RESUMO

The use of continuous glucose monitors (CGMs) in individuals living without diabetes is increasing. The purpose of this study was to profile various CGM metrics around nutritional intake, sleep and exercise in a large cohort of physically active men and women living without any known metabolic disease diagnosis to better understand the normative glycemic response to these common stimuli. A total of 12,504 physically active adults (age 40 ± 11 years, BMI 23.8 ± 3.6 kg/m2; 23% self-identified as women) wore a real-time CGM (Abbott Libre Sense Sport Glucose Biosensor, Abbott, USA) and used a smartphone application (Supersapiens Inc., Atlanta, GA, USA) to log meals, sleep and exercise activities. A total of >1 M exercise events and 274,344 meal events were analyzed. A majority of participants (85%) presented an overall (24 h) average glucose profile between 90 and 110 mg/dL, with the highest glucose levels associated with meals and exercise and the lowest glucose levels associated with sleep. Men had higher mean 24 h glucose levels than women (24 h-men: 100 ± 11 mg/dL, women: 96 ± 10 mg/dL). During exercise, the % time above >140 mg/dL was 10.3 ± 16.7%, while the % time <70 mg/dL was 11.9 ± 11.6%, with the remaining % within the so-called glycemic tight target range (70-140 mg/dL). Average glycemia was also lower for females during exercise and sleep events (p < 0.001). Overall, we see small differences in glucose trends during activity and sleep in females as compared to males and higher levels of both TAR and TBR when these active individuals are undertaking or competing in endurance exercise training and/or competitive events.

Using a large database of continuous glucose monitoring (CGM) data, this study aimed to gain insights into the association between pre-exercise food ingestion timing and reactive hypoglycemia. A group of 6,761 users self-reported 48,799 pre-exercise food ingestion events and logged minute-by-minute CGM, which was used to detect reactive hypoglycemia (<70 mg/dL) in the first 30 min of exercise. A linear and a non-linear binomial logistic regression model was used to investigate the association between food ingestion timing and the probability of experiencing reactive hypoglycemia. An analysis of variance was conducted to compare the predictive ability of the models. On average, reactive hypoglycemia was detected in 8.34 ± 3.04% of the total events, with <15% of individuals experiencing hypoglycemia in >20% of their events. The majority of the reactive hypoglycemia events were found with pre-exercise food timing between ∼30 and ∼90 min, with a peak at ∼60 min. The superior accuracy (62.05 vs 45.1%) and F-score (0.75 vs 0.59) of the non-linear vs the linear model were statistically superior (P < 0.0001). These results support the notion of an unfavourable 30-to-90 min pre-exercise food ingestion time window which can significantly impact the likelihood of reactive hypoglycemia in some individuals.


Large datasets of self-reported continuous glucose monitoring and food events are used here for the first time to get insights into reactive hypoglycemia, a condition often regarded as negative for endurance performance eventsUsing a binomial non-linear logistic regression model, the association between pre-exercise food ingestion timing and reactive hypoglycemia revealed the presence of an unfavourable window, when reactive hypoglycemia is more likely to occur.Results confirm an individual predisposition to reactive hypoglycemia and, for 8 in 100 individuals, the pre-exercise food ingestion timing can meaningfully impact the likelihood of experiencing reactive hypoglycemia.

BACKGROUND: Physical exercise exerts a positive effect on many chronic conditions, specifically lifestyle-related diseases such as overweight and obesity, type 2 diabetes mellitus (T2DM), cardiovascular conditions and osteoarthritis (OA). As a result of common risk factors, most of these patients present with multiple conditions. Exercise- and disease-related biomarkers, such as adipokines, are emerging tools in training supervision and regulation; however, their significance in subjects with multimorbidities is unknown. SUBJECTS AND METHODS: To address this issue, adipokines leptin, adiponectin and resistin were assessed in a cohort of subjects with multimorbidities (n = 39) presenting with at least two of the abovementioned conditions or relevant risk factors before and after a six-month exercise and lifestyle intervention program ('MultiPill-Exercise'), and correlated with training adaptation, namely changes in relative maximum oxygen uptake (V·O2max). RESULTS: There was a significant negative correlation between baseline leptin concentrations and training effect for relative V·O2max (after three months: rho = -0.54, p = 0.020 *; after six months: rho = -0.45, p = 0.013 *), with baseline leptin explaining 35% of the variance in delta relative V·O2max after three months and 23% after six months. CONCLUSIONS: Leptin might be a suitable surrogate biomarker in the context of exercise-based lifestyle intervention programs in subjects with multimorbidity.

