RESUMO

X-ray free-electron lasers (XFELs) deliver intense x-ray pulses that destroy the sample in a single shot by a Coulomb explosion. Experiments using XFEL pulse trains or the new generation of high-repetition rate XFELs require rapid sample replacement beyond those provided by the systems now used at low repletion-rate XFELs. We describe the development and characterization of a system based on a spinning disk to continuously deliver a solid sample into an XFEL interaction point at very high speeds. We tested our system at the Linac Coherent Light Source and European XFEL hard x-ray nano-focus instruments, employing it to deliver a 25 µm copper foil sample, which can be used as a gain medium for stimulated x-ray emission for the proposed x-ray laser oscillator.

RESUMO

The dynamics of a nuclear open quantum system could be revealed in the correlations between the breakup fragments of halo nuclei. The breakup mechanism of a proton halo nuclear system is of particular interest as the Coulomb polarization may play an important role, which, however, remains an open question. Here we use a highly efficient silicon detector array and measure the correlations between the breakup fragments of 8B incident on 120Sn at near-barrier energies. The energy and angular correlations can be explained by a fully quantum mechanical method based on the state-of-the-art continuum discretized coupled channel calculations. The results indicate that, compared to the neutron halo nucleus 6He, 8B presents distinctive reaction dynamics: the dominance of the elastic breakup. This breakup occurs mainly via the short-lived continuum states, almost exhausts the 7Be yield, indicating the effect of Coulomb polarization on the proton halo state. The correlation information reveals that the prompt breakup mechanism dominates, occurring predominantly on the outgoing trajectory. We also show that, as a large environment, the continuum of 8B breakup may not significantly influence elastic scattering and complete fusion.

RESUMO

In the present study, microplastics (MPs) and metal concentrations were studied in the widely consumed tilapia (Oreochromis niloticus) fishes (n = 15) collected from a metropolitan reservoir of the Atoyac River basin, Mexico. Nearly 139 fibers were extracted from the gastrointestinal tracts and assessed using optical microscopy to evaluate their physical characteristics. The colour distribution of the fibers was mainly black (40%), blue (19%), red and white (14%). SEM images represented the surface morphology, while the elemental composition of the fibers was studied using EDX spectra. Polymer characterization using µFTIR aided in confirming the fibers as plastics (polyamide, polyester, and synthetic cellulose) and non-plastics (natural cellulose). Henceforth, ∼33% of the fibers, provisionally thought to be plastics, were natural fibers. The total metal concentrations were higher in the liver (259.24 mg kg-1) than the muscle (122.56 mg kg-1) due to diverse metabolic functions in the hepatic tissues. Human health risk assessment in terms of Hazard Index (HI) presented Pb and Zn values above unity in both adults and children, prompting regulatory measures. Statistical tests between MPs and fish biometry did not present any substantial correlations. The present study also affirmed that the presence of MPs and metals in fishes of a highly contaminated region is not only governed by their bioavailabilities, but also on the physiological characteristics of the individual organism.

BACKGROUND: Influenza is a highly prevalent disease and is responsible of three to five million cases of severe illness every year, leading to 0.3-0.6 million of deaths and important health costs. Therefore, it is important to analyze which factors, complications and other characteristics could be related with patients admitted with flu who die. METHODS: Single-center, observational, cross-sectional and retrospective study of patients admitted with influenza at La Paz University Hospital during 2013-2014 and 2014-2015 seasons, with analysis of the variables collected in the clinical history and in the documents of "notifiable diseases". RESULTS: 16 patients admitted with influenza out of 179 died in the 2013-14 and 2014-15 seasons, which represent 8.9%. These were older patients, and more frequently suffered from some type of immunodeficiency. Eleven had pneumonia, five respiratory distress, seven multi-organ failure and four of them were admitted to the Intensive Care Unit. The duration of antiviral treatment was shorter in the living patients at discharge. Only six were vaccinated. According to the multivariate analysis, advanced age, suffering from any type of immunodeficiency, the development of pneumonia and respiratory distress are factors independently associated with mortality. CONCLUSIONS: Advanced age, suffering from any type of immunodeficiency, the development of pneumonia and respiratory distress are independent risk factors for mortality in patients admitted with influenza

