STATEMENT OF PROBLEM: The congruence of surgical implant placement with the preplanned position is important for anatomic and prosthetic precision, minimal complications, and increased longevity. The influence of implant placement timing on the surgical deviations in single posterior implants is unclear. PURPOSE: The purpose of this clinical study was to compare deviations between preplanned and single posterior immediate and delayed implants placed using computer-guided digital light processing (DLP) surgical guides fabricated using intraoral scanning and cone beam computed tomography (CBCT). MATERIAL AND METHODS: Implant surgery was performed on 24 participants requiring single immediate or delayed implants in the posterior maxillary and mandibular regions, for which the surgical site data were obtained from CBCT and intraoral scanning. Subsequently, virtual implant planning and DLP surgical guides were fabricated. Preimplant and postimplant placement CBCT scans were overlapped, and mean deviations for the immediate and delayed implant groups were calculated. The groups were compared with unpaired t tests (α=.05). RESULTS: A total of 24 implants were placed, 12 in each group. In participants who received immediate implant placement, the mean ±standard deviation angular deviation, linear deviation at shoulder, linear deviation at apex, and vertical deviation were 1.03 ±0.70 degrees, 0.26 ±0.30 mm, 0.23 ±0.24 mm, and 0.39 ± 0.34 mm, respectively. In participants who received delayed implant treatment, the deviations were 0.53 ±0.60 degrees, 0.15 ±0.18 mm, 0.25 ±0.33 mm, and 0.17 ±0.10 mm, respectively. Significant differences between the 2 groups were found in the vertical deviation (P<.05). CONCLUSIONS: The timing of the single posterior placement was associated with different deviations in the vertical direction. All deviations obtained were below the recommended values. DLP surgical guides fabricated from intraoral and CBCT scans provided accurate implant placement in immediate and delayed single posterior implants.

Squamous odontogenic tumor is a rare epithelial odontogenic tumor. Only a few cases have been reported in the literature, out of which central cases occurring within the jawbones are more common, and very rarely peripheral cases have been reported. Here, we report a rare case of a peripheral squamous odontogenic tumor occurring in the left lower gingivobuccal sulcus region in a 63-year-old female patient and presented as a firm and fibrotic swelling clinically. Radiographic examination revealed no evidence of a central lesion in the bone. Excisional biopsy of the lesion revealed the characteristic histopathological picture of peripheral squamous odontogenic tumor (PSOT). The healing was found to be satisfactory upon periodic evaluation of the patient done to study the healing and for any recurrence of the lesion.

BACKGROUND: Post-Covid mucormycosis is associated with very high morbidity and reasonably high mortality. The three main causes seem to be covid infection, excessive steroid usage and uncontrolled diabetes. METHODS: Of the three risk factors causing post-Covid mucormycosis, the only risk factor which can be modified quickly is uncontrolled diabetes. Checking urine-sugar levels of susceptible population asymptomatic for mucormycosis can be done rapidly and at a relatively low cost. Urine sugar positive persons can then be investigated and managed for uncontrolled diabetes. RESULTS: None CONCLUSIONS: Mass urine sugar testing to assess and regulate diabetes(MUSTARD) can help identify and manage uncontrolled diabetes in populations with high prevalence of diabetes.

RESUMO

Fibrodysplasia ossificans progressiva (FOP) is a rare autosomal dominant disease. It is characterized by widespread soft tissue ossification and congenital stigmata of the extremities, affecting all ethnic backgrounds. The worldwide reported prevalence is approximately 1/2,000,000. Based on history and clinicoradiological findings, FOP should be diagnosed as early as possible and noninvasively. The hallmark of diagnosis of FOP is bilateral great toe anomaly present from birth. Misdiagnosis may lead to inadvertent managements, such as manipulations, biopsies, and surgery. Surgery, till now, does not seem to have any role in the management of this condition and may lead to further trauma and disease progression. FOP may be precipitated due to trauma to muscle. In masticatory muscle, it mainly affects masseter muscle and presents with symptom of trismus. Herein, we present a case of FOP which presented to us with trismus after wooden thorn injury and immobility of the left leg. This article also emphasize on diagnosis, precaution, and treatment of disease.

