Mental health is a global public concern that contributes raising disability and premature death. Anxiety undertakes around 3.6% of the global population, while psychological stress is a condition associated to anxiety with a prevalence of 36.5%. Treatment for both mental conditions consist mainly of psychological therapy and pharmacotherapy, but the long-term drugs use can trigger adverse effects. Growing evidence shows the effect of specific food compounds on stress and anxiety treatment. The aim of this systematic review is to describe the molecular mechanisms related to dietary polyphenols administration from food matrix (considering food, juices or herbal/food extracts) and their effects on stress and/or anxiety, as well as review the available clinical evidence. Search was based on PRISMA Guidelines using peer-reviewed journal articles sourced from PubMed and Web of Science. A total of 38 articles were considered as eligible. The major effects for anxiety management were: reduction of oxidative stress and inflammation; HPA axis modulation; and regulation of some serotonergic/adrenergic pathways. There is a very limited evidence to conclude about the real effect of dietary polyphenols on stress. Although pharmacological treatment for mood disorders is essential, alternative therapies are necessary using non-pharmacological compounds to improve the long-term treatment effectiveness.

INTRODUCTION: The US deports more Mexicans to Tijuana than any other borderland city. Returning involuntarily as members of a stigmatized underclass, many find themselves homeless and de-facto stateless. Subject to routinized police victimization, many take refuge in the Tijuana River Canal (El Bordo). Previous reports suggest Tijuana River water may be contaminated but prior studies have not accessed the health effects or contamination of the water closest to the river residents. METHODS: A binational, transdisciplinary team undertook a socio-environmental, mixed methods assessment to simultaneously characterize Tijuana River water quality with chemical testing, assess the frequency of El Bordo residents' water-related diseases, and trace water contacts with epidemiological survey methods (n = 85 adults, 18+) in 2019, and ethnographic methods in 2019-2021. Our analysis brings the structural violence framework into conversation with an environmental injustice perspective to documented how social forces drive poor health outcomes enacted through the environment. RESULTS: The Tijuana River water most proximate to its human inhabitants fails numerous water-quality standards, posing acute health risks. Escherichia coli values were ∼40,000 times the Mexican regulatory standard for directly contacted water. Skin infections (47%), dehydration (40%) and diarrhea (28%) were commonly reported among El Bordo residents. Residents are aware the water is contaminated and strive to minimize harm to their health by differentially using local water sources. Their numerous survival constraints, however, are exacerbated by routine police violence which propels residents and other people who inject drugs into involuntary contact with contaminated water. DISCUSSION: Human rights to drinking water, sanitation and hygiene are routinely violated among El Bordo inhabitants. This is exacerbated by violent policing practices that force unhoused deportees to seek refuge in waterways, and drive water contacts. Furthermore, US-Mexico 'free-trade' agreements drive rapid growth in Tijuana, restrict Mexican environmental regulation enforcement, and drive underinvestment in sewage systems and infrastructure.

About 90% of cases of acute pericarditis have an idiopathic or viral etiology. In some cases, it is possible to identify high-risk patients for whom hospital admission and specific etiology research are mandatory for adequate treatment. Bacterial pericarditis is uncommon and responsible for less than 1% of cases. Only 10 cases of pericarditis due to Campylobacter fetus have been documented worldwide. This case highlights the importance of good cardiac imaging, with the right clinical and microbiology-integrated approach in high-risk cases of pericardial disease.


Environ 90 % des cas de péricardite aiguë sont d'étiologie idiopathique ou virale. Dans certains cas, il est possible de déterminer les patients exposés à un risque élevé pour qui l'admission à l'hôpital et la recherche d'une étiologie précise sont indispensables pour offrir le bon traitement. La péricardite bactérienne est rare et représente moins de 1 % des cas. Seuls 10 cas de péricardite à Campylobacter fetus ont été consignés dans le monde. Ce cas illustre l'importance d'une bonne imagerie cardiaque selon une bonne approche clinique et microbiologique intégrée chez les cas exposés à un risque élevé de maladies péricardiques.

