RESUMO

While immigrants in the US suffer poor access to healthcare in general, access within immigrant populations varies notably by legal status and employment. Intersections between immigration, employment, and healthcare policy have shaped immigrants' access or exclusion from healthcare; however, little research has examined how immigrants experience and navigate these intersections. Drawing on social exclusion theory and the theory of bounded agency, we aimed to investigate Mexican and Chinese immigrants' experiences of exclusion from healthcare as one key dimension of social exclusion-and how this was shaped by interactions with the institutions of immigration and employment. The examination of two ethnic immigrant groups who live under the same set of policies allows for a focus on the common impacts of policy. We selected Mexican and Chinese immigrants as the two largest subgroups in California's Latinx and Asian immigrant population. We use a policy lens to analyze qualitative data from the mixed-methods Research on Immigrant Health and State Policy (RIGHTS) Study, involving 60 in-depth interviews with Mexican and Chinese immigrants in California between August 2018-August 2019. We identified two primary themes: pathways of social exclusion and access, and strategies used to address social exclusion. Findings show that immigrants' exclusion from healthcare is fundamentally linked to legal status and employment, and that immigrants navigate difficult choices between opportunities for improved employment and changes in legal status. We argue that multiple categories of legal status affect immigrants' employment opportunities and social position, which, in turn, translates to stratified healthcare access. Our findings support the literature establishing legal status as a mechanism of social stratification but challenge legal-illegal binary paradigms.

RESUMO

OBJECTIVE: The purpose of this report was to explore the views of chiropractors about their career paths and recommendations for Chiropractic Day. METHODS: For this narrative inquiry, selected individuals from diverse backgrounds were invited to share their experiences about becoming chiropractors and their recommendations for Chiropractic Day. Each doctor of chiropractic was asked to provide a response to a set of structured questions. The responses were collated into a descriptive narrative, and themes were identified from these narratives. RESULTS: Five chiropractors from diverse backgrounds, who graduated from chiropractic programs in the United States, participated in this project. Each reported a different path to becoming a chiropractor. From these different career paths, common themes emerged, which included (1) a drive to serve others, (2) a personal experience or quest, (3) and a propensity to be problem solvers and inquisitive learners. Their recommendations for celebrating Chiropractic Day paralleled their individual qualities and values, which included providing educational seminars to patients and other health care providers, mentoring students, reading scientific papers about health issues related to diversity, providing chiropractic services to underserved populations, appreciating our predecessors and mentors, and performing community outreach. CONCLUSION: This narrative provides insight into the thoughts of people from diverse backgrounds about why they may choose chiropractic as a profession and their paths to a career in chiropractic. This article demonstrates that chiropractors come to the profession for a variety of reasons but may have similar themes such as that the choice may generate from a personal experience, a quest for knowledge, an interest in helping others, and a joy for service. This narrative provides unique insights about the authors' values for chiropractic, which were congruent with their recommendations for celebrating Chiropractic Day.

RESUMO

OBJECTIVE: to explore the perception and experience of fathers of children with disabilities in caring for their children; to know their role and how these tasks impact their daily life, health and physical, mental and/or emotional well-being. METHODOLOGY: qualitative approach study with phenomenological design. The sample consisted of seven young fathers of underage children with various diagnoses. The data were collected through semi-structured interviews; the discourse analysis was carried out through open and axial coding processes. Three themes emerged from the results of the analysis: (1) shared responsibilities, (2) somewhat difficult to fit in, and (3) either you join or you split. RESULTS: Fathers must readjust their work schedule, reduce their working hours, or give up their job altogether to take care of their children, as well as give up their social life. They lack time to enjoy their leisure time, to spend time with their partner, to take care of themselves. This involvement in caregiving generates an important occupational imbalance that has repercussions above all on their mental health. CONCLUSIONS: The sharing of caregiving tasks also impacts significantly on parents' lives, it also takes away time and opportunities, and sometimes health and quality of life.

RESUMO

AIM: To examined the efficacy and safety of treatment with boceprevir, PEGylated-interferon and ribavirin (PR) in hepatitis C virus genotype 1 (HCVGT1) PR treatment-failures in Asia. METHODS: The Boceprevir Named-Patient Program provided boceprevir to HCVGT1 PR treatment-failures. Participating physicians were invited to contribute data from their patients: baseline characteristics, on-treatment responses, sustained virological response at week 12 (SVR12), and safety were collected and analysed. Multivariate analysis was performed to determine predictors of response. RESULTS: 150 patients were enrolled from Australia, Malaysia, Singapore and Thailand (Asians = 86, Caucasians = 63). Overall SVR12 was 61% (Asians = 59.3%, Caucasians = 63.5%). SVR12 was higher in relapsers (78%) compared with non-responders (34%). On-treatment responses predicted SVR, with undetectable HCVRNA at week 4, 8 and 12 leading to SVR12s of 100%, 87%, and 82% respectively, and detectable HCVRNA at week 4, 8 and 12, leading to SVR12s of 58%, 22% and 6% respectively. Asian patients were similar to Caucasian patients with regards to on-treatment responses. Patients with cirrhosis (n = 69) also behaved in the same manner with regards to on-treatment responses. Those with the IL28B CC genotype (80%) had higher SVRs than those with the CT/TT (56%) genotype (P = 0.010). Multivariate analysis showed that TW8 and TW12 responses were independent predictors of SVR. Serious adverse events occurred in 18.6%: sepsis (2%), decompensation (2.7%) and blood transfusion (14%). Discontinuations occurred in 30.7%, with 18.6% fulfilling stopping rules. CONCLUSION: Boceprevir can be used successfully in PR treatment failures with a SVR12 > 80% if they have good on-treatment responses; however, discontinuations occurred in 30% because of virological failure or adverse events.

Sex education starts from home and family since parents arethe first educators of their own children. They also have the right to make decisions related to sexualitywithout suffering from discrimination, violence, or abuse. The date from the 2009 National Health Surveyshows that 55.2% of adolescents between 15 and 19 have had sex. About two million adolescents sufferfrom Aids worldwide. The critical factor to reduce morbidity and mortality among adolescents and yourchild is timely and thorough prenatal care. Education, from childhood as well as schooling, is a main baseto help successfully guide and teach children about all issues related to sexuality.

RESUMO

Purification of the root extract of Lonchocarpus xuul resulted in the isolation and identification of dihydrospinochalcone A (1) and epi-flemistrictin B (2) as additional natural isocordoin derivatives. Identification was based on the analysis of their spectroscopic data and by comparing these with those of previously reported metabolites.

RESUMO

The Psychology as Science scale was used to assess whether Brazilian students believe psychology is a science. 190 undergraduates from four universities in Brazil participated. Analysis showed that 54% of students strongly agree that psychology is a science, 26% strongly agree that psychological research is necessary and that training in methodology is important, and 25% strongly agree that behavior is predictable.