RESUMO

BACKGROUND: China and Spain are world leaders in the consumption of edible bulbs (garlic and onion), but there are few references to their capacity to cause allergic symptoms. The target was to study allergic sensitization and clinical symptoms associated with garlic and onion consumption in a large sample of allergic patients. METHODS: An observational cross-sectional study was conducted, testing garlic and onion extracts in 8109 patients of all ages seen by our allergy clinic in 2018. Forty-four aeroallergens and foods were tested, including garlic and onion, with prick test and determination of specific IgE. Oral provocation and contact tests were performed if a delayed reaction was suspected. Western Blot was performed in the serum of patients positive to garlic and onion. RESULTS: We conducted 356,798 skin tests and 4254 specific IgE determinations. Of the 8109 patients tested, 2508 (30.92%) presented with symptoms associated with food intake and, in these patients, food hypersensitivity was detected by skin test, positive specific IgE or provocation in 924 patients, and was caused by garlic or onions in 27, indicating a prevalence of 2.92%. Immunodetection showed an association between the symptoms and a specific LTP to these bulbs, without cross-reactivity with other LTPs in the Mediterranean diet (peach, wheat). CONCLUSIONS: Allergic hypersensitivity to garlic and onions should not be underestimated and, given their high consumption, should be included in the diagnostic food allergy battery

RESUMO

Targeted knock-in (KI) can be achieved in embryos by clustered regularly interspaced short palindromic repeats (CRISPR)-assisted homology directed repair (HDR). However, HDR efficiency is constrained by the competition of nonhomologous end joining. The objective of this study was to explore whether CRISPR-assisted targeted KI rates can be improved in bovine embryos by exposure to the HDR enhancer RS-1. In vitro produced zygotes were injected with CRISPR components (300 ng/µl Cas9 messenger RNA and 100 ng/µl single guide RNA against a noncoding region) and a single-stranded DNA (ssDNA) repair template (100 ng/µl). ssDNA template contained a 6 bp XbaI site insert, allowing targeted KI detection by restriction analysis, flanked by 50 bp homology arms. Following microinjection, zygotes were exposed to 0, 3.75, or 7.5 µM RS-1 for 24 hr. No differences were noted between groups in terms of development or genome edition rates. However, targeted KI rates were doubled in the group exposed to 7.5 µM RS-1 compared to the others (52.8% vs. 25% and 23.1%, for 7.5, 0, and 3.75 µM, respectively). In conclusion, transient exposure to 7.5 µM RS-1 enhances targeted KI rates resulting in approximately half of the embryos containing the intended mutation, hence allowing direct KI generation in embryos.

RESUMO

BACKGROUND: Although the Barcelona Clinic Liver Cancer (BCLC) staging system has been largely adopted in clinical practice, recent studies have emphasized the need for further refinement and subclassification of this system. METHODS: Patients who underwent hepatectomy with curative intent for BCLC-0, -A or -B hepatocellular carcinoma (HCC) between 2000 and 2017 were identified using a multi-institutional database. The tumour burden score (TBS) was calculated, and overall survival (OS) was examined in relation to TBS and BCLC stage. RESULTS: Among 1053 patients, 63 (6·0 per cent) had BCLC-0, 826 (78·4 per cent) BCLC-A and 164 (15·6 per cent) had BCLC-B HCC. OS worsened incrementally with higher TBS (5-year OS 77·9, 61 and 39 per cent for low, medium and high TBS respectively; P < 0·001). No differences in OS were noted among patients with similar TBS, irrespective of BCLC stage (61·6 versus 58·9 per cent for BCLC-A/medium TBS versus BCLC-B/medium TBS, P = 0·930; 45 versus 13 per cent for BCLC-A/high TBS versus BCLC-B/high TBS, P = 0·175). Patients with BCLC-B HCC and a medium TBS had better OS than those with BCLC-A disease and a high TBS (58·9 versus 45 per cent; P = 0·005). On multivariable analysis, TBS remained associated with OS among patients with BCLC-A (medium TBS: hazard ratio (HR) 2·07, 95 per cent c.i. 1·42 to 3·02, P < 0·001; high TBS: HR 4·05, 2·40 to 6·82, P < 0·001) and BCLC-B (high TBS: HR 3·85, 2·03 to 7·30; P < 0·001) HCC. TBS could also stratify prognosis among patients in an external validation cohort (5-year OS 79, 51·2 and 28 per cent for low, medium and high TBS respectively; P = 0·010). CONCLUSION: The prognosis of patients with HCC varied according to the BCLC stage but was largely dependent on the TBS.

