OBJECTIVE: To evaluate the effect of an octenidine (OCT)-based antiseptic on the gingival inflammation and microbial composition of subgingival dental plaque in patients with fixed orthodontic appliances. PATIENTS AND METHODS: Thirty-three orthodontic patients were randomized into 2 groups. The control group patients were given standard oral hygiene and dietary advice, while the experimental group patients used an OCT-based antiseptic together with standard oral hygiene and dietary recommendations. The periodontal status was evaluated using the following indices: the plaque index (PI), the gingival index (GI), the papilla bleeding index (PBI) and the probing pocket depth (PD). Next Generation Sequencing of the 16S rRNA amplicons was performed in order to assess the subgingival microbiome. RESULTS: The PD values obtained were significantly lower in the experimental group after one month, as well as PBI. The microbiological analysis showed a significant increase in the occurrence of the genus Prevotella in the control group, while the number of other periodontopathogens remained stable in both groups. The changes in the abundance of the bacteria not directly associated with periodontal disease were also observed. CONCLUSIONS: The use of an OCT-based antiseptic has a positive effect on the prevention of gingival inflammation. Additionally, it also prevents a likely increase in numbers of periodontopathogens of the subgingival dental plaque in the first three months of fixed orthodontic treatment.

PURPOSE: The purpose of this study is to give an overview of studies investigating the role of fludeoxyglucose F18 ((18)F-FDG) positron emission tomography (PET) scanning in patients with aortic aneurysms with a focus on molecular characteristics of the aneurysm wall. METHODS: MEDLINE, EMBASE, and the Cochrane database were searched for relevant articles. After inclusion and exclusion, we selected 18 relevant articles reporting on (18)F-FDG PET scanning of aortic aneurysms. RESULTS: The sample size of studies is limited, and there are no standardized imaging protocols and quantification methods. (18)F-FDG PET scanning was shown to display molecular characteristics of the aortic wall. Different studies showed contradictory findings of aortic (18)F-FDG uptake in aneurysm patients compared to controls. CONCLUSIONS: Non-invasively determining molecular characteristics of aortic wall weakening might lead to better rupture and growth prediction. This might influence the decision of the surgeon between conservative and surgical treatment of aneurysms. To date, there is conflicted evidence regarding the use of (18)F-FDG PET scanning to predict aneurysm rupture and growth. The role of (18)F-FDG PET scanning in rupture risk prediction needs to be further investigated, and standardized imaging protocols and quantification methods need to be implemented.

Augmenting the nasal dorsum is often a difficult task, because of the irregularities that might be visible under the thin dorsal nasal skin. Saddle noses are especially difficult to correct because of the need to provide strong structural support to the nose while at the same time achieving an aesthetic dorsum. Stable reconstruction of the cartilaginous septum is the critical challenge in the operative treatment of such deformities with both functional and morphological implications. Treatment depends on the degree of saddling. Autologous cartilage is widely recognized to be the standard against which other materials must be judged. It is the most acceptable and reliable long-term graft. We present a case of unusually severe posttraumatic saddle nose resolved with a single rather large costal cartilaginous graft and a columellar strut. Classic extended spreader grafts could not be used due to the lack of bony and cartilaginous support of the nasal pyramid and, thus, no possibility of stabilization. A normal nasal dorsum, as well as breathing improvement, were achieved and the patient was extremely satisfied with the result. We had no complications except a slight asymmetry of the nostrils. Graft resorbtion or displacement was not observed on the 14-months follow-up.

BACKGROUND: Malignant melanoma (MM) is an extremely malignant tumor with an unpredictable profile of spread and variable periods of remission. CASE: We describe an unusual case of malignant melanoma metastatic to the omentum occurring seven years after diagnosis and treatment of cutaneous malignant melanoma in the patient's arm. She received surgery and chemoimmunotherapy. To date, nine months after detection of malignant melanoma metastatic to the omentum, the patient is alive with no clinical and radiological metastatic disease. CONCLUSIONS: The diagnosis of omentum malignant melanoma in a living patient is uncommon, thus very few individuals and referral centers can build up an adequate experience of handling this disease. Optimal management has been a challenge and a subject of debate and has not yet been established.

