RESUMO

Recent research has indicated that lead in water of private wells is in the range of that which caused problems in Flint, Michigan. However, there is limited understanding of the mechanisms for water lead release in these systems. We evaluated water lead at the homes of two children with elevated blood lead in Macon County (North Carolina), which did not have identifiable lead paint or lead dust hazards, and examined water lead release patterns among 15 private wells in the county. Water lead release patterns differed among the 15 private wells. Problems with lead release were associated with (1) dissolution of lead from plumbing during periods of stagnation; (2) scouring of leaded scales and sediments during initial water use; and (3) mobilization of leaded scales during continued water use. Accurate quantification of water lead was highly dependent on sample collection methods, as flushing dramatically reduced detection of lead hazards. The incidence of high water lead in private wells may be present in other counties of North Carolina and elsewhere in the United States. The underestimation of water lead in wells may be masking cases of elevated blood lead levels attributed to this source and hindering opportunities to mitigate this exposure.

OBJECTIVE: To measure the zinc, copper, selenium and manganese blood levels in a cohort of 68 preterm infants, and to establish any associations with growth and/or dietary intake. DESIGN: Blood samples were collected at an infant's expected date of delivery (term) and 6 months later. Serum zinc, plasma copper and whole blood manganese were analysed by atomic absorption spectrometry, plasma and red cell selenium were determined by mass spectrometry. Growth and dietary intake determinations have been previously published. SETTING: Hampshire, England. RESULTS: Mean (SD) birth weight of the infants was 1.47 (0.434) kg and mean gestation was 31.4 (2.9) weeks. Mean blood levels at term and 6 months were: serum zinc 12.0 (2.6) micromol/l and 13.8 (2.5) micromol/l; plasma copper 10.1 (2.6) micromol/l and 19.2 (3.6) micromol/l; plasma selenium 0.49 (0.15) micromol/l and 0.72 (0.14) micromol/l; red blood cell selenium 1.68 (0.40) micromol/l and 1.33 (0.19) micromol/l; and blood manganese 320 (189) nmol/l and 211 (68) nmol/l, respectively. There were no significant associations between levels of zinc and copper and dietary intakes of those nutrients at either age (dietary intakes of selenium and manganese were not determined). Only copper levels at term were significantly associated (r = 0.31; p = 0.05) with a growth parameter (head circumference). CONCLUSION: These results provide new information about trace element status in this vulnerable population.

Before 2001, the World Health Organization (WHO) recommended that infants be exclusively breastfed for 4-6 mo with the introduction of complementary foods (any fluid or food other than breast milk) thereafter. In 2001, after a systematic review and expert consultation, this advice was changed, and exclusive breastfeeding is now recommended for the first 6 mo of life. The systematic review commissioned by the WHO compared infant and maternal outcomes for exclusive breastfeeding for 3-4 mo versus 6 mo. That review concluded that infants exclusively breastfed for 6 mo experienced less morbidity from gastrointestinal infection and showed no deficits in growth but that large randomized trials are required to rule out small adverse effects on growth and the development of iron deficiency in susceptible infants. Others have raised concerns that the evidence is insufficient to confidently recommend exclusive breastfeeding for 6 mo for infants in developed countries, that breast milk may not meet the full energy requirements of the average infant at 6 mo of age, and that estimates of the proportion of exclusively breastfed infants at risk of specific nutritional deficiencies are not available. Additionally, virtually no data are available to form evidence-based recommendations for the introduction of solids in formula-fed infants. Given increasing evidence that early nutrition and growth have effects on both short- and longer-term health, it is vital that this issue be investigated in high-quality randomized studies. Meanwhile, the consequences of the WHO recommendation should be monitored in different settings to assess compliance and record and act on adverse events. The policy should then be reviewed in the context of new data to formulate evidence-based recommendations.

