BACKGROUND AND OBJECTIVES: A low risk of gastrointestinal disorders in the adult population has long been linked to consuming a Mediterranean diet (MD). The study aims to evaluate the effects of the Mediterranean diet educational program on employee's knowledge regarding functional gastrointestinal disorders. MATERIALS AND METHODS: A quasi-experimental study design of two study groups (pre-test and post-test) was carried out at four colleges in the University of Baghdad, Iraq of the period between January 10, 2022 and May 31, 2022. A purposive (non-probability) sample was used with a sample size of (160) employees divided into two groups (80 employees each). The study group of 80 employees was exposed to the Mediterranean Diet educational program while the control group with was not. RESULTS: The results showed a positive effect of the Mediterranean diet educational program on the employees' functional gastrointestinal disorders (FGIDs) knowledge in the study group. The study results indicated that there are highly significant mean differences between the study and control groups in the pre-test and the post-test in the specific and general knowledge area of employees' functional gastrointestinal disorders (t = 17.002, df = 158; t = 24.19, df = 158; P value ≤ 0.01). Findings indicated that there were no significant statistical relationships between the level of knowledge of functional gastrointestinal disorders and demographic characteristics except employees' level of education with a high significant association P value ≤ 0.01). CONCLUSION: The Mediterranean diet educational program effectively improves the employees' knowledge of functional gastrointestinal disorders. These results support the benefit of establishing educational programs for adult employees on the Mediterranean diet to improve individuals' knowledge about the gastrointestinal diseases.

RESUMO

The Advanced Ion Source for Hadrontherapy (AISHa) is an electron cyclotron resonance ion source operating at 18 GHz, developed at the Istituto Nazionale di Fisica Nucleare-Laboratori Nazionali del Sud, with the aim of producing high intensity and low emittance highly charged ion beams for hadrontherapy purposes. Moreover, thanks to its unique peculiarities, AISHa is a suitable choice for industrial and scientific applications. In the framework of the INSpIRIT and IRPT projects, in collaboration with the Centro Nazionale di Adroterapia Oncologica, new candidates for cancer treatment are being developed. In particular, the paper presents the results of the commissioning of four ion beams of interest for hadrontherapy: H+, C4+, He2+, and O6+. Their charge state distribution in the best experimental conditions, their emittance, and brightness will be critically discussed, along with the role of ion source tuning and space charge effects in beam transport. Perspectives for further developments will also be presented.

The surgical treatment of the intermediate-risk DTC (1-4 cm) remains still controversial. We analyzed the current practice in Italy regarding the surgical management of intermediate-risk unilateral DTC to evaluate risk factors for recurrence and to identify a group of patients to whom propose a total thyroidectomy (TT) vs. hemithyroidectomy (HT). Among 1896 patients operated for thyroid cancer between January 2017 and December 2019, we evaluated 564 (29.7%) patients with unilateral intermediate-risk DTC (1-4 cm) without contralateral nodular lesions on the preoperative exams, chronic autoimmune thyroiditis, familiarity or radiance exposure. Data were collected retrospectively from the clinical register from 16 referral centers. The patients were followed for at least 14 months (median time 29.21 months). In our cohort 499 patients (88.4%) underwent total thyroidectomy whereas 65 patients (11.6%) underwent hemithyroidectomy. 151 (26.8%) patients had a multifocal DTC of whom 57 (10.1%) were bilateral. 21/66 (32.3%) patients were reoperated within 2 months from the first intervention (completion thyroidectomy). Three patients (3/564) developed regional lymph node recurrence 2 years after surgery and required a lymph nodal neck dissection. The single factor related to the risk of reoperation was the histological diameter (HR = 1.05 (1.00-1-09), p = 0.026). Risk stratification is the key to differentiating treatment options and achieving better outcomes. According to the present study, tumor diameter is a strong predictive risk factor to proper choose initial surgical management for intermediate-risk DTC.

RESUMO

The Advanced Ion Source for Hadrontherapy (AISHa) has been designed to generate high brightness multiply charged ion beams for hadron therapy applications. AISHa is a compact electron cyclotron resonance ion source whose hybrid magnetic system consists of a permanent Halbach-type hexapole magnet and a set of independently energized superconducting coils. This has allowed us to achieve high performances in a cost effective way. During the commissioning phase, a few criticalities have been observed and fixed in 2018/19; the improvements will be briefly described and the results of the operations with a single 18 GHz generator will be presented. Particular relevance will be given to the production of high intensity beams of oxygen, argon, and carbon, the latter having huge importance for hadron therapy applications. Perspectives for further improvements, including double frequency heating, will also be highlighted.