This paper presents a "policy-informed" life cycle assessment of a cross-border electricity supply chain that links the impact of each unit process to its governing policy framework. An assessment method is developed and applied to the California-Mexico energy exchange as a unique case study. CO2-equivalent emissions impacts, water withdrawals, and air quality impacts associated with California's imports of electricity from Mexican combined-cycle facilities fueled by natural gas from the U.S. Southwest are estimated, and U.S. and Mexican state and federal environmental regulations are examined to assess well-to-wire consistency of energy policies. Results indicate most of the water withdrawn per kWh exported to California occurs in Baja California, most of the air quality impacts accrue in the U.S. Southwest, and emissions of CO2-equivalents are more evenly divided between the two regions. California energy policy design addresses generation-phase CO2 emissions, but not upstream CO2-eq emissions of methane during the fuel cycle. Water and air quality impacts are not regulated consistently due to varying U.S. state policies and a lack of stringent federal regulation of unconventional gas development. Considering local impacts and the regulatory context where they occur provides essential qualitative information for functional-unit-based measures of life cycle impact and is necessary for a more complete environmental impact assessment.

INTRODUCTION: Determination of antinuclear antibodies (ANA) by indirect immunofluorescence (IIF) is usually the initial test for the diagnosis of systemic rheumatic diseases (SRD). Assigning predictive values to positive and negative results of the test is vital because lack of knowledge about ANAs and their usefulness in classification criteria of SRD leads to inappropriate use. METHODS: Retrospective study, ANA tests requested by different specialties, correlation to patients' final diagnosis. RESULTS: The prevalence of autoimmune disease was relatively low in our population yielding a low PPV and a high NPV for the ANA test. 40% of the patients had no clinical criteria applied prior to test. Coexistence of two or more autoimmune disorders affects prevalence and predictive values. CONCLUSION: Application of the test after careful evaluation for clinical criteria remarkably improves the positive likelihood ratio for the diagnosis.

Cardiovascular imaging is one of the disciplines in cardiology with the most recent advances. This means that the teaching of Cardiology must evolve in the same way. In 2009, the American College of Cardiology published a statement, which points out that all of the cardiology residents must have basic training in every one of the cardiovascular imaging modalities available. Ischemic heart disease is the main cause of death in the world, including Mexico. Up to 43% of the patients that suffered a myocardial infarction and up to 31% of the patients with sudden cardiac death had an almost normal nuclear myocardial perfusion study in the year before the event, thus evidencing the importance of a multi-imaging approach. With the better understanding of the pathophysiological processes of coronary artery disease, new techniques have been developed that allows the detection of this disease almost from the beginning, through the detection of endothelial dysfunction by Positron Emission Tomography. Later on, when the patient develops diffuse atherosclerosis, we can rely on the use of de coronary calcium score and the detection of atherosclerotic plaques with coronary computed tomography angiography. To detect the presence of myocardial ischemia, two methods are widely used: echocardiography and nuclear medicine. Other options to identify myocardial ischemia are magnetic resonance imaging and computed tomography, due to the development of the "Dual Source" and "Flash" technologies. After an acute coronary event, cardiovascular imaging is useful for risk stratification and detection of myocardial viability, being the positron emission tomography the gold standard.

La imagen cardiovascular es una de las disciplinas que más ha evolucionado en el campo de la cardiología. Ante esto, la enseñanza de la cardiología debe moverse a la par. En 2009, el Colegio Americano de Cardiología decidió publicar una declaración en la que señala que: todos los residentes de cardiología deben llevar un entrenamiento básico en cada una de las técnicas de imagen cardiovascular disponibles. La cardiopatía isquémica es la principal causa de muerte en casi todo el mundo, incluido México. Hasta 43% de los pacientes que habían sufrido un infarto del miocardio y 31% de los pacientes con muerte súbita de origen cardiaco, tenían un estudio de perfusión por medicina nuclear prácticamente normal en el año previo al desenlace, poniendo en evidencia la importancia del abordaje por medio de distintos métodos de imagen. Con el mejor entendimiento de los procesos fisiopatológicos de la enfermedad arterial coronaria, se han desarrollado técnicas diagnósticas que nos permiten identificar esta patología prácticamente desde su inicio, a través de la detección de disfunción endotelial por medio de la tomografía por emisión de positrones. Más adelante, cuando los pacientes desarrollan ateroesclerosis manifiesta, contamos con herramientas como el score de calcio y la detección de las placas ateroscleróticas por medio de la tomografía computarizada. Para detectar la presencia de isquemia miocárdica contamos con dos métodos ampliamente utilizados: la ecocardiografía en estrés con dobutamina o dipiridamol y la medicina nuclear. Otras opciones para la identificación de isquemia son la resonancia magnética y la tomografía computada, gracias a la tecnología Dual Source y Flash. Posterior a un evento coronario, la imagen cardiovascular tiene como funciones la estratificación de riesgo y la detección de tejido miocárdico viable, siendo hoy en día el método de elección la tomografía por emisión de positrones.

