Objective The Body Image Disturbance Questionnaire for Scoliosis (BIDQ-S) for scoliosis derives from the Body Image Disturbance Questionnaire (BIDQ) with specific adaptation for scoliosis patients. Despite its significance and applicability, this instrument has never been translated into Brazilian Portuguese. The current study aimed to translate, transculturally adapt, and validate the BIDQ-S into Brazilian Portuguese. Methods BIDQ-S was translated and culturally adapted into Brazilian Portuguese using the American Association of Orthopedic Surgeons (AAOS) criteria. The questionnaire validation relied on internal consistency and comparison with the Cobb angle, Pediatric Quality of Life Inventory (PedsQL), and Scoliosis Research Society (SRS-22). The Brazilian version (BR-)BIDQ-S validation occurred in a sample of 35 adolescents with scoliosis waiting for specialized treatment. Results Internal consistency of the BR-BIDQ-S was 0.899 according to the Cronbach's index (i.e., virtually perfect). Although BR-BIDQ-S did not correlate with the Cobb angle, it presented correlations with the Physical, Emotional, and Social domains from the PedsQL and the Function/Activity domain from the SRS-22. Conclusion BR-BIDQ-S was reliable in evaluating the body image of adolescents with scoliosis, presenting an internal consistency of 0,899 (virtually perfect). Moreover, similar to the original instrument, it correlated with PedsQL and SRS-22.

Abstract Objective: To analyze the association between osteolysis at the prosthesis interfaces, as determined by magnetic resonance imaging (MRI) with multiacquisition variable-resonance image combination selective (MAVRIC-SL) sequences, and clinical severity after knee or hip arthroplasty, as well as to assess interobserver and intraobserver agreement on periprosthetic bone resorption. Materials and Methods: This was a cross-sectional study of 47 patients (49 joints) under postoperative follow-up after knee or hip arthroplasty, with chronic pain, between March 2019 and August 2020. All of the patients completed the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) questionnaire. The component interfaces were evaluated and ordered into two groups: osseointegrated and osteolytic. Nonparametric tests were used. Results: There were significant differences between the two groups in terms of the mean WOMAC scores: total (p = 0.010); stiffness domain (p = 0.047); and function (p = 0.011) domains. There was substantial interobserver and intraobserver agreement for most analyses of the components. Conclusion: Periprosthetic osteolysis appears to be associated with clinical complaints of pain in the post-arthroplasty scenario, and MAVRIC-SL provides reproducible assessments. It could prove to be an important tool for orthopedists to use in the evaluation of challenging cases of chronic pain after arthroplasty.


Resumo Objetivo: Analisar associação entre osteólise nas interfaces protéticas por ressonância magnética com sequências MAVRIC-SL e a gravidade clínica dos pacientes submetidos a artroplastias de joelho ou quadril. Determinar concordância intraobservador e interobservador na reabsorção óssea peri-implante. Materiais e Métodos: Foi realizado estudo transversal entre março de 2019 e agosto de 2020, com 47 pacientes (49 articulações) em seguimento pós-operatório de artroplastias de joelho ou quadril, com dor crônica, que responderam ao questionário WOMAC. As interfaces dos componentes foram avaliadas e definiram dois grupos: osteointegrado e osteólise. Testes não paramétricos foram usados. Resultados: Houve diferença significativa na média do escore WOMAC entre os grupos (p = 0,010), assim como nos domínios rigidez (p = 0,047) e função (p = 0,011). Houve concordância substancial interobservador e intraobservador para a maioria dos componentes analisados. Conclusão: Osteólise periprótese parece estar associada com a queixa clínica de dor pós-artroplastia, com avaliação reprodutível pela MAVRIC-SL. Isto pode ser uma importante ferramenta para o ortopedista na avaliação de casos desafiadores de dor crônica pós-artroplastia.