OBJECTIVES: SAPHO syndrome (synovitis, acne, pustulosis, hyperostosis and osteitis) is a rare disease combining skin, bone and joint manifestations. In recent years new therapeutic strategies have been tried, among them TNF-alpha-blocking agents. We report our experience with infliximab in four cases of SAPHO syndrome refractory to conventional therapies. METHODS: Between 2002 and 2005, four cases of SAPHO syndrome (two females and two males; mean age 49.7 yr) responding poorly to conventional drugs were treated with infliximab. The dose was 5 mg/kg, according to the protocol used in spondyloarthropathies, with infusions at 0, 2 and 6 weeks followed by 6 weeks intervals. No active cutaneous manifestations were present at the time of starting therapy. RESULTS: Complete remission of osteoarticular involvement was achieved after the second or third infusion, and the positive response was maintained for up to 12 months. A patient relapsed after discontinuation of infliximab, because of infectious complication. Palmoplantaris pustulosis relapsed in two patients after three and six infusions, respectively; there was slight improvement after discontinuation of anti-TNF-alpha drugs. CONCLUSIONS: Infliximab seems to be a very effective therapy for osteoarticular complaints of SAPHO syndrome. Cutaneous involvement responded less favourably, palmoplantaris pustulosis relapse being a possible complication.

Transient osteoporosis of the hip and regional migratory osteoporosis are uncommon and probably underdiagnosed bone diseases characterized by pain and functional limitation mainly affecting weight-bearing joints of the lower limbs. These conditions are usually self-limiting and symptoms tend to abate within a few months without sequelae. Routine laboratory investigations are unremarkable. Middle aged men and women during the last months of pregnancy or in the immediate post-partum period are principally affected. Osteopenia with preservation of articular space and transitory edema of the bone marrow provided by magnetic resonance imaging are common to these two conditions, so they are also known by the term regional transitory osteoporosis. The appearance of bone marrow edema is not specific to regional transitory osteoporosis but can be observed in several diseases, i.e. trauma, reflex sympathetic dystrophy, avascular osteonecrosis, infections, tumors from which it must be differentiated. The etiology of this condition is unknown. Pathogenesis is still debated in particular the relationship with reflex sympathetic dystrophy, with which regional transitory osteoporosis is often identified. The purpose of the present review is to remark on the relationship between transient osteoporosis of the hip and regional migratory osteoporosis with particular attention to the bone marrow edema pattern and relative differential diagnosis.

The purpose of this study was to correlate soybean dust (SD) exposure, skin reactivity to soybean hull (SH) allergens, and symptoms of asthma and/or allergic rhinitis. A group of 365 subjects with asthma and/or allergic rhinitis and a control group of 50 individuals without respiratory symptoms were studied. The level of exposure to SD is defined as follows: 1) direct (DE); 2) indirect (ID), and 3) urban (UE). All subjects completed standard questionnaires. Skin tests with a SH extract and with common allergens were performed by the prick technique (SPT). Fifty-six (15.3%) patients and no subjects from control group had positive SPT (histamine index > or = 0.5) with a SH allergen extract. The percentages of positive SPT to SH extract were 38.7%, 20.3% and 8.4% in subjects with DE, IE and UE, respectively (p < 0.001). Monosensitization to SH was absent in all subjects. The percent of subjects with positive SPTs to mites (p < 0.01), pollen (p < 0.01) and molds (p < 0.05) were higher in subjects with a positive SPT to SH versus those with a negative SPT to SH. Sixty-six percent of subjects with DE and 13.6% of subjects with IE or UE reported respiratory symptoms after SD inhalation (Odds Ratio: 12.67 [2.4-74.9], p < 0.001). Compared to subjects exclusively sensitized to mites, patients sensitized to SH presented significantly different clinical characteristics. Soybean production has been increasing in Argentina during the last 20 years, determining an increase in the population exposed to chronic SD inhalation. This fact determines a high risk of sensitization and triggering of respiratory symptoms in atopic subjects. This study demonstrates that there is: 1) a high prevalence of skin reactivity to SH in subjects with asthma and/or allergic rhinitis from Argentina and that this prevalence is associated with the level of exposure to SD, and 2) an association between sensitivity to SH and severity of asthma. Measures to avoid release and inhalation of SD in rural areas from Argentina are needed.

