RESUMO

Management of scaphoid nonunion after failed surgery for acute scaphoid fracture presents a unique treatment challenge. Prior surgery complicates patient evaluation and increases the technical difficulty of future procedures. Healing of nonunion is crucial to prevent carpal collapse and progressive arthritis. A thorough workup is required to identify technical factors or treatment decisions that may have resulted in a poor outcome after initial fixation attempts. CT is particularly useful for characterizing nonunion and planning revision surgery. Several studies have described the use of bone grafts and fixation devices for scaphoid nonunion repair, including nonvascularized and vascularized bone grafts, screws, pins, and plates. Reliable rates of union have been achieved using nonvascularized bone graft supplemented with screw or wire fixation, particularly in the absence of osteonecrosis. Although vascularized grafts are more technically challenging, they improve the odds of union in the setting of osteonecrosis.

BACKGROUND: The purpose of this study was to test the effect of (1) titanium versus stainless steel metal and (2) locked unicortical versus bicortical shaft fixation on stiffness of Less Invasive Stabilization System (LISS) constructs in a mechanically unstable nonosteoporotic distal femur fracture model. METHODS: An AO/OTA 33-A3 fracture was created in 24 sawbone femur specimens that simulated normal bone quality. Testing was performed on three groups: titanium unicortical LISS fixation, stainless steel unicortical LISS fixation, and bicortical stainless steel LISS fixation. Specimens were tested in axial, torsional, and cyclic axial loading in a materials testing machine. The outcome measurement was stiffness in axial and torsional loading, total deformation, and irreversible (plastic) deformation in cyclical axial loading. RESULTS: No difference was found in stiffness in axial loading or plastic and total deformation between all three groups. There was a small decrease in stiffness in torsional loading in the titanium group compared with the stainless steel groups (p < 0.0001) and a small increase in stiffness in torsional loading with bicortical versus unicortical proximal fixation (p = 0.04). CONCLUSIONS: Unstable distal femur fracture constructs, created in sawbone specimens, fixed with stainless steel and titanium LISS implants, with or without bicortical locking screws proximally, show similar biomechanical properties under loading. Although there are numerous considerations when selecting implants for these fractures, our results indicate that stiffness is not a significant factor in choosing between the constructs tested.

RESUMO

Plastic deformation of the forearm is a rare injury in young adults that occurs when a slow bending or rotational force is applied to the arm, most commonly in the setting of an industrial workplace accident. There are currently no guidelines for treatment of the residual forearm deformity that often results in limitations of forearm supination and pronation. We present 2 cases demonstrating that deformity correction with single cortex, double-level osteotomies combined with rigid plate fixation and early range of motion exercise that results in good functional outcomes.

This paper reviews the relationships between trance or altered state effects, suggestibility, and expectancy as these concepts are defined in the theorizing of Weitzenhoffer (2002), Holroyd (2003), Kirsch (1991), and others, for the purpose of demonstrating how these concepts can be assessed with the PCI-HAP (Phenomenology of Consciousness Inventory: Hypnotic Assessment Procedure; Pekala, 1995a, b). In addition, how the aforementioned variables may relate to the nature of hypnosis/hypnotism as a function of self-reported hypnotic depth are discussed, along with how the PCI-HAP may be used as a means to measure hypnotic responsivity from a more phenomenological state perspective, in contrast to more traditional behavioral trait assessment instruments like the Harvard, the Stanford C, or the HIP. A follow-up paper (Pekala, Kumar, Maurer, Elliott-Carter, Moon, & Mullen, 2010) will present research data on the PCI-HAP model and how this model can be useful for better understanding hypnotism.

This study sought to determine if self-reported hypnotic depth (srHD) could be predicted from the variables of the Phenomenology of Consciousness Inventory - Hypnotic Assessment Procedure (PCI-HAP) (Pekala, 1995a, 1995b; Pekala & Kumar, 2007; Pekala et al., 2010), assessing several of the processes theorized by researchers to be associated with hypnotism: trance (altered state effects), suggestibility, and expectancy. One hundred and eighty participants completed the PCI-HAP. Using regression analyses, srHD scores were predicted from the PCI-HAP pre-hypnotic and post-hypnotic assessment items, and several other variables. The results suggested that the srHD scores were found to be a function of imagoic suggestibility, expectancy (both estimated hypnotic depth and expected therapeutic efficacy), and trance state and eye catalepsy effects; effects that appear to be additive and not (statistically) interactive. The results support the theorizing of many investigators concerning the involvement of the aforementioned component processes with this particular aspect of hypnotism, the self-reported hypnotic depth score.

Positive and negative affect generated while using the Phenomenology of Consciousness--Hypnotic Assessment Procedure (PCI-HAP) on a sample of drug and alcohol users were predicted using several variables. The results were then cross-validated on a second, smaller sample. The results suggest that, although some negative affect was reported, the PCI-HAP was more likely to generate positive, rather than negative, affect. Positive affect was related to the vividness of a suggested hypnotic dream during hypnosis and also hypnotic depth; these findings were replicated upon cross-validation. Although negative affect correlated with the Dissociative Experiences Scale scores and falling asleep, these results did not replicate upon cross-validation. Mild transient negative effects (e.g., headache) were reported by about 10% of the participants in a smaller, second sample. Implications of the results are discussed.