OBJECTIVE: To systematically establish whether there is an association between polymorphisms and avascular necrosis in patients with sickle cell disease. DATA SOURCE: The review, conducted according to PRISMA guidelines and registered with PROSPERO, was based on research of studies in PubMed, SciELO, LILACS, BVS databases and in the gray literature (Google Scholar and Open Gray) published until June 2020. The STROBE initiative was used to analyze the articles' quality. DATA SYNTHESIS: Ten articles were selected from the databases and two were included through manual search, totaling 12 studies. All samples gathered 2,362 patients. According to STROBE, seven studies fully and/or partially covered more than 70% of the essential items and two studies reached less than 60%, with an overall variation of 86.4-54.5%. The results indicate that polymorphisms in the genes of the bone morphogenetic protein 6 (BMP6), Klotho (KL) and Annexin A2 (ANXA2) may be associated with osteonecrosis in the context of sickle cell disease. Six articles addressed the polymorphism in the MTHFR enzyme gene, but only one found a positive association. Polymorphisms associated with the DARC receptor, the ITGA4 gene, CD36 and thrombophilia protein genes were not associated in any of the studies. CONCLUSIONS: The results indicate that the polymorphisms in BMP6, Klotho and ANXA2 genes may be associated with avascular necrosis in patients with sickle cell disease. However, in order to confirm these genetic changes as risk factors, further studies with greater statistical power and methodological rigor are needed.

RATIONALE AND OBJECTIVES: To compare bone marrow fat quantification using magnetic resonance spectroscopy (MRS) and six-point DIXON (6PD) techniques in patients with sickle cell disease (SCD) and healthy subjects. MATERIALS AND METHODS: Prospective study, with 43 SCD patients (24 homozygous [SS], 19 double heterozygous [SC), and 41 healthy subjects paired by age, weight and sex with SCD patients. All participants underwent magnetic resonance imaging with 6PD and single voxel MRS in the L3 vertebral body. Pearson's correlation, ROC curve, and bland-altman analysis were performed, p-values ​​≤0.05 were considered statistically significant for all tests. RESULTS: Significant linear correlation was found between fat fraction (FF) by 6PD and Total Lipids (TL) (r = 0.932; p < 0.001) and Saturated Lipids (SL) (r = 0.934; p < 0.001), in all subjects. Strong correlations were also identified considering subjects of the SS/SC subgroups. Despite high correlations, no significant difference was observed only between FF and SL in the SS subgroup (Bland-Altman analysis), indicating excellent agreement between the fat estimations in this specific situation. Significant differences were observed in all variables (FF, TL, SL) comparing the SCD and healthy subjects. The ROC curve between SCD and healthy subjects showed the following areas under the curve: FF(0.924) > TL(0.883) > SL(0.892). CONCLUSIONS: The comparison between fat quantification by the 6PD with MRS demonstrated an excellent correlation in SCD patients, especially in the SS subgroup, which usually has a higher degree of hemolysis. The diagnostic performance of 6PD and MRS is similar, with advantages of shorter imaging processing time and larger studied area with the 6PD.

Resumo Objetivo: Estabelecer, de modo sistemático, se existe associação entre polimorfismos e a necrose avascular em pacientes com doença falciforme. Fontes de dados: A revisão, conduzida segundo as diretrizes Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) e registrada no International Prospective Register of Systematic Reviews (PROSPERO), foi baseada na busca de estudos nas bases de dados PubMed, Scientific Electronic Library Online (SciELO), Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS), Biblioteca Virtual em Saúde (BVS) e na literatura cinza (Google Scholar e Open Gray) até junho de 2020. A análise da qualidade dos artigos foi baseada nos critérios do Strengthening the Reporting of Observational Studies in Epidemiology (STROBE). Síntese dos dados: Dez artigos foram selecionados nas bases de dados e dois incluídos por meio da busca manual, totalizando 12 estudos elencados. As amostras resultaram em 2.362 pacientes incluídos. Com base na iniciativa STROBE, sete estudos atenderam total e/ou parcialmente mais de 70% dos itens essenciais e dois atingiram menos que 60% deles, com variação geral de 86,4-54,5%. Os resultados mostram que os polimorfismos nos genes da proteína morfogenética óssea 6 (BMP6), da Klotho (KL) e da Anexina A2 (ANXA2) podem ter associação com osteonecrose no contexto da doença falciforme. Seis artigos estudaram o polimorfismo no gene da enzima MTHFR, mas apenas um obteve associação positiva. Os polimorfismos associados ao receptor DARC, ao gene ITGA4, ao CD36 e aos genes de proteínas trombofílicas não demonstraram associação em nenhum dos estudos. Conclusões: Os polimorfismos nos genes BMP6, KL e ANXA2 estão possivelmente associados com a necrose avascular em indivíduos com doença falciforme. Entretanto, para a confirmação dessas alterações genéticas como fatores de risco, é necessário que mais estudos com maior poder estatístico e com maior rigor metodológico sejam realizados.

