RESUMO

Calcifying odontogenic cysts (COCs) exhibit a diverse clinical course, commonly developing between the second and third decades of life, displaying no gender predilection. A 15-year-old female without medical history was under observation for a mixed lesion in the maxilla associated with an impacted tooth. She presented to the emergency department with sudden onset and worsening swelling of the left midface. Radiographic findings in the panoramic radiograph and a CT scan revealed a well-circumscribed mixed lesion localized in the left maxilla, extending into the left maxillary sinus and reaching the orbital floor. After an intercurrent infection of the cyst, the patient was hospitalized, received intravenous antibiotics, and went for surgical intervention under general anesthesia. Lesions that combine histological characteristics of two or more odontogenic tumors or individual cysts in the same location are called hybrid odontogenic lesions. This type of lesion poses a challenge for both pathologists and surgeons, because of its controversial histogenesis and poorly understood clinical evolution. The most common of these lesions are COCs associated with odontoma. Our case represents an exceptionally rare entity among odontogenic cysts.

A network of 137 cultivated fields covering the wide diversity of soils, crop rotations and cropping practices throughout the region of Brittany (France) was monitored to collect data on soil organic nitrogen (SON) mineralization and to identify the factors that explain the observed variability. The dataset presented in this article contains all of the information about the soils, which were subjected to pedological description and in-depth analysis of their topsoil properties. The topsoil (0-30 cm) was sampled by mixing 30 samples to obtain one composite per field, which was divided into one sub-sample sieved at 5 mm to analyze soil microbial biomass (SMB) and SON mineralization via anaerobic incubation, and one subsample dried at 40 °C and sieved at 2 mm. The physico-chemical analyses included the particle-size distribution of five fractions; organic matter (OM); organic C; organic N; pH (water); pH KCl; CEC (Metson); CEC (hexamminecobalt); exchangeable Al, Ca, Fe, K, Mg, Mn and Na (hexamminecobalt); Olsen P; Dyer P; and total Al, Ca, Fe, K, Mg, Mn, Na and P. Physical OM fractionation was used to characterize the 200-2000 µm and 50-200 µm fractions of particulate organic matter (POM). Finally, three chemical methods were used to determine extractable organic nitrogen (EON): hot KCl, hot water and phosphate buffer tests. This dataset covers a wide range of pedological situations and cropping systems, and is of great interest to scientists searching for soil properties that can explain SON mineralization. It provides original data on EON indices, SMB and multiple forms of P. This paper supports and supplements information presented in a previous article [1].

Familial partial lipodystrophies (FPLD) are rare diseases characterized by selective loss of subcutaneous adipose tissue at different sites. This cross-sectional observational study aimed to estimate adipose tissue in the bone marrow (BMAT), intra (IMCL) and extra-myocyte lipids (EMCL), and define the bone phenotype in the context of FPLD2/Dunnigan syndrome (DS). The subjects comprised 23 controls (C) and 18 DS patients, matched by age, weight and height. Blood samples, dual-energy X-ray absorptiometry for bone mineral density (BMD) and trabecular bone score (TBS) and 1H-spectroscopy using magnetic resonance to estimate BMAT in the lumbar spine, IMCL, EMCL and osteoclastogenesis were assessed. The prevalence of diabetes mellitus was 78% in DS patients. Glucose, HbA1c, triglycerides, insulin and HOMA-IR levels were elevated in DS, whereas HDLc, 25(OH)D, PTH and osteocalcin levels were reduced. BMD was similar between groups at all sites, except 1/3 radius, which was lower in DS group. TBS was reduced in DS. DS presented increased osteoclastogenesis and elevated BMAT, with greater saturation levels and higher IMCL than the C group. HOMA-IR and EMCL were negatively associated with TBS; osteocalcin and EMCL were correlated negatively with BMD. This study contributes to refining the estimation of adipose tissue in DS by showing increased adiposity in the lumbar spine and muscle tissue. DXA detected lower TBS and BMD in the 1/3 radius, suggesting impairment in bone quality and that bone mass is mainly affected in the cortical bone.

