RESUMO

The intervertebral disc (IVD) has a complex and multiscale extracellular matrix structure which provides unique mechanical properties to withstand physiological loading. Low back pain has been linked to degeneration of the disc but reparative treatments are not currently available. Characterising the disc's 3D microstructure and its response in a physiologically relevant loading environment is required to improve understanding of degeneration and to develop new reparative treatments. In this study, techniques for imaging the native IVD, measuring internal deformation and mapping volumetric strain were applied to an in situ compressed ex vivo rat lumbar spine segment. Synchrotron X-ray micro-tomography (synchrotron CT) was used to resolve IVD structures at microscale resolution. These image data enabled 3D quantification of collagen bundle orientation and measurement of local displacement in the annulus fibrosus between sequential scans using digital volume correlation (DVC). The volumetric strain mapped from synchrotron CT provided a detailed insight into the micromechanics of native IVD tissue. The DVC findings showed that there was no slipping at lamella boundaries, and local strain patterns were of a similar distribution to the previously reported elastic network with some heterogeneous areas and maximum strain direction aligned with bundle orientation, suggesting bundle stretching and sliding. This method has the potential to bridge the gap between measures of macro-mechanical properties and the local 3D micro-mechanical environment experienced by cells. This is the first evaluation of strain at the micro scale level in the intact IVD and provides a quantitative framework for future IVD degeneration mechanics studies and testing of tissue engineered IVD replacements. STATEMENT OF SIGNIFICANCE: Synchrotron in-line phase contrast X-ray tomography provided the first visualisation of native intact intervertebral disc microstructural deformation in 3D. For two annulus fibrosus volumes of interest, collagen bundle orientation was quantified and local displacement mapped as strain. Direct evidence of microstructural influence on strain patterns could be seen such as no slipping at lamellae boundaries and maximum strain direction aligned with collagen bundle orientation. Although disc elastic structures were not directly observed, the strain patterns had a similar distribution to the previously reported elastic network. This study presents technical advances and is a basis for future X-ray microscopy, structural quantification and digital volume correlation strain analysis of soft tissue.

BACKGROUND: Human skin has the crucial roles of maintaining homeostasis and protecting against the external environment. Skin offers protection against mechanical trauma due to the reversible deformation of its structure; these biomechanical properties are amenable to dynamic testing using noninvasive devices. OBJECTIVES: To characterize the biomechanical properties of young, black African/African-Caribbean and white Northern European skin from different anatomical sites, and to relate underlying skin architecture to biomechanical function. METHODS: Using cutometry and ballistometry, the biomechanical properties of buttock and dorsal forearm skin were determined in black African/African-Caribbean (n = 18) and white Northern European (n = 20) individuals aged 18-30 years. Skin biopsies were obtained from a subset of the volunteers (black African/African-Caribbean, n = 5; white Northern European, n = 6) and processed for histological and immunohistochemical detection of the major elastic fibre components and fibrillar collagens. RESULTS: We have determined that healthy skin from young African and white Northern European individuals has similar biomechanical properties (F3): the skin is resilient (capable of returning to its original position following deformation, R1), exhibits minimal fatigue (R4) and is highly elastic (R2, R5 and R7). At the histological level, skin with these biomechanical properties is imbued with strong interdigitation of the rete ridges at the dermoepidermal junction (DEJ) and candelabra-like arrays of elastic fibres throughout the papillary dermis. Dramatic disruption to this highly organized arrangement of elastic fibres, effacement of the rete ridges and alterations to the alignment of the fibrillar collagens is apparent in the white Northern European forearm and coincides with a marked decline in biomechanical function. CONCLUSIONS: Maintenance of skin architecture - both epidermal morphology and elastic fibre arrangement - is essential for optimal skin biomechanical properties. Disruption to underlying skin architecture, as observed in the young white Northern European forearm, compromises biomechanical function.

PURPOSE: The Achilles tendon (AT) must adapt to meet changes in demands. This study explored AT adaptation by comparing properties within the jump and non-jump legs of jumping athletes. Non-jumping control athletes were included to control limb dominance effects. METHODS: AT properties were assessed in the preferred (jump) and non-preferred (lead) jumping legs of male collegiate-level long and/or high jump (jumpers; n=10) and cross-country (controls; n=10) athletes. Cross-sectional area (CSA), elongation, and force during isometric contractions were used to estimate the morphological, mechanical and material properties of the ATs bilaterally. RESULTS: Jumpers exposed their ATs to more force and stress than controls (all p≤0.03). AT force and stress were also greater in the jump leg of both jumpers and controls than in the lead leg (all p<0.05). Jumpers had 17.8% greater AT stiffness and 24.4% greater Young's modulus in their jump leg compared to lead leg (all p<0.05). There were no jump versus lead leg differences in AT stiffness or Young's modulus within controls (all p>0.05). CONCLUSION: ATs chronically exposed to elevated mechanical loading were found to exhibit greater mechanical (stiffness) and material (Young's modulus) properties.