ABSTRACT BACKGROUND AND OBJECTIVES: Chronic pain causes functional and social disability, resulting in emotional impact. This study's objective was to describe the main impairments of activities of daily living, anxious and depressive symptoms, and quality of life in patients with chronic pain. METHODS: Cross-sectional study with chronic pain outpatients. Analysis of clinical and sociodemographic variables, as well as activities of daily living. Assessment of pain, anxious and depressive symptoms by the Hospital Anxiety and Depression Scale, quality of life by the Medical Outcomes Study 36 - Item Short Form Health Survey questionnaire and data analysis by the SPSS statistical software. RESULTS: The study observed limitations in work, movement, leisure and home activities, quality of life below the median and worse for the physical and emotional domains. Sleep was very impaired, followed by partial difficulty with appetite and sexual activity. Although they had anxious and depressive symptoms, most participants were moderately satisfied with their treatment. CONCLUSION: Chronic pain has a very significant impact on quality of life, impairing and limiting daily activities more intensely in individuals with anxious and depressive symptoms.


RESUMO JUSTIFICATIVA E OBJETIVOS: Dor crônica acarreta incapacidade funcional e social, impactando emocionalmente os indivíduos. O objetivo deste estudo foi descrever os principais comprometimentos das atividades de vida diária, sintomas ansiosos e depressivos e qualidade de vida em pacientes com dor crônica. MÉTODOS: Estudo transversal com pacientes de ambulatório de dor crônica. Análise das variáveis clínicas e sociodemográficas, assim como das atividades de vida diária. Avaliação de dor, de sintomas ansiosos e depressivos pela Hospital Anxiety and Depression Scale, de qualidade de vida pelo questionário Medical Outcomes Study 36 - Item Short Form Health Survey e a análise dos dados no programa estatístico SPSS. RESULTADOS: Foi evidenciada limitação para o trabalho, movimento, lazer e atividade domiciliar, qualidade de vida abaixo da mediana e pior para os domínios aspectos físicos e emocionais. O sono foi muito comprometido, seguido de dificuldade parcial para apetite e atividade sexual. Embora apresentassem sintomas ansiosos e depressivos, a maioria dos participantes estavam moderadamente satisfeitos com o tratamento. CONCLUSÃO: A dor crônica impacta de forma muito significativa na qualidade de vida, comprometendo e limitando as atividades diárias de modo mais intenso nos portadores de sintomas ansiosos e depressivos.

BACKGROUND: The objective of this study was to determine whether individuals with hip osteoarthritis who undergo THA have higher HRQOL scores than those awaiting the procedure; An additional aim was to investigate associated factors exerting an impact on HRQOL. MATERIALS AND METHODS: A cross-sectional study was conducted. Participants were divided into two groups: pre-THA and post-THA. Functional capacity was assessed using the Harris Hip Score, and HRQOL was measured using a validated and adapted version of the WOMAC questionnaire. A multivariate analysis of the WOMAC results was used to identify the main variables associated with HRQOL in both groups. RESULTS: The post-THA group had higher HRQOL scores (100.4 ± 88.5) than the pre-THA group (197.8 ± 54.1). The domains Pain (68.0 ± 19.0 versus 34.7 ± 30.8), Stiffness (61.2 ± 28.9 versus 29.9 ± 33.1), and Physical Activity (68.6 ± 16.1 versus 36.6 ± 30.2) also had higher scores in the post-THA group. Pain was the variable most frequently associated with the increase in post-THA HRQOL scores. Other variables such as occupation, time of diagnosis, joint function, and body weight also had an impact on the HRQOL of both groups. CONCLUSIONS: 1. Individuals who underwent THA had higher scores for HRQOL compared to those still awaiting the procedure. 2. Pain was the variable most strongly associated with quality of life.