Xilooligossacarídeos (XOS) são reconhecidos pelo seu potencial prebiótico relevante para diversos setores industriais e foram obtidos após o pré-tratamento hidrotérmico da biomassa lignocelulósica residual de galhos de eucalipto. Subprodutos inibitórios são gerados durante o processo de solubilização dos oligossacarídeos e acabam comprometendo a utilização do licor em microrganismos. Neste trabalho, o processo de destoxificação, hidrólise enzimática e atividade estimulantes de crescimento da bactéria Staphylococcus xylosus foram estabelecidos. Os resultados mostraram que a adsorção com carvão ativado em pó removeu cerca de 55% do ácido acético e mais de 90% do ácido fórmico, compostos fenólicos, lignina solúvel, furfural e 5-hidroximetilfurfural, e que a soma dos oligossacarídeos xilobiose (X2) e xilotriose (X3) foram maximizadas de 0,57 g/L para 1,21 g/L com 110 U/gXOS da enzima endoxilanase e 6,3% do licor destoxificado na hidrólise enzimática. O consumo de cerca de 63% de X2 e de 46% de X3 pela bactéria em meio basal deficiente em fontes de carbono, mas acrescido com os oligômeros, proporcionou maior crescimento celular em relação aos meios basais com alta composição de carbono, com e sem XOS, revelando seu potencial prebiótico pelo efeito estimulante de crescimento. Xylooligosaccharides (XOS) are recognized for their prebiotic potential relevant to several industrial sectors and were obtained after hydrothermal pretreatment of residual lignocellulosic biomass from eucalyptus branches. Inhibitory by-products are generated during the solubilization process of oligosaccharides and end up compromising the utilization of the liquor in microorganisms. In this work, the detoxification process, enzymatic hydrolysis and growth stimulating activity of Staphylococcus xylosus bacteria were established. The results showed that adsorption with powdered activated carbon removed about 55% of acetic acid and more than 90% of formic acid, phenolic compounds, soluble lignin, furfural, and 5-hydroxymethyl furfural and the sum of the oligosaccharides xylobiose (X2) and xylotriose (X3) were maximized from 0.57 g/L to 1.21 g/L with 110 U/gXOS of the enzyme endoxylanase and 6.3% of the detoxified liquor in the enzymatic hydrolysis. The consumption of X2 and X3 were about 63% and 46%, respectively, by the bacteria in basal medium deficient in carbon sources, but in medium added with the oligomers, provided higher cell growth compared to basal medium with high carbon composition, with and without XOS, revealing its prebiotic potential by its growth-stimulating effect. The hosing instability poses a feasibility risk for plasma-based accelerator concepts. We show that the growth rate for beam hosing in the linear regime (which is relevant for concepts that use a long driver) is a function of the centroid perturbation wavelength. We demonstrate how this property can be used to damp centroid oscillations by detuning the plasma response sufficiently early in the development of the instability. We also develop a new theoretical model for the early evolution of hosing. These findings have implications for the general control of an instability's growth rate.

This study examined the relationship between the quality of the kindergarten physical and social environment to promote physical activity (PA) and preschoolers' motor and social-emotional competence. Two Portuguese kindergartens (Gondomar city) were selected from a pool of seventeen with an assessment of kindergarten PA best practices (one with high PA practices, the other with low). Thirty-six children (M = 4.42; SD = 1.00 years) without neuromotor disorders participated in this study. Motor and social-emotional competence were assessed with standardized motor skills tasks and parent report of child behaviors. Children from the kindergarten with higher compliance with PA best practices showed significantly better motor competence. No statistically significant differences were found for social-emotional competence scores. These findings emphasize the critical importance of kindergarten in promoting preschoolers' motor competence by assuring a physical and social environment that enhances their PA practice. This is a particularly relevant concern for directors and teachers during the post-pandemic period, given the developmental delays and decreases in physical activity preschool children experienced across the pandemic period.