PURPOSE: Facially malformed patients often present a variable degree of facial imbalance even after basal bone correction, and resolution of the residual hard- and soft-tissue deficiencies and asymmetries of the face are of great importance for achieving a satisfactory post-implant procedure result. The use of polyether ether ketone (PEEK) implants for maxillofacial reconstruction has been documented in the literature, but the number of patients who have received them is limited. The aim of this study was to evaluate the overall volumetric results in patients with facial imbalance after onlay custom implant positioning for mandible and fronto-orbital reconstructions. Analysis was performed by confronting volumes with the use of three-dimensional (3D) photogrammetry. METHODS: Fifteen patients were eligible for PEEK implant placements, eight for mandibular angle reconstruction, and seven for fronto-orbital reconstruction. Pre- and post-surgical 3D images of each patient's face were acquired. Facial asymmetry was analyzed by comparing each face with its mirrored copy. RESULTS: Three-dimensional analyses have shown that some degree of volume imbalance was still present in the patients with only 1.32 ± 1.02 mm residual discrepancy after treatment. CONCLUSION: Results of the study were found to fall within clinically acceptable limits since an asymmetry rate of < 3 mm is considered to fall into the norm.

RESUMO

The surgery first approach (SFA) is a therapeutic strategy used in orthognathic surgery that is constantly evolving. With this approach, the pre-surgical orthodontic treatment can be eliminated, the maxilla and the mandible are surgically repositioned into the desired position, and the therapy is ended with a short orthodontic phase. Several studies have reported that the SFA is an acceptable approach, but postoperative stability is unclear. In this study, a systematic review on the SFA was performed. The PubMed, Google Scholar, Scopus, LexisNexis, Web of Science, and Cochrane Library databases were accessed. Studies from which data could be extracted on skeletal stability based on specific cephalometric points were included. The search yielded 2766 publications. Application of the selection criteria resulted in a final group of 14 articles. Five hundred and sixty patients with class III malocclusion underwent orthognathic surgery, 339 with the SFA. Study parameters such as evaluation time points and reference planes varied, making it impossible to perform a meta-analysis. The studies suggest that surgery with the SFA is as stable as surgery with the conventional approach. However, all articles described stability using a penultimate time point of 'after surgery' and not 'after debonding'; hence orthodontic movements and consequent mandibular movements could have influenced cephalometric measurements. Thus, to verify the real stability of the SFA, further research with longer follow-up periods is required, with evaluation at the same time points.

RESUMO

The increase of sheep meat competitiveness in international markets can be attributed to the rise of the quantity and the improvement of the quality of the edible portion of sheep carcasses. Usually, carcass yield is established after the slaughter of the animals. Yet, when carcass yield is determined in vivo, it can be both a costly and subjective method. This study proposes models for predicting the physical characteristics of lamb carcass using bioimpedance analysis (BIA) in live animals. Thirty-one Texel × Ile de France crossbreed ram lambs were slaughtered at 20, 26, 32 or 38 kg of BW. Before the slaughter, values of resistance (Rs) and reactance (Xc) were collected using a single-frequency BIA equipment (Model RJL Quantum II Bioelectrical Body Composition Analyzer). Then, BIA main variables such as body bioelectrical volume (V), phase angle (PA), resistive density (RsD) and reactive density (XcD) were calculated. After slaughter, cold carcass weight (CCW), cold carcass yield (CCY), subcutaneous fat thickness (SFT), soft tissue weight (STW) and soft tissue yield (STY) were also measured. Multiple regression analyses were carried out using the physical characteristics as dependent variables and the bioimpedance values as independent variables. Predictive performance of the models was assessed using leave-one-out cross-validation. The prediction model of CCW was obtained using the V, PA and RsD (R2 = 0.97), STW through the V, RsD and XcD (R2 = 0.97), CCY by Rs, Z and XcD (R2 = 0.69), STY by V and XcD (R2 = 0.67), and SFT only for XcD (R2 = 0.84). The results indicated that BIA has the potential to predict carcass characteristics of lambs at different body masses.

