Tanto la lesión hepática inducida por drogas (DILI), así como la lesión hepática inducida por hierbas (HILI), son una preocupación creciente en la atención sanitaria contemporánea que plantea importantes desafíos clínicos debido a sus variadas etiologías, presentaciones clínicas y posibles resultados potencialmente mortales. Presentamos el caso de un paciente masculino de 38 años con antecedentes de cálculos renales que consultó por dolor lumbar y hematuria. Al ingreso presentó ictericia, hepatomegalia, dolor a la palpación en fosa ilíaca derecha y no tenía signos de hepatopatía crónica, con pruebas de función hepática anormales, que mostraron un patrón hepatocelular asociado con hiperbilirrubinemia. Se descartó obstrucción biliar, trombosis portal, hepatitis autoinmune y viral, con panel autoinmune negativo. El paciente refirió haber consumido un remedio herbario para los cálculos renales llamado "vino rompe cálculos (chancapiedra)", que se supone contiene Phyllanthus niruri, cinco días antes del inicio de los síntomas. Una biopsia hepática reveló hepatitis aguda con infiltrado inflamatorio mixto. Debido al empeoramiento de las pruebas de función hepática y la sospecha de DILI idiosincrásico, se inició un ensayo terapéutico con corticosteroides, que resultó en una mejoría clínica y del perfil hepático. La gravedad de este caso nos recuerda la necesidad de incrementar el seguimiento por parte de las autoridades reguladoras de medicamentos, implementar campañas educativas para los pacientes e informar a la comunidad sobre productos con alertas activas.


Both drug-induced liver injury (DILI) and herb-induced liver injury (HILI) are a growing concern in contemporary healthcare that poses significant clinical challenges due to their varied etiology, clinical presentations, and potential life-threatening outcomes. We present the case of a 38-year-old male patient with a history of kidney stones who consulted for low back pain and hematuria. On admission he presented with jaundice, hepatomegaly, pain on palpation in the right iliac fossa and no signs of chronic liver disease, with abnormal liver function tests, which showed a hepatocellular pattern associated with hyperbilirubinemia. Biliary obstruction, portal thrombosis, autoimmune and viral hepatitis were ruled out, with negative autoimmune panel. The patient reported consuming an herbal remedy for kidney stones called "stone-breaking wine (chancapiedra)", presumed to contain Phyllanthus niruri, five days before the onset of symptoms. A liver biopsy revealedacute hepatitis with mixed inflammatory infiltrate. Due to worsening liver function tests and suspicion of idiosyncratic DILI, a therapeutic trial with corticosteroids was initiated, which resulted in clinical and liver profile improvement. The severity of this case reminds us of the need to increase follow-up by drug regulatory authorities, implement educational campaigns for patients, and inform the community about products with active alerts.

After orthotopic liver transplantation, several complications can arise, such as arterial or venous thrombosis, stenosis, biliary leakage, ischemia, or ascites. Although refractory ascites are not a common complication, they can have a significant impact on patients' prognosis and quality of life. This condition can be caused by multiple mechanisms, both intrahepatic and extrahepatic, and one of them is the splenic artery steal syndrome. In this article, we present the case of a patient with advanced cirrhosis who developed refractory ascites following liver transplantation. Despite management with diuretics and paracentesis, the ascites did not respond to conventional treatment. The diagnosis of splenic artery steal syndrome was confirmed through angiography, and subsequent embolization of the splenic artery resulted in symptom resolution.

Abstract Continuous home oxygen therapy (CHOT) is a widely used intervention but has scant evidence regarding its indications, benefits and the clinical predictors which could affect its requirement in patients with cardiovascular disease. Objective: to describe the patients with cardiovascular disease and the need for home oxygen therapy at the Fundación Cardioinfantil throughout 2018. Methods: a descriptive observational study. Hospitalized patients at Fundación Cardioinfantil from January 1, 2018, to December 31, 2018, diagnosed with cardiovascular disease and presenting their first request for home oxygen. Sociodemographic, clinical, laboratory, and imaging variables along with inpatient days and length of stay in the ICU, were collected. Results: 192 patients were studied. The median age was 74 years (IQR: 64, 25 - 81). The BMI was 25.98 kg/m2 (IQR: 22.74 - 29.67). A total of 47.9% of the population had coronary disease; 49.5% had heart failure; 27.1% had PH, 23% had OSAH, 6.3% had PTE and 16.7% had COPD. The average BNP was 1,430 SD 1,302. The average LVEF was 35.43% SD 15.35 and the median PASP was 53 mmHg (IQR 43 - 65). Altogether, 71.9% required ICU treatment. The average hospital stay was 14.08 days SD 10.1 days. A total of 20.8% of the patients had a PaO2 less than 55 mmHg; 77.1% belonged to the contributive social security regimen. Conclusions: we found that only a minority of the study patients for whom CHOT was ordered met the usually accepted criteria. Half of the patients had heart failure, most with reduced LVEF and elevated BNP, criteria which are not currently used for prescribing CHOT. More studies should be performed on the indications for supplementary oxygen therapy in this population. (Acta Med Colomb 2022; 47. DOI:https://doi.org/10.36104/amc.2022.2207).