microRNAs (miRs) have been proposed as a promising new class of biomarkers in the context of training adaptation. Using microarray analysis, we studied skeletal muscle miR patterns in sedentary young healthy females (n = 6) before and after a single submaximal bout of endurance exercise ('reference training'). Subsequently, participants were subjected to a structured training program, consisting of six weeks of moderate-intensity continuous endurance training (MICT) and six weeks of high-intensity interval training (HIIT) in randomized order. In vastus lateralis muscle, we found significant downregulation of myomiRs, specifically miR-1, 133a-3p, and -5p, -133b, and -499a-5p. Similarly, exercise-associated miRs-23a-3p, -378a-5p, -128-3p, -21-5p, -107, -27a-3p, -126-3p, and -152-3p were significantly downregulated, whereas miR-23a-5p was upregulated. Furthermore, in an untargeted approach for differential expression in response to acute exercise, we identified n = 35 miRs that were downregulated and n = 20 miRs that were upregulated by factor 4.5 or more. Remarkably, KEGG pathway analysis indicated central involvement of this set of miRs in fatty acid metabolism. To reproduce these data in a larger cohort of all-female subjects (n = 29), qPCR analysis was carried out on n = 15 miRs selected from the microarray, which confirmed their differential expression. Furthermore, the acute response, i.e., the difference between miR concentrations before and after the reference training, was correlated with changes in maximum oxygen uptake (V̇O2max) in response to the training program. Here, we found that miRs-199a-3p and -19b-3p might be suitable acute-response candidates that correlate with individual degrees of training adaptation in females.

In exercise physiology, laboratory components help students connect theoretical concepts to their own exercise experiences and introduce them to data collection, analysis, and interpretation using classic techniques. Most courses include a lab protocol that involves exhaustive incremental exercise during which expired gas volumes and concentrations of oxygen and carbon dioxide are measured. During these protocols, there are characteristic alterations in gas exchange and ventilatory profiles that give rise to two exercise thresholds: the gas exchange threshold (GET) and the respiratory compensation point (RCP). The ability to explain why these thresholds occur and how they are identified is fundamental to learning in exercise physiology and requisite to the understanding of core concepts including exercise intensity, prescription, and performance. Proper identification of GET and RCP requires the assembly of eight data plots. In the past, the burden of time and expertise required to process and prepare data for interpretation has been a source of frustration. In addition, students often express a desire for more opportunities to practice/refine their skills. The objective of this article is to share a blended laboratory model that features the "Exercise Thresholds App," a free online resource that eliminates postprocessing of data and provides a bank of profiles on which end-users can practice threshold identification skills with immediate feedback. In addition to including prelaboratory and postlaboratory recommendations, we present student accounts of understanding, engagement, and satisfaction following completion of the laboratory experience and introduce a new quiz feature of the app to assist instructors with evaluating student learning.NEW & NOTEWORTHY We present a laboratory to study exercise thresholds from gas exchange and ventilatory measures that features the "Exercise Thresholds App," a free online resource that eliminates postprocessing of data and provides a bank of profiles on which end-users can practice threshold identification skills. Background: The use of medicinal plants is common among a significant part of the elderly population. In addition, at present, the use of medicinal plants has been introduced in Primary Health Care centers, and is becoming increasingly popular, especially in this population. Objectives: The present study aimed to determine the main medicinal plants consumed by the elderly attending a primary care unit (Puente Alto, Chile), considering main indications for use, preparation, frequency of use, place of obtaining and the possibility of drug interactions. Methods: Observational, descriptive and cross-sectional study conducted in a primary care unit. The questionnaire on the use of medicinal plants was applied in self-reliant elderly patients. Results: Were interviewed 310 elderly users who attended primary health care centers in Puente Alto (Chile) and met the eligibility criteria. About 83% were women and 17% men. The first 10 most used plants comprise 70% of the total citations and correspond to mint (15.80%), chamomile (7.96%), rue (7.96%), matico (6.98%), plantain (6.85%), boldo (5.99%), lemon balm (5.80%), pennyroyal (5.06%), paico (4.69%), and lemon verbena (2.72%). The reasons for the use of medicinal plants were mainly gastrointestinal, nervous system, dermal, respiratory, metabolic and genitourinary problems. Home-growing was the most cited method and infusion the most popular form of preparation. The main plants mentioned have some possible interaction already described. Conclusions: The results reinforce the importance of knowing the plant species used by the population and the correct orientation on their rational use, potential benefits, adverse effects and interactions.

