RESUMO

Primary immunodeficiencies (PIDs)/Inborn errors of immunity (IEI) consist of a complex genetic group of disorders that cause susceptibility to infections, inflammation, immune dysregulation, autoimmunity, and malignancy. One of the key steps to reach an early diagnosis is improving knowledge of PID among the medical community. In this study, a web-based survey was conducted among 355 Japanese physicians, consisting of 121 pediatricians, 116 hematologists, and 118 general internal medicine physicians, to assess their awareness and knowledge about the diagnostic flow of PID. One of the major problems this study identified was the unawareness of optimal IgG trough levels among the physicians, while around half the physicians knew about the symptoms of PID. Results from the hypothetical case study revealed that over 70% of physicians considered PID after obtaining the past medical history of patients and 75.2% of physicians showed interest in gaining more knowledge about PID. The survey findings revealed that proper questioning to understand the exact medical history of patients may lead to basic immunological examination. There is a need to improve knowledge about PID, e.g., the '10 warning signs of PID' and '4 stages of testing for PID', and to motivate physicians to ensure earlier diagnosis of PID.

The purpose of this study was to optimize X-ray conditions for full spine X-ray examinations in slot-scan digital radiography (SSDR). Follow-up of spinal deformities, such as scoliosis, typically involves many radiographs of the patient throughout childhood and adolescence. The Radiation doses for the full spine X-ray examination should be minimized. Recently, SSDR has been introduced for full spine as well as lower extremity examinations. This system utilizes slot scanning geometry to acquire the radiographic images. X-ray energy characteristics of direct amorphous Selenium (a-Se) digital fluoroscopy and short time X-ray tube loads of this system were investigated. Orthopedists evaluated the irradiation time for full spine radiographs in various conditions using an observer study. Patient doses were measured with radiophotoluminescence glass dosimeters, which were implanted in various tissue and organ positions within an anthropomorphic phantom. In the optimization of conditions for SSDR, patient doses are reduced by approximately 60% compared to default setting.

Our purpose in this study was to evaluate the preliminary clinical efficacy of soft-copy reading of digital mammography, for a 15-mega-sub-pixel (MsP) and a 9-MsP super-high-resolution liquid-crystal display (SHR-LCD) by use of an independent sub-pixel driving technology. We performed three kinds of phantom observation studies by six radiological technologists. Detectability of a contrast-detail phantom and simulated small objects (SSOs) resembling microcalcifications (MCLs), and shape discrimination ability of SSOs with round and square shapes, were examined and compared with a 5-MP conventional LCD (5-MP LCD). In each study, four types of display magnification ratio were used. The detectability and the shape discrimination ability of the 15-MsP SHR-LCD were highest among the three LCDs of most of the display magnification ratios. The 9-MsP SHR-LCD indicated a higher or equal performance as compared with the 5-MP LCD in the SSO detection and shape studies. The results of our study demonstrated that the SHR-LCDs had good potential to detect MCLs and to evaluate the shape in high-resolution digital mammography.
