mm or presented hydrocephalus or intracranial hypertension (ICH) as initial manifestation of MEN1. Case Description: A 21-years-old man presented with ICH as the first clinical manifestation of MEN1. He harbored a MEN1 germline mutation but refused periodic vigilance after normal hormonal screening at age 14 years. During investigation, magnetic resonance imaging (MRI) of the skull showed an expansive sellar/parasellar lesion (75 × 44 × 36 mm) with moderate to severe supratentorial obstructive hydrocephalus and an extremely high serum prolactin (PRL) of 10,800 ng/mL, without combined hypersecretion of other pituitary hormones. He was diagnosed with giant prolactinoma, and cabergoline was initiated. The patient evolved with early improvement of clinical complaints for hydrocephalus and ICH and PRL reached normal values (11 ng/mL) in association with significant tumoral shrinkage after 18 months on cabergoline. After 2 months of cabergoline, cerebrospinal fluid leakage was diagnosed and corrective surgery was provided. The mean dose of cabergoline was 3 mg/week throughout treatment. Conclusion: We reported the first case with hydrocephalus and ICH as the initial clinical manifestation of a giant prolactinoma in MEN1. From our knowledge, this is the largest MEN1-related prolactinoma reported so far. Notably, all four MEN1-related giant prolactinomas cases reported were younger than 21 years strengthening the importance to routine MEN1 genetic testing for prolactinoma in this age group. Also, they all had initial effective response with dopamine agonist ensuring this drug as first-line treatment for MEN1-related giant prolactinoma. However, the scarce number of treated patients and progression of cabergoline resistance in two of them suggest strict surveillance.

Habitat loss is one of the main threats to biodiversity. Fragmentation resulting from this process may restrict available habitat for primates, limiting their persistence in a given region. In this study, we aimed to quantify deforestation and fragmentation by identifying forest remnants capable of maintaining populations of the endangered crested capuchin monkey (Sapajus robustus) within its distributional limits in the states of Bahia, Espírito Santo, and Minas Gerais, Brazil. We used Landsat images from 1985 to 2010 to generate three time-interval mosaics classified by natural and non-natural vegetation. We then calculated the size of the forest remnants and the distance of each fragment of natural vegetation to its nearest neighbor. Between the first (1985-1990) and second (1995-2000) mosaics there was a vegetation loss of 41.4%, and between the second and third (2005-2010) mosaics there was a natural vegetation gain of 1%. The third mosaic showed smaller and more isolated fragments in comparison to the first mosaic. Across the three periods, Bahia was the state with the highest rate of deforestation. While Espírito Santo had fewer identified forest remnants, it retained the most natural vegetation within these small fragments. The landscape within the currently recognized distributional limits of the crested capuchin monkey is dominated by agriculture and livestock. Both decreasing fragment size and increasing species isolation by distance between fragments can negatively affect primate populations, increasing their risk of extinction. Isolation makes it difficult for migration and recolonization events to occur. Therefore, habitat restoration should be prioritized to reduce isolation between populations. Our findings may serve as a resource for future conservation efforts and management of the crested capuchin monkey.

BACKGROUND: Few data are available regarding blood loss during cleft palate surgery. This study assessed the effect of using tranexamic acid in the reduction of intraoperative bleeding. METHODS: A double-blind, randomized clinical trial compared intraoperative bleeding and the incidence of oronasal fistulas after palatoplasties in a control group that was given placebo and an intervention group that was given 10 mg/kg tranexamic acid followed by a continuous infusion of 1 mg/kg/h of the same until the end of surgery. Patients who underwent primary palatoplasty with no known or suspected coagulation disorders at our institution during the study period were included in the study. RESULTS: 70 patients were allocated, 66 received the intervention. Blood loss was reduced by 11.9%, without statistical significance. The incidence of fistulas in the intervention and control groups was 12.9% and 18.75%, respectively. The reduction of 5.8% (CI 95%: 12%-23.8%) was not significant. CONCLUSIONS: The reduction of intraoperative blood loss was lower than expected and not statistically significant; a larger sample is needed to confirm the observed reduction. The drug did not seem to have negative effects on flap viability.