Aldehydes, particularly acetaldehyde, are carcinogenic molecules and their concentrations in foodstuffs should be controlled to avoid upper aerodigestive tract (UADT) and liver cancers. Highly reactive, acetaldehyde forms DNA and protein adducts, impairing physiological functions and leading to the development of pathological conditions. The consumption of aged beer, outside of the ethanol metabolism, exposes habitual drinkers to this carcinogen, whose concentrations can be over-increased due to post-brewing chemical and biochemical reactions. Storage-related changes are a challenge faced by the brewing industry, impacting volatile compound formation and triggering flavor instability. Aldehydes are among the volatile compounds formed during beer aging, recognized as off-flavor compounds. To track and understand aldehyde formation through multiple pathways during beer storage, consequent changes in flavor but particularly quality losses and harmful compound formation, this systematic review reunited data on volatile compound profiles through gas chromatography analyses from 2011 to 2021. Conditions to avoid flavor instability and successful methods for reducing beer staling, and consequent acetaldehyde accumulation, were raised by exploring the dynamic conversion between free and bound-state aldehydes. Future research should focus on implementing sensory analyses to investigate whether adding aldehyde-binding agents, e.g., cysteine and bisulfite, would contribute to consumer acceptance, restore beer flavor, and minimize acetaldehyde-related health damage.

Globally, one in 11 adults has diabetes mellitus of which 90% have type 2 diabetes. The numbers for osteoporosis are no less staggering: 1 in 3 women has a fracture after menopause, and the same is true for 1 in 5 men after the age of 50 years. Aging is associated with several physiological changes that cause insulin resistance and impaired insulin secretion, which in turn lead to hyperglycemia. The negative balance between bone resorption and formation is a natural process that appears after the fourth decade of life and lasts for the following decades, eroding the bone structure and increasing the risk of fractures. Not incidentally, it has been acknowledged that diabetes mellitus, regardless of whether type 1 or 2, is associated with an increased risk of fracture. The nuances that differentiate bone damage in the two main forms of diabetes are part of the intrinsic heterogeneity of diabetes, which is enhanced when associated with a condition as complex as osteoporosis. This narrative review addresses the main parameters related to the increased risk of fractures in individuals with diabetes, and the mutual factors affecting the treatment of diabetes mellitus and osteoporosis.

This work studied the influence of hydrogel's physical properties (geometry and hierarchical roughness) on the in vitro sorption/release profiles of molecules. To achieve this goal, chitosan (CS) solutions were cast in 3D-printed (3DP) molds presenting intricate shapes (cubic and half-spherical with/without macro surface roughness) and further immersed in alkaline solutions of NaOH and NaCl. The resulting physically crosslinked hydrogels were mechanically stable in aqueous environments and successfully presented the shapes and geometries imparted by the 3DP molds. Sorption and release profiles were evaluated using methyl orange (MO) and paracetamol (PMOL) as model molecules, respectively. Results revealed that distinct MO sorption/PMOL release profiles were obtained according to the sample's shape and presence/absence of hierarchical roughness. MO sorption capacity of CS samples presented both dependencies of hierarchical surface and geometry parameters. Hence, cubic samples without a hierarchical surface presented the highest (up to 1.2 × greater) dye removal capacity. Moreover, PMOL release measurements were more dependent on the surface area of hydrogels, where semi-spherical samples with hierarchical roughness presented the fastest (~1.13 × faster) drug delivery profiles. This work demonstrates that indirect 3DP (via fused filament fabrication (FFF) technology) could be a simple strategy to obtain hydrogels with distinct sorption/release profiles.

OBJECTIVES: to assess the association between breastfeeding and diseases prevalent in the first two years of a child's life. METHODS: a retrospective cross-sectional study that analyzed electronic medical records of 401 children. Data on birth, growth, breastfeeding and medical care in the first two years of life were collected. In the analysis, Poisson regression with robust variance was used. RESULTS: 27.9% of children were exclusively breastfed until six months, and, at 24 months, 93.3% had already had some prevalent childhood disease. In the crude analysis, 5-minute Apgar association, length, weight at 12 months, exclusive and non-exclusive breastfeeding time had association. In the adjusted analysis, only the variable breastfeeding at six months maintained the association with prevalent childhood diseases. CONCLUSIONS: children who were not breastfed, exclusively or not, up to six months of age, had a higher prevalence of diseases compared to breastfed children.