OBJECTIVE: To evaluate the association between bone changes due to vaso-occlusive events in sickle cell disease (SCD) revealed by conventional MRI sequences and the fat fraction obtained using a 6-point DIXON technique (FFdix), in an attempt to use quantitative data as a biomarker for bone complications. METHODS: Cross-sectional study, with 48 SCD patients, 26-homozygous (HbSS), and 22-compound heterozygous (HbSC). Forty-eight healthy individuals paired by age, weight, and sex with SCD patients. All participants underwent lumbar spine and pelvis MRI. Conventional sequences: bone complications related to vaso-occlusive events-femoral head avascular necrosis, bone infarctions, "H"-shaped vertebrae, bone marrow necrosis. Six-point DIXON technique: quantitative evaluation of the bone marrow at pre-established sites (lumbar vertebrae, sacrum, iliacs, femoral heads, greater femoral trochanters, femoral necks). Pearson's correlation, ROC curve, and binary logistic regression analysis were performed. RESULTS: The most frequent findings in the SCD group included femoral head avascular necrosis (75%), bone infarctions (58.3%), "H"-shaped vertebrae (58.3%), and typical imaging findings of bone marrow necrosis (8.3%). Cortical bone thickness in the proximal femoral diaphysis in patients with SCD was moderately negatively correlated with FFdix in lumbar vertebrae, iliacs, femoral necks, and first sacral vertebrae. The ROC curves and odds ratios demonstrated excellent performance of FFdix in all the evaluated anatomical sites and identified patients having bone complications. CONCLUSIONS: FFdix could serve as a potential biomarker in SCD because of its association with bone complications secondary to vaso-occlusive events in patients with SCD, especially in femoral heads, femoral necks, and iliacs.

PURPOSE: The aim of this study is to present a case series of patients with congenital Zika syndrome (CZS), giving special attention to the more relevant aspects for the paediatric orthopaedic surgeon. METHODS: An analytical cross-sectional study was performed in a group of 47 patients affected by CZS with maternal Zika serology confirmation. Clinical features of the maternal Zika infection, prenatal and perinatal complications, and patient demographic data were recorded. In addition, clinical and radiographic evaluations of the lower limb (hip, knee, and foot) were performed. RESULTS: Maternal Zika infection occurred at an average of 12.5 (± 6.4) weeks of gestation. The rates of complications during pregnancy and delivery were 37% and 17.4%, respectively. At the time of the clinical evaluation, spasticity was appreciated in 77.3% of the cases, and arthrogryposis was present in 15.2%. Foot deformities were appreciated in 41.2% of the cases, hip alterations in aggregate in 29.8% (adduction contracture in 20.2%; displacement in 7.5%), and knee alterations in 12.8% (displacement in 6.4%). The deformities in arthrogrypotic patients were both more prevalent and severe. All arthrogrypotic patients were simultaneously spastic. CONCLUSIONS: The current study demonstrates a high prevalence of musculoskeletal alterations in patients with CZS. Affected patients were able to be classified as belonging to one of three types at the time of evaluation: Type I patients presented with a low incidence of musculoskeletal complications; Type II demonstrated progressive deformities; Type III presented with simultaneously the most prevalent and severe deformities. This classification system could help guide medical and surgical management of CZS patients based on the frequency and severity of the deformities at the time of initial evaluation.

Abstract Mucopolysaccharidoses (MPS) are inborn errors of metabolism caused by deficient lysosomal enzymes, leading to organomegaly, hip osteonecrosis, coarse facial features, bone deformities, joint stiffness, cardiac and pulmonary symptoms (MPS VI) or hypermobility (MPS IVA). Some patients may present with non-classical forms of the disease in which osteoarticular abnormalities are the initial symptoms of non-classical forms. As orthopedists and surgeons are the specialists most frequently consulted before the diagnosis, it is critical that MPS may be considered as a differential diagnosis for patients with bone dysplasia. Experts in Latin America reviewed medical records focusing on disease onset, first symptoms and the follow-up clinical and surgical outcomes of non-classical MPS VI and IVA patients. All patients displayed orthopedic issues, which worsened over time, followed by cardiac and ophthalmological abnormalities. Our findings enlighten the necessity of including non-classical MPS as possible diagnosis for patients who report osteoarticular abnormalities in absence of inflammation.

