RESUMO

AIM: In response to the SARS-CoV-2 emergency, the Competence Centre on digital health 'TrentinoSalute4.0' has developed TreC_Televisita, a tele visit solution that meets the needs of the Trentino healthcare system and maintains high-quality patient-doctor interactions while respecting social distancing. This paper highlights how 'TreC_Televisita' was integrated into the Trentino healthcare system and its potential to become a structural and durable solution for the future local healthcare service provisioning. SUBJECT AND METHODS: This paper presents the multifactorial context that TreC_Televisita has faced for its implementation and the strategies adopted for its structural integration into the healthcare system. The analysis focuses on the main issues faced for the integration of the tele visits (e.g. privacy, payments) and how the context of TrentinoSalute4.0 permitted responding quickly to its implementation during the pandemic. It also describes how TreC_Televisita fits into the healthcare continuum from the organisational and technological standpoint, the end-user perspective and the barriers that could hamper the solution scalability. RESULTS: TreC_Televisita has demonstrated to be a technological solution that can be contextualised for different clinical domains beyond SARS-CoV-2. Moreover, it has shown its potential to scale up the solution beyond the COVID-19 emergency to the whole healthcare provisioning system in the long term. CONCLUSION: Being a positive experience in the first months of its implementation, the long-term goal is to transform TreC_Televisita into a structural pillar of the Trentino healthcare system, setting the bases for a sustainable, win-win situation for all the stakeholders involved in healthcare service provisioning.

RESUMEN Introducción: La lobectomía temporal anterior (LTA) es un procedimiento quirúrgico comúnmente utilizado para el tratamiento de la epilepsia del lóbulo temporal refractario al tratamiento médico, con altas tasas de éxito en el control de las crisis. Sin embargo, se ha descrito una asociación importante con enfermedades psiquiátricas que puede afectar al resultado posquirúrgico en estos pacientes. Métodos: Se exponen 2 casos representativos de pacientes que sufrieron complicaciones psiquiátricas en el posoperatorio de lobectomía temporal anterior, a pesar del control exitoso de las crisis. Resultados: Un varón sin antecedentes de enfermedad mental que sufre un episodio depresivo mayor en el periodo posoperatorio mediato, y una mujer con psicosis previa que evidencia exacerbación de su afección como complicación quirúrgica. Conclusiones: La enfermedad psiquiátrica se puede presentar en pacientes posoperatorios de epilepsia de lóbulo temporal tanto con antecedentes de enfermedad mental como sin ellos. Las alteraciones más frecuentes reportadas son depresión, ansiedad, psicosis y trastornos de la personalidad. La inclusión de evaluaciones psiquiátricas en los protocolos prequirúrgicos y posquirúrgicos pueden llevar a una mejora en el pronóstico de los resultados neurológicos y mentales de los pacientes sometidos a la intervención.

Methods: A series of 2 cases of patients who, despite successful crisis control, developed psychiatric complications in the postoperative period of anterior temporal lobectomy. Results: The cases included a male patient with no history of previous mental illness, who developed a major depressive episode in the postoperative period, and a female patient with previous psychosis who presented as a surgical complication exacerbation of psychosis, diagnosed with paranoid schizophrenia. Conclusions: Psychiatric disorders can occur in postoperative temporal lobe epilepsy patients with or without a history of mental illness. The most frequent alterations reported are depression, anxiety, psychosis and personality disorders. The inclusion of psychiatric evaluations in the pre- and post-surgical protocols can lead to an improvement in the prognosis of the neurological and mental outcomes of the patients undergoing the intervention.

RESUMO

The law n. 81/2008 gathers the regulations about the health safeguard and safety in the workplaces. It provides the duty of risk management, the designation of a responsible for the prevention and protection services, the designation of a specialist doctor for the surveillance of the operators and of the workplaces and a training focused on safety for all the operators. In the healthcare workplaces usually we can distinguish between the risks for the workers safety and the risks for the workers health. The art.43 of the law 81/2008 also provides the legal guarantee both for other people attending workplaces and patients. In fact the law n.24/2017 at the art. 1 confirms that the care safety is part of the health right.

RESUMO

Apesar de toda carga que deve suportar a articulaçäo do joelho, é relativamente fraca do ponto de vista mecânico, devido as configuraçôes de suas superfícies articulares, ficando sua resistência na dependência dos ligamentos que unem o fêmur à tíbia. No estudo realizado, foram acompanhados dois pacientes do sexo masculino, ambos com idade de 26 anos que sofreram lesäo total do ligamento cruzado anterior. Como tratamento fisioterápico, utilizou-se o denominado conservador, recuperando a funçäo que após dez sessöes de fisioterapia distribuídas em um período de 20 dias. Inicialmente enfocou-se nessas sessöes o alívio do quadro agudo (sinais inflamatórios, inclusive perda da funçäo) através de eletroterapia associada com exercícios passivos e ativos livres, variando de duas a quatro sessöes semanais. A seguir trabalhou-se a reeducaçäo proprioceptiva e fortalecimento muscular. Constatou-se através dos resultados que o tratamento conservador nas lesöes de ligamento cruzado anterior näo foi eficaz para restabelecer a estabilidade articular compatível para a realizaçäo de atividades físicas intensas que sobrecarreguem a articulaçäo do joelho, sendo entretanto, eficaz para as atividades físicas rotineiras necessárias às atividades de vida diária.
