RESUMO

Proton-pump inhibitors (PPIs) are one of the most prescribed drugs in the world. Although they are remarkably safe, with minimal adverse effects, it has rarely been reported as a cause of anaphylaxis. Hence, we report the case of a 69-year-old patient who experienced intravenous pantoprazole-induced anaphylaxis during peribulbar block anesthesia for mechanical vitrectomy.

RESUMO

INTRODUCTION: Near infrared spectroscopy is a non-invasive method to assess regional oxygenation and is being used in transcatheter aortic valve implantation to document periods of cerebral hypoperfusion, where cerebrovascular events are one of the most feared complications. Alkaptonuria is a rare metabolic disease characterized by accumulation of homogentisic acid in tissues and body fluids. The accumulation of pigment might interfere with the absorption of near infrared light, used in near infrared spectroscopy monitoring. We present a case of near infrared spectroscopy failing to accurately monitor cerebral oximetry in a woman, with alkaptonuria, undergoing a transcatheter aortic valve implantation.

RESUMO

Kounis syndrome is an acute coronary syndrome occurring in the setting of a hypersensitivity reaction. It occurs as a consequence of mast-cell activation and degranulation causing coronary artery spasm, atherosclerotic plaque erosion, or stent thrombosis. We report the case of a man who presented to the emergency department with typical angina that started 20 minutes after coronavirus disease 2019 (COVID-19) vaccine administration. Electrocardiogram showed inferior ST-elevation myocardial infarction, and coronary angiography confirmed right coronary artery stent thrombosis. Five months before, he had an acute coronary syndrome after influenza vaccine administration. Both vaccines share a common excipient, polysorbate 80. Considering the reproducibility of the reaction and the temporal association between vaccine administration and coronary events, a hypersensitivity reaction to polysorbate 80 was admitted and the patient was instructed to further avoid drugs containing it. To our knowledge, this constitutes the first reported case of type 3 Kounis syndrome after COVID-19 vaccine administration. Kounis syndrome is an uncommon disease, often undiagnosed. Its recognition and proper management are crucial to prevent future events. .

Aim:Schools have proven to be favorable environments for the dissemination and sharing of health information. This study aimed to report the extension actions promoted by the Project "Oral Health Promotion" in two public schools in Belo Horizonte/MG. Methods:A total of 105 students, 8 to 15 years of age, participated in the study. The project team consisted of students and professors from UFMG. Three interventions were carried out using educational information on oral health. The students answered a questionnaire to evaluate their daily habits and knowledge about oral health before and after the interventions. Descriptive analyzes, as well as McNemar and Wilcoxon tests, were performed (p < 0.05). Results:Interventions were able to improve students' knowledge and attitudes about oral health. In relation to the first assessment, there was a significant reduction in the reported amount of toothpaste placed on the toothbrush (p < 0.001). In addition, the schoolchildren began to use dental floss more frequently, there was a reduction in the frequency of the intake of candies and less interest in using piercings and false orthodontic appliances. Conclusion:The extension activities resulted in greater knowledge of the students about oral health and highlighted the need for continuous actions so that good health habits are solidified. (AU)

RESUMO

Planktonic and attached cells of strains of Candida albicans, C. glabrata and C. krusei with varied susceptibilities to fluconazole (FCZ) were compared for their relative susceptibilities to Ag+ via cell recovery and flow cytometric analyses. All strains lost membrane permeability and were non-recoverable upon culture after 1h exposure in morpholino-ethanesulfonic acid (MES) buffer fortified with

RESUMO

Wild-type and efflux pump-deficient cells of Candida albicans adhering to silicone were compared with planktonic cells by flow cytometry for their relative resistance to fluconazole (FCZ). Flow cytometry data on cells carrying a fusion of green fluorescent protein to efflux pump promoters confirmed that enhanced tolerance of attached cells to FCZ was due in part to increased expression of CaMDR1 and CDR1 promoters. Within 2 h of their attachment to silicone, the adherent cells demonstrated levels of FCZ tolerance shown by cells from 24-h biofilms. Following their mechanical detachment, this subset of cells retained a four- to eightfold increase in tolerance compared with the tolerance of planktonic cells for at least two generations. Enhanced efflux pump tolerance to FCZ appeared to be induced within the initial 15 min of attachment in a subset of cells that were firmly attached to the substrata.
