BACKGROUND: Whether patients over 85 years old with gastrointestinal cancer should undergo surgery remains controversial. We aimed to describe the changing trends of characteristics to provide more information to decision makers, and strive to find appropriate surgical plan. AIM: To describe the changing trends of characteristics to provide more information to decision makers, and strive to find appropriate surgical plan. METHODS: A total of 218 gastric cancer (GC) patients and 563 colorectal cancer (CRC) patients who underwent surgery between 2001 and 2021 were enrolled in this retrospective analysis. Changes in clinicopathological features, surgical treatments, and survival status were analyzed longitudinally at 5-year intervals. RESULTS: Only 14 GC patients underwent laparoscopic surgery where 219 CRC patients had this procedure. Cardia and esophagogastric junction cancer increased in GC patients, and the proportion of sigmoid colon cancer decreased in CRC patients. Pulmonary infection gradually became the most common postoperative complication, its incidence in period 4 reached 48.79%. However, the incidence of anastomotic leakage decreased from 26.79% to 9.38% (P < 0.01). Additionally, 30-d mortality significantly decreased from 32.14% to 9.01%. Increases were observed in 5-year overall survival (OS) in GC patients from period 1 to period 4 (18.18% vs 33.32%, respectively) and CRC patients (0 vs 36.32%, respectively). Disease-free survival (DFS) also increased in GC and CRC patients (7.14% vs 27.74% and 0 to 36.03%, respectively). The average survival time of GC patients following radial lymphadenectomy was higher than in patients that underwent limited lymphadenectomy (26 vs 22 mo, respectively), the same was seen in CRC patients (44 vs 33 mo, respectively). This advantage was particularly evident in patients with TNM I, but not in patients with TNM II/III period cancer. CONCLUSION: The safety as well as effectiveness of surgery in ultra-elderly patients is increasing. Radical lymphadenectomy has advantages in patients with TNM I gastrointestinal cancer, but not TNM II/III.

Vascular dementia(VD) is a condition of cognitive impairment due to acute and chronic cerebral hypoperfusion. The available therapies for VD mainly focus on mitigating cerebral ischemia, improving cognitive function, and controlling mental behavior. Achievements have been made in the basic and clinical research on the treatment of VD with traditional Chinese medicine(TCM) active components, including Ginkgo leaf extract, puerarin, epimedium, tanshinone, and ginsenoside. Most of these components have anti-inflammatory, anti-apoptotic, anti-oxidant, and neuroprotective effects, and puerarin demonstrates excellent performance in mitigating cholinergic nervous system disorders and improving synaptic plasticity. Puerarin, ginkgetin, and epimedium are all flavonoids, while tanshinone is a diterpenoid. Puerariae Lobatae Radix, pungent in nature, can induce clear Yang to reach the cerebral orifices and has the wind medicine functions of ascending, dispersing, moving, and scurrying. Puerariae Lobatae Radix entering collaterals will dredge blood vessels to promote blood flow, and that entering the sweat pore will open the mind, which is in line with the TCM pathogenesis characteristics of VD. This study reviews the progress in the mechanism of puerarin, the main active component of Puerariae Lobatae Radix, in treating VD. Puerarin can ameliorate cholinergic nervous system disorders, reduce excitotoxicity, anti-inflammation, inhibit apoptosis, alleviate oxidative stress injury, enhance synaptic plasticity, up-regulate neuroprotective factor expression, promote cerebral circulation metabolism, and mitigate Aβ injury. The pathways of action include activating nuclear factor erythroid 2-related factor 2(Nrf2)/antioxidant response element(ARE), vascular endothelial growth factor(VEGF), extracellular regulated protein kinases(ERK), phosphatidylinositol-3-kinase(PI3K)/protein kinase B(Akt), Janus-activating kinase 2(JAK2)/signal transducer and activator of transcription 3(STAT3), AMP-activated protein kinase(AMPK), as well as inhibiting the tumor necrosis factor α(TNF-α), transient receptor potential melastatin 2(TRPM2)/N-methyl-D-aspartate receptor(NMDAR), p38 mitogen-activated protein kinase(p38 MAPK), Toll-like receptor 4(TLR4)/nuclear factor-kappaB(NF-κB), early growth response 1(Egr-1), and matrix metalloproteinase 9(MMP-9). By reviewing the papers about the treatment of VD by puerarin published by CNKI, Wanfang, VIP, PubMed, and Web of Science in the last 10 years, this study aims to support the treatment and drug development for VD.