Resumen La oxigenoterapia domiciliaria continua (OCD) es una intervención altamente utilizada, pero con escasa evidencia sobre sus indicaciones, beneficios y predictores clínicos que puedan influir sobre su requerimiento en pacientes con enfermedad cardiovascular. Objetivo: caracterizar los pacientes con enfermedad cardiovascular y requerimientos de oxígeno domiciliario hospitalizados en la Fundación Cardioinfantil durante el año 2018. Métodos: estudio observacional descriptivo, hospitalizados en la Fundación Cardioinfantil del 1° de enero del 2018 a 31 de diciembre del 2018 con diagnóstico de enfermedad cardiovascular y con primera solicitud de oxígeno domiciliario. Se tomaron variables sociodemográficas, clínicas, paraclínicas, imagenológicas, días de hospitalización y estancia de UCI. Resultados: se estudiaron 192 pacientes. La mediana de edad fue 74 años (RIQ: 64, 25-81). El IMC fue 25.98 Kg/m2 (RIQ: 22.74-29.67). El 47.9% de la población tenía enfermedad coronaria. El 49.5% de los pacientes presentaban falla cardiaca. El 27.1% tenía HTP, 23% SAHOS, 6.3% TEP y 16.7% EPOC. El promedio del BNP fue de 1430 DS 1302. La FEVI promedio fue de 35.43% DS 15.35 y la mediana PSAP fue de 53 mmHg (RIQ 43-65). El 71.9% requirió manejo en UCI. El promedio de estancia hospitalaria fue de 14.08 días DS 10.1 días. El 20.8% de los pacientes tenían una PaO2 menor a 55mmHg. El 77.1% pertenecían al régimen contributivo. Conclusiones: encontramos que sólo una minoría de pacientes estudiados a los que se le indicó OCD cumplían con los criterios actualmente aceptados. La mitad de los pacientes presentaban falla cardiaca, la mayoría con FEVI reducida y BNP elevado, criterios que actualmente no son utilizados para la prescripción de OCD. Introduction Different factors are critical when assessing COVID-19 mortality, and can explain why severity differs so widely among populations. However, there is little information regarding prognostic factors and mortality in COVID-19 from Latin American countries. Objectives To determine prognostic factors in hospitalized COVID-19 patients and to evaluate the impact of tocilizumab use in patients with hyperinflammatory syndrome and severe disease defined by the National Early Warning Score 2 (NEWS2) with a value greater than or equal to seven points. Materials and methods This retrospective cohort study included hospitalized COVID-19 patients from May to July 2020. A multivariate logistic regression analysis was performed to determine independent factors associated with mortality. Results A total of 136 patients required hospital admission. In-hospital mortality was 39.7%. Mortality was observed to be potentiated by older age, LDH serum levels and the presence of type 2 diabetes mellitus. Lymphopenia and lower PaO2/FiO2 ratio were more common in these patients. Similarly, patients who died were classified more frequently with severe disease. The independent factors associated with in-hospital mortality were age greater than 65 years, type 2 diabetes mellitus, NEWS2 greater than or equal to seven points and LDH greater than 400U/L. The use of Tocilizumab alone was not related with decreased in-hospital mortality. Subgroup analysis performed in patients with hyperinflammation and severe disease showed similar results. Conclusions COVID-19 mortality in hospitalized patients was high and mainly related with older age, comorbidities, LDH and the severity of disease at hospital admission.

La pseudohemorragia subaracnoidea es un fenómeno infrecuente que se caracteriza por hallazgos sugestivos de hemorragia subaracnoidea en la tomografía computarizada simple de cráneo, sin evidencia de la misma en estudios adicionales. Se ha asociado a múltiples causas, de las cuales la principal es la encefalopatía hipóxico-isquémica posparo cardiaco y reanimación cardiopulmonar. El contexto clínico y los niveles de atenuación medidos en Unidades Hounsfield (UH) se deben tener en cuenta al hacer el diagnóstico diferencial entre ambas entidades. Se presenta el caso de una paciente con pseudohemorragia subaracnoidea de etiología multifactorial.


Pseudo-subarachnoid hemorrhage (PSAH) is an infrequent entity characterized by findings in non- contrast head computed tomography that mimic subarachnoid hemorrhage, but without evidence of blood products in further studies. It has been associated with multiple etiologies, with hypoxic ischemic encephalopathy following cardiac arrest and cardiopulmonary resuscitation as the leading cause in literature. Clinical context and attenuation levels measured in Hounsfield Units should be taken into consideration when establishing the differential diagnosis between these entities. The case of a patient with PSAH of multifactorial etiology is presented.
