RESUMO

Finite-temperature phases of many-body quantum systems are fundamental to phenomena ranging from condensed-matter physics to cosmology, yet they are generally difficult to simulate. Using an ion trap quantum computer and protocols motivated by the quantum approximate optimization algorithm (QAOA), we generate nontrivial thermal quantum states of the transverse-field Ising model (TFIM) by preparing thermofield double states at a variety of temperatures. We also prepare the critical state of the TFIM at zero temperature using quantum-classical hybrid optimization. The entanglement structure of thermofield double and critical states plays a key role in the study of black holes, and our work simulates such nontrivial structures on a quantum computer. Moreover, we find that the variational quantum circuits exhibit noise thresholds above which the lowest-depth QAOA circuits provide the best results.

Computational quantum technologies are entering a new phase in which noisy intermediate-scale quantum computers are available, but are still too small to benefit from active error correction. Even with a finite coherence budget to invest in quantum information processing, noisy devices with about 50 qubits are expected to experimentally demonstrate quantum supremacy in the next few years. Defined in terms of artificial tasks, current proposals for quantum supremacy, even if successful, will not help to provide solutions to practical problems. Instead, we believe that future users of quantum computers are interested in actual applications and that noisy quantum devices may still provide value by approximately solving hard combinatorial problems via hybrid classical-quantum algorithms. To lower bound the size of quantum computers with practical utility, we perform realistic simulations of the Quantum Approximate Optimization Algorithm and conclude that quantum speedup will not be attainable, at least for a representative combinatorial problem, until several hundreds of qubits are available.

BACKGROUND: Although Th2 cells are well known to play important roles in allergic diseases including allergic rhinitis (AR), the factors that induce and sustain the pathogenesis of AR remain unclear. The recent development of sublingual immunotherapy (SLIT) is expected to allow changes to the underlying pathogenesis of AR. However, which Th2 cell subsets are important in house dust mite-induced AR (HDM-AR), the influence of SLIT on the pathogenic Th2 cells, and the association of Th2 cell subsets with SLIT efficacy have not been clarified. METHODS: The cytokine production and frequency of HDM-reactive T-cell subsets in peripheral blood mononuclear cells (PBMCs) were evaluated using flow cytometry in 89 HDM-AR patients (placebo [n = 43] and HDM 300 IR [n = 46]) who participated in a placebo-controlled study of SLIT with HDM tablets. All patients provided samples both before treatment as a baseline and at the end of the 52-week study. The PBMCs were stained with CellTrace™ Violet (CTV) before culture with HDM extract, and HDM-reactive T cells were detected as the proliferated cells with diminished CTV. RESULTS: HDM-reactive IL-5+ IL-13+ CD27- CD161+ CD4+ cells and ST2+ CD45RO+ CD4+ cells were observed in the peripheral blood from each patient with HDM-AR; these cells significantly decreased after SLIT in the group treated with active tablets. HDM-reactive ST2+ CD45RO+ CD4+ cells were significantly lower in active-responders. CONCLUSION: Allergen-reactive ST2+ CD45RO+ CD4+ cells or those combined with IL-5+ IL-13+ CD27- CD161+ CD4+ cells may be useful as markers indicating the successful treatment of SLIT. These cells may play a crucial role in the pathogenesis of AR as pathogenic memory Th2 cells.

BACKGROUND AND PURPOSE: This double-blind, randomized, placebo-controlled study investigated the beneficial effects of repetitive transcranial magnetic stimulation (rTMS) to patients with motor paresis in acute subcortical stroke on functional recovery and electrophysiological measures. METHODS: Twenty patients with acute stroke were randomized into real rTMS (n = 10) or sham (n = 10) groups. Patients received five daily sessions of rTMS with 1200 pulses at 1 Hz for 20 min or sham stimulation over the contralesional motor cortex. Movement-related cortical potential MRCP, consisting of the Bereitschaftpotential, negative slope (NS') and motor potential (MP), was recorded during self-paced wrist extension of the affected limb associated with assessment of the Fugl-Meyer assessment (FMA) of the upper extremity, the pegboard test and the grip strength before and after the rTMS session. RESULTS: Real rTMS improved the FMA and pegboard test scores compared to the sham group in the affected hand. This improvement was associated with increases in the MP and NS' over the front-central sites in the ipsilesional hemisphere, whereas the sham group did not show significant changes in MRCP components by rTMS. CONCLUSIONS: Our findings suggest that low-frequency rTMS to the contralesional motor cortex facilitates functional recovery of paretic limbs in acute stroke patients through enhancing the the neuronal activity of ipsilesional motor and pre-motor areas.