RESUMO

PURPOSE: The aim is discussing the origins of worsening of external eye condition (EEC) and of tear film (TF) instability after wear of silicone-hydrogel contact lenses (CLs) with hydrogen-peroxide (H2O2) care system. METHODS: EEC and TF stability were evaluated before and after 15days of wear combined with different care systems: (1) H2O2, (2) detergent solution and H2O2, (3) multipurpose solution (MPS), (4) H2O2 and artificial tears. In-vitro cell mortality tests were performed after 24h cell incubation with CLs treated with H2O2. Photon correlation spectroscopy (PCS) was carried out on tears of non-wearers and CL wearers who used MPS or H2O2 solution. RESULTS: Worsening of EEC was observed only for the group using H2O2 (group 1). In-vitro, cell mortality was found higher for worn CL than for unworn CLs. Worsening of TF stability was observed regardless of care system and also PCS results on tears of CL wearers were found different compared to non-wearers regardless of care system. The only observed remedy for tear instability of CL wearers was found to be the administration of artificial tears. CONCLUSIONS: Worsening of EEC of CL wearers using H2O2 is attributed to H2O2 scarce cleaning efficacy, which can be solved by adding a CL detergent solution. The origin of TF instability is found to be different. A remedy was found to be the administration of artificial tears, whose effect could be attributed either to the role of specific components or to rinsing and replacement of TF during wear.

This research objective was to evaluate the effect of private supplementation with concentrated or legumes, on the carcass characteristics and body components from suckling lambs kept on ryegrass pasture. Twenty seven suckling lambs were used, with about 17 days of age and weighing 9,91±0,594kg, which were distributed in the treatments that corresponded to feeding systems: suckling lambs kept on ryegrass pasture, suckling lambs kept on ryegrass pasture and supplemented with concentrated in private feeder (creep feeding) and suckling lambs kept on ryegrass pasture and supplemented with legume in the private pasture (creep grazing). The concentrate supplement was composed by corn, soybean meal and limestone, and was supplied ad libitum. The supplementary pasture was white clover legume. The experimental design was completely randomized, where the results were submitted to analysis of variance and means compared by Tukey test at 5% of significance level. The gastrointestinal+bile+urine content and the proportion of esophagus were higher (P<0,05) in lambs kept on ryegrass. The liver and large intestine proportions were higher (P<0,05) in lambs supplemented with concentrate. The tested alimentary systems produce carcasses with similar characteristics. The supplementation with legume or concentrate cause changes in body components of suckling lambs grazing ryegrass.(AU)

The objective of this study was to evaluate the carcass and non carcass components characteristics of lambs finished in feedlot with different proportions of cottonseed on the diet. Forty five lambs, non castrated males, from Ile de France race. The treatments were constituted by levels of inclusion of cottonseed on dry matter (DM) of the total diet (0%; 10%; 20%; 30% or 40%), being the diet composed by corn silage (Zea mays L.), ground corn grain (Zea mays L.), soybean meal (Glycine max L.), cottonseed (Gossypiumhirsutum L.), limestone and mineral salt in a roughage: concentrate proportion of 40:60. The animals were slaughtered with a pre- established slaughter weight. The hot and cold carcass weight, as well as the carcass chilling index, the compactness index, the conformation and the fattening state of the carcass were not influenced (P> 0.05) by the level of inclusion of cottonseed on the diet. The hot and cold carcass yield decreased linearly (P≤ 0.05) as the cottonseed was included on the diet. Regarding the subcutaneous fat thickness it is possible to observe that there was a crescent quadratic behavior and in relation to the rib eye area variable, the behavior observed was linear decrescent. Regarding the proportions of the commercial cuts of the carcass, the leg weight decreased linearly (P≤ 0.05) while the percentage of leg was quadratic influenced (P≤ 0.05) by the inclusion of cottonseed on the diets. The full gastrointestinal tract and the gastrointestinal tract content increased linearly (P≤ 0.05) with the increase on the level of inclusion of cottonseed on the diet. Some of the carcass and non carcass components characteristics had negative influence of level fiber content and dietary lipids as it included cottonseed in the total diet.(AU)