An acute session of sprint interval training (SIT) is a potent stimulus for the metabolic and cardiovascular systems. However, the feasibility of SIT in older adults and its effectiveness to acutely improve aerobic function by transiently accelerating the rate of adjustment of oxidative phosphorylation quantified by V̇O2 kinetics (τV̇O2) are unknown. This study evaluated the time course of changes of τV̇O2 in response to different doses of SIT in older inactive adults compared to their young counterparts. Eight older (age: 67 ± 3 years) and eight young (age: 30 ± 3 years) adults completed three separate SIT sessions consisting of either one (SIT1), three (SIT3), or five (SIT5) consecutive bouts of SIT. Each SIT intervention was interspersed by a two-week recovery phase. The bike resistance during the sprints was set at 0.065 kg·kg-1 body mass for older and 0.075 kg·kg-1 body mass for young adults. Moderate-intensity step-transitions were performed to assess τV̇O2 before (PRE) and one (1d), two (2d) and three (3d) days post each SIT intervention. Older adults attained lower peak power outputs, average power output, and blood lactate concentrations across all sprints of each SIT intervention compared to young (P < 0.01). Following SIT1, τV̇O2 was faster at 1d (-13.6%; P = 0.008) and 2d (-12.7%; P = 0.017) and returned to values similar to PRE at 3d (+0.4%; P > 0.05) in both older and young. Following SIT3, τV̇O2 was faster at 1d (-20.6%; P < 0.001), 2d (-18.5%; P = 0.011), and 3d (-14.5%; P = 0.045) compared to PRE in both older and young. Following SIT5, τV̇O2 became faster in older (at 1d, 2d, and 3d; ~25%; P < 0.05) but remained unchanged in young with respect to PRE (P > 0.05). These findings indicate that SIT has the potency to acutely improve aerobic function by speeding the rate of adjustment of oxidative phosphorylation. However, only older adults were able to maintain these beneficial effects when the volume of SIT was maximized (SIT5). Future studies are warranted to evaluate the long-term feasibility of SIT in older adults.

Small, non-coding RNAs (microRNAs) have been shown to regulate gene expression in response to exercise in various tissues and organs, thus possibly coordinating their adaptive response. Thus, it is likely that differential microRNA expression might be one of the factors that are responsible for different training responses of different individuals. Consequently, determining microRNA patterns might be a promising approach toward the development of individualized training strategies. However, little is known on (1) microRNA patterns and their regulation by different exercise regimens and (2) possible correlations between these patterns and individual training adaptation. Here, we present microarray data on skeletal muscle microRNA patterns in six young, female subjects before and after six weeks of either moderate-intensity continuous or high-intensity interval training on a bicycle ergometer. Our data show that n = 36 different microRNA species were regulated more than twofold in this cohort (n = 28 upregulated and n = 8 downregulated). In addition, we correlated baseline microRNA patterns with individual changes in VO2 max and identified some specific microRNAs that might be promising candidates for further testing and evaluation in the future, which might eventually lead to the establishment of microRNA marker panels that will allow individual recommendations for specific exercise regimens.

During incremental exercise, two thresholds may be identified from standard gas exchange and ventilatory measurements. The first signifies the onset of blood lactate accumulation (the lactate threshold, LT) and the second the onset of metabolic acidosis (the respiratory compensation point, RCP). The ability to explain why these thresholds occur and how they are identified, non-invasively, from pulmonary gas exchange and ventilatory variables is fundamental to the field of exercise physiology and requisite to the understanding of core concepts including exercise intensity, assessment, prescription, and performance. This review is intended as a unique and comprehensive theoretical and practical resource for instructors, clinicians, researchers, lab technicians, and students at both undergraduate and graduate levels to facilitate the teaching, comprehension, and proper non-invasive identification of exercise thresholds. Specific objectives are to: (1) explain the underlying physiology that produces the LT and RCP; (2) introduce the classic non-invasive measurements by which these thresholds are identified by connecting variable profiles to underlying physiological behaviour; (3) discuss common issues that can obscure threshold detection and strategies to identify and mitigate these challenges; and (4) introduce an online resource to facilitate learning and standard practices. Specific examples of exercise gas exchange and ventilatory data are provided throughout to illustrate these concepts and a novel online application tool designed specifically to identify the estimated LT (θLT) and RCP is introduced. This application is a unique platform for learners to practice skills on real exercise data and for anyone to analyze incremental exercise data for the purpose of identifying θLT and RCP.