The aim of this study was to determine the loading capacity of a trotting Taishuh pony by gait analysis using a motion analysis system. Seven Taishuh Ponies (5 mares and 2 geldings) and their rider were fitted with a marker (70 mm in diameter placed on their chest) and recorded by 2 high-resolution digital DVD cameras (at a sampling frequency of 60 Hz) as they were trotting along a straight course. Each horse performed 7 tests: 1 test with a loaded weight of 70 kg, 5 tests with random loaded weights between 80 kg and 120 kg, and a final test with a loaded weight of 70 kg again. Three-dimensional movements of each marker were analyzed using motion capture system. The time series of the vertical displacements of the marker was subjected to spectrum analysis by the maximum entropy method, and the autocorrelation coefficient was calculated. The first 2 peaks of the autocorrelation were defined as symmetry and regularity of the gait, and the sum of symmetry and regularity was defined as stability. The cross-spectrum analysis (Blackman-Tukey method) also was performed to analyze the time lag and cross-correlation coefficient between the time series of both pony and rider. Among ponies, symmetry in the 120 kg test (0.54) was significantly lower than that in the first 70 kg test (0.75, P < 0.05) and stabilities in the 100 kg (1.17) and 120 kg (1.17) tests were significantly less than that in the first 70 kg (1.46, P < 0.05). Regarding the rider, there were no significant differences in symmetry, regularity, and stability between loaded weights. The time lag between the time series of horse and rider in the 120 kg test (47.6 ms) was significantly greater than that in the first 70 kg (14.3 ms, P < 0.05) test. These results suggests that the maximum permissible load weight of the Taishuh pony trotting at 3.0 m/s over a short distance was less than 100 kg, which is 43% of the BW.

INTRODUCTION: Preeclampsia is characterized as an increase in vascular tone due to the disorder of endothelial cell function. It was found that action of nitric oxide (NO) derived from endothelial cell might be decreasing in preeclamptic women. OBJECTIVES: In the present study, we investigated whether or not the supplementation of L-arginine (LARG, NO synthase substrate) and folic acid (FA) might improve the reduced action of NO seen in preeclampsia. METHODS: Change in brachial artery diameter by hyperemia (%FMD) was measured to evaluate the endothelial function using ultrasound. Twelve of 25 normal pregnant women seen with reduced %FMD (<110) at less than 16weeks of gestation were given the supplementation of FA 0.8mg+LARG1g/day throughout pregnancy. The concentrations of FA+LARG in erythrocyte, and cGMP (a second messenger of NO) in serum were measured. The informed consent was obtained from each patient. This investigation was approved by the Ethics Committee of Nagoya City East and West Medical Center. RESULTS: Reduced % FMD was seen in 25 pregnant women. One of 12 pregnant women in the supplementation group developed mild, late onset preeclampsia, while 8 of 13 pregnant women in no supplement group developed onset preeclampsia (4 cases were severe). Following the supplementation, %FMD had been increasing as well as the concentrations of FA and LARG in erythrocyte, while the concentration of cGMP in serum had not changed. CONCLUSION: The supplementation of FA+LARG might prevent preeclampsia due to improvement of the reduced endothelial function in high risk pregnant women of preeclampsia.

INTRODUCTION: It was found that eclampsia might be caused by vasogenic edema as a result of breaking autoregulation of cerebral blood flow. The finding of PRES (posterior reversible encephalopathy syndrome) was seen on MRI (T2 FLAIR) in eclamptic women. PRES was also seen in women complicated by severe hypertension. Therefore, severity of hypertension might depend on the onset of eclampsia in pregnant women. However, it remains to be unclear with other factors except severity. OBJECTIVES: In the present study, we estimated the pathophysiological mechanisms focusing on changes of blood pressure in preeclamptic women. METHODS: From May to December 2011, 5 eclamptic women were present in Nagoya City West Medical center (795 pregnant women, including 22 preeclamptic women). Four of preeclamptic women developed eclampsia. We recorded changing blood pressure and analyzed them for 4 days (before and after 2 days). RESULTS: Three were severe hypertension (more than 160/110mmHg) before 2days. Just before, all showed wide-oscillation of blood pressure (more than 30/15mmHg). One was severe preeclampsia in 37weeks of gestation. After cesarean section, blood pressure was decreasing (100/50mmHg), due to blood loss. Taking blood transfusion, blood pressure subsequently increased and disease occurred. Double product, which means a working index of heart function was low and was in turn low after it. Furthermore, shock index, which means a blood volume was upset. CONCLUSION: From these results, it was suggested that a wide-difference of blood pressure might induce elampsia in women complicated by preeclampsia.