Objective To compare the imaging findings of anatomical alterations using multi-slice computed tomographic arthrography in the evaluation of rotator cuff tears in the shoulder, correlating them with the arthroscopy (the gold standard diagnostic test) findings. Materials and Methods A longitudinal, prospective, comparative study of diagnostic accuracy performed in the period between June 2016 and June 2017 in patients of both sexes, aged between 40 and 70 years, with shoulder rotator cuff tendon tears and therapeutic need to undergo shoulder arthroscopy. Patients with contraindication to magnetic resonance imaging were included. After multi-slice computed tomographic arthrography, all patients underwent arthroscopy. Results To obtain the results, the following parameters were determined: sensitivity, specificity, accuracy, positive predictive value, negative predictive value, and Kappa coefficient, and contrast between the imaging method and arthroscopy. Abstract Objective To compare the imaging findings of anatomical alterations using multi-slice computed tomographic arthrography in the evaluation of rotator cuff tears in the shoulder, correlating them with the arthroscopy (the gold standard diagnostic test) findings. Materials and Methods A longitudinal, prospective, comparative study of diagnostic accuracy performed in the period between June 2016 and June 2017 in patients of both sexes, aged between 40 and 70 years, with shoulder rotator cuff tendon tears and therapeutic need to undergo shoulder arthroscopy. Patients with contraindication to magnetic resonance imaging were included. After multi-slice computed tomographic arthrography, all patients underwent arthroscopy. Results To obtain the results, the following parameters were determined: sensitivity, specificity, accuracy, positive predictive value, negative predictive value, and Kappa coefficient, and contrast between the imaging method and arthroscopy. Conclusion In the impossibility of performing magnetic resonance imaging (the gold standard imaging technique), multi-slice computed tomographic arthrography is an imaging examination capable of evaluating/diagnosing rotator cuff tears.


Resumo Objetivo Comparar os achados por imagem das alterações anatômicas da artrotomografia computadorizada multi-slice na avaliação das lesões do manguito rotador do ombro e correlacioná-los com os achados da artroscopia (exame diagnóstico padrão-ouro). Materiais e Métodos Estudo longitudinal, prospectivo, comparativo de acurácia diagnóstica, feito de junho de 2016 a junho de 2017, em pacientes de ambos os sexos, com idades entre 40 e 70 anos, com lesão dos tendões do manguito rotador do ombro, e que tinham necessidade terapêutica de fazer artroscopia do ombro. Foram incluídos pacientes com contraindicação à realização de ressonância magnética. Após a artrotomografia computadorizada multi-slice, todos os pacientes foram submetidos a artroscopia. Resultados Para a obtenção dos resultados, os seguintes parâmetros foram considerados: sensibilidade, especificidade, acurácia, valor preditivo positivo, valor preditivo negativo, coeficiente Kappa, e contraposição do método de imagem com a artroscopia. Conclusão Na impossibilidade da realização da ressonância magnética (exame de imagem padrão-ouro), a artrotomografia computadorizada multi-slice se mostra um exame de imagem capaz de avaliar/diagnosticar as lesões do manguito rotador.

Mucopolysaccharidosis (MPS) is a rare and neglected disorder. Only a few studies to date have focused on humanistic impacts of the disease, particularly health-related quality of life (HRQOL). The objective of our research is to (1) evaluate HRQOL in Brazilian patients with MPS and (2) assess the validity of the PedsQL 4.0 Generic Core Scales (PedsQL) in this specific disease. We performed an analytical cross-sectional study using the Brazilian Portuguese version of the PedsQL in 22 patients with MPS between the ages of 8 and 21. With regard to assessing the validity of the PedsQL for MPS, we evaluated internal consistency using Cronbach's α coefficient and reliability using the Spearman-Brown estimate of agreement. The mean HRQOL score in our sample was 63.6 points. The worst score was obtained in the Psychosocial domain (61.9) on account of interference with school (56.1), while the Physical Health domain had the highest score (67.6). The total PedsQL internal consistency was .764 points. The Physical Health domain obtained the highest internal consistency (.914), whereas the Psychosocial Health domain obtained the lowest one (.754). MPS was demonstrated to decrease HRQOL, and PedsQL seems to be a valid instrument to perform this kind of analysis.