PURPOSE: Personal mastery is a psychological resource recognized as a domain of quality of life that helps individuals cope with life strains. Despite its relevance, there is a lack surrounding the relationship between mobility disability and personal mastery. This study aimed to investigate the influence of personal mastery on mobility disability among older adults and identify the mechanisms that underlie this influence. METHODS: A systematic review was conducted by searching four databases (i.e., PubMed, Scopus, ScienceDirect, and the Virtual Health Library of the Latin American and Caribbean Center on Health Sciences Information) using combinations of the following words: mastery, personal mastery, self-control, mobility, disability, older adults, older people, and aging. We summarized and analyzed the findings of cross-sectional and longitudinal/prospective studies. RESULTS: Twenty-one studies were included. There was high heterogeneity in how the studies had assessed mobility disability. Although a majority of the studies had used the Pearlin's Self-Mastery Scale (PSMS) to assess personal mastery, there were variations in the number of response anchors and scoring strategies that were used. Nevertheless, findings revealed that personal mastery influences mobility disability by acting as a protective resource against adverse outcomes among older adults. The authors of the reviewed articles had provided physiological and behavioral explanations for their findings. CONCLUSIONS: Higher level of personal mastery can protect older adults from mobility disability, because it confers a sense of personal control, which in turn promotes positive physiological functioning and health behaviors. This knowledge contributes to the maintenance of physical functioning in old age.

Osmotic demyelination syndrome is an uncommon neurologic condition, characterized by noninflammatory demyelination involving the pons and other areas of the central nervous system. As chronic hyponatremia is frequently associated with cirrhosis, patients undergoing liver transplantation are at an increased risk for developing this condition. We report the case of a patient who developed refractory hypernatremia and osmotic demyelination syndrome after liver transplantation. The patient was a 40-year-old man, who underwent liver transplantation for the treatment of cryptogenic cirrhosis, and had a preoperative sodium level of 128 mmol/L. Although there were no intraoperative complications, the patient showed signs of mental confusion and drowsiness in the second postoperative day, and we noticed an increase to 136 mmol/L in his serum sodium. Treatment with 5% dextrose and desmopressin was initiated, but his serum sodium continued to increase steadily, while his neurologic condition gradually worsened. Serum sodium rose to 157 mmol/L, and a magnetic resonance imaging of the brain showed extensive lesions consistent with osmotic demyelination syndrome. The clinical condition of the patient continued to deteriorate until his death 17 days after the transplant. Although the occurrence of this syndrome after liver transplantation is well described, the steady increase in serum sodium despite early treatment, as described in this case, is highly unusual, and highlights the great attention that must be taken with monitoring and control of serum sodium in patients who undergo liver transplant in the context of chronic hyponatremia. Hemophagocytic lymphohistiocytosis (HL) is a rare syndrome characterized by a hyperinflammatory state, resulting from an excessive but ineffective immune response. There is a continuous stimulation of TCD8 + lymphocytes, associated with an uncontrolled release of cytokines, causing the infiltration of multiple organs by histiocytes and activated lymphocytes. HL can be a primary condition as a consequence of genetic disorder that most often affects children, or it can be secondary to neoplasms, autoimmune conditions or various infectious diseases in patients of all ages. HL caused by infection by Mycobacterium tuberculosis is highly unusual, with few cases reported in the literature. There is no clinical manifestation or laboratorial finding that is specific to HL, and a high index of clinical suspicion is necessary for the correct diagnosis, which is usually confirmed by biopsy. Treatment consists of controlling the causative event and the use of immunosuppressant drugs such as corticosteroids, etoposide, and cyclosporine to suppress the exacerbated immune response. We report the case of a patient who developed HL 2 months after liver transplantation. The initial presentation was persistent fever, prompting a search for a site of infection and the use of broad-spectrum antibiotics. As the clinical condition of the patient continued to deteriorate, HL was diagnosed through a bone marrow biopsy, and a cerebrospinal fluid culture positive for M. tuberculosis established the diagnosis of disseminated tuberculosis. Despite optimal treatment with immunosuppressors and antituberculosis drugs, there was no significant response and the patient died. Without any realistic prospect of comprehensive global vaccine coverage and lasting immunity, control of pandemics such as COVID-19 will require implementation of large-scale, rapid identification and isolation of infectious individuals to limit further transmission. Here, we describe an automated, high-throughput integrated screening platform, incorporating saliva-based loop-mediated isothermal amplification (LAMP) technology, that is designed for population-scale sensitive detection of infectious carriers of SARS-CoV-2 RNA. Central to this surveillance system is the "Sentinel" testing instrument, which is capable of reporting results within 25 min of saliva sample collection with a throughput of up to 3840 results per hour. It incorporates continuous flow loading of samples at random intervals to cost-effectively adjust for fluctuations in testing demand. Independent validation of our saliva-based RT-LAMP technology on an automated LAMP instrument coined the "Sentinel", found 98.7% sensitivity, 97.6% specificity, and 98% accuracy against a RT-PCR comparator assay, confirming its suitability for surveillance screening. This Sentinel surveillance system offers a feasible and scalable approach to complement vaccination, to curb the spread of COVID-19 variants, and control future pandemics to save lives.

