STATEMENT OF PROBLEM: Applications of polyetheretherketone (PEEK) as a promising alternative to ceramic materials have extended to include post-and-cores. However, information regarding the available fabrication techniques for producing custom-made PEEK post-and-cores and their effect on accuracy is still scarce. PURPOSE: The purpose of this in vitro study was to evaluate the trueness and intaglio fit of custom-made PEEK post-and-cores fabricated using different techniques. MATERIAL AND METHODS: Ten mandibular second premolars were collected, decoronated, and endodontically treated. For the fabrication of custom-made post-and-cores, each post space was prepared, scanned, and designed by using the exocad software program. Each reference design was used to fabricate PEEK post-and-cores with 3 different techniques (N=30): group P, heat pressing; group S, subtractive manufacturing, and group A, additive manufacturing. All the fabricated restorations were scanned, providing the test data as standard tessellation language (STL) files, which were imported with the reference data into a reverse engineering software program to evaluate the 3D trueness of each fabrication technique. To evaluate the intaglio fit, restorations coated with silicone material were scanned to provide the test data as STL files, which were superimposed on the STL files of the scanned restorations. One-way analysis of variance (ANOVA) and 2-way mixed model ANOVA tests were used to analyze the data (α=.05). RESULTS: For 3D trueness, root mean square (RMS) values representing the deviations between the scans of the fabricated restorations and the reference designs varied significantly among different groups (P<.001), with the highest mean deviation found in group A (102.8 ±31 µm), while the lowest mean deviation was found in group S (42.2 ±11 µm). For the intaglio fit, the deviations between the scans of the fabricated restorations and the scans of the coated restorations varied significantly among different groups (P<.001), with the highest mean deviation found in group A (228.4 ±47 µm), while the lowest mean deviation was found in group S (96.2 ±14 µm). Simple effects comparisons showed the highest value of deviation in the apical area in group A (P<.001). CONCLUSIONS: Custom-made PEEK post-and-cores fabricated by using subtractive manufacturing and heat pressing techniques showed better trueness and intaglio fit compared with additively manufactured restorations. Before printed PEEK restorations can be considered a suitable alternative to pressed or milled ones, improvements are required to confirm their reliability.

Cs-137 is the most released fission product in the marine environment. It is important to develop a robust in situ technique for its monitoring. The existing diffusive gradients in thin films (DGT) passive sampling techniques for in situ measurement of Cs+ have some limitations due to the ion competition and high pH of seawater. A new DGT sampler based on potassium zinc hexacyanoferrate (KZFCN) as a binding layer has been developed and investigated for the measurement of the time-weighted average concentration of Cs-137 in seawater. This binding layer proved a working pH range of 2-12 and an ionic strength of up to 0.75 M. Two types of diffusive gels were tested and agarose gel (AGE) was chosen for the KZFCN-DGT sampler. The measured Cs+ diffusion coefficient (1.71 × 10-5 cm2·s-1 at 25 °C) in the diffusive gel from seawater was within the expected range published in the literature. The measured concentrations of Cs-137 in seawater obtained by laboratory deployments of the KZFCN-DGT samplers for up to 4 weeks showed good precision (RSD = 13%) and accuracy (relative error = 8.5%) values. The performance test results demonstrated that the KZFCN-DGT sampler is suitable for long-term monitoring of Cs-137 in seawater due to its high capacity and resistance to ion competition and high pH.

STATEMENT OF PROBLEM: Trial restorations are a versatile tool for visualizing the esthetic treatment plan and should be an accurate replica of the planned smile design. The accuracy of conventionally fabricated trial restorations has been investigated; however, studies on the accuracy of different digital workflows for trial restoration fabrication are lacking. PURPOSE: The purpose of this in vitro study was to evaluate the accuracy of computer-aided design (CAD) trial restorations fabricated with different digital workflows. MATERIAL AND METHODS: A patient in need of additive esthetic treatment was examined, and intraoral and extraoral photographs and intraoral digital scans were made, followed by 3D digital smile design using the exocad software program. The reference virtual design was converted into trial restorations (N=40) by using different digital workflows: subtractive manufacturing, additive manufacturing, additive manufacturing of 3D designed index, and silicone index on an additively manufactured cast. Fabricated trial restorations of all groups were scanned with a desktop scanner, providing a standard tessellation language (STL) file serving as the test data for each trial restoration. All the STL files of the test and reference data were imported into a reverse engineering software program to evaluate the 3D trueness of each workflow. Also, analog and digital 2D linear measurements (maxillary left central incisor height, maxillary left central incisor width, and intercanine width) were made to assess any dimensional alterations between the fabricated trial restorations and the reference digital smile design. A 1-way analysis of variance (ANOVA) was used to analyze the data followed by the Tukey post hoc test (α=.05). RESULTS: For 3D trueness measurements, root mean square (RMS) values representing the deviations between the fabricated trial restorations and the reference digital smile design varied significantly among the different groups (P<.001), with the highest mean deviation found in the group of additive manufacturing of 3D designed index (0.21 ±0.01 mm), while the lowest mean value of deviation was found in the group of subtractive manufacturing (0.11 ±0.02 mm). For both the digital and analog 2D linear measurements, post hoc pairwise comparisons showed the group of additive manufacturing of 3D designed index to have significantly higher values of deviation than the other groups in all assigned measurements (P<.001). CONCLUSIONS: The accuracy of CAD trial restorations was affected by the fabrication technique, and implementing conventional steps in the digital workflows of trial restoration fabrication may result in discrepancies that affect accuracy when compared with the reference design.