OBJECTIVE: To verify the impact of avascular necrosis of the femoral head on the quality of life of children and adolescents with Perthes disease and sickle cell disease. METHODS: A comparative study including 24 children between eight and 18 years old with avascular necrosis of the femoral head secondary to Perthes disease and sickle cell disease (Group with Necrosis) and 24 children considered asymptomatic (Group without Necrosis). Clinical and sociodemographic data were collected and the PedsQL 4.0 and the Charnley score for hip dysfunction were applied. RESULTS: There was no difference in the overall score and in any domain when comparing the Perthes group (global = 73.1) and the sickle cell disease group (global = 65.9). When comparing the groups with necrosis and without necrosis, the Perthes group had a lower overall score only for the Physical Functioning domain (87.5 versus 68.5); sickle cell disease group had a lower overall score (64.9 versus 79.4) and in the Physical Functioning (68.5 versus 87.5) and School Functioning (62.9 versus 73.7) domains. CONCLUSION: Avascular necrosis of the femoral head produces lower quality of life scores both in the global evaluation and in the domains Physical Functioning and School Functioning. Necrosis, bilateral lesion, and hip function were found to be associated with the loss of quality of life. Level of Evidence III, Sectional comparative study.

MÉTODOS: Estudo comparativo com 24 crianças entre oito e 18 anos com necrose avascular da cabeça do fémur secundária à doença de Perthes e à anemia falciforme (grupo com necrose) e 24 crianças consideradas assintomáticas (grupo sem necrose). Foram coletados dados clínicos e sociodemográficos e aplicado o instrumento PedsQL 4.0 e o escore de Charnley para disfunção do quadril. RESULTADOS: Não houve diferença no escore global e em nenhum domínio comparando os grupos Perthes (global = 73,1) e anemia falciforme (global = 65,9). Quando comparados os grupos sem necrose e com necrose, nota-se que o grupo Perthes tem escore inferior apenas para o domínio Capacidade Física (87,5 versus 68,5); já o grupo anemia falciforme tem escore global inferior (64,9 versus 79,4) e também nos domínios Capacidade Física (68,5 versus 87,5) e Aspecto Escolar (62,9 versus 73,7). CONCLUSÃO: A necrose avascular da cabeça do fémur produz escores de qualidade de vida inferiores tanto na avaliação global, como nos domínios Capacidade Física e Aspecto Escolar. METHODS: A retrospective observational study was performed, including 121 patients suffering from open fracture of the appendicular skeleton treated at an emergency hospital. The fractures were classified according to Gustilo and Tscherne systems during the initial treatment, and ratings were subsequently confirmed or rectified during hospitalization. Sensitivity, specificity, positive and negative predictive values, and accuracy were calculated according to each classification adopted. RESULTS: The results of this study demonstrated that both classifications of Gustilo and Tscherne are associated with the clinical outcome of infection in open fractures. The Gustilo classification achieved sensitivity of 76.7%, specificity of 53.8%, and accuracy of 59.5%. Tscherne's classification had a sensitivity of 56.7%, specificity 82.4%, and accuracy of 76.1%. CONCLUSION: The Tscherne system showed better accuracy, including specificity as a predictor of infection in open fractures, when compared with the Gustilo system.