The endothelium plays an important role in the regulation of the intracellular fluid, vascular permeability, and modulation of vascular focal tone and angiogenesis. Endothelial dysfunction is manifested by the loss of the endothelium ability to modulate physiology changes in its vascular bed, and actually it is considered a prognostic marker of coronary artery disease. The relevance of assessing endothelial dysfunction relies in that it has been observed in different pathologies like DM, dyslipidemia, hypertension, tabaquism and in immunologic diseases like antiphospholipid syndrome and systemic lupus. PET is a non invasive method that allows the absolute quantification of myocardial blood flow during rest, stress and adrenergic stimulation, which allows to asses endothelial function. Therefore PET is a useful diagnostic technique to identify patients with endothelial dysfunction, and in the assessment of its response to administered therapy, allowing an optimal control and prevention of secondary adverse events of these diseases.

Certain criteria are examined for infarctions currents defined as inferior or inferolateral. To do this, certain considerations on the anatomical aspects of isolated and in situ heart are laid out. The topographical relationship of the in situ heart with other adjacent thoracic organs is described. The heart is schematically represented as a pyramid with a triangular base and its walls and borders are related to walls of the thorax. The spatial orientation of the main resulting vectors from ventricular depolarization and repolarization are summarized also. Usefulness of registering the unipolar thoracic leads V7, V8, V9 or a complete electrocardiographic thoracic circle, is underlined. This method allows to detect for of the existence of an acute myocardial infarction in the inferior and inferolateral segments in as third basal and mid cardiac regions previously denominated posterolateral. On the base of previous electroanatomical comparisons, it is concluded that the thoracic posterior leads V7 - V9, as well as the magnetic resonance images, explore the same heart regions. Therefore, these two methods: electrocardiography which is an essentially functional method and magnetic resonance that especially focus on structural changes are not contradictory but rather complementary tests."

Se examinan algunos criterios acerca de los infartos definidos en la literatura actual como inferiores e inferolaterales. Con este fin, se describen los aspectos anatómicos del corazón aislado e in situ. Se analizan también las relaciones de este último con otros órganos torácicos circunvecinos. Se representa, de manera esquemática, el corazón como una pirámide con base triangular y sus lados y bordes en relación con las paredes de la caja torácica. Se resume, además, la orientación espacial de los principales vectores resultantes de los procesos de despolarización y repolarización ventriculares. Se subraya la utilidad del registro de las derivaciones unipolares torácicas posteriores V7, V8, V9 y del círculo torácico electrocardiográfico. Esto permite detectar la existencia de un infarto miocárdico agudo en regiones medias y básales de la pared inferolateral del corazón y su verdadera extensión. Con base en cotejos electroanatómicos previos, se concluye que las derivaciones V7 - V9 exploran los segmentos inferior e inferolateral en su tercio basal y medio, anteriormente denominados regiones cardiacas posterolaterales. Los registros electrocardiográficos proporcionan datos esencialmente funcionales y las imágenes obtenidas por resonancia magnética dan una información sobre todo estructural. Por lo tanto, ambos procedimientos de exploración cardiaca no deben contraponerse, sino integrarse.

El endotelio juega un papel importante en la regulación del líquido intracelular, la permeabilidad vascular, en la modulación del tono vascular focal y la angiogénesis. La disfunción endotelial se manifiesta por la pérdida de la capacidad del endotelio de modular el comportamiento fisiológico del lecho vascular y actualmente se considera un marcador pronóstico de la enfermedad arterial coronaria. La relevancia de estudiar la disfunción endotelial radica en que ésta se ha observado en diversas patologías como diabetes mellitas (DM), dislipidemia, hipertensión arterial sistémica, tabaquismo o en enfermedades inmunológicas como síndrome antifosfolípido y lupus eritematoso sistémico. La Tomografía por Emisión de Positrones (PET) es un método no invasivo que permite cuantificar en términos absolutos el flujo miocárdico en reposo, esfuerzo y durante la estimulación adrenérgica, siendo considerado en la actualidad el estándar de oro para valorar la función endotelial. Por lo tanto el PET es una herramienta diagnóstica muy útil en identificar a los pacientes con disfunción endotelial y en evaluar la respuesta a la terapia administrada en enfermedades que se acompañen de ésta. Permitiendo un control óptimo y prevención de eventos adversos de estas enfermedades.