Affect experienced during an exercise session is supposed to predict future exercise behavior. However, empirical evidence reveals high variability in affective response to different exercise modalities. Thus, the purpose of the present study was to compare acute affective response and its variation during three different endurance exercise modalities: (a) moderate-intensity continuous exercise (MICE), (b) vigorous-intensity continuous exercise (VICE), and (c) high-intensity interval exercise (HIIE). Using the dual-mode theory as a theoretical framework, cognitive and interoceptive factors were considered as potential predictors of in-task affective response. In a within-subject design, 40 insufficiently active healthy participants (aged from 20 to 40 years) attended three sessions per exercise modality on a cycle ergometer. Affective valence (measured by the Feeling Scale), two cognitive factors (perceived competence and awareness of interoceptive cues), and one interoceptive factor (heart rate) were assessed before, during, and after each exercise session. Mixed models with three levels (subject, exercise session, and time point) revealed more positive affective valence during MICE compared with VICE (p < 0.001) and HIIE (p < 0.01), while there was no significant difference between the latter two. Levene's test results showed the highest variability of in-task affective valence during VICE (ps < 0.01). Regarding the course across the session, MICE was associated with a constant slight increase in affective valence from pre- to post-exercise (p < 0.05), whereas VICE and HIIE caused a decline in pleasure, followed by an affective rebound immediately after exercise termination (ps < 0.01). The highest importance of cognitive and interoceptive factors for in-task affective valence was observed in VICE (ps < 0.05). The current findings provide support for the tenets of the dual-mode theory, however, indicating that there may be differences in the affect-intensity relationship between continuous and interval exercise. In conclusion, the study results concerning previously insufficiently active individuals extend the knowledge of how exercise can positively shape affective well-being depending on exercise modality and psychophysiological influences. This knowledge enables public health practitioners to design more individualized activity recommendations, thereby improving the subjective experience of exercise.

Regular exercise fosters fitness-enhancing benefits. We assume that exercise interventions become successful and sustainable if physiological benefits of exercise are also subjectively perceivable. The goal of this study was to examine how young inactive adults physiologically respond to an exercise intervention and how those responses are subjectively experienced. Furthermore, we aimed to assess whether the sequence of two distinct endurance-based exercise modes has an impact on physiological and subjectively experienced physical fitness. Thirty-one young inactive adults were assessed for this substudy of the larger iReAct study. Participants were randomly assigned to a high-intensity interval training (HIIT) or a moderate-intensity continuous training (MICT) group for 6 weeks and subsequently switched groups for a second training period. Physiological fitness data was collected at baseline, follow-up I, and follow-up II using a graded cardiopulmonary exercise test. Subjectively reconstructed (i.e., retrospective constructions) experiences relating to physical fitness were assessed at follow-up II using a biographical mapping method. A repeated-measures one-way ANOVA on each training group was performed to see whether physiological and subjectively experienced fitness differed across training periods. The rate of change between all variables was calculated for the first and the second training period in order to compare the agreement between physiological and subjective fitness improvements. Participants increased their fitness across the intervention period both physiologically and subjectively. However, the rate of change depended on the sequence of the two training modes. While VO2max increased significantly in both training periods in the MICT-HIIT sequence, a significant increase in VO2max in the HIIT-MICT sequence was only observed in the HIIT period. Participants similarly perceived those increases subjectively in their exercise-related physical fitness, although they experienced a significant decrease in the second period of the HIIT-MICT sequence. For subjectively perceived physical fitness relating to everyday activities, significant increases were only observed for the first period of the MICT-HIIT sequence. Young inactive adults can improve both their physiological and their subjectively perceived fitness through regular exercise. However, exercise modes and their sequence can make a substantial difference regarding measured and perceived physical fitness. Additionally, despite a favorable tendency toward HIIT over MICT, inter- and intra-individual variability, particularly in the subjective experiences of fitness, reiterates the necessity of individualized approaches to exercise.