MÉTODOS: Foi feito um estudo observacional retrospectivo com 121 indivíduos acometidos por fratura exposta do esqueleto apendicular atendidos em uma unidade de emergência hospitalar. As fraturas expostas foram classificadas segundo os dois sistemas durante o atendimento inicial; as classificações eram posteriormente confirmadas ou retificadas durante o internamento. Foram calculados sensibilidade, especificidade, valores preditivos positivos e negativos e acurácia, segundo cada classificação adotada. Purpose: To analyze the location of the femoral tunnel by three-dimensional computed tomography (3D CT) of the lateral condyle in patients who underwent arthroscopic single-bundle anterior cruciate ligament (ACL) reconstruction, performed using a modified transtibial technique, and to compare the results with data from the literature. Materials and Methods: Seventeen patients with ACL lesions underwent modified transtibial ACL reconstruction. Postoperatively, 3D CT examinations were performed and the images were analyzed by the quadrant system described by Bernard and Hertel to define the femoral tunnel coordinates. Results: The mean value of femoral tunnel location coordinates was 37.1±5.9 in the horizontal plane and 23.9±7.5 in the vertical plane. Compared with other studies using the transtibial or transportal technique, femoral positioning was improved in the vertical plane. The outside-in technique provided the best results both in the vertical and horizontal planes. Conclusions: The modified transtibial technique was not effective for systematically anatomic femoral tunnel positioning; however, it was relatively better than the conventional transtibial technique.

ABSTRACT Objective The aim of this study is to verify the prevalence of avascular necrosis (AVN) in pediatric patients with sickle cell anemia and hip dysfunction, and to evaluate the presence of associated risk factors. Method A cross-sectional study was conducted in a group of 92 patients with sickle cell disease and hip dysfunction. Clinical and sociodemographic characteristics were collected, and laboratory variables were evaluated. All the subjects underwent radiographic and clinical evaluation of the hip. The participants were divided into two groups: the "AVN Group" consisting of patients with AVN, and the "Comparison Group" without AVN. Both groups were evaluated in search of factors associated with osteonecrosis of the femoral head. Results 43 (46.7 %) out of 92 individuals presented hip dysfunction, and 13 were diagnosed with AVN (30.2 %). Comparison between groups showed significant differences in time of diagnosis, previous trauma, presence of pain, and mean values of functional scores. Higher percentage rates of fetal hemoglobin, higher platelet counts and lower rates of total hemoglobin were perceived in the Comparison Group. Conclusions Pediatric patients with sickle cell anemia with hip dysfunction present a prevalence of 39.4 % of osteonecrosis of the femoral head. This was associated with a longer time of diagnosis (97 months), previous trauma in 92 % of patients, and a mean Charnley score of 15 points. Also, an association with lower rate of fetal hemoglobin (7.2 versus 11.8) was found, which supports the hypothesis that fetal hemoglobin may function as a protective factor against avascular necrosis.(AU)


RESUMEN Objetivo El objetivo de este estudio es confirmar la prevalencia de necrosis avas-cular (AVN) en pacientes con anemia de células falciformes, así como disfuncio-nes de cadera en la infancia, además de evaluar la presencia de factores de ries-go en estos individuos. Métodos Se realizó un estudio de corte transversal en un grupo de 92 pacientes con enfermedad falciforme y disfunción de cadera. Se hizo una recolección de caracte-rísticas clínicas y sociodemográficas, y se realizó un análisis de las variables encon-tradas en laboratorio. Todos los sujetos se sometieron a evaluación clínica y radio-gráfica de la cadera, y fueron divididos en dos grupos: el "Grupo AVN" que consistía de pacientes con AVN y el "Grupo Comparativo" sin AVN. Se evaluó ambos grupos con el fin de encontrar factores asociados con la osteonecrosis de cabeza femoral. Resultados 43 (46.7 %) de los 92 individuos presentaron disfunción en la cadera y 13 de ellos recibieron un diagnóstico de AVN (30.2 %). La comparación entre los dos grupos mostró diferencias importantes en el tiempo de diagnóstico, trauma previo, presencia de dolor, y media de valores en los puntajes funcionales. El Grupo Com-parativo evidenció valores porcentuales más altos de hemoglobina fetal, así como un conteo de plaquetas más alto y valores menores de hemoglobina total. Conclusiones Entre los pacientes pediátricos con anemia falciforme y con disfun-ción de cadera se presenta una prevalencia de 39.4 % de osteonecrosis de cabeza femoral, lo que está asociado a un tiempo mayor de diagnóstico (97 meses), trau-ma previo en 92 % de los pacientes, y valores de puntaje medio de Chanrley de 15 puntos. También se encontró una asociación con valores menores de hemog-lobina fetal (7.2 versus 11.8), lo cual respalda la hipótesis de que la hemoglobina fetal puede funcionar como un factor de protección contra la necrosis avascular.(AU)