Treatment of the complex injury to the spine produced by a gunshot wound remains controversial. Treatment depends on the physician's ability to understand mechanism of injury, principles of medical management, diagnostic imaging, and surgical options. Antibiotics are an important component of treatment and should be continued for a minimum of 7 days in cases of wounds that both perforate the colon and injure the spine. Corticosteroids do not affect neurologic outcome and therefore should not be used. Decompression and removal of intracanal bullets at T12 and below may improve motor function. In select cases of cervical injuries, removal of intracanal bullet fragments may be justified, particularly with incomplete lesions. Regardless of injury level, new-onset or progressive neurologic deterioration is an indication for urgent decompression. Optimal surgical timing remains a controversial issue, and more study is needed to develop treatment guidelines. Intrathecal migratory missiles represent a very rare subset of the gunshot wounds to the spine, and their treatment should be individualized. In this article, we review the literature and then describe the case of a migratory intrathecal bullet in the lumbar spine of a patient who presented with cauda equina-type symptoms.

Procedures for estimating hypnotic depth have been used for more than 70 years. This study predicted self-reported hypnotic depth from the phenomenological and behavioral variables of the Phenomenology of Consciousness Inventory-Hypnotic Assessment Procedure (PCI-HAP). Participants were divided into 2 groups; 1 was used to generate regression equations, and the other group was used for cross-validation. Both imagery vividness during hypnosis (imagoic suggestibility) and the PCI pHGS measure of hypnotic depth (hypnoidal state) accounted for most of the variance in self-reported hypnotic depth. The above results, further supported by correlational and 3-D visual analyses, are consistent with other researchers' observations that ratings of hypnotic depth are a function of: (a) alterations in subjective experience, and (b) the perception of responsiveness to suggestions. The findings are also congruent with J. Holroyd's hypothesis that suggestibility and altered-state effects interact to produce hypnotic effects.

PURPOSE: Avascular necrosis (AVN) is one of the most serious complications of femoral neck fractures in children. Variability in the reported rates of AVN and lack of statistical evidence has minimized the prognostic power of individual studies. The purpose of our study was to review our own cases as well as those from the literature in an effort to identify the risk factors for AVN. METHODS: We conducted a computerized search of medical records to identify all children with femoral neck fractures treated at our institution from 1980 to 2003. Records were reviewed to determine fracture type (Delbet type I to IV), displacement, age, treatment, and incidence of AVN. A structured search of PubMed was also performed to identify all reports published in English, on femoral neck fractures in children, from 1965 to 2003. Hand searches of major orthopedic journals and reference lists of publications identified additional cases. Of the 275 citations initially identified through our computerized search, 47 met the criteria for further evaluation. These citations were reviewed by 2 investigators, and data was abstracted from 20 reports that provided patient-level data and met our criteria for inclusion. RESULTS: Twenty-four patients with 25 femoral neck fractures from our institution were identified including 12 boys and 12 girls with an average age of 8 years (range 1.5 to 16). Over 300 patients who met our study criteria were also identified from the literature, and a total of 360 patients were included in the analysis. Fracture type, displacement, age, and treatment were all statistically significant independent predictors of AVN with P values

This study evaluated the effectiveness of a self-hypnosis protocol with chronic drug and alcohol patients in increasing self-esteem, improving affect, and preventing relapse against a control, a transtheoretical cognitive-behavioral (TCB), and a stress management (attention-placebo) group. Participants were 261 veterans admitted to Substance Abuse Residential Rehabilitation Treatment Programs (SARRTPs). Participants were assessed pre- and postintervention, and at 7-week follow-up. Relapse rates did not significantly differ across the 4 groups at follow-up; 87% of those contacted reported abstinence. At follow-up, the participants in the 3 treatment conditions were asked how often they practiced the intervention materials provided them. Practicing and minimal-practicing participants were compared against the control group for each of the 3 interventions via MANOVAs/ANOVAs. Results revealed a significant Time by Groups interaction for the hypnosis intervention, with individuals who played the self-hypnosis audiotapes "at least 3 to 5 times a week" at 7-week follow-up reporting the highest levels of self-esteem and serenity, and the least anger/impulsivity, in comparison to the minimal-practice and control groups. No significant effects were found for the transtheoretical or stress management interventions. Regression analyses predicted almost two-thirds of the variance of who relapsed and who did not in the hypnosis intervention group. Hypnotic susceptibility predicted who practiced the self-hypnosis audiotapes. The results suggest that hypnosis can be a useful adjunct in helping chronic substance abuse individuals with their reported self-esteem, serenity, and anger/impulsivity.

The process of vascular smooth muscle cell (vSMC) differentiation is critical to embryonic angiogenesis. However, despite its importance, the vSMC differentiation program remains largely undefined. Murine gene disruption studies have identified several gene products that are necessary for vSMC differentiation, but these methodologies cannot establish whether or not a factor is sufficient to initiate the differentiation program. A gain-of-function system consisting of normal vSMC progenitor cells would serve as a useful complement to whole animal loss-of-function studies. We use such a system here, namely freshly isolated rat neural crest stem cells (NCSCs), to show that activation of the calcineurin signaling pathway is sufficient to drive these cells toward a smooth muscle fate. In addition, we present data suggesting that transforming growth factor (TGF)-beta1, which also causes NCSCs to differentiate into smooth muscle, activates calcineurin signaling in NCSCs, leading to a model in which activation of calcineurin signaling is the mechanism by which TGF-beta1 causes SMC differentiation in these cells.
