RESUMO
Twenty-six cases of hallux valgus deformity, in 16 children with cerebral palsy, were managed with great toe metatarsophalangeal (MTP) arthrodesis. Mean age at the time of surgery was 16 years (range, 10 years and 11 months to 21 years and 11 months), and mean follow-up was 4 years and 11 months (range, 2 years and 1 month to 10 years). Significant improvement in the hallux valgus angle (preoperative, 36.3 degrees; follow-up, 9.6 degrees; p < 0.05), the intermetatarsal angle (preoperative, 12.3 degrees; follow-up, 8.4 degrees; p < 0.05), and lateral metatarsophalangeal angle (preoperative, 4.8 degrees; follow-up, 25.8 degrees; p < 0.05), were achieved and maintained after MTP arthrodesis. Functional outcome was documented by significant improvement in the modified American Orthopaedic Foot and Ankle Society Hallux Metatarsophalangeal-Interphalangeal Scale (preoperative mean score, 46.2; follow-up mean score, 90.9; p < 0.05). Patient/ parent/caregiver satisfaction (as determined by a questionnaire), with improvements in cosmesis, footwear, hygiene, activity, and pain were high, ranging from 81% to 100%. Hallux valgus deformity in children with cerebral palsy is best managed by MTP arthrodesis, in conjunction with other surgical procedures that address segmental foot malalignment and dynamic gait deviations.

Clinical and pathological studies of a neurological disorder of 3 adult Rottweiler dogs are described. Dogs developed proprioceptive losses involving the limbs, leading to progressively worsening ataxia and hypermetria. Blood and CSF fluid analyses were normal and radiographs and myelograms showed no compressive lesions of the spinal cord. At necropsy, severe demyelination of the dorsal and lateral funiculi of the cervical spinal cord was visible grossly. Microscopically there was more extensive, symmetrical involvement of the spinal cord and brain than was apparent grossly, with demyelination of white matter tracts and sparing of axons in affected areas. These changes were considered diagnostic for leucoencephalomyelopathy. This condition has not been previously reported in Australia, and is probably transmitted genetically, presumably as an autosomal recessive trait.

A case of bicipital bursitis in a six-year-old mare is described. The horse was presented with acute lameness, pyrexia and hot swelling over the right shoulder region. Although antibiotic treatment relieved these signs, a firm cold swelling remained which had to be removed by surgical resection and drainage.

The successful treatment of a single case of traumatic reticulopericarditis is described. Under general anaesthesia the pericardial sac was approached by resection of the distal part of the fifth rib and allowed to drain directly to the exterior. The discussion reviews the surgical technique.

The surgical techniques employed in the repair of femoral neck fractures in two dogs and a Charolais calf are described. All the fractures were extracapsular; one of the dogs also had a fractured femoral diaphysis. For both dogs, exposure of the femoral neck was made from a dorsal approach after removal of the greater trochanter, while for the calf, a gluteal transection technique was used. All the fractures were successfully fixed using cortical screws, and in the case of the femoral diaphysis, a vitallium plate was used. All the animals were walking with minimal lameness eight weeks after surgery.

Observations on a series of 38 cases (35 Doberman Pinschers and three Great Danes) of the canine wobbler syndrome are described. Radiographic examinations suggested that the primary lesion is an intervertebral instability at C6/7, but with time, secondary changes of the disc degeneration and prolapse and vertebral body malformation causing stenosis of the vertebral canal occur. Cases detected at a young age and showing only the primary lesion were treated successfully, by disc fenestration alone or by disc fenestration and intervertebral screwing. The success rate in older cases with secondary lesions was poor and in this type of case, cord decompression by dorsal laminectomy appears necessary.

Two cases of bilateral atresia of the nasolacrimal orifice are described. The presenting sign was a persistent ocular discharge, clear in the one case and purulent in the other. Treatment by the creation of a fistula between the distal extremity of the blind ending duct and the nasal cavity was successful in both cases.

Seven cases of chronic tenosynovitis affecting the extensor carpi radialis or lateral digital extensor tendon and their sheaths are described. All cases were chronic and in 4 of the cases previous treatment by drainage and local corticosteroid injection had been ineffective. Surgical exploration of 6 cases demonstrated a distinct lesion and specific surgical treatment was carried out.

Two cases of osteochondrosis dissecans of the head of the humerus in foals are described. The lesions were gross compared with those seen in dogs. Arthrotomy and curettage of the affected area resulted in marked clinical improvement.

Indirect arterial blood pressure was determined on 33 horses prior to surgical intervention for the diagnosis and/or correction of acute abdominal disorders and a relationship between low systolic blood pressure and non-survival was established. It is suggested that blood pressure determination should be used to augment other methods of clinical and laboratory examination in cases of equine colic.

Observations based on the case histories of 39 dogs and bitches (38 Dobermans and 1 Great Dane) diagnosed as "wobblers" are presented. Cases diagnosed at a young age were shown to have cervical vertebral instability particularly at the C6/C7 articulation. Older animals showed a variety of secondary degenerative lesions. Treatment by fenestration of intervertebral disc was effective in cases which showed only vertebral instability but was much less effective in older animals with associated degenerative changes.