OBJECTIVE: The purpose of this study was evaluating the biaxial strength of bi-layered PEEK restorations before and after aging using different veneering materials in different thickness ratios. MATERIAL AND METHODS: Ninety specimens of thickness 1.5 mm were divided into three groups according to their veneering material. Group (CAD LD): BioHPP discs veneered with CAD milled lithium disilicate (n=30), group (CAD C): BioHPP discs veneered with CAD milled composite (n=30), and group (LC): BioHPP discs veneered with conventionally layered composite (n=30). Each group was subdivided into 3 subgroups (n=10) according to the different thickness ratios between the core and the veneering material (TC:TV). Subgroup 1: TC:TV=1:0.5, subgroup 2: TC:TV=0.7:0.8, and subgroup 3: TC:TV=0.5:1. Half of the specimens of each subgroup were subjected to thermocycling, and the bi-axial flexural strength of all specimens was tested before and after aging. Three-way ANOVA followed by Bonferroni's post hoc test were used for data analysis. The significance level was set at P ≤ 0.05. RESULTS: Material, thickness ratio, and aging all had a significant effect on biaxial flexural strength. (LC) group had the highest biaxial flexural strength. TC:TV=0.5:1 showed the lowest biaxial flexural strength. All groups showed significant decrease in biaxial flexural strength after aging. CONCLUSIONS: Veneering material for PEEK together with the thickness ratio between the core and veneering material greatly affect the flexural strength of bi-layered restorations. Thermocycling negatively impacts the flexural strength of PEEK bi-layered restorations. CLINICAL SIGNIFICANCE: According to the results of that study, PEEK cores are best veneered with conventionally layered composite with core to veneering thickness ratio being 1:0.5.

PURPOSE: Although endocrown is a successful restorative approach for endodontically treated molars, its survival rate in endodontically treated premolars with extensive loss of coronal structure has been debated. The aim of this study was to evaluate the biomechanical behavior of endodontically treated maxillary premolars restored with different lithium disilicate endocrown designs. MATERIALS AND METHODS: Based on cone-beam computed tomography (CBCT) of an intact maxillary premolar, five models were designed. Model A: fiber post, core, and crown; Model B: endocrown, 5 mm pulpal extension and butt margin; Model C: endocrown, 5 mm pulpal extension and axial extension; Model D: Endocrown, 3 mm pulpal extension and butt margin; Model E: Endocrown, 3 mm pulpal extension and axial extension. The bone geometry was simplified as a cylinder of compact and trabecular bone. All models were imported into finite element analysis (FEA) software, where the base of the bone cylinder was chosen as fixed support. Axial and oblique loads of 100 N each were applied separately to each model, and static structural analysis was performed. RESULTS: Regardless of the design of the endocrown, the resulting von Mises stresses were far below the yield strength of the tooth structure and the flexural strength of the ceramic material. The generated von Mises stresses on the restoration decreased by 15% in the models with 3 mm pulp extension (D and E) compared to the 5-mm pulpal extension models. In addition, the resulting von Mises stresses on the tooth structure decreased also by 15% in models C and E with the axial extension compared to models B and D with the butt margin. CONCLUSION: Endocrown is a suitable restoration for endodontically treated maxillary premolars. Furthermore, reducing the depth of the pulpal extension to 3 mm with the addition of an axial extension resulted in a more favorable stress distribution within the tooth-restoration interface.

