UNLABELLED: There is an urgent need to create adapted for the Russian population of the recommendations in order to improve the efficiency and automation of the process of informing patients about the correct (sanogennykh) behavior in the heat. The proposed health WHO recommendations in 2010 year are difficult to perception, understanding and subsequent patient use without consulting a doctor. THE GOAL: creating and testing adapted for the Russian population recommendations for sanogennykh behavior during the summer heat on the basis of the recommendations of the WHO in 2010. MATERIALS AND METHODS: 155 persons older than 30 years, 93 people were adapted recommendations (group 1), 62--"classic" WHO recommendations 2010 (group 2). The groups were comparable in terms of the main clinical and demographic characteristics. Results: in the summer 2014 heat waves observed 2: 4 and 10 days, the maximum temperature of 32.7 °C. On the "deterioration of health during a heat wave", complained 20.6% of patients receiving tailored recommendations, and 40%--classic (p = 0.008). The number of cardiovascular complications in group 1 was 0.0 (0.3, 1.4) in group 2--0.0 (0.6, 2.4; p = 0.000). Most effective recommendations noted patients with coronary heart disease and hypertension (87.5%; 9 of them were adapted recommendations 4--"classic") and chronic heart failure (80%; all they got adapted recommendations). CONCLUSION: customized recommendations sanogennykh behavior during heat waves can be recommended for use in clinical practice for patients with cardiovascular diseases.

AIM: To study the impact of cold waves on disease course, hemodynamics, lipid and carbohydrate metabolisms, oxidative stress, and blood rheological properties in patients with cardiovascular diseases (CVD). SUBJECTS AND METHODS: 24 men and 36 women (their mean age was 62.9±9.7 years) were examined; coronary heart disease (CHD) and hypertension were present in 40 and 95% of the patients, respectively; selected therapy remained unchanged throughout the entire period. The investigators measured blood pressure and pulse wave velocity (PWV), carried out biochemical blood tests, estimated plasma oxidized low-density lipoproteins (oxLDL) and malondialdehyde (MDA) and erythrocyte superoxide dismutase (SOD) activity, calculated a MDA/SOD ratio, determined blood viscosity; as well as assessed quality of life using a visual analogue scale (VAS) and a specially developed questionnaire. RESULTS: Female sex, CHD, type 2 diabetes mellitus (DM-2) were independent predictors of cardiovascular events (CVEs) in the frost period. The persons who had experienced CVEs in frost had higher baseline PWV. CVEs, such as hypertensive crisis, emergency calls, cardiac arrhythmias, and the larger number of adverse reactions, were more commonly recorded in frost. There was an increase in blood glucose levels, a decrease in oxLDL, a rise in η2/η1, and a reduction in plasma viscosity during frost and elevated glycation end product levels at visit 2. Conclusion. The cold wave is associated with the larger number of CVEs in some patients with CVD during selected therapy. CHD, DM-2, female sex are independent predictors of CVE in patients with CVD during the winter period. In this period, there were increases in the levels of glucose, glycation end products, and erythrocyte aggregation, and a reduction in plasma viscosity.

The performance of a great variety of electronic devices--ranging from semiconductor transistors to superconducting qubits--is hampered by low-frequency noise with spectra proportional to 1/f. The ubiquity and negative impact of 1/f noise has motivated intensive research into its cause, and it is now believed to originate from a bath of fluctuating two-level defect states (TLSs) embedded in the material. This phenomenon is commonly described by the long-established standard tunnelling model (STM) of independent TLS. A key prediction of STM is that the noise should vanish at low temperatures. Here we report measurements on superconducting microresonators over previously unattainable, very long time scales that show an increase in 1/f noise at low temperatures and low microwave power, contrary to the STM. We propose a new generalised tunnelling model that includes significant interaction between multiple TLSs, which fully describes these observations, as well as recent studies of individual TLS lifetimes in superconducting qubits.

Given that the effects of frost can play the role of independent stress factor influencing the course of cardiovascular disease (CVD), it is reasonable to supplementation of drugs that increase the body's resistance to cold stress. Aim: To evaluate the possibility of using meldonium to prevent unwanted seasonal changes in CVD patients in the winter. The study included 49 patients with CVD aged 38-75 years. Patients were randomized into 2 groups: active management (M), in which in addition to the basic therapy received during the winter 3 months meldonium 1000 mg/day, and a control (K). We measured office blood pressure, heart rate, blood chemistry, determination of glycosylation end products (DGP). Filled with a visual analogue scale (VAS) to assess the quality of life (QOL). During frost marked increase in blood glucose (p = 0.02) in group K, persisting throughout the winter, and an increase in tissue DGP in March (p = 0.002). In group M glucose and DGP not significantly raised. In group M at the peak of cold showed a reduction in cholesterol levels. Admission meldonium associated with improved quality of life, in the dynamics of the group K was negative [Δ +10.0 VAS scores in group M versus -7.5 points in the group K in the cold (p = 0.04) and Δ +10,0 points vs -5.0 points, respectively, in March 2014 (p = 0.055)]. Adding to the basic treatment of patients with CVD meldonium in a dose of 1000 mg/day in winter, accompanied by improved quality of life, as well as let negative changes in carbohydrate metabolism.