El objetivo del presente trabajo fue evaluar la prevalencia de la sensibilización a Lepidoglyphus destructor (Ld), Tyrophagus putrescentiae (Tp) y Chortoglyphus arcuatus (Cha) en pacientes con asma y/o rinitis que residen exclusivamente en Rosario y compararla con otros ácaros estudiados previamente. En tres centros privados y tres hospitales públicos, se estudiaron 216 pacientes, 54,6 por ciento de sexo femenino, con edades comprendidas entre los 5 y 55 años (x 23,3 años). El 20,8 por ciento de ellos presentaba asma, el 38,9 por ciento rinitis y el 40,3 por ciento ambas patologías. Se confeccionó un cuestionario tipo y se realizaron prick tests (SPT) con extractos de Ld, Tp, Cha, Dermatophagoides pteronsyssinus (Dp) y Blomia tropicalis (Bt) obtenidos a partir de aislamiento y cultivos locales, y con una batería de aeroalergenos comerciales. El 79,6 por ciento (n = 172) de los pacientes presentó al menos un STP (+) y de ellos, el 94,8 por ciento mostró sensibilidad a algún ácaro y el 48,3 por ciento a otros alergenos. La prevalencia de sensibilización a alguno de los tres ácaros en estudio fue de 76,2 por ciento (Ld 69,2 por ciento, Cha 64,0 por ciento y Tp 61,6 por ciento), siendo de 66,1 por ciento en los pacientes con rinitis, de 78,9 por ciento en asma y de 82,7 por ciento para ambas patologías. No se observaron diferencias entre centros públicos y privados. El 54,6 por ciento de los pacientes (n=89) tuvo SPT (+) para los 5 ácaros testeados, el 6 por ciento (n=13) fue (-) para Dp y (+) para alguno de los otros 4 ácaros y el 3 por ciento (n=6) fue (+) sólo a Ld, Tp ó Cha. Se concluye que la prevalencia de sensibilización a Ld, Tp y Cha hallada en Rosario justifica ampliamente su inclusión en el panel de aeroalergenos, lo que permitiría identificar sensibilizaciones inaparentes

El 79,6 por ciento (n = 172) de los pacientes presentó al menos un STP (+) y de ellos, el 94,8 por ciento mostró sensibilidad a algún ácaro y el 48,3 por ciento a otros alergenos. La prevalencia de sensibilización a alguno de los tres ácaros en estudio fue de 76,2 por ciento (Ld 69,2 por ciento, Cha 64,0 por ciento y Tp 61,6 por ciento), siendo de 66,1 por ciento en los pacientes con rinitis, de 78,9 por ciento en asma y de 82,7 por ciento para ambas patologías. No se observaron diferencias entre centros públicos y privados. El 54,6 por ciento de los pacientes (n=89) tuvo SPT (+) para los 5 ácaros testeados, el 6 por ciento (n=13) fue (-) para Dp y (+) para alguno de los otros 4 ácaros y el 3 por ciento (n=6) fue (+) sólo a Ld, Tp ó Cha. OBJECTIVE: To investigate whether a new flow-triggered (FT) system can reduce the patient's inspiratory effort compared to a traditional pressure-triggered (PT) system during weaning from mechanical ventilation. DESIGN: Prospective study. SETTING: Intensive care unit of a General Hospital. PATIENTS AND PARTICIPANTS: 10 mechanically ventilated patients, without chronic airway disease, ready to wean. MEASUREMENTS: Minute ventilation, breathing pattern, lung mechanics, inspiratory work of breathing (WI) and pressure time product (PTP) of Ppl were obtained in two conditions: 1) unsupported spontaneous breathing through the ventilator circuit (SB); 2) spontaneous breathing with continuous positive airway pressure set at 5 cmH2O (CPAP). Two triggering systems, namely PT and FT, were used in each condition. RESULTS: Though there was no change in breathing pattern, minute ventilation, and lung mechanics, the magnitude of the inspiratory effort decreased significantly with FT compared to PT in both instances. The added resistance (total flow resistance minus pulmonary resistance) decreased by 37% on average when FT replaced PT. PTP decreased, on average, 27% and 15% during SB and CPAP, respectively, with FT compared to PT (p < 0.05). A similar significant decrease was observed in WI. CONCLUSION: The new FT system, i.e. flow-by system, reduces the unintentional ventilatory workload upon the patients' inspiratory muscles compared to traditional PT system during weaning from mechanical ventilation.