Métodos: estudio transversal retrospectivo que analizó las historias clínicas electrónicas de 401 niños. Se recogieron datos sobre nacimiento, crecimiento, lactancia y atención médica en los dos primeros años de vida. En el análisis se utilizó la regresión de Poisson con varianza robusta. Resultados: el 27,9% de los niños fueron amamantados exclusivamente hasta los seis meses de edad y, a los 24 meses, el 93,3% ya había tenido alguna enfermedad infantil prevalente. En el análisis crudo presentaron asociación de Apgar al minuto 5, longitud, peso a los 12 meses, tiempo de lactancia materna exclusiva y no exclusiva. En el análisis ajustado, sólo la variable lactancia materna a los seis meses mantuvo la asociación con las enfermedades prevalentes de la infancia. Conclusiones: los niños que no fueron amamantados, exclusivamente o no, hasta los seis meses de edad, presentaron mayor prevalencia de enfermedades en comparación con los niños amamantados.


RESUMO Objetivos: avaliar a associação do aleitamento materno e as doenças prevalentes nos primeiros dois anos de vida da criança. Métodos: estudo transversal retrospectivo, que analisou prontuários eletrônicos de 401 crianças. Foram coletados dados sobre nascimento, crescimento, aleitamento materno e atendimentos médicos nos dois primeiros anos de vida. Na análise, utilizou-se Regressão de Poisson com variância robusta. Resultados: receberam aleitamento exclusivo até os seis meses 27,9% das crianças, e, aos 24 meses de vida, 93,3% já haviam tido alguma doença prevalente da infância. Na análise bruta, apresentaram associação Apgar no 5º minuto, comprimento, peso aos 12 meses, tempo de aleitamento exclusivo e não exclusivo. Na análise ajustada, apenas a variável aleitamento materno aos seis meses manteve a associação com as doenças prevalentes da infância. Conclusões: as crianças que não foram amamentadas, exclusivamente ou não, até os seis meses, apresentaram maior prevalência de doenças em relação às amamentadas.

Knee pain is a condition recognized for its high functional impact in the older population. Knowledge about the characteristics of these demands in Primary Health Care (PHC) is scarce, which makes it difficult to plan lines of care aimed at pain management in this population. The objective of this study was to investigate characteristics of knee pain among older adults users of primary health care, according to sociodemographic aspects. A cross-sectional study was carried out with 201 older adults in three randomly selected health units. Knee pain was classified as chronic (>3 months) and acute (<3 months). The Analog Pain Scale was applied to assess the intensity, scored from 0 to 10, and questions about the impact of pain on daily life and demand for health services. Gender, age, presence of a partner, and education were recorded. Chronic knee pain was reported by 39.8% of participants; 24.9% reported functional impact and 23.9% sought a health service. Presence of pain (p=0.021) and functional impact (p=0.016) were more frequent among those who did not have a partner and the search for health services was more frequent among those with higher education. Acute knee pain was reported by 37.8% of participants, mean pain intensity was 2.4 (SD:3.6), and was associated with higher education and not having a partner. The demand for care for older adults with knee pain in PHC is high and is characterized as being functionally impactful and stimulating the demand for health services, in addition to being unequal in terms of education and the presence of a partner.