RESUMO

The calcifying epithelial odontogenic tumour (CEOT) is a rare but well recognized benign odontogenic tumour for any Pathologist. Histologically, it shows sheets of polyhedral epithelial cells with prominent intercellular bridges, hyperchromatic nuclei and globules of amyloid-like material among tumour cells which undergoes calcification. We present a case of CEOT with classical histopathological picture in a 37-year-old HIV positive patient with long evolution history. Its association with HIV positivity, long clinical history and delay in surgical treatment make it a challenging case to report.

RESUMO

Prosthetic rehabilitation of atrophic maxilla and large maxillary defects can be done successfully by zygomatic implant-supported prosthesis. Zygomatic implants are an avant-garde to complex and invasive-free vascularised osteocutaneous flaps, distraction osteogenesis, and the solution to flap failures. A treated case of tuberculous osteomyelitis, with a class II (Aramany's classification) maxillary defect, reported to oral maxillofacial department, Government Dental College (RUHS-CODS). The defect in this group was unilateral, retaining the anterior teeth. The patient was previously rehabilitated with a removable maxillary obturator. Inadequate retention affected essential functions like speaking, mastication, swallowing, esthetics, and so on due to lack of sufficient supporting tissues. A fixed prosthetic rehabilitation of posterior maxillary defect was done with obturator supported with two single-piece zygomatic implants. At 1-year follow-up, the patient was comfortable with the prosthesis, and no further complaints were recorded.

RESUMO

Patients taking anticoagulant medication pose a challenge for the clinician. Dentists are often required to manage bleeding as part of routine oral surgery or dental procedures, and altered hemostasis can lead to complications. Nevertheless, use of these medications is generally important for the patient's health and any alteration in the anticoagulant regimen may have untoward sequelae. In addition, several medications can affect the clotting mechanism, potentially compromising hemostasis. This article will review a variety of anticoagulant medications and the medical conditions that necessitate their use.

RESUMO

Success of endodontic treatment depends on the proper identi-fication of all the canals, thorough chemomechanical preparation followed by three-dimensional obturation with hermetic seal. Failure of any of these steps may occur due to unusual tooth morphology. Usually mandibular molars have two roots with three canals but in few teeth, the number of roots and canals vary. The variation in the number of roots, if extra root located lingually called radix entomolaris (RE) or located bucally called radix paramolaris (RP). This article presents successful endo-dontic treatment of two mandibular frst molars with extra roots one is with radix entomolaris and other is with radix paramolaris, both of which are rare microstructures.

PURPOSE: To present a case series of primary intraosseous squamous cell carcinoma arising in odontogenic cysts and review the published data to understand the pathogenesis of the lesion. MATERIALS AND METHODS: The files of the Department of Oral and Maxillofacial Surgery, Government Dental College and Hospital (Jaipur, India) were reviewed, and 3 cases of primary intraosseous squamous cell carcinoma ex odontogenic cyst diagnosed from 2006 to 2011 were identified. The diagnostic inclusion criteria were the presence of a completely intraosseous lesion; the absence of ulceration of the oral mucosa, except when caused by other factors such as tooth extraction or trauma; and the absence of a distant primary tumor. The malignant transformation of the benign cystic lining of an odontogenic cyst to squamous cell carcinoma was identified, and the diagnosis of primary intraosseous squamous cell carcinoma ex odontogenic cyst was confirmed in all cases. The relevant English data were reviewed to determine the possible etiopathologic factors associated with the lesion. RESULTS: It was observed that long-standing chronic inflammation in the benign odontogenic cyst was the most probable etiopathologic factor responsible for malignant transformation of a benign cystic lining. CONCLUSIONS: The signs of chronic inflammation in odontogenic cysts should be taken into consideration, and such lesions should be treated promptly. By doing so, we might reduce the rate of malignant transformation of benign odontogenic cysts.