The element-specific electronic structure of the organic semiconductor aluminum tris-8-hydroxyquinoline (Alq(3)) has been studied using a combination of resonant x-ray emission spectroscopy, x-ray photoelectron spectroscopy, x-ray absorption spectroscopy, and density functional theory (DFT) calculations. Resonant and nonresonant x-ray emission spectroscopy were used to measure directly the carbon, nitrogen and oxygen 2p partial densities of states in Alq(3), and good agreement was found with the results of DFT calculations. Furthermore, resonant x-ray emission at the carbon K-edge is shown to be able to measure the partial density of states associated with individual C sites. Finally, comparison of previous x-ray emission studies and the present data reveal the presence of clear photon-induced damage in the former.

RESUMO

Heartstring is a useful device. However, the device failure at the time of loading the seal into the delivery device is a troublesome issue. To avoid this problem, we invent a new method using 2 tourniquets made of 5 mm-wide woven Teflon tapes and plastic tubes. Using our method, the loading procedure became easier and more reliable.

BACKGROUND AND OBJECTIVES: Sodium bicarbonate is the most physiological alkalinizing agent. The effect of a new bicarbonated Ringer's solution (BRS) containing Mg2+, on metabolic acidosis and serum magnesium abnormality were evaluated and compared with those of acetated Ringer's (ARS), lactated Ringer's (LRS) and Ringer's (RS) solutions in an experimental haemorrhagic shock model with dogs. METHODS: Animals were randomly divided into six groups (n = 6 in each group), a sham-operated group, an operated group without infusion, and 4 operated groups with infusion (BRS, ARS, LRS and RS groups). Each RS was intravenously administered at 60 mL kg(-1) h(-1) for 1.5 h. Arterial blood gases, plasma electrolytes and cardiovascular parameters were analysed. RESULTS: BRS significantly improved blood base excess values, which were decreased by blood-letting, faster and more markedly than did LRS and RS (BRS--6.3 +/- 0.5 mEq L(-1); LRS--9.2 +/- 1.1 mEq L(-1); RS--12.4 +/- 1.0 mEq L(-1) at the end of infusion). The alkalinizing effect of BRS tended to be better than that of ARS but not significantly so. The serum Mg2+ concentration was well-maintained by BRS as compared to other RS (BRS 1.5 +/- 0.0 mgdL(-1); ARS 1.2 +/- 0.0mgdL(-1); LRS 1.1 +/- 0.0mgdL(-1); RS 1.3 +/- 0.1 mgdL(-1), at the end of infusion). CONCLUSIONS: These results suggest that BRS is a suitable perioperative solution for metabolic acidosis and serum electrolyte balance among RS tested.

BACKGROUND AND OBJECTIVE: The effects of bicarbonated Ringer's solution were evaluated and compared with those of acetated Ringer's, lactated Ringer's and Ringer's solutions in partially hepatectomized rabbits. METHOD: Animals were randomly divided into six groups (n = 6 in each group): a sham-operated group, an operated group without infusion, and four operated groups with infusions of each of the four Ringer's solutions. Each Ringer's solution was intravenously administered at 40 mL kg(-1) h(-1) for 1.5 h. Arterial blood gases, plasma magnesium concentrations and cardiovascular parameters were analysed. RESULTS: The partial hepatectomy-induced decrement of base excess was inhibited by bicarbonated Ringer's solution more remarkably than by either lactated or plain solutions (P < 0.01). The alkalinizing effect of bicarbonated Ringer's solution tended to be more marked than that of the acetated solution but not significantly so. Plasma magnesium concentrations were well maintained by bicarbonated solution as compared to the other solutions (P < 0.01). CONCLUSIONS: These results suggest that bicarbonated Ringer's solution is the most suitable perioperative solution for metabolic acidosis and plasma electrolyte balance among the Ringer's solutions tested.