Genipa americana is a tropical tree species that is widely distributed in the humid tropical and subtropical regions of Central and South America. This study investigated Mendelian inheritance, genetic linkage, and genotypic disequilibrium at six microsatellite loci developed for G. americana. Adult trees (188) and regenerants (163) were sampled and genotyped in a fragmented population of the species. We also genotyped open-pollinated seeds from 12 seed-trees during reproductive events in 2010 and 2011. Significant deviations from the expected 1:1 Mendelian segregation were detected in 29.5% of the tests. Significant genetic linkage between pairwise loci was detected in 54.4% of the tests, but no genotypic disequilibrium was detected between pairwise loci for adult trees and regenerants. Overall, the results indicate that the six loci analyzed may be used in studies of G. americana's genetic diversity and structure, its mating system, and in parentage analyses.

Leptospirosis is an infectious disease caused by the bacterium Leptospira spp. In goats, the productive impact of leptospirosis is not well known and totally unknown in Santa Catarina (SC), Brazil. This study aimed to investigate leptospirosis seroprevalence and its risk factors in goats in the west side of SC. A total of 654 blood samples were analyzed using the microscopic agglutination technique and 35.47% (232) of the animals were seropositives. Except for serogroup Autumnalis, positive samples for all other serogroups were found as follows: Sejroe (Hardjo, Wolffi), Grippotyphosa (Grippotyphosa), Canicola (Canicola), Icterohaemorrhagiae (Icterohaemorrhagiae, Copenhageni), Australis (Australis, Bratislava) and Pomona (Pomona). The contact among sheep and goats, and the addition of concentrate as food supplement were found to be risk factors for leptospirosis. Based on these results, we conclude that there is a high occurrence of anti-Leptospira antibodies in goats in the Western part of Santa Catarina State.

INTRODUÇÃO: Os cirurgiões plásticos têm recebido em seus consultórios e ambulatórios um número cada vez maior de pacientes com queixa de deformidade do contorno corporal após maciças perdas ponderais secundárias a cirurgias bariátricas. Todavia, em função das restrições alimentares e disabsorções cirúrgicas, são frequentes complicações metabólicas e distúrbios nutricionais no pós-operatório tardio das gastroplastias. O objetivo deste estudo é detectar a prevalência dos principais distúrbios nutricionais relacionados à cicatrização em pacientes de ambulatório especializado em cirurgia plástica do paciente pós-gastroplastia. MÉTODO: é um estudo observacional, transversal e descritivo, em que os pacientes foram avaliados clinicamente e através de protocolo laboratorial. RESULTADOS: foram avaliados 32 pacientes com gastroplastia há pelo menos dois anos e estabilidade ponderal há no mínimo seis meses. Os principais distúrbios detectados foram: anemia (56% dos pacientes), com carências na bioquímica do ferro (31,2%) e da vitamina B12 (25%), bem como deficiências dos íons zinco (18,7%) e cobre (3,1%). Houve carências discretas no perfil proteico e ausência de déficits na bioquímica básica e lipidograma. CONCLUSÃO: é dever do cirurgião plástico conhecer a fundo a prevalência dos principais distúrbios nutricionais relacionados à cicatrização na população de pacientes pós-gastroplastia no sentido de corrigir eventuais carências e prevenir complicações. Sabe-se, contudo, que ainda são necessários mais estudos para correlacionar qualquer deficiência alimentar às intercorrências observadas no pós-operatório deste grupo de pacientes.


INTRODUCTION: Plastic surgeons have received in their outpatient clinics an increasing number of patients complaining of body contour defects due to the massive weight loss after a bariatric surgery. However, owing to dietary restrictions and surgical desorption, metabolic complications and nutritional disorders are commonly observed in the late postoperative period after gastroplasty. The aim of this study was to determine the prevalence of major nutritional disorders related to healing in patients at a specialized outpatient plastic surgery clinic who underwent gastroplasty. METHOD: This is an observational, transversal, and descriptive study, in which a laboratory protocol was used to clinically evaluate and assess the patients. RESULTS: Thirty-two patients who underwent gastroplasty at least 2 years previously with weight stability for at least 6 months were evaluated. The major disorders identified included anemia (56% of patients) and deficiency in iron (31.2%), vitamin B12 (25%), zinc ions (18.7%), and copper (3.1%). Mild deficiencies in the protein profile were also observed; however, no deficiencies in basic and lipid biochemistry were noted. CONCLUSION: The plastic surgeon should have an in-depth knowledge about the prevalence of the major nutritional disorders related to healing in postbariatric patients, to allow correction of possible deficiencies and prevention of complications. However, more studies would be required to correlate any dietary deficiency to the complications observed in the postoperative period in this group of patients.