The evaluation of the maximal oxygen uptake ( V˙O2max ) following exercise training is the classical assessment of training effectiveness. Research has lacked in investigating whether individuals that do not respond to the training intervention ( V˙O2max ), also do not improve in other health-related parameters. We aimed to investigate the cardiovascular and metabolic adaptations (i.e., performance, body composition, blood pressure, vascular function, fasting blood markers, and resting cardiac function and morphology) to exercise training among participants who showed different levels of V˙O2max responsiveness. Healthy sedentary participants engaged in a 6-week exercise training program, three times a week. Our results showed that responders had a greater increase in peak power output, second lactate threshold, and microvascular responsiveness, whereas non-responders had a greater increase in cycling efficiency. No statistical differences were observed in body composition, blood pressure, fasting blood parameters, and resting cardiac adaptations. In conclusion, our study showed, for the first time, that in addition to the differences in the V˙O2max , a greater increase in microvascular responsiveness in responders compared to non-responders was observed. Additionally, responders and non-responders did not show differences in the adaptations on metabolic parameters. There is an increasing need for personalized training prescription, depending on the target clinical outcome.

PURPOSE: We investigated the cardiovascular individual response to 6 weeks (3×/week) of work-matched within the severe-intensity domain (high-intensity interval training, HIIT) or moderate-intensity domain (moderate-intensity continuous training, MICT). In addition, we analyzed the cardiovascular factors at baseline underlying the response variability. METHODS: 42 healthy sedentary participants were randomly assigned to HIIT or MICT. We applied the region of practical equivalence-method for identifying the levels of responders to the maximal oxygen uptake (V̇O2max) response. For investigating the influence of cardiovascular markers, we trained a Bayesian machine learning model on cardiovascular markers. RESULTS: Despite that HIIT and MICT induced significant increases in V̇O2max, HIIT had greater improvements than MICT (p < 0.001). Greater variability was observed in MICT, with approximately 50% classified as "non-responder" and "undecided". 20 "responders", one "undecided" and no "non-responders" were observed in HIIT. The variability in the ∆V̇O2max was associated with initial cardiorespiratory fitness, arterial stiffness, and left-ventricular (LV) mass and LV end-diastolic diameter in HIIT; whereas, microvascular responsiveness and right-ventricular (RV) excursion velocity showed a significant association in MICT. CONCLUSION: Our findings highlight the critical influence of exercise-intensity domains and biological variability on the individual V̇O2max response. The incidence of "non-responders" in MICT was one third of the group; whereas, no "non-responders" were observed in HIIT. The incidence of "responders" was 11 out of 21 participants in MICT, and 20 out of 21 participants in HIIT. The response in HIIT showed associations with baseline fitness, arterial stiffness, and LV-morphology; whereas, it was associated with RV systolic function in MICT.

PURPOSE: We aimed to investigate differences between high-intensity interval exercise (HIIE, including high-intensity interval training and sprint interval training) and moderate-intensity continuous training (MICT) on physical fitness, body composition, blood pressure, blood lipids, insulin and glucose metabolism, inflammation, and endothelial function. METHODS: Differences between HIIE and MICT were summarized using a random-effects meta-analysis on the effect size (Cohen's d). A meta-regression was conducted using the following subgroups: population, age, training duration, men ratio, exercise type, baseline values (clinical relevant ranges), and type of HIIE. Studies were included if at least one of the following outcomes were reported: maximal oxygen uptake (V˙O2max), flow-mediated dilation (FMD), body mass index (BMI), body mass, percent body fat, systolic and diastolic blood pressure, high-density lipoprotein (HDL), low-density lipoprotein (LDL), triglycerides, total cholesterol, C-reactive protein (CRP), fasting glucose and insulin, glycated hemoglobin (HbA1c), and insulin resistance (HOMA-IR). A total of 55 studies were included. RESULTS: Overall, HIIE was superior to MICT in improving V˙O2max (d = 0.40, P < 0.001) and FMD (d = 0.54, P < 0.05). Oppositely, MICT was superior to HIIE in improving HbA1c (d = -0.27, P < 0.05). No differences were observed in BMI (d = -0.02), body mass (d = -0.05), percent body fat (d = 0.04), systolic blood pressure (d = -0.04), diastolic blood pressure (d = 0.03), HDL (d = -0.05), LDL (d = 0.08), triglycerides (d = 0.03), total cholesterol (d = 0.14), CRP (d = -0.11), fasting insulin (d = 0.02), fasting glucose (d = 0.02), and HOMA-IR (d = -0.04). Moderator analyses indicated that the difference between HIIE and MICT was affected by different subgroups. CONCLUSION: Overall, HIIE showed to be more effective in improving cardiovascular health and cardiorespiratory fitness, whereas MICT was superior in improving long-term glucose metabolism. In the process of personalized training counseling, health-enhancing effects of exercise training may be improved by considering the individual risk profiles.