OBJECTIVES: To describe feeding patterns and mothers' perceptions of desirable feeding practices in low-birth-weight (LBW) infants after hospital discharge in England and to test for the association of inappropriate practices/perceptions with identifiable demographics. DESIGN: Postal questionnaires from 198 mothers of LBW infants in London, Liverpool and Winchester were analysed regarding infant demographics, mothers' demographics, infant milks used, solid feeding practices and mothers' perceptions of infant feeding practices. RESULTS: At birth, the median weight, independent of gestational age, was 1.80 kg (range 0.6-2.50 kg) and 37.1% of infants received breast milk exclusively; 83.7% were breast-fed at some stage, a practice favoured more by first-time mothers (90.2%) than multiparous mothers (73.4%) (P = 0.004) as well as by mothers of higher social groups (P = 0.019). The median age of solid food introduction was 17 postnatal weeks (range 8-36 weeks); the timing correlated strongly with infant birth weight (P < 0.001). A high-fibre diet and a low-fat diet were incorrectly considered important for their infants by 67.1% and 51.6% of mothers, respectively. Regarding a high-fibre diet as important was associated with being a multiparous mother (P = 0.006), while regarding a low-fat diet as important was associated with low social group (P = 0.018). A quarter of mothers did not consider 'plenty of calories' to be important for their infants, reflecting similar attitudes in 1990 for mothers of term infants; this incorrect attitude exhibited an association with being a first-time mother (P = 0.047). CONCLUSIONS: Infant feeding practices were very variable, showing poor concordance with national and international recommendations for term infants. In selecting foods for their infants, many mothers appeared to be applying principles more appropriate to planning a healthy diet for adults. There is an urgent need to develop and implement evidence-based guidelines for feeding LBW infants after hospital discharge, with special attention given to multiparous mothers as well as mothers from lower social groups.

Current understanding of the use of exclusion diets in the management of asthma in children is limited and controversial. The aim of this study was to examine the effects of excluding eggs and milk on the occurrence of symptoms in children with asthma and involved 22 children aged between three and 14 years clinically diagnosed as having mild to moderate disease. The investigation was single blind and prospective, and parents were given the option of volunteering to join the 'experiment' group, avoiding eggs, milk and their products for eight weeks, or the 'control' group, who consumed their customary food. Thirteen children were recruited to the experimental group and nine to the control group. A trained paediatrician at the beginning and end of the study period assessed the children. A seven-day assessment of food intake was made before, during and immediately after the period of dietary intervention in both groups. A blood sample was taken from each child for determination of food specific antibodies and in those children who could do so, the peak expiratory flow rate (PEFR) was measured. Based on the recommended nutrient intake (RNI), the mean percentage energy intake of the children in the experimental group was significantly lower (p < 0.05) in the experimental group. After the eight-week study period and compared with baseline values, the mean serum anti-ovalbumin IgG and anti-beta lactoglobulin IgG concentrations were statistically significantly reduced (p < 0.05) for both in the experimental group. In contrast, the values for anti-ovalbumin IgG in the control group were significantly increased and those for anti-beta lactoglobulin IgG were practically unchanged. The total IgE values were unchanged in both groups. Over the study period, the PEFR in those children in the experimental group able to perform the test was significantly increased, but no such change was noted in the children in the control group who could do the test. These results suggest that even over the short time period of eight weeks, an egg- and milk-free diet can reduce atopic symptoms and improve lung function in asthmatic children.

The objective of the study was to determine whether iron and micronutrient status is improved with an increased amount of meat in the diet. To this end, a longitudinal prospective study with infants recruited at 4 mo and followed until 24 mo of age was undertaken. One hundred ninety-eight infants formed the original study cohort; 48 withdrew before the end of the study. Subjects were classified as nonmeat eaters or as mixed (red and white)-meat eaters subgrouped into tertiles depending on the meat content reported in diet diaries. Seven-day weighed food records were recorded at 4, 8, 12, 16, 20, and 24 mo. Blood samples taken at 4, 12, and 24 mo were analyzed for parameters of iron and micronutrient status. Iron intake increased during the first year, thereafter remaining constant. The percentages of subjects with hemoglobin values below 110 g/L were 34.1, 23.1, and 13.4 at 4, 12, and 24 mo, respectively. For parameters of iron status, the number of results below the reference range was determined for each diet group and a significant negative relationship between serum iron and meat intake at 12 mo of age was seen (p<0.023). There was a trend for hemoglobin concentrations to be inversely related to the meat intake, at the same age (p<0.068). No effects on zinc or copper status were seen. We conclude that a weak association between dietary meat and iron/Hb suggests a positive role for red meat. There was no disadvantage to the nonmeat-eating infants with respect to zinc or copper.

Using a rice-based model weaning food, the effect of Lactococcus lactis on the growth and survival of a range of enteric pathogens has been investigated. The starter organism used produces the bacteriocin nisin and the physiological L-lactate isomer, thus avoiding the risk of D-lactate acidosis when consumed by infants. L. lactis was a less effective antagonist than stronger acid producers such as the DL lactate producer, Lactobacillus plantarum , and only produced a potentially useful inhibition of pathogens when present in a large numerical superiority (>105:1). Prefermentation of the weaning food with L. lactis for 24 h produced a product with a pH of 3.7-3.8 containing ≈ 0.25% lactate (>96% L-lactate). The prefermented product was bactericidal for pathogens introduced subsequently. Despite the production of 100-150 international units nisin per g during fermentation, the inhibition of pathogens could be ascribed to acid production alone.