Skin is a multifunctional organ but, alongside every other organ system, is subject to both intrinsic (chronological) and extrinsic (environmental) aging, resulting in a loss of functional capacity. Cutaneous aging manifests as an observable change in the external appearance of the skin, the major accelerator of the aging process being our interactions with our environment, such as chronic exposure to solar irradiation (UV, IR or visible wavelengths of light). The aim of this contribution, therefore, was to provide a review of the pathological mechanisms which may play roles in the development of extrinsic, mainly photo-, aging and to review how these molecular changes impact on the structure of the organ as a whole, resulting in loss of function. Finally, we will describe the advances which are occurring in imaging techniques which may allow further characterisation of aged skin.

An experiment involving 25 experiment stations in the North Central and Southern regions (NCR-42 and S-288, respectively) was conducted to assess the degree of uniformity of diet mixing among stations and to assess the variability among station laboratories in chemical analysis of mixed diets. A fortified corn-soybean meal diet was mixed at each station using a common diet formula (except for vitamin and trace-mineral additions). The diet was calculated to contain 14% crude protein (CP), 0.65% Ca, 0.50% P, and 125 ppm Zn (based on 100 ppm added Zn). After mixing, samples were collected from the initial 5% of feed discharged from the mixer, after 25, 50, and 75% was discharged, and from the final 5% of discharged feed. The five samples were sent to the University of Kentucky, finely ground, and divided into subsamples. Each set of five subsamples from each station was distributed to three randomly selected stations for analysis of CP, Ca, P, and Zn (i.e., each station analyzed five diet sub-samples from three other stations). In addition, two commercial and two station laboratories analyzed composites of the five subsamples from each of the 25 mixed diets. Based on the laboratories that analyzed all diets, means were 13.5, 0.65, and 0.52%, and 115 ppm for CP, Ca, P, and Zn, respectively. Ranges of 11.8 to 14.6% CP, 0.52 to 0.85% Ca, 0.47 to 0.58% P, and 71 to 182 ppm of Zn were found among the 25 diet mixes. The coefficients of variation among the 25 diet samples for CP, Ca, P, and Zn were 4.3, 9.3, 4.1, and 17.4%, and among the 25 laboratories were 3.6, 12.5, 10.7, and 11.1%, respectively. Overall analyses of the five sub samples were, respectively, CP: 13.4, 13.6, 13.4, 13.5, and 13.4% (P < 0.06); Ca: 0.66, 0.67, 0.67, 0.66, and 0.67%; P: 0.50,0.51,0.51,0.50, and 0.50%; and Zn: 115, 116, 112, 113, and 120 ppm (P < 0.001). Diets were not uniformly mixed at all stations (station x sample No. was P < 0.08 for Ca and P < 0.01 for CP, P, and Zn). Among stations, the range of the five samples, expressed as a percentage of the mean and averaged for CP, Ca, P, and Zn, varied from +/- 1.1% (i.e., 98.9 to 101.0%) to +/- 12.9% (84.6 to 110.4%), with an overall average of +/- 5.2%. Neither type nor volume of mixers was related to mixing uniformity. The results suggest that uniformity of diet mixes varies among experiment stations, that some stations miss their targeted levels of nutrients (especially Zn), and that the variability among experiment station laboratories in analysis of dietary Ca, P, and Zn in mixed diets is quite large.

Patients with a primary family history of colon cancer were recommended to have full colonoscopy for screening. The results of 125 such patients who also were asymptomatic, had no prior history of neoplasms, and had negative fecal occult blood, showed 15 patients (12 percent) with neoplasms. Only 6 (5.2 percent) had neoplasms that were detectable only by colonoscopy (i.e., above 55 cm). These results suggest that colonoscopy may not be necessary to screen patients with a primary history of colon cancer.

