RESUMO

This study deals with the characterization of low alloyed steels of different yield strengths (varying in the range of 235-1100 MPa) via Barkhausen noise emission. The study investigates the potential of this technique to distinguish among the low alloyed steels and all significant aspects contributing to Barkhausen noise, such as the residual stress state, microstructure expressed in terms of dislocation density, grain size, prevailing phase, as well as associated aspects of the domain wall substructure (domain wall thickness, energy, their spacing and density in the matrix). Barkhausen noise in the rolling as well as transversal direction grows along with the yield strength (up to 500 MPa) and the corresponding grain refinement of ferrite. As soon as the martensite transformation occurs in a high strength matrix, this evolution saturates, and remarkable magnetic anisotropy is developed when Barkhausen noise in the transversal direction grows at the expense of the rolling direction. The contribution of residual stresses as well as the domain wall thickness is only minor, and the evolution of Barkhausen noise is driven by the density of the domain walls and their realignment.

Abdominal obesity represents an independent risk factor for subsequent severe cardiovascular events. It is one of the important diagnostic criteria for Metabolic Syndrome whose predictive value for severe cardiovascular events is similar to that of elevated LDL-cholesterol levels. The prevalence of abdominal obesity in the context of Metabolic Syndrome still has not been systematically monitored in Slovakia. The IDEA program evaluated a sample of 4183 patients in 103 centres in Slovakia. It has shown that only one in three adult inhabitants of Slovakia has a normal body weight. Almost every second inhabitant of Slovakia has intraabdominal obesity (by ATP III criteria). Obesity was recorded in every third woman (34.7 %) and in every fourth man (27.0 %). The prevalence of the principal risk factors (hypertension, hyperlipoproteinemia and diabetes mellitus) grew in proportion to the increase in waist circumference.

BACKGROUND: HLA class II gene pattern and IA-2A antibody positivity, revealing selected risks for prediction of type 1 diabetes mellitus (DM1), were previously shown to be cycling roughly in parallel with seasonally fluctuating frequencies of births of future diabetic children. Is this also true for insulin genotyping? Is this also true for births of the healthy close relatives of children with DM1? PATIENTS AND METHODS: In 98 Slovak children with DM1 and in 60 healthy parents and siblings, two single nucleotide polymorphisms of the insulin gene (-23 Hph I; +1127 Pst I) were typed. Results were expressed as score 0 (maximal risk) up to 2 (minimal risk or protection), or 0-4 for the sum of both scores. Birth seasonality of these risk scores was tested by Halberg cosinor regression. RESULTS: Results were similar for the two alleles; therefore, the summed risk score was evaluated. The DM1 risk from the insulin gene pattern in births of future diabetics cycled significantly semi-annually, with maxima around the spring and autumn equinoxes. In the relatives, a significant annual and extremely pronounced quarterly peaking rhythm was found, with maximum risk in births around the autumn equinox and shortly after the winter solstice. Significant protection peaks are present in late winter, spring and summer. CONCLUSION: The highest frequency of Coxsackie infections in the Slovakian child population, and of 'diabetic' births and manifestations, found earlier together with maximal risks from HLA II and IA-2A antibodies in late summer and early autumn, is now confirmed also for the insulin gene pattern in diabetic children and even also for their healthy close relatives.

RESUMO

The aim of this work was to study and monitor the patterns in incidence of childhood type 1 diabetes in Slovakia. The incidence of type 1 diabetes per 100,000/year from 1985 to 2000 was determined in children 0 - 14 years old. A total of 1,818 cases were diagnosed during this time. Poisson regression models were used to study incidence trends. The overall incidence of type 1 diabetes varied from 5.62/100,000 per year in year 1986 to 14.46/100,000 in year 1998. This represents a 2.57-fold increase. The incidence increased with age and was the highest among children 10 - 14 years of age. The incidence of type 1 diabetes in Slovakia is increasing very dramatically, especially in age group 0 - 4 years.

Malate dehydrogenases (MDH, L-malate:NAD oxidoreductase, EC 1.1.1.37), catalyze the NAD/NADH-dependent interconversion of the substrates malate and oxaloacetate. This reaction plays a key part in the malate/aspartate shuttle across the mitochondrial membrane, and in the tricarboxylic acid cycle within the mitochondrial matrix. They are homodimeric molecules in most organisms, including all eukaryots and the most bacterial species. The enzymes share a common catalytic mechanism and their kinetic properties are similar, which demonstrates a high degree of structural similarity. The three-dimensional structures and elements essential for catalysis are conserved between mitochondrial and cytoplasmic forms of MDH in eukaryotic cells even though these isoenzymes are only marginally related at the level of primary structure.

The practice environment of healthcare in the United States is fluid, dynamic, and sometimes chaotic. Statutory, regulatory, and credentialing requirements are critical elements of this changeable environment affecting clinical nurse specialists (CNSs) and other advanced practice nurses (APNs). CNSs are estimated to comprise approximately 54,000 APNs, based on the 2000 National Sample Survey of Registered Nurses. Although statutes and regulations related to APNs are reviewed regularly, there has not been a critical analysis of statutes and regulations specifically governing CNS practice in the United States. This lack is largely due to the absence of a national association devoted to CNSs before 1995. However, recently there has been a resurgence of interest and need for CNS services. Therefore, it is particularly important that any statutory and/or regulatory barriers to CNS practice be removed. In 1998, the National Association of Clinical Nurse Specialists Legislative and Regulatory Committee embarked on a critical analysis of state statutes and regulations governing CNS practice. All 50 state boards of nursing and the District of Columbia board of nursing were requested to send copies of their statutes and regulations governing CNS practice. Responses were received from 48 states. This article summarizes the state of regulation of CNS practice and identifies significant barriers to CNS practice that should be systematically addressed. The patchwork quilt of CNS regulation that varies from state to state results in over-restrictive and underinclusive provisions that preclude ease of reciprocity and deprive the public full access to CNS services. Recommendations for addressing the barriers are made.

RESUMO

The mode, location, and focus of health care services are changing rapidly, especially delivery of psychiatric services. The high prevalence of psychiatric and medical comorbidity, the national shift in health care to a managed care arrangement with one professional designated as principle provider, and problems with access to comprehensive services for individuals with psychiatric problems interact to create a compelling need for a clear definition of advanced psychiatric-mental health nursing practice. This article is, in part, a response to the national dialogue and debate sparked by the beginning development of a psychiatric nurse practitioner certification exam. However, this debate will be used merely as a starting point to articulate and document the need for a flexible, diverse, and evolving definition of advanced psychiatric-mental health nursing practice that can inform and shape educational programs in the discipline.