Wilson disease (WD) is an autosomal recessive disorder of copper transport, resulting in copper accumulation and toxicity to the liver and brain. There is no evidence that the WD patient's immune system attacks copper accumulated hepatocytes. Here we describe that the frequency and absolute number of Valpha24+Vbeta11+ natural killer T (NKT) cells were significantly increased in 3 cases of WD, whereas those of CD3+CD161+ NKT cells were within the normal range. Patients no. 1 and 2 had a presymptomatic form of WD. Their tissue specimens showed pathological changes of mild degeneration of hepatocytes with a few infiltrating mononuclear cells and a low degree of fatty change. Patient no. 3 displayed fulminant hepatitis with Coombs-negative haemolytic anaemia. The tissue specimens of patient no. 3 showed macronodular cirrhosis with thick fibrosis, inflammatory infiltrates and spotty necrosis. Human Valpha24+Vbeta11+ NKT cells are almost equal to CD1d-restricted NKT cells. Therefore we investigated CD1d-restricted NKT cells in the LEC rat as an animal model of WD. In LEC rats before hepatitis onset, the number and phenotype of liver NKT cells were normal. At about 4 months of age all LEC rats developed acute hepatitis accompanied by acute jaundice, and CD161high NKT cells developed in their livers. CD161highalphabetaTCRbright NKT cells developed in some of them. Their hepatitis was severe. CD161highalphabetaTCRbright NKT cells expressed an invariant rat Valpha14-Jalpha281 chain, which is CD1d-restricted. Furthermore, liver lymphocytes in the acute jaundiced LEC rats with CD161highalphabetaTCRbright NKT cells had significant and CD1d-specific cytotoxic activity.

RESUMO

A 62-year-old man with supraventricular/ventricular tachycardia associated with myocardial infarction developed tachycardia during beating coronary artery bypass grafting (CABG). Intravenous administration of an ultra short acting beta-blocker, landiolol hydrochloride, controlled heart rate and improved tachyarrhythmia without significant change of blood pressure. Landiolol hydrochloride is effective and useful for the treatment of tachyarrhythmia during beating CABG.

Contemporary mushroom factories are places where there is a substantial risk of the occurrence of respiratory allergy. The aims of this investigation were to estimate its causative agents and to evaluate the contribution of innate immune response in mushroom workers who cultivate Hypsizigus marmoreus (Bunashimeji). Cross-sectional and follow-up studies were performed in the factory. We investigated CD1b, CD3, CD4, CD8, CD14, CD45RO, CD62L and CD161 expression in peripheral blood mononuclear cells (PBMC) by flow cytometry, and serum levels of interleukin (IL-2), IL-4, granulocyte-macrophage colony stimulating factor (GM-CSF), IL-13 and interferon (IFN)-gamma by enzyme-linked immunosorbent assay (ELISA). Co-culture experiments of PBMC with spore extracts were also performed. Percentages of CD1b+ monocytes, natural killer (NK), NK T and CD4+ T cells were increased in the workers compared with controls. Increases in Th2 type cells, Th2/Th1 ratio and serum IL-13 and decreased IFN-gamma were detected, indicating a Th2-biased status of the workers. The follow-up study showed that monocytes and NK cells increased soon after employment while CD4+ T, Th2 and NK T cells increased gradually as employment time lengthened. Serum precipitating antibody to the mushroom antigen could be detected at a later stage. Co-cultivation of PBMC with the spore extracts induced much higher CD1b expression, and suppressed secretion of Th1 cytokine in culture supernatants. These results indicate that the mushroom antigen contains highly immunogenic substances which stimulate PBMC into a Th2-biased in vivo status, and innate immune cells might also play a critical role in developing respiratory allergy in mushroom workers.

As total arterial revascularization in coronary artery bypass grafting (CABG) has been recommended, a sequential bypass technique using arterial grafts has been induced. We evaluate whether a sequential bypass graft can be functioned or not by using a simple simulation model for coronary circulation analogous to the electrical circuit based on Ohm law. The ratio of flow between graft and native coronary artery was determined by the severity of stenosis in the bypassed vessels and the graft diameter. In selection for sequential bypass technique, these factors should be taken into consideration.

Identification of factors influencing success of Helicobacter pylori (HP) eradication is important for clinical practice. We have prospectively conducted an HP eradication study in the Aichi Cancer Center with a total of 142 patients available for analysis. The overall success rate was 61.3% (95% confidence interval 52.7-69.3%). Smoking during the medication for eradication significantly decreased the success rate (42.9%), whereas smoking cessation during the treatment was associated with a similar rate as for non-smokers (66.7%). We also examined links between an eradication outcome and polymorphisms of Le, Se, IL1A, IL1B, IL1RN and MPO genes, but with one exception none showed any association. The non-functional le allele of Le polymorphisms, leading to decreased expression of Le(b) antigen to which HP attaches with adhesin, showed a beneficial effect for success. Although further clarification is necessary, our study indicated that smoking cessation and Le gene polymorphisms may affect the success rate of HP eradication.