The major reason of human morbidity and mortality is obesity and related diseases. Brown adipose tissue (BAT) is associated with low total adipose tissue content and a lower risk of type 2 diabetes mellitus. Studies have shown that exercise and cold expose may induce browning. In this study, we verified (1) whether exercise and/or cold exposure can improve the expression level of ucp4c, serca, ampkα, camkII, sirt1, octß3r, and hamlet; (2) if these interventions can save cardiac dysfunction induced by a high-fat diet (HFD) in Drosophila. w1118 (wild-type) virgin female flies collected within 8 h after eclosion were divided into eight groups: the normal feed control group (NFD-C), the normal feed exercise group (NFD-E), the normal feed cold exposure group (NFD-CA), the normal feed exercise/cold exposure group (NFD-EC), the HFD control group (HFD-C), the HFD exercise group (HFD-E), the HFD cold exposure group (HFD-CA), and the HFD exercise/cold exposure group (HFD-EC). After exercise and/or cold exposure for 7 days, the mRNA expression levels of ucp4c, serca, ampkα, camk II, sirt1, octß3r, and hamlet were tested by qRT-PCR, and m-mode was used to assess cardiac function. In addition, we assessed the triacylglycerol (TAG) levels, motor ability, fat mass (by Oil Red O [ORO] staining), and morphological features. The results of TAG, ORO staining, and morphological features all indicate that after interventions, body size of Drosophila was smaller compared with the control group, irrespective of the feeding patterns. The mRNA expression levels of ucp4c, serca, octß3r, hamlet, ampkα, camkII, and sirt1 were changed to varying degrees under different intervention states (exercise and/or cold exposure). Cold exposure and exercise/cold exposure partly improved cardiac function and the normal fruit flies' cardiac function and exercise ability. However, after exercise intervention, exercise ability and heart function were improved in both HFD and normal-fat diet (NFD) fruit flies. In conclusion, different intervention states (exercise and/or cold exposure) can change the mRNA expression levels of ucp4c, serca, octß3r, hamlet, ampkα, camkII, and sirt1. Exercise is the most effective way to restore HFD-induced cardiac dysfunction.

Aim To establish gefitinib resistant epidermal growth factor receptor(EGFR) mutant lung cancer cell lines and to explore the changes of downstream signaling pathway of EGFR. Methods Gefitinib concentration gradient method was used to induce the establishment of H3255 and HCC827 resistant cell lines, and CCK8 assay was used to detect the proliferation of gefitinib resistant cell lines H3255/GR and HCC827/ GR. Western blot was used to detect the changes of EGFR downstream signals in H3255, HCC827, H3255/GR and HCC827/GR cells. Results The proliferation of H3255/GR and HCC827/GR cells was significantly lower than that of non-drug resistant cells. The phosphorylation signal molecules p-AKT, p-ERK1/2 and p-STAT3 of H3255/GR and HCC827/ GR drug-resistant cell lines were no significantly changed compared with their non-drug-resistant cell lines. There was no significant change in the expression of p-STAT3 and p-ERKl/2 in H3255/GR cells treated with gefitinib, but the expression of p-AKT was significantly down-regulated, while gefitinib only slightly inhibited the expression of p-ERKl/2 in drugresistant H3255/GR cells. In HCC827/GR cells, p-AKT and p-STAT3 were not inhibited by gefitinib, while p-ERKl/2 was moderately inhibited. Conclusion There are significant differences in the signal mechanism of drug resistance between H3255/GR and HCC827/GR cell lines.