BACKGROUND: Physical activity is a substantial promoter for health and well-being. Yet, while an increasing number of studies shows that the responsiveness to physical activity is highly individual, most studies focus this issue from only one perspective and neglect other contributing aspects. In reference to a biopsychosocial framework, the goal of our study is to examine how physically inactive individuals respond to two distinct standardized endurance trainings on various levels. Based on an assessment of activity- and health-related biographical experiences across the life course, our mixed-method study analyzes the responsiveness to physical activity in the form of a transdisciplinary approach, considering physiological, epigenetic, motivational, affective, and body image-related aspects. METHODS: Participants are randomly assigned to two different training programs (High Intensity Interval Training vs. Moderate Intensity Continuous Training) for six weeks. After this first training period, participants switch training modes according to a two-period sequential-training-intervention (STI) design and train for another six weeks. In order to analyse baseline characteristics as well as acute and adaptive biopsychosocial responses, three extensive mixed-methods diagnostic blocks take place at the beginning (t0) of the study and after the first (t1) and the second (t2) training period resulting in a net follow-up time of 15 weeks. The study is divided into five modules in order to cover a wide array of perspectives. DISCUSSION: The study's transdisciplinary mixed-method design allows to interlace a multitude of subjective and objective data and therefore to draw an integrated picture of the biopsychosocial efficacy of two distinct physical activity programs. The results of our study can be expected to contribute to the development and design of individualised training programs for the promotion of physical activity. TRIAL REGISTRATION: The study was retrospectively registered in the German Clinical Trials Register on 12 June 2019 (DRKS00017446).

Embolic event's exist during hip arthroplasty surgery and specially those who had been caused by hip fracture. Complications depends mainly on the preoperative clinical conditions, and may even be subclinical if the patient was previously healthy. Thus, thanks to the technology progress, especially transesophageal echocardiography (TEE), it has been possible to identify these embolic phenomena at an early stage, allowing to be anticipated and treated an eventual cardiorespiratory compromise. A report was made of 2 patients with hip fracture, who underwent hip arthroplasty surgery (HA), in which through transthoracic echocardiography (TTE), it was possible to visualize embolic events, mainly during the acetabular reaming and the press-fit impact of the Cup. One patient presented hemodynamic impact that was early treated. This emphasizes how critical that period of the surgery is, and that when the anesthesiologist must be especially vigilant. There could be different causes: the energy made by the surgeons to make the acetabular reaming or to insert the cup; or even the destruction of the surrounding tissue during the fracture moment. So, in that way blood thrombus, air, fat or bone can get into the blood vessels and produce eventual complications. Therefore, special attention is required to the hemodynamic changes that may occur in these stages of surgery. Particularly, in these cases, in whom the patient's receive regional anesthesia and were positioned on lateral decubitus, it has to monitored by TTE and not by TEE, which gave more limitations at the moment to get the different echocardiograph Windows, thus it was chosen to use the subxiphoid window to see the inferior vena cava. In both cases, it was visualized embolic's events. But as it's said before, only one presented hemodynamic compromised. Also stands out, the relevance that there is more access to TTE in most of the operating centers of the country, unlike the TEE, which the transducer use for it, is more difficult for the anesthesiologist to get access to it and also requires more training.