RESUMO

This clinical report describes the digital workflow for implementing different virtual and digital aids for the restoration of tooth wear and loss of occlusal vertical dimension (OVD). This conservative approach was implemented in an adult with pseudo class III malocclusion by using computer-aided design and computer-aided manufacturing (CAD-CAM) monolithic lithium disilicate ceramic restorations.

Objective: To evaluate the effect of scaling procedures using different ultrasonic tips on the surface roughness, color stability and bacterial accumulation of lithium disilicate ceramic. Material and Methods: Scaling procedure was carried out using ultrasonic scaler (Satalec, Acteon, North America) with stainless-steel tip (US), titanium tip (UT) and plastic tip (UP), on disc shaped lithium disilicate samples cemented into a cavity prepared onto the labial surface of freshly extracted bovine teeth (10 samples per group). The samples were stored in coffee solution in an incubator at 37°C for 12 days, which is equivalent to 1 year of coffee consumption. The surface roughness was measured before and after the scaling procedure using a profilometer and atomic force microscopy. The color parameters were measured before and after scaling and staining procedures using VITA Easyshade Advance 4.0 according to the CIE L*a*b* color order system. The samples were then incubated with Streptococcus mutans (S. mutans) suspension. After incubation, the plates with 30 to 300 typical colonies of S. mutans were counted in a colony counter and mean values of colony forming units were obtained (CFU/mL). Results:The titanium scaling tip showed a statistically significant higher mean values of change in surface roughness ΔRa and bacterial count than the plastic scaling tip. Color changes (ΔE) were not a statistically significant among the groups. The results showed a statistically significant positive (direct) correlation between surface roughness and color change (p = 0.012) and also between surface roughness and bacterial count (p = 0.00). Conclusion: Within the limitations of this study, titanium scaling instruments cause irreversible surface alterations of lithium disilcate ceramics which was in direct correlation to the color changes and bacterial accumulation; therefore, dentists should proceed with caution when scaling lithium disilicate surfaces. The findings of the current study may indicate the need for instruments or equipment that can remove plaque and calculus without causing surface damage (AU)


Introdução: Avaliar o efeito de procedimentos de raspagem com diferentes pontas de ultrassom na rugosidade superficial, estabilidade de cor e acúmulo bacteriano em cerâmica de dissilicato de lítio. Material e Métodos: O procedimento de raspagem foi realizado usando um aparelho de ultrassom (Satalec, Acteon, América do Norte) com ponta de aço inoxidável (US), ponta de titânio (UT) e ponta de plástico (UP), em amostras de dissilicato de lítio em forma de disco cimentadas em uma cavidade preparada na superfície vestibular de dentes bovinos recém-extraídos (10 amostras por grupo). As amostras foram armazenadas em solução de café em incubadora a 37 ° C por 12 dias, o que equivale a 1 ano de consumo de café. A rugosidade da superfície foi medida antes e após o procedimento de raspagem usando um perfilômetro e um microscópio de força atômica. Os parâmetros de cor foram medidos antes e depois dos procedimentos de raspagem e armazenagem no café usando VITA Easyshade Advance 4.0 de acordo com o sistema de ordem de cores CIE L*a*b*. As amostras foram incubadas com suspensão de Streptococcus mutans (S. mutans). Após a incubação, as placas com 30 a 300 colônias típicas de S. mutans foram contadas em contador de colônias e obtidos os valores médios das unidades formadoras de colônias (UFC / mL). Resultados: A ponta de titânio mostrou valores estatisticamente maiores de mudança na rugosidade da superfície ΔRa e contagem de bactérias do que a ponta de raspagem de plástico. A mudança de cor (ΔE) não foi estatisticamente significativa entre os grupos. Os resultados mostraram uma correlação positiva (direta) estatisticamente significativa entre rugosidade superficial e alteração de cor (p = 0,012) e também entre rugosidade superficial e contagem bacteriana (p = 0,00). Conclusão: Dentro das limitações deste estudo, os instrumentos de raspagem de titânio causam alterações irreversíveis na superfície das cerâmicas de dissilicato de lítio que estão em correlação direta com as mudanças de cor e o acúmulo de bactérias. Portanto, os dentistas devem proceder com cautela ao realizar raspagem em superfícies de dissilicato de lítio. Os resultados deste estudo podem indicar a necessidade de instrumentos ou equipamentos que possam remover a placa e cálculo sem causar danos à superfície. (AU)

