RESUMO

Two young patients with Von Recklinghausen's disease developed an obstructive aortic arch syndrome. Follow-up over several years showed progressive changes in the clinical signs with no development of symptoms. Angiography demonstrated great changes in the anatomical relationship of the supraaortic trunks which lead to surgical repair. In one case, the histology of operative biopsies established a relation between the neurofibromatous and the arterial lesions. Modern investigative procedures and therapeutic possibilities are reviewed in the light of these two cases. These two patients seem to be the first reported cases of neurofibromatosis causing the aortic arch syndrome.

RESUMO

An arterio-venous fistula in the neck arising from the right vertebral artery is described. Selective angiography showed a single communication. A Fogarty catheter, introduced by the occipital artery and left in situ was used as a means of embolisation. This procedure was simple and effective.

RESUMO

The authors present a case of sub-annular aneurysm of the left ventricle in a child of 7. It was associated with aortic incompetence, a ventricular septal defect, and coarctation of the aorta which were discovered during investigation of streptococcal septicaemia. As a first stage procedure, the aortic valve was repaired, the aneurysm was excised, and the ventricular septal defect closed. At a later stage, the coarctation was resected. The question of whether this lesion was congenital or acquired is discussed. After one year, the result remains satisfactory.