OBJECTIVE: The aim of this study was to evaluate the cause and treatment of atrial flutter after coronary artery bypass grafting (CABG). METHODS: Between July 1999 and July 2000, 96 CABGs were performed at the Cardiovascular Center of Aichi Prefectural Owari Hospital. We compared two groups of patients: those with atrial flutter after CABG (group A) and those without atrial flutter (group B). RESULTS: We treated the atrial flutter group with medication, electrical defibrillation, and overdrive pacing. Six of these cases later required radiofrequency catheter ablation (RFCA). These were all with common-type atrial flutter, treated by RFCA for the posterior isthmus, without difficulty. The frequency of direct cannulation to the coronary sinus for retrograde cardioplegia was significantly higher in group A. CONCLUSIONS: Incidence of atrial flutter after CABG was influenced by surgical damage arising from direct cannulation to the coronary sinus for retrograde cardioplegia. Preoperative viability of the myocardium (in addition to good myocardial protection during surgery) also seems to be associated with an increased likelihood of postoperative atrial flutter.

AIMS: The 'metaplastic' polyp of the colorectum, a synonym for the hyperplastic polyp, was named based only on features of the crypt epithelium. It is considered non-neoplastic, but the precise cellular differentiation status remains to be proven. METHODS AND RESULTS: Forty-eight hyperplastic polyps, 12 serrated adenomas, 45 tubular adenomas and five juvenile polyps were studied for their phenotypic expression using gastric (foveolar or pyloric gland cell), small intestinal (goblet cell), and colonic (goblet cell) cellular markers by immunohistochemical and mucin histochemical techniques. Gastric foveolar cell-type differentiation was significantly expressed in hyperplastic polyps, while colonic differentiation was also consistently preserved. Neither gastric pyloric-type nor small intestinal differentiation was observed. The same cell differentiation status as hyperplastic polyps was observed in serrated adenomas but not in tubular adenomas or juvenile polyps. CONCLUSIONS: A large proportion of hyperplastic polyps are composed of hybrid epithelium, with bidirectional differentiation to both gastric foveolar and colonic epithelial cells in the same crypt. Therefore hyperplastic polyps might be interpreted as the outcome of abnormal cell differentiation of stem cells. The same phenotypic expression suggests that hyperplastic polyps and serrated adenomas share the same cell lineage.

Various types of assist devices have been developed for severe heart failures. Among them, intra-aortic balloon pumping (IABP) has achieved popularity, mainly in the treatment of patients with either severe cardiac infarction or low cardiac output syndrome (LOS) after open-heart surgery. However, IABP has a limitation in that although it acts as a pressure support, it cannot directly support flow volume. In cases of severe pump failure, stronger pump support would be required. However, there is a limit in the clinical use of temporary mechanical support using a pump because of the costs of the expensive equipment. With this in mind, we came up with a new idea in which the motion of a balloon in IABP system is converted into a power source for creating a one-way stream. In order to realize our idea, we made a J-shaped sample model of pulsating chamber incorporating two ball valves. In the hydrodynamic experiment using our new device, flows over 2500 ml/min (Max. 3475 ml/min) were obtained. The percent changes in parameters such as TTI, DPTI and bypass flows obtained by the experiment of left ventricular bypass have demonstrated that our assist system is effective for reducing cardiac work.

Although improving energy metabolism in ischemic brain has been accepted for the treatment of cerebrovascular diseases, administration of glucose, as an energy substrate, would aggravate ischemic brain damage via activating anaerobic glycolysis, which leads to lactate accumulation. Beta-hydroxybutyrate (BHB) is one of the ketone bodies that can be utilized as an energy source during starvation. The purpose of our study was to define the protective effects of BHB on brain damage induced by hypoxia, anoxia and ischemia. The isotonic solution of BHB administered 30 min before the induction of ischemia at doses over 50 mg x kg(-1) x h(-1) showed remarkable protective effects against hypoxia and anoxia. BHB administered immediately after a bilateral carotid artery ligation at a dose of 30 mg x kg(-1) x h(-1) significantly suppressed the elevation of cerebral water and sodium contents as well as maintaining high ATP and low lactate levels. In contrast, glycerin, a hypertonic agent, substantially reduced the water content but did not show any significant effect on other parameters. We demonstrated that BHB, unlike glycerin, when used as an energy substrate in ischemic brain, has protective effects on cerebral hypoxia, anoxia and ischemia-induced metabolic change.