Results from the occultation of the sun by Neptune imply a temperature of 750 +/- 150 kelvins in the upper levels of the atmosphere (composed mostly of atomic and molecular hydrogen) and define the distributions of methane, acetylene, and ethane at lower levels. The ultraviolet spectrum of the sunlit atmosphere of Neptune resembles the spectra of the Jupiter, Saturn, and Uranus atmospheres in that it is dominated by the emissions of H Lyman alpha (340 +/- 20 rayleighs) and molecular hydrogen. The extreme ultraviolet emissions in the range from 800 to 1100 angstroms at the four planets visited by Voyager scale approximately as the inverse square of their heliocentric distances. Weak auroral emissions have been tentatively identified on the night side of Neptune. Airglow and occultation observations of Triton's atmosphere show that it is composed mainly of molecular nitrogen, with a trace of methane near the surface. The temperature of Triton's upper atmosphere is 95 +/- 5 kelvins, and the surface pressure is roughly 14 microbars.

Four experiments involving 3- to 4-wk-old, ad libitum-fed weanling pigs (n = 809) were conducted to determine the effects of initial pig weight, co-mingling of litters, numbers of pigs/pen and room temperature on pig performance in a conventionally equipped nursery. Pigs that were largest at birth were largest (P less than .01) at weaning (r = .66) and also at 28 d postweaning (r = .62). Light-weight pigs (3.8 kg) at weaning gained more slowly (P less than .05) for 4 wk postweaning than heavy weight pigs (6.5 kg). Growth curves for the light and heavy groups of pigs remained parallel throughout the 4-wk nursery period, indicating that small pigs were not making compensatory gains. Growth rates of average-weight pigs (5.2 kg) were intermediate to and not different (P greater than .05) from light- and heavy-weight pigs. Penning 8, 16 or 24 pigs together while maintaining constant flooring area and feeder and waterer space/pig did not reduce (P greater than .05) intakes, gains or feed:gain ratios. Co-mingled litters grew comparably (P greater than .05) to pigs reared as littermates. In temperature studies, pigs were reared either in a room maintained at 29 +/- 2.8 C throughout the 28-d trials or at 29 +/- 2.8 C the first week postweaning and 24 +/- 2.8 C thereafter. Reducing the nursery temperature to 24 +/- 2.8 C after 1 wk depressed (P less than .05) intakes, gains and feed efficiencies as compared with the warmer temperature regimen.

Data from solar and stellar occultations of Uranus indicate a temperature of about 750 kelvins in the upper levels of the atmosphere (composed mostly of atomic and molecular hydrogen) and define the distributions of methane and acetylene in the lower levels. The ultraviolet spectrum of the sunlit hemisphere is dominated by emissions from atomic and molecular hydrogen, which are kmown as electroglow emissions. The energy source for these emissions is unknown, but the spectrum implies excitation by low-energy electrons (modeled with a 3-electron-volt Maxwellian energy distribution). The major energy sink for the electrons is dissociation of molecular hydrogen, producing hydrogen atoms at a rate of 10(29) per second. Approximately half the atoms have energies higher than the escape energy. The high temperature of the atmosphere, the small size of Uranus, and the number density of hydrogen atoms in the thermosphere imply an extensive thermal hydrogen corona that reduces the orbital lifetime of ring particles and biases the size distribution toward larger particles. This corona is augmented by the nonthermal hydrogen atoms associated with the electroglow. An aurora near the magnetic pole in the dark hemisphere arises from excitation of molecular hydrogen at the level where its vertical column abundance is about 10(20) per square centimeter with input power comparable to that of the sunlit electroglow (approximately 2x10(11) watts). An initial estimate of the acetylene volume mixing ratio, as judged from measurements of the far ultraviolet albedo, is about 2 x 10(-7) at a vertical column abundance of molecular hydrogen of 10(23) per square centimeter (pressure, approximately 0.3 millibar). Carbon emissions from the Uranian atmosphere were also detected.

RESUMO

Four hundred forty-one patients who had closed hemorrhoidectomy with local anesthesia were followed for one to seven years postoperatively to assess long-term results and patient satisfaction. Seven and one-half per cent needed further treatment of hemorrhoids, 7.7 per cent developed other anorectal or colonic pathology, and 0.5 per cent had lasting incontinence. Lateral internal sphincterotomy did not predispose to incontinence. Patient satisfaction was 92.6 per cent.

Between 1955 and 1979, a modification of the compression arthrodesis of the ankle was performed in twenty-eight patients at The Campbell Clinic. The ages of the patients ranged from twenty-one to seventy-seven years, with an average of 48.4 years. The modification narrows both the medial and lateral sides of the ankle, resulting in a much better cosmetic appearance as the fused ankle is essentially the same width as the normal ankle. The distal end of the tibia and the talus are easily approached through anteromedial and anterolateral incisions, and all articular cartilage can be removed without difficulty. A solid arthrodesis was achieved in 92.8 per cent of the ankles, a higher percentage than in most reported series. Not only is the final appearance of the ankle more pleasing to the patient, but the modification also eliminates the complication of irritation and abrasion of the soft tissue beneath the malleoli by the top of the shoe.