Objective: TThe study evaluated in vitro the changes in roughness, color stability, and bacterial count of a CAD/CAM Resin Nano-Ceramic material surface treated by various scaling procedures. Material and Methods: 70 disks (5mm diameter, 0.5 mm thickness) of Resin Nano-Ceramic (Lava ™Ultimate, 3M, ESPE) material were cemented in standardized cavities prepared in bovine teeth. A custom-made scaling apparatus of a double pan balance was used for different scaling methods, simulating standard clinical conditions. The specimens were assigned to three main groups: no scaling(C), ultrasonic scaling (U), and manual scaling (M). Each group was then divided into three subgroups according to scaling tip material; stainless steel tip (St), plastic tip (P), and titanium tip (Ti). The surface texture was analyzed quantitatively and qualitatively with a tactile profilometer and atomic force microscopy. A spectrophotometer was used for color measurement. Streptococcus mutans were counted in a colony counter. All the data were tabulated and statistically analyzed. Results: Two-way ANOVA was used to study the effect while One-way ANOVA was performed to compare between study groups. The significance level was set at p ≤ 0.05. The ultrasonic titanium tip(UTi) revealed the significant highest mean value of alterations (p < 0.001). The integrity of the material surface was altered in the form of deep scratches on the ultrasonically scaled surfaces and numerous smaller scratches on the hand-scaled surfaces. Conclusion: The plastic instrument would appear to be the instrument of choice during a routine maintenance procedure for Resin Nano-Ceramic materials (AU).


Objetivo: Este estudo avaliou in vitro as alterações na rugosidade, estabilidade de cor e quantidade de bactérias da superfície de uma resina nano-cerâmica produzida em CAD/CAM tratada por diferentes procedimentos de raspagem. Material e Métodos: 70 discos (5 mm diâmetro, 0,5 espessura) de resina nano-cerâmica (Lava ™Ultimate, 3M, ESPE) foram cimentadas em cavidades padronizadas preparadas em dentes bovinos. Um aparato customizado de raspagem com pratos duplos de balança foram usados para os diferentes métodos de raspagem, simulando uma condição clínica padronizada. Os espécimes foram distribuídos em três principais grupos: Sem raspagem (C), raspagem ultrassônica (U) e raspagem manual (M). Cada grupo foi dividido em três subgrupos de acordo com a ponta do material de raspagem; ponta de aço inoxidável (St), ponta plástica (P), ponta de titânio (Ti). A textura da superfície foi analisada qualitativamente e quantitativamente por um perfilômetro tátil e microscopia de força atômica. Um espectrofotometro foi usado para a mensuração da cor. Estreptococos mutans foram contados em um contador de colônias. Todos os dados foram tabulados e analisados estatisticamente. Resultados: Anova dois fatores foi utilizado para estudar os efeitos, enquanto ANOVA um fator foi utilizado para comparar os grupos experimentais. O nível de significância foi estabelecido em p≤ 0,05. A ponta de ultrassom de titânio (UTi) revelou o valor significante mais alto de alterações (p < 0,001). A integridade da superfície d matérias foi alterada na forma ranhuras profundas, nas superfícies raspada por ultrassom e numerosas ranhuras menores nas superfícies raspadas à mão. Conclusão: O instrumento plástico poderia apresentar-se como um instrumento de escolha durante o pocedimento de manutenção routineira para materiais de resina nano-ceramica. (AU)

Objective: Despite the higher translucency of nanocrystalline Y-TZP than the conventional Y-TZP, it couldn't be used in the anterior region of the mouth in its fully anatomical form without veneering for better esthetics. This study evaluated the color change (ΔE) and translucency parameter (TP) of the bilayered nano-crystalline zirconia restorations after different treatments of the zirconia surface before and after accelerated aging. Material and Methods: Forty samples of inCoris TZI were cut in the form of discs 15 mm (diameter) and 1mm (thickness) so that after sintering each disc dimensions' would be 12 mm (diameter) and 0.8 mm (thickness). All discs were colored by dipping in aqueous zirconia coloring solution of shade A2, then divided into four groups according to the type of surface treatment received before sintering; being tribochemical silica coating, zirconia powder deposition, glass grading, and a control untreated group. After sintering, all discs were veneered with a layer of porcelain 0.7 mm (thickness) and 12 mm (diameter). (ΔE) and TP were measured using VITA Easyshade Compact spectrophotometer before and after accelerated autoclave aging. Results: Different surface treatments applied had significant effect (P<0.05) over ΔE and TP. Glass graded group showed the highest ΔE value and the least TP value among both un-aged and aged sub-groups. Conclusion: Different surface treatments applied affected the color and translucency negatively where the control untreated group showed the highest TP and the least color deviation from the A2 color parameters. On the other hand, accelerated autoclave aging had no effect over translucency or color. (AU)