Durante las cirugías de prótesis de cadera y especialmente aquellas por fractura de cadera pueden presentar eventos embólicos. Las complicaciones dependen principalmente de la condición clínica preoperatoria, pudiendo incluso ser subclínico si el paciente es sano. Así, gracias al progreso de la tecnología, particularmente la ecocardiografía transesofágica (ETE) y transtoraxica (ETT), se ha logrado identificar precozmente dichos fenómenos embólicos, permitiendo anticipar y tratar oportunamente una eventual descompensación cardiorrespiratoria. Se realizó un reporte de 2 pacientes con fractura de cadera, que se sometieron a una cirugía de artroplastía de cadera (AC), en los cuales mediante la vigilancia de ETT, se logró visualizar estos eventos, principalmente durante la fenestración e impactación del cotilo en el acetábulo, presentando uno de estos pacientes un impacto hemodinámico que se trató precozmente. Las causas de las embolías pueden ser variadas, como la presión ejercida por el cirujano para fenestrar e introducir la prótesis o la destrucción del tejido circundante a la lesión durante el momento de la fractura, entre otros. Así, se pueden producir embolias de trombos, grasa, hueso o incluso aire que pueda entrar al torrente sanguíneo y producir eventualmente complicaciones. Con estos hallazgos, ecográficos y en algunas oportunidades clínicos, se podría demostrar lo crítico que es aquel período de la cirugía, en donde se requiere especial atención a los cambios hemodinámicos que se puedan producir, y en el que el anestesiólogo debe estar especialmente vigilante. Particularmente, en estos casos, en que las pacientes recibieron anestesia regional y se intervino en posición de decúbito lateral, se tuvo que realizar monitoreo mediante ETT y no ETE, lo cual entrego más limitantes a la hora de conseguir buena calidad de ventanas cardiacas y por lo tanto, después de visualizar las distintas ventanas se optó por utilizar la ventana subxifoidea para visualizar la vena Cava inferior y así observar los distintos eventos embólicos circulantes. En ambas pacientes se pudieron apreciar eventos embólicos, pero en sólo una paciente tuvo compromiso cardiorrespiratorio con hipotensión, aumento de la frecuencia cardiaca y desaturación. Junto con la demostración de la posibilidad de visualización de embolías con ETT, se destaca la relevancia de que existe mayor acceso a este tipo de ecografía en gran parte de los pabellones de nuestro país, a diferencia de la ETE, en la que la sonda utilizada es de más difícil acceso por parte de los anestesistas y requiere un mayor entrenamiento.

In chronometric mental rotation tasks, sex differences are widely discussed. Most studies find men to be more skilled in mental rotation than women, which can be explained by the holistic strategy that they use to rotate stimuli. Women are believed to apply a piecemeal strategy. So far, there have been no studies investigating this phenomenon using eye-tacking methods in combination with electroencephalography (EEG) analysis: the present study compared behavioral responses, EEG activity, and eye movements of 15 men and 15 women while solving a three-dimensional chronometric mental rotation test. The behavioral analysis showed neither differences in reaction time nor in the accuracy rate between men and women. The EEG data showed a higher right activation on parietal electrodes for women and the eye-tracking results indicated a longer fixation in a higher number of areas of interest at 0° for women. Men and women are likely to possess different perceptual (visual search) and decision-making mechanisms, but similar mental rotation processes. Furthermore, men presented a longer visual search processing, characterized by the greater saccade latency of 0°-135°. Generally, this study could be considered a pilot study to investigate sex differences in mental rotation tasks while combining eye-tracking and EEG methods.

OBJECTIVES: The maximal lactate steady state (MLSS) represents the highest exercise intensity at which an elevated blood lactate concentration ([Lac]b) is stabilized above resting values. MLSS quantifies the boundary between the heavy-to-very-heavy intensity domains but its determination is not widely performed due to the number of trials required. DESIGN: This study aimed to: (i) develop a mathematical equation capable of predicting MLSS using variables measured during a single ramp-incremental cycling test and (ii) test the accuracy of the optimized mathematical equation. METHODS: The predictive MLSS equation was determined by stepwise backward regression analysis of twelve independent variables measured in sixty individuals who had previously performed ramp-incremental exercise and in whom MLSS was known (MLSSobs). Next, twenty-nine different individuals were prospectively recruited to test the accuracy of the equation. These participants performed ramp-incremental exercise to exhaustion and two-to-three 30-min constant-power output cycling bouts with [Lac]b sampled at regular intervals for determination of MLSSobs. Predicted MLSS (MLSSpred) and MLSSobs in both phases of the study were compared by paired t-test, major-axis regression and Bland-Altman analysis. RESULTS: The predictor variables of MLSS were: respiratory compensation point (Wkg-1), peak oxygen uptake (V˙O2peak) (mlkg-1min-1) and body mass (kg). MLSSpred was highly correlated with MLSSobs (r=0.93; p<0.01). When this equation was tested on the independent group, MLSSpred was not different from MLSSobs (234±43 vs. 234±44W; SEE 4.8W; r=0.99; p<0.01). CONCLUSIONS: These data support the validity of the predictive MLSS equation. We advocate its use as a time-efficient alternative to traditional MLSS testing in cycling.