PURPOSE: The quantitative real-time polymerase chain reaction (qRT-PCR) is used in the detection of molecular events involved in leukemogenesis, such as the Bcr-Abl gene translocation, the most important factor in the pathogenesis of chronic myeloid leukaemia (CML). The main aim of our study was to test the reproducibility, specificity and sensitivity of the qRT-PCR in the detection of Bcr-Abl gene translocation. METHODS: In complementary (c)DNA, isolated from K562 Bcr-Abl positive cell line, we performed qRT-PCR analysis with Bcr-Abl specific primers. For qRT-PCR analysis, we used serial dilutions of the newly synthesized cDNA in order to establish the detection threshold of this method. RESULTS: Using the specific primers for the Bcr-Abl translocation, we obtained the specific translocation product in cDNA sample of K562 human erythroid leukemia cell line. qRT- PCR showed significant sensitivity with the detection threshold for the Bcr-Abl fluorescent signal, which enabled the precise detection that was accurate within a 10-fold dilution range, and a dynamic range of 5 orders of magnitude. CONCLUSION: The results of our study showed that the application of the qRT-PCR is the optimal method for the detection of Bcr-Abl gene translocation, characterized by high reproducibility, specificity and sensitivity.

Zinc-oxide powder mixed with eugenol or cariofillorum oil and sterilized cotton is frequently used in dental practice as surgical bandage for eliminating or dry socket. Osteonecrosis of alveolar bone caused by free eugenol is rare but possible complication. The aim of this paper is to present a patient with maxillary osteonecrosis complicated with maxillary sinusitis caused by a surgical paste of zinc-oxide. A 47-year-old female patient was admitted to our department after unsuccessful treatment of dry socket with a paste of zinc-oxide. Intense use of zinc-oxide paste as bandage over the extraction sockets caused two separately osteonecrosis of maxillary alveolar bone. Surgical resection of necrotic bone, extraction of adjacent teeth and revision surgery of maxillary sinus was treatment of choice. Postoperative course was uneventful and 6 months postoperatively there weren't present further signs of maxillary necrosis. In preparation of a paste of zinc-oxide as surgical bandage it is necessary that all the powder be filled up with intense mixing with liquid. In present case osteonecrosis was caused by free eugenol in surgical zinc-oxide bandage as well as due to the long term and frequently used of zinc-oxide. In resistant dry sockets we recommended combined method of treatment and not only the use of zinc-oxide paste especially if there was no analgesic and therapeutic effects.

Deep vein thrombosis is important social and health care problem for its high incidence, pulmonary embolism-related mortality, and long-term sequelae which may be disabling (post-thrombotic syndrome and ulceration). The aim of our work was to determine the diagnostic potential and importance of D dimer in the early detection of deep vein thrombosis of lower extremities. Prospectively we have analyzed a group of 47 patients who were admitted to vascular department of Surgical Clinic in Nis during the period from 1.8.2001. to 31.3.2003. with echosonographically verified deep vein thrombosis of no more than 48 hours of duration. D dimer was measured before anticoagulant treatment started using commercial Nycomed test. The activation of coagulation process was detected with increase in D dimer values in 44 out of 47 examined patients (93.61%). D dimer is highly sensitive diagnostic tool that should be routinely used in the patients with suspected deep vein thrombosis of leg.