State-of-the-art multidetector computed tomographic (CT) technology has replaced invasive angiography for evaluation of patients suspected to have aortic disease. Although most aortic disease is associated with atherosclerosis (ie, aneurysms and dissection), the spectrum of aortic disease is vast and includes various congenital and acquired entities. Radiologists should also be familiar with uncommon aortic diseases, which are divided into those that are congenital in origin and acquired disorders, and with their findings at multidetector CT. The first group includes patent ductus arteriosus, aortic hypoplasia, aortic coarctation, interrupted aortic arch, aortopulmonary window, common arterial trunk, supravalvular aortic stenosis, and vascular rings. The acquired disorders include aortic dissection due to extension of a coronary artery dissection, Marfan syndrome, large-vessel vasculitis such as Takayasu arteritis, and mycotic aneurysms. Finally, specific conditions associated with therapeutic maneuvers--such as recoarctation, stent-graft rupture, and endoleaks--can also be assessed with multidetector CT. Multidetector CT is an alternative tool helpful in establishing the primary diagnosis, defining anatomic landmarks and their relationships, and identifying associated cardiovascular anomalies. It is also an adjunct in the evaluation of complications during follow-up.

Concern about increasing levels of trace elements in the environment has led to the development and implementation of a global programme to determine the current baseline levels of these chemicals in the Earth's surface. The FORum of European Geological Surveys (FOREGS) has recently published a geochemical database for Europe, while progress on similar databases is continuing in other major regions of the world. The FOREGS database comprises multimedia samples collected at a resolution of approximately 72x72 km from 26 European countries. This enables the investigation of the factors governing geochemical variation on a continental scale, potentially allowing contributions of natural processes to be appreciated prior to setting environmental quality standards. This paper investigates the variation in European topsoil geochemistry using factorial kriging analysis, which performs principal components analysis at different spatial scales. The results are interpreted with the aid of a GIS database. Four spatial scales were identified: a nugget component representing variation over a range less than the sampling density; a 'short' scale component with a range of 296 km; an 'intermediate' scale component (875 km); and a 'long' scale component (1750 km). The first three principal components (PCs) of the nugget covariance matrix explained 22.2% of the overall variance, representing local variation in geology, land use, weathering and organic matter content. The first two PCs of the short range structure explained 12.6% of the variance, representing variation according to the major structural divisions of Europe, and to carbonate content. The first PC of the intermediate structure explained 7.2% of the variance and was found to relate to glacial history and Quaternary deposition. Finally, the first three PCs of the long range structure explained 29.6% of the variance and represented variation due to mineralisation, soil texture, climate and possibly anthropogenic contamination.

BACKGROUND: Systemic vasculitis are a group of heterogeneous diseases characterized by inflammation and necrosis of blood vessel walls. The etiology is not known, but geographic and environmental factors are implicated. AIM: To describe the clinical features of microscopic polyangiitis (MPA) and Wegener's granulomatosis (WG) in a Chilean cohort of patients. PATIENTS AND METHODS: Retrospective review of the medical records of 123 patients with the diagnosis of systemic vasculitis (65 MPA and 58 WG), seen from 1990 to 2001. The diagnosis were made based on the American College of Rheumatology and Chapel Hill criteria. RESULTS: The mean follow-up for MPA was 15 months (1-120) and for WG, 20 months (1-120). The median age (years) at diagnosis for MPA was 61 (19-82) and WG 50 (20-82). Gender distribution was similar in both groups (male: 68% and 57% respectively). The main clinical features in the MPA group were renal involvement (68%), peripheral nervous system involvement (57%), pulmonary hemorrhage (28%), and skin disease (32%). In the WG group were alveolar hemorrhage (62%), renal involvement (78%), paranasal sinus involvement (57%), and ocular disease (26%). In both, creatinine levels above 2.0 mg/dl were associated with a higher mortality (p< 0.01). ANCA by immunofluorescence was performed in 56 MPA patients (75% had pANCA, 4% had cANCA and 21% were ANCA negative) and in 55 WG patients (17% had pANCA, 79% had cANCA and 4% were ANCA negative). Global mortality was 18% and 17% respectively, and the most common causes of death were infections. CONCLUSIONS: The clinical features of our patients are similar to other published data. In our WG and MPA patients the main predictor for death was a serum creatinine above 2 mg/dl.