Several studies using neuroimaging techniques have established a positive relationship between physical activity (PA) and brain structure and function in older populations. However, the use of subjective measures of PA and the lack of multimodal neuroimaging approaches have limited the understanding of this association. This study aims to explore the associations between PA and brain structure and function by objectively evaluating PA. Community-dwelling cognitively healthy older adults (without diagnosed cognitive, neurological or degenerative disease) were recruited from local health centers and local gyms. In a cross-sectional design, participants were evaluated regarding cognitive, clinical, anthropometric, physical performance, and lifestyle characteristics. A 3 T magnetic resonance imaging (MRI) was performed for structural and functional brain measures. PA time and level was assessed via a Xiaomi Mi Band 2® worn for 15 consecutive days. Participants (n = 110, after inclusion/exclusion criteria and completion of all evaluations) were 58 females (56%), with an average age of 68.42 years old (SD = 3.12), most were active. Multiple regression analysis revealed that higher time spent in vigorous PA associated with larger left parahippocampal gyrus and right hippocampus volumes. Furthermore, the analysis of the functional connectome indicated a greater functional connectivity (FC) between the frontal gyrus, cingulate gyrus, occipital inferior lobe for light, moderate, and total PA time, and sedentary time associated with lower FC in the same networks. Overall, the structural and functional findings may provide evidence on the relevant association between PA and brain health in aging.

As repercussões emocionais do câncer podem atingir o cônjuge com quase a mesma intensidade que atingem o paciente, justificando um paralelo com a noção de trauma para a Psicanálise. Já a capacidade dos cônjuges em ressignificar este trauma permite uma aproximação com o conceito de resiliência. O presente estudo transversal, qualitativo, exploratório e descritivo objetivou compreender a perspectiva do cônjuge sobre o adoecimento por câncer do(a) parceiro(a), analisando sua relação com os conceitos de trauma, ressignificação e resiliência para a Psicanálise. Entrevistas semiestruturadas foram realizadas com 11 cônjuges e analisadas por meio da Análise de Conteúdo Temática, obtendo-se 4 eixos temáticos (Complexidade emocional da vivência; Acesso ao traumático; Vicissitudes do processo de ressignificação; Expectativas para o futuro). Concluiu-se que, devido à complexidade da vivência, o reconhecimento do trauma e sua ressignificação nem sempre são possíveis, indicando a importância dos cônjuges como objetos de cuidado e estudo dos profissionais da saúde.


The emotional repercussions of cancer can reach the spouse with almost the same intensity as the patient, justifying a parallel with the notion of trauma for Psychoanalysis. The spouses' ability to reframe this trauma allows an approximation with the concept of resilience. This cross-sectional, qualitative, exploratory and descriptive study aimed to understand the perspective of the spouse about the partner's cancer illness, analyzing its relation with the concepts of trauma, resignification and resilience for Psychoanalysis. Semi-structured interviews were conducted with 11 spouses and analyzed through Thematic Content Analysis, obtaining 4 thematic axes (Emotional complexity of experience; Access to traumatic; Vicissitudes of the resignification process; Expectations for the future). It was concluded that, due to the complexity of the experience, the recognition of trauma and its resignification are not always possible, indicating the importance of spouses as objects of care and study of health professionals.

SUMMARY The occurrence of fractures in young individuals is frequently overlooked by physicians, especially when associated with exercise or trauma. Nevertheless, multiple fractures should always be investigated since underlying conditions can predispose to such events. We describe here the case of a young, healthy woman who sustained multiple fractures in the lower limbs, which were initially considered to be "stress fractures". Further investigation, including a panel of genes associated with osteogenesis imperfecta, revealed that the patient is a heterozygous carrier of a SERPINF1 variant. According to criteria recommended by the American College of Medical Genetics and Genomics and the Association for Molecular Pathology, this variant is classified as likely benign (PM2, PP3, PP4, BP1, and BP4). The patient's mother and brother were also asymptomatic carriers of the variant and had sustained previous minor fractures. The patient had normal biochemical profile and bone density. This condition has been rarely described and is not associated with low bone mineral density or altered bone turnover markers. This case highlights the importance of investigating multiple fractures in young patients who are otherwise healthy since these may be a warning sign of rare genetic conditions associated with fragility fractures.