A mixed matrix modifier of nickel and strontium nitrates was used as a chemical modifier for the determination of selenium in wines by Zeeman electrothermal atomic absorption spectrometry. Wine samples were heated on a boiling water bath with small amounts of nitric acid and hydrogen peroxide. For complete elimination of interference, especially from sulfates and phosphates, selenium is complexed with ammonium pyrolidinedithiocarbamate (APDTC), extracted into methyl isobutyl ketone (MIBK), and measured by ETAAS. The graphite furnace temperature program was optimized for both aqueous and organic solutions. Pyrolysis temperatures of 1300 degrees C and 800 degrees C were chosen for aqueous and organic solutions, respectively; 2700 degrees C and 2100 degrees C were used as optimum atomization temperatures for aqueous and organic solutions, respectively. The optimum modifier mass established is markedly lower than those presented in the literature. The platform atomization ensures pretreatment stabilization up to 1100 degrees C and 1600 degrees C, respectively, for organic and aqueous selenium solutions. The procedure was verified by the method of standard addition. The investigated wine samples originated from the different regions of the Republic of Macedonia. The selenium concentration varied from not detectable to 0.93 microg L(-1).

A comparative study was conducted with soybean material presenting symptoms of the Diaporthe/Phomopsis complex that was collected in two distant geographical regions of the world: Beltsville, MA, USA, and Vojvodina, Yugoslavia. Contrasting with earlier findings, great variability in the disease symptoms was observed, and one or more Phomopsis species could be isolated from lesions presenting similar characteristics. Among the thirty-three isolates obtained from the lesions the following species were identified: D. phaseolorum var. caulivora, P. phaseoli (teleomorph D. phaseolorum var. sojae, rare), P. longicolla (found for the first time on the soybean fields of Yugoslavia), Phomopsis sp., and one culture showing intermediate characters of D. phaseolorum var. caulivora and D. phaseolorum var. sojae. Much diversity was also found in the cultural characters of the the isolates from both localities, presumably indicating evolutionary and adaptation processes.

Thallium(III) forms a ternary complex of yellow colour with iodoacetic acid and organic bases, especially hexamethylenetetramine. The complex is soluble in water and various organic solvents. Zinc, copper, cadmium and iron do not form complexes with these reagents, so that a spectrophotometric determination of thallium in zinc and its alloys is possible. The limit of detection is 15 microg g(-1), the molar absorption coefficient about 2000 mol(-1) l(-1) cm(-1).

OBJECTIVE: To assess the safety and effectiveness of leflunomide versus placebo in patients with active rheumatoid arthritis (RA) treated for 6 months. METHODS: Four hundred two patients were randomly assigned to receive placebo or leflunomide at 5 mg, 10 mg, or 25 mg daily. A washout period of 6-12 weeks from prior second-line therapy was required. RESULTS: Statistically significant improvement in primary and secondary outcome measures, as well as by responder analyses, occurred in the 10-mg and 25-mg dosage groups compared to placebo. Twenty-one patients (7.0%) in the active treatment groups withdrew due to adverse events (AEs). The incidence of AEs was higher with leflunomide than with placebo. Gastrointestinal symptoms, weight loss, allergic reactions, skin rash, and reversible alopecia were more common in the 10-mg and 25-mg dosage groups. The incidence of infections was similar between the treatment and placebo groups; no opportunistic infections were seen. Transient elevations in liver function studies were noted in a small number of patients. CONCLUSION: Leflunomide is effective in daily doses of 10 mg and 25 mg in patients with active RA. Improved efficacy at the 25-mg dose was associated with a higher incidence of AEs. Randomized, placebo-controlled trials using daily doses of 10 mg and 20 mg are under way in the US and Europe to confirm these positive results.

Stress response measured as the cortisol secretion rate as well as urinary 17 oxogenic steroid (17 OGS) and 17 oxosteroid (17 OS) excretion was investigated in a group of 28 patients with stroke (11 with hypertensive intracerebral hemorrhage and 17 with ischemic cerebrovascular insult). Significantly higher cortisol secretion values were found in the group of patients who died (p less than 0.01). Similarly, the first day urinary 17 OGS excretion pointed to the greater adrenocortical response in patients who died (p less than 0.05). Urinary 17 OS excretion was normal or below normal in the majority of patients and no difference was found between either male or female patients who survived or died. Our findings indicate that adrenocortical changes which maximize the production of cortisol are operative soon after the onset of stroke. The cortisol secretion rate appeared to be a good indicator of the severity of the stress caused by stroke and may be useful in predicting the prognosis of the illness.