BACKGROUND: Bariatric surgery is the only predictable method to obtain weight loss in severe obesity. Poor physical performance of obese individuals may be mediated by the peripheral metaboreflex, which controls blood flow redistribution to exercising muscles. Weight reduction improves exercise capacity through several possible mechanisms that are insufficiently understood. We hypothesized that the metaboreflex is one among the causes of improvement in exercise capacity after weight loss. This study thus aimed to examine the effect of bariatric surgery on exercise performance and metaboreflex. METHODS: Severely obese patients were assessed before and 3 months after bariatric surgery. Metaboreflex was evaluated by the technique of selective induction by post-exercise circulatory occlusion (PECO+) after isometric handgrip exercise at 30% of maximum voluntary contraction. The exercise capacity was assessed by 6-min walking test. RESULTS: Seventeen patients completed the protocol. Body mass index decreased from 46.4 ± 2 to 36.6 ± 2 kg/m2 (P < 0.001). The distance walked in 6 min increased from 489 ± 14 to 536 ± 14 m (P < 0.001). The peripheral metaboreflex activity, expressed by the area under the curve of vascular resistance, was lower after than before bariatric surgery (42 ± 5 to 20 ± 4 units, P = 0.003). Heart rate, blood pressure, and vascular resistance were also significantly decreased. The correlation between change in distance walked in 6 min and change in peripheral metaboreflex activity was not significant. CONCLUSIONS: Weight loss after bariatric surgery increases exercise capacity and reduces peripheral metaboreflex, heart rate, and blood pressure. Further investigation on the role of metaboreflex regarding mechanisms of exercise capacity of individuals with obesity is warranted.

BACKGROUND: It is not known whether smoking by mothers during pregnancy is associated with headache in their offspring. METHODS: Two prospective cohorts of 869 children aged 10-11 years from Ribeirão Preto (RP) and 805 children aged 7-9 years from São Luís (SL) were studied. Data on maternal smoking were collected at birth. Primary headache was defined as a reporting of ≥2 episodes of headache in the past 2 weeks, without any associated organic symptoms. RESULTS: Prevalence of headache was 28.1% in RP and 13.1% in SL as reported by the mothers and 17.5% in RP and 29.4% in SL as reported by the children. Agreement between mothers' report and children's self-report of primary headache in the child was poor. After adjustment, children whose mothers smoked ≥10 cigarettes per day during pregnancy presented higher prevalence of primary headache than their counterparts in both cohorts, as reported by the mother and in RP as reported by the children. CONCLUSIONS: Maternal smoking during pregnancy was associated with headache in 7- to 11-year-olds. With one exception, the consistency of the results, despite poor agreement between maternal and children reports of headache, indicates that maternal smoking during pregnancy may contribute to headaches in their children.

OBJECTIVE: The diagnosis of subclinical central hypothyroidism in hypothalamic-pituitary patients cannot be established by serum markers of thyroid hormone action. Myocardial function by echocardiography has been shown to reflect thyroid hormone action in primary thyroid dysfunction. We evaluated the performance of echocardiography in diagnosing subclinical central hypothyroidism. DESIGN: Cross-sectional and before and after. METHODS: Echocardiography and serum thyroid hormones were assessed in overt primary (n=20) and central (n=10) hypothyroidism, subclinical primary hypothyroidism (n=10), hypothalamic-pituitary disease with normal free thyroxine (FT(4); n=25), and controls (n=28). Receiver operating characteristic (ROC) curves were generated using overt hypothyroidism patients and selected cut-off values were applied to detect both primary and central subclinical hypothyroidism. After levothyroxine (l-T(4)) intervention, patients were echocardiographically reevaluated at predefined targets: normal thyrotropin (TSH) in primary hypothyroidism, normal FT(4) in overt central hypothyroidism, and higher than pretreatment FT(4) in echo-defined subclinical central hypothyroidism. RESULTS: Parameters with highest areas under the ROC curves (area under the curve (AUC) ≥0.94) were as follows: isovolumic contraction time (ICT), ICT/ejection time (ET), and myocardial performance index. Highest diagnostic accuracy (93%) was obtained when at least one parameter was increased (positive and negative predictive values: 93%). Hypothyroidism was echocardiographically diagnosed in eight of ten patients with subclinical primary hypothyroidism and in 14 of 25 patients (56%) with hypothalamic-pituitary disease and normal serum FT(4).