The early diagnosis of children with autism spectrum disorders (ASD) is essential. Electroencephalography (EEG) is one of most commonly used neuroimaging techniques as the most accessible and informative method. In this study, approximate entropy (ApEn), sample entropy (SaEn), permutation entropy (PeEn) and wavelet entropy (WaEn) were extracted from EEGs of ASD child and a control group, and Student's -test was used to analyze between-group differences. Support vector machine (SVM) algorithm was utilized to build classification models for each entropy measure derived from different regions. Permutation test was applied in search for optimize subset of features, with which the SVM model achieved best performance. The results showed that the complexity of EEGs in children with autism was lower than that of the normal control group. Among all four entropies, WaEn got a better classification performance than others. Classification results vary in different regions, and the frontal lobe showed the best performance. After feature selection, six features were filtered out and the accuracy rate was increased to 84.55%, which can be convincing for assisting early diagnosis of autism.

BACKGROUND: Many randomized controlled trials have investigated the effects of exercise on the rehabilitation of patients with breast cancer. However, the exercise forms used in most previous studies were monotonous. Therefore, we designed a protocol to estimate the effects of combined exercise intervention using Internet and social media software on the rehabilitation of postoperative patients with BC. METHODS/DESIGN: This study protocol is a randomized control trial with an intervention time of 12 weeks. After completing baseline questionnaire and physical fitness tests, the participants are randomized to the study group or the control group. Procedure contents of exercise intervention in the study group include: via phone step-recording app, ask the individuals to complete the target number of steps within a specified period of exercise, four times per week; face-to-face remote video guidance of individuals on muscle training, three times per week; common knowledge of physical exercise BC rehabilitation will be pushed regularly by social media apps every day. The control group will receive normal treatment and rehabilitation according to daily specifications of the hospital. The primary outcome will be the quality of life. The secondary outcomes are physical fitness and social cognitive indicators. DISCUSSION: This study is a clinical trial to estimate the effects of combined exercise intervention based on the Internet and social media software for postoperative patients with breast cancer (BC). If expected results are achieved in this study, measures and methods of BC rehabilitation will be enriched. TRIAL REGISTRATION: Chinese Clinical Trial Register, ChiCTR-IPR-17012368 . Registered on 14 August 2017.

OBJECTIVE: The purpose of this study was to investigate the effects of Qigong on type 2 diabetes mellitus (DM) using the systematic review and meta-analysis. METHODS: All prospective, randomized, controlled clinical trials published in English or Chinese and involving the use of Qigong by patients with DM were searched in 7 electronic databases from their respective inception to June 2016. The meta-analysis was conducted using the Revman 5.2. The quality of the included trials was assessed using the Jadad rating scale. Two researchers independently completed the inclusion, data extraction, and quality assessment. RESULTS: Twenty-one trials with 1326 patients met the inclusion criteria and were reviewed. The meta-analysis demonstrated that, compared with no exercise, the Qigong had significant effects on fasting blood glucose (MD = -0.99, 95% CI (-1.23, 0.75), P < 0.0001), HbA1c (MD = -0.84, 95% CI (-1.02, -0.65), P < 0.0001), and postprandial blood glucose (MD = -1.55, 95% CI (-2.19, -0.91), P < 0.00001). CONCLUSION: The Qigong training can improve the blood glucose status of the type 2 DM patients and has positive effects on the management of type 2 DM. However, future research with better quality still needs to be conducted to address the effects of Qigong on type 2 DM.