RESUMO

An analysis of results of treatment of 425 patients aged from 15 to 85 years with penetrating wounds of the neck was made. In 40 (8.1%) patients there were wounds of the cervical part of the esophagus, 29 of them had associated injuries. The proposed active surgical strategy consisted in a necessary thorough revision of the penetrating wounds of the neck using general anesthesia and then directly in the operation room using subsidiary methods of examination such as urgent x-ray analysis with a water-soluble contrast, esophagoscopy, larengotracheoscopy. This approach allowed detection of injuries of the cervical part of the esophagus, even in cases of the absence of clinical symptoms of esophagus injuries. In all the patients the wounds of the esophagus were sutured and followed by active aspiration drainage and antibacterial treatment with modern antimicrobial medicines.

RESUMO

The authors made an analysis of results of treatment of 339 patients with wounds and closed injuries of the neck. Laryngotracheal injuries were found in 27 (7.9%) patients, in 15 patients the injuries being of combined character. Active surgical strategy including the necessary thorough revision of the penetrating wounds of the neck during operation under general narcosis anesthesia, by instrumental methods of examination. Such strategy allowed timely detection and adequate elimination of injuries of the organs and vessels of the neck and improvement of the results of treatment of such patients.

RESUMO

The authors present the results of surgical treatment of 106 elderly and senile patients. In 30 patients admitted in a extremely severe condition the acute intestinal obstruction was combined with peritonitis. Postoperative mortality in this group of patients was 86.7% in spite of intensive treatment. In 28 patients generalization of the process was detected and they were given palliative interventions for liquidation of acute intestinal obstruction. Postoperative mortality in this group was 39.2%. Conventional radical operations were fulfilled on 48 patients. Postoperative mortality in this group was 20.8%. Later on the intestinal patency was reestablished in 15 patients. The data obtained show the possibility to widen the indications to radical operative interventions and to improve results of treatment of such patients.

We report on the observation of optical gain and lasing at the trion transition of n-doped ZnSe quantum wells. Specifically, the (stimulated) emission-absorption net rate of this transition is controlled by the difference of trion and electron occupation in momentum space. As the mass of the trion is larger than that of the electron, gain occurs on the low-energy side of the line center without degeneracy and inversion in the total particle numbers. The scenario is reminiscent of a three-level system. At higher injection levels, carrier heating sets on and limits the available gain to values of about 10(4) cm(-1).

RESUMO

The aim of this work is to describe a case of necrotising enteritis as a first presentation of a systemic lupus erythematosus (SLE). The patient has undergone two laparatomies with resection of a part of the small intestine because of a clinically relevant intestinal obstruction. Distended intestinal loops, inflammed serosa and mesenterial lymphadentis have been found during the operations. The histological examination has proved phlegmonous enteritis with hyperplasia of the lymph follicles and infiltration with plasmocytes, lymphocytes and eosinophils. Numerous biochemical, immunological, microbiological, ultrasound and X-ray investigations have not been helpful for the diagnosis. The appearance of a nephrotic syndrome in the course of the disease and the consequent renal biopsy have led to confirmation of the diagnosis SLE. The immunosupressive and corticosteroid treatment have resulted in fully recovery of the patient.

The effect of intraperitoneal administration of perfluorocarbon emulsion, an inducer of cytochrome P-450-dependent monoxygenase system of the liver, on the resistance of rodents to the action of CCl4 and organophosphorus pesticides was studied. Perfluorocarbon emulsion potentiated CCl4 toxicity decreasing LD50 from 4.5 to 3.7 mg/kg mouse body weight without changing susceptibility of rats to organophosphorus pesticides. A preliminary administration of perfluorocarbon emulsion effectively increased the protective action of antidotes (atropine + dipyroxime) providing the resistance of the animals to 12-fold, 20-fold and 20-fold LD50 of dichlophos, methaphos and butiphos, respectively.

A new type of system is made for determination of chemical pollutants in the air--indicator tubes with noncylindrical indicator filling--a subject of invention. The effect of the noncylindrical form on the metrological characteristics of the indicator tubes is studied. The noncylindrical filling is a way to influence these characteristics without changing the chemical receipt. There are a number of advantages: increasing more of 10x the sensitivity; linearization of the logarithmic characteristics of the indication dusts; possibility to lay two contrary scales on the conical indicator tubes for sucking respectively from the one or the other side of the asymmetric indicator filling, thus realising opportunities for defining of very low or very high accuracy. A new type of sampling system is established in practice. The dependences are given graphically.