Combined analysis of helium (584 angstroms) airglow and the atmospheric occultations of the star delta Scorpii imply a vertical mixing parameter in Saturn's upper atmosphere of K (eddy diffusion coefficient) approximately 8 x 10(7) square centimeters per second, an order of magnitude more vigorous than mixing in Jupiter's upper atmosphere. Atmospheric H(2) band absorption of starlight yields a preliminary temperature of 400 K in the exosphere and a temperature near the homopause of approximately 200 K. The energy source for the mid-latitude H(2) band emission still remains a puzzle. Certain auroral emissions can be fully explained in terms of electron impact on H(2), and auroral morphology suggests a link between the aurora and the Saturn kilometric radiation. Absolute optical depths have been determined for the entire C ring andparts of the A and B rings. A new eccentric ringlet has been detected in the C ring. The extreme ultraviolet reflectance of the rings is fairly uniform at 3.5 to 5 percent. Collisions may control the distribution of H in Titan's H torus, which has a total vertical extent of approximately 14 Saturn radii normal to the orbit plane.

The global hydrogen Lyman alpha, helium (584 angstroms), and molecular hydrogen band emissions from Saturn are qualitatively similar to those of Jupiter, but the Saturn observations emphasize that the H(2) band excitation mechanism is closely related to the solar flux. Auroras occur near 80 degrees latitude, suggesting Earth-like magnetotail activity, quite different from the dominant Io plasma torus mechanism at Jupiter. No ion emissions have been detected from the magnetosphere of Saturn, but the rings have a hydrogen atmosphere; atomic hydrogen is also present in a torus between 8 and 25 Saturn radii. Nitrogen emission excited by particles has been detected in the Titan dayglow and bright limb scans. Enhancement of the nitrogen emission is observed in the region of interaction between Titan's atmosphere and the corotating plasma in Saturn's plasmasphere. No particle-excited emission has been detected from the dark atmosphere of Titan. The absorption profile of the atmosphere determined by the solar occultation experiment, combined with constraints from the dayglow observations and temperature information, indicate that N(2) is the dominant species. A double layer structure has been detected above Titan's limb. One of the layers may be related to visible layers in the images of Titan.

Extreme ultraviolet spectral observations of the Jovian planetary system made during the Voyager 2 encounter have extended our knowledge of many of the phenomena and physical processes discovered by the Voyager 1 ultraviolet spectrometer. In the 4 months between encounters, the radiation from Io's plasma torus has increased in intensity by a factor of about 2. This change was accompanied by a decrease in plasma temperature of about 30 percent. The high-latitude auroral zones have been positively associated with the magnetic projection of the plasma torus onto the planet. Emission in molecular hydrogen bands has been detected from the equatorial regions of Jupiter, indicating planetwide electron precipitation. Hydrogen Lyman alpha from the dark side of the planet has been measured at an intensity of about 1 kilorayleigh. An observation of the occultation of alpha Leonis by Jupiter was carried out successfully and the data are being analyzed in detail.

Observations of the optical extreme ultraviolet spectrum of the Jupiter planetary system during the Voyager 1 encounter have revealed previously undetected physical processes of significant proportions. Bright emission lines of S III, S IV, and O III indicating an electron temperature of 10(5) K have been identified in preliminary analyses of the Io plasma torus spectrum. Strong auroral atomic and molecular hydrogen emissions have been observed in the polar regions of Jupiter near magnetic field lines that map the torus into the atmosphere of Jupiter. The observed resonance scattering of solar hydrogen Lyman alpha by the atmosphere of Jupiter and the solar occultation experiment suggest a hot thermosphere (>/= 1000 K) wvith a large atomic hydrogen abundance. A stellar occultation by Ganymede indicates that its atmosphere is at most an exosphere.

Regulations to limit the use of trichloroethylene as a degreasing solvent have led to an increased use of the more photochemically inert solvent methyl chloroform as a substitute. Model calculations show that about 15 percent of the methyl chloroform released into the atmosphere will reach the stratosphere. Time scenarios based on past production figures and reasonable projections for future release rates lead to a steady-state ozone depletion due to this solvent about 20 percent as large as those resulting from the continuous release of chlorofluoromethanes at 1973 rates.