Autism spectrum disorders (ASD) is a complex developmental disorder characterized by impairments in social communication and stereotyped behaviors. Electroencephalograph (EEG), which can measure neurological changes associated with cortical synaptic activity, has been proven to be a powerful tool for detecting neurological disorders. The main goal of this study is to explore the effects of repetitive transcranial magnetic stimulation (rTMS) on behavioral response and EEG. We enrolled 32 autistic children, rTMS group ( = 16) and control group ( = 16) and calculated the relative power of the δ, θ, α, β rhythms in each brain area by fast Fourier transform and Welch's method. We also compared Autism Behavior Checklist (ABC) scores of the patients before and after rTMS. The results showed a significant decrease in the relative power of the δ band on right temporal region and parietal region and also a decreased coherence on frontal region after rTMS intervention. The study proves that rTMS could have positive effects on behavior of attention, execution ability, and language ability of children and could reduce their stereotyped behavior and radical behavior.

Objective To determine the expression levels of serum miR-25 in the patients with non-small cell lung cancer (NSCLC),and evaluate its clinical significance in the screening of NSCLC.Methods Serum samples from 82 untreated NSCLC patients,including 4 with TNM satge Ⅰ,10 with TNM stage Ⅱ,11 with TNM stage Ⅲ,53 with TNM stage Ⅳ and 4 with unknown stage,and 82 healthy controls with matched age and gender were collected,and the levels of miR-25 in these samples were determined by quantitative reverse transcription PCR (qRT-PCR).The diagnostic value of miR-25 in NSCLC was evaluated by the receiver operating characteristic (ROC) curve.Results Serum miR-25 levels in NSCLC patients were significantly higher than that in healthy controls (0.017 ± 0.028 vs 0.004 ±0.004,t =4.098,P <0.01).The area under the ROC curve (AUGROC),sensitivity and specificity of miR-25 for the diagnosis of NSCLC were 0.818 (95% CI:0.753-0.882),70.7% and 80.7%,respectively.In addition,the AUCROC,sensitivity and specificity of serum miR-25 for the screening of stage Ⅰ/Ⅱ NSCLC were 0.852 (95% CI:0.728-0.976),78.6% and 87.8%,respectively.Logistic regression analysis showed that miR-25 was an independent risk factor of NSCLC (OR =10.84,95% CI:5.07-23.19,P < 0.01).Conclusion Serum miR-25 levels in NSCLC patients increase significantly,indicating that it may be a novel molecular biomarker for the screening of NSCLC.

The objective of this study was to investigate the effects of suspended moxibustion upon tender point on delayed onset muscle soreness. The study was a randomized double-blind controlled trial with 50 subjects in each group. Subjects in moxibustion group received suspended moxibustion upon tender point (by palpation) twice. The controls received no treatment. Each subject received treatments twice, 24 hr and 48 hr after the delayed onset muscle soreness induction. The measurements of Pain Visual Analog Scale, maximum isometric muscle strength and circumference were made immediately after delayed onset muscle soreness induction, before and after every treatment session and 72 hr after delayed onset muscle soreness induction. There were no significantly statistical differences between two groups at all measurement time points on maximum isometric muscle strength and circumference. However, the Pain Visual Analog Scale scores after first treatment session and 72 hr after delayed onset muscle soreness induction in moxibustion group was less than the control group, with significant difference (p < 0.05). The suspended moxibustion failed to show the effect on delayed onset muscle soreness. However, results need to be cautiously interpreted because of the pilot character of this study. The suspended moxibustion has a potential effect of pain relief on DOMS from the analysis of the data of this study. A large sample size trial is needed to confirm the effect of the suspended moxibustion on DOMS in the future.