RESUMO

The extraction of mandibular third molars can lead to injury to the inferior alveolar nerve. Hence, it is important to assess the proximity of the root to the inferior alveolar canal before extraction. The classification system of Pell and Gregory and the Winter classification are commonly used to evaluate the positional relationship of the third molar based on radiographs. This retrospective study involving 105 mandibular third molars was performed to assess whether these systems reflect the proximity of the root to the canal (based on computed tomography images), and to identify risk factors for nerve injury. Regarding the prediction of computed tomography-verified canal invasion, the sensitivity, specificity, and positive and negative predictive values were high for each Pell and Gregory category when there was radiographic evidence. The mean distance of invasion was significantly greater in class III than in class I. However, there were no significant differences between the Winter inclination categories. The mean distance differed significantly between a horizontal inclination angle to the buccal side of >5° and an angle of ≤5°. Thus, a horizontal inclination angle >5° represents a novel risk factor for nerve injury.

RESUMO

OBJECTIVE: To apply an e-nose system for monitoring headspace volatiles in biological samples from Egyptian patients with active pulmonary tuberculosis (TB) and healthy controls (HCs) and compare them with standard sputum analysis. DESIGN: The study population comprised 260 (140 males, 120 females) newly diagnosed TB patients and 240 (120 males, 120 females) HCs matched by age and socio-economic level admitted to hospitals specialising in chest diseases in Alexandria, Behera, Giza and Damietta Governorates, Egypt. Participants provided a history of TB and underwent clinical examinations, chest X-ray, and microbiological and e-nose analyses. Biological samples (blood, breath, sputum and urine) were collected. RESULTS AND CONCLUSION: Being a confirmed TB patient was directly proportional to e-nose 10-sensor responses. Principal component analysis clusters showed a clear distinction between TB and HC groups, with variances of 93%, 85%, 75% and 95% for blood, breath, sputum and urine samples, respectively. Overall accuracy, sensitivity and specificity of the artificial neural network (ANN) analysis for classifying samples were >99%. The e-nose successfully distinguished TB patients from HC participants for all measured biological samples with great precision. With urine samples gaining broader acceptance for clinical diagnosis, an e-nose-based ANN can be a very useful tool for low-cost mass screening and early detection of TB patients in developing countries.

RESUMO

Angular distributions of the elastic, inelastic, and breakup cross sections of the halo nucleus ^{11}Be on ^{197}Au were measured at energies below (E_{lab}=31.9 MeV) and around (39.6 MeV) the Coulomb barrier. These three channels were unambiguously separated for the first time for reactions of ^{11}Be on a high-Z target at low energies. The experiment was performed at TRIUMF (Vancouver, Canada). The differential cross sections were compared with three different calculations: semiclassical, inert-core continuum-coupled-channels and continuum-coupled-channels ones with including core deformation. These results show conclusively that the elastic and inelastic differential cross sections can only be accounted for if core-excited admixtures are taken into account. The cross sections for these channels strongly depend on the B(E1) distribution in ^{11}Be, and the reaction mechanism is sensitive to the entanglement of core and halo degrees of freedom in ^{11}Be.

RESUMO

OBJECTIVE: To date, no systematic review has been undertaken to identify the complications of segmental osteotomies. The aim of the present systematic review was to analyze the type and incidence of complications of segmental osteotomies, as well as the time of subjective and/or clinical onset of the intra- and post-operative problems. MATERIALS AND METHODS: A search was conducted in two electronic databases (MEDLINE - Pubmed database and Scopus) for articles published in English between 1 January 2000 and 30 August 2015; only human studies were selected. Case report studies were excluded. Two independent researchers selected the studies and extracted the data. Two studies were selected, four additional publications were recovered from the bibliography search of the selected articles, and one additional article was added through a manual search. RESULTS: The results of this systematic review demonstrate a relatively low rate of complications in segmental osteotomies, suggesting this surgical approach is safe and reliable in routine orthognathic surgery. CONCLUSIONS: Due to the small number of studies included in this systematic review, the rate of complication related to surgery first approach may be slightly higher than those associated with traditional orthognathic surgery, since the rate of complications of segmental osteotomies must be added to the complication rate of basal osteotomies. A surgery-first approach could be considered riskier than a traditional one, but further studies that include a greater number of subjects should be conducted to confirm these findings.