Objective To analyze the significance of levels and combined detection of plasma homocysteine(Hcy),high-sensitivity C reactive protein(hs-CRP) and lipoprotein-associated phospholipase A2(Lp-PLA2) in patients with ischemic cerebral stroke(ICS).Methods A total of 140 patients with ICS were selected into ICS group,and 100 healthy individuals were recruited in the control group.The plasma levels of Hcy,hs-CRP,Lp-PLA2 were examined,then the positive rates of them were statistically analyzed.Analyzed the value of the levels and joint detection of Hcy,hs-CRP and Lp-PLA2.Results The plasma levels and the positive rates of Hcy,hs-CRP and Lp-PLA2 in ICS group were statistically higher than those in the control group(P<0.05).The area under the receiver-operating characteristic curve of combined detection of Hcy,hs-CRP and Lp-PLA2 in diagnosis of ICS was 0.949,the sensitivity was 0.850,and the specificity was 0.807,which were higher than Hcy,hs-CRP and Lp-PLA2 alone.Conclusion The plasma levels of Hcy,hs-CRP and Lp-PLA2 increase in ICS patients,and the combined detection of the three indicators might improve the early diagnostic value of ICS.

Objective To investigate the association of Tim-1 protein expression and its gene polymorphism with nutritional parameters in patients with systemic lupus erythematosus (SLE).Methods Peripheral blood samples were collected from 126 patients with SLE.Serum Tim-1 protein levels were detected by ELISA,and the Tim-1 gene-416G>C,-1454G>A polymorphism was detected by PCR-RFLP.Prealbumin,ceruloplasmin,and retinol conjugated protein levels were determined by immunoturbidimetry.Ferritin and 1,25-dihydroxy vitamin D3 levels were detected by electrochemiluminescence.Results Concentrations of serum Tim-1 protein,prealbumin,ceruloplasmin,retinol binding protein,ferritin,and 1,25-dihydroxy vitamin D3 were 249.7±30.2 pg/mL,226±42 μg/mL,363±95 μg/mL,29.4± 13.2 μg/mL,355± 164 ng/mL,and 26.4-± 11.5 ng/mL,respectively.In the-416G>C site,GG,GC,and CC genotypes accounted for 11.9%,57.1%,and 31.0%,respectively.In the-1454G>A site,GG,GA,and AA genotypes accounted for 67.5%,26.2%,and 6.3%,respectively.The Tim-1 protein concentration did not differ significantly between the different genotypes of the-416G>C site (F=0.575,P=0.564) or-1454G>A site (F=1.255,P=0.289).Tim-1 level was significandy negatively correlated with prealbumin (r =-0.176,P =0.033),and positively correlated with ceruloplasmin (r =0.205,P =0.014) and 1,25-dihydroxy vitamin D3 (r=0.166,P=0.042).The serum prealbumin level decreased significantly (P=0.027) in patients harboring the GG genotype in the-1454G>A site,whereas the serum 1,25-dihydroxy vitamin D3 level decreased significandy (P =0.024) in patients with the AA genotype in the-1454G>A site.Conclusion Serum Tim-1 protein level and the-1454G>A polymorphism of Tim-1 gene are associated with the nutritional function of patients with SLE.

Objective: To explore how to set up the service management center of ventilators depending on department of respiratory medicine , how to perfect daily management of ventilators, how to rationally and timely allocate ventilators ,how to reduce the fault of ventilators and ensure the safe use of ventilators, and how to cost savings of purchasing ventilators. Methods: To figure out and compare data of before and after the establishment of the service management center of ventilators in 2010 and in 2012. Results: Non machine failure rate was 42.86% in 2012 and 69.35% in 2010, had remarkably decreased. At the same time, the total failures have decreased significantly. Conclusion: Setting up the service management center of ventilators depending on department of respiratory medicine have achieved timely allocation ventilators, have improved the level of medical personnel of using ventilators, have ensured the safe use of ventilators, have cost savings of purchase ventilators, have reduced the fault of ventilators.

PURPOSE: This study was aimed to evaluate effect of the desensitizing pretreatments on the micro-tensile bond strengths (microTBS) to eroded dentin and sound dentin. MATERIALS AND METHODS: Forty-two extracted molars were prepared to form a flat dentin surface, and then they were divided into two groups. Group I was stored in distilled water while group II was subjected to a pH cycling. Each group was then subdivided into three subgroups according to desensitizing pretreatment used: a) pretreatment with desensitizer (Gluma); b) pretreatment with CO2 Laser (Ultra Dream Pluse); c) without any pretreatment. All prepared surfaces were bonded with Single Bond 2 and built up with resin composite (Filtek Z250). The micro-tensile bond test was performed. Fracture modes were evaluated by stereomicroscopy. Pretreated surfaces and bonded interfaces were characterized by scanning electron microscope (SEM). The data obtained was analyzed by two-way ANOVA (alpha=0.05). RESULTS: For both sound and eroded dentin, samples treated with desensitizer showed the greatest microTBS, followed by samples without any treatment. And samples treated with CO2 laser showed the lowest microTBS. SEM study indicated that teeth with eroded dentin appeared prone to debonding, as demonstrated by existence of large gaps between adhesive layers and dentin. CONCLUSION: Pretreatment with Gluma increased the microTBS of Single Bond 2 for eroded and sound teeth. CO2 laser irradiation weakened bond performance for sound teeth but had no effect on eroded teeth.

Objective To explore the association between genetic variants in intron 4 of the surfactant protein B(SP-B) gene and neonatal respiratory distress syndrome(NRDS) in preterm infants.Methods Using 1 ∶ 1 casc-control study method,a cohort of 52 preterm infants with NRDS and 52 preterm infants without NRDS(non-NRDS) were recruited.Within 1 week after the patients hospitalized,2 mL venous blood was collected.Genomic DNA was extracted and polymerase chain reaction(PCR) was performed to amplify the special region with conserved sequence motifs and intermotif dinucleotide repeats(CA) n in intron 4 of SP-B gene.PCR products was demonstrated by agarose gel electrophoresis and variants were identified by product size.Results A 550 bp target band could be seen in all PCR products (including NRDS group and non-NRDS group).Besides,an additional band(about 650 bp) could be seen in 3 cases of NRDS and 1 case of non-NRDS,which represented insertion variation.The variation incidence of intron 4 of SP-B gene in NRDS group and non-NRDS group were 5.77% and 1.92%,respectively,there was significant differences (x2 =44.18,P < 0.05).Conclusion Genetic variants in intron 4 of SP-B gene may contribute to risk of preterm infants with NRDS.

OBJECTIVE: To examine risk factors of chronic obstructive pulmonary disease (COPD) deaths in Chinese military elderly men. METHODS: A cohort analytic study was carried out in Xi'an, China. A total of 1268 retired military males aged 55 or older were examined in 1987 and followed for 18 years. Main outcome measures were all causes and COPD deaths. RESULTS: The total person-years of follow-up from 1987 until June 2005 was 18 766.28. The mean follow-up time was 14.35 years; A total of 491 had died, with 748 alive and 29 lost of follow-up. COPD was the second cause of death in all deaths (16.90%). Results Univariate analysis of Cox model showed that age, number of smoking cigarettes per day, duration of smoking, negative affairs and existing COPD were risk factors of COPD deaths and the relative risks [95% confidence intervals (CI)] were 1.13 (1.09-1.17), 1.04 (1.02-1.06), 1.03 (1.01-1.04), 1.81 (2.85-6.77) and 4.39 (2.85-6.77) respectively. Data from Multivariate analysis of Cox model showed that age, number of smoking cigarettes per day and existing COPD were risk factors of COPD death with relative risks [95% confidence intervals (CI)] as 1.10 (1.06-1.15), 1.03 (1.01-1.06) and 3.07 (1.90-4.98) respectively. The risks for deaths increased significantly with increasing amount and duration of smoking resulting from all causes and COPD. Compared with current smokers, former smokers had lower risks of total mortality(excess risk reduction of 66.67%). CONCLUSION: COPD was the second cause among all deaths in this cohort. Age, number of smoking cigarettes per day and existing COPD were the risk factors of COPD deaths which called for further survey to examine the relationship between quitting smoking and COPD deaths in this cohort.