Objetivo: Apesar da maior translucidez da Y-TZP nanocristalina em relação à Y-TZP convencional, ela não pode ser usada na região anterior da boca em sua forma totalmente anatômica, sem o recobrimento cerâmico para uma melhor estética. Este estudo avaliou a mudança de cor (ΔE)eoparâmetrodetranslucidez(PT)das restauraçõesde zircônia nano-cristalina em duas camadas após diferentes tratamentos de superfície, antes e após o envelhecimento. Material e Métodos: Quarenta amostras de inCoris TZI foram cortadas na forma de discos de 15 mm (diâmetro) e 1 mm (espessura) para que, após sinterização, cada diâmetro dos discos fosse de 12 mm (diâmetro) e 0,8 mm (espessura). Todos os discos foram corados por imersão em solução aquosa de zircônia corante da sombra A2, divididos em quatro grupos de acordo com o tipo de tratamento de superfície recebido antes da sinterização; sendo revestimento de sílica químico-química, deposição de pó de zircônia, jateamento de vidro e um grupo de controle sem tratamento. Após a sinterização, todos os discos foram revestidos com uma camada de porcelana de 0,7 mm (espessura) e 12 mm (diâmetro). ΔE e TP foram medidos usando o espectrofotômetro VITA Easyshade Compact antes e depois do envelhecimento em autoclave. Resultados: Diferentes tratamentos de superfície aplicados tiveram efeito significativo (P <0,05) sobre ΔE o e maior TP. valor O grupo com de ∆E e o menor jateamento de vidro valor de TP apresentou entre os subgrupos não envelhecidos e envelhecidos. Conclusão: Diferentes tratamentos de superfície aplicados afetaram negativamente a cor e a translucidez, onde o grupo controle sem tratamento apresentou o maior TP e o menor desvio de cor dos parâmetros de coloração A2. Por outro lado, o envelhecimento acelerado da autoclave não teve efeito sobre a translucidez ou a cor. (AU)

The present study aimed to assess the radiological parameters from gamma rays due to the uncontrolled disposal of porcelain waste to the environment. Qualitative and quantitative identification of radionuclides in the investigated samples was carried out by means of a high-purity germanium (HPGe) detector. The average activity concentrations of the local porcelain samples were measured as 208.28 Bq/kg for 226Ra, 125.73 Bq/kg for 238U, 84.94 Bq/kg for 232Th and 1033.61 Bq/kg for 40K, respectively. The imported samples had an average activity of 240.57 Bq/kg for 226Ra, 135.56 Bq/kg for 238U, 115.74 Bq/kg for 232Th and 1312.49 Bq/kg for 40K, respectively. Radiological parameters and the radium equivalent Raeq for the investigated samples were calculated. The external and internal hazard indices, representative level index (Iγ), alpha index (Iα), and the exemption level (Ix), were estimated to be higher than the recommended value (unity), while the average activity concentrations for the studied samples were higher than recommended levels. In conclusion, we are concerned that disposal of porcelain in the environment might be a significant hazard.

PURPOSE: To evaluate the fracture mechanics of cemented versus fused CAD-on veneers on customized zirconia implant abutments. MATERIALS AND METHODS: Forty-five identical customized CAD/CAM zirconia implant abutments (0.5 mm thick) were prepared and seated on short titanium implant abutments (Ti base). A second scan was made to fabricate 45 CAD-on veneers (IPS Empress CAD, A2). Fifteen CAD-on veneers were cemented on the zirconia abutments (Panavia F2.0). Another 15 were fused to the zirconia abutments using low-fusing glass, while manually layered veneers served as control (n = 15). The restorations were subjected to artificial aging (3.2 million cycles between 5 and 10 kg in a water bath at 37°C) before being axially loaded to failure. Fractured specimens were examined using scanning electron microscopy to detect fracture origin, location, and size of critical crack. Stress at failure was calculated using fractography principles (alpha = 0.05). RESULTS: Cemented CAD-on restorations demonstrated significantly higher (F = 72, p < 0.001) fracture load compared to fused CAD-on and manually layered restorations. Fractographic analysis of fractured specimens indicated that cemented CAD-on veneers failed due to radial cracks originating from the veneer/resin interface. Branching of the critical crack was observed in the bulk of the veneer. Fused CAD-on veneers demonstrated cohesive fracture originating at the thickest part of the veneer ceramic, while manually layered veneers failed due to interfacial fracture at the zirconia/veneer interface. CONCLUSIONS: Within the limitations of this study, cemented CAD-on veneers on customized zirconia implant abutments demonstrated higher fracture than fused and manually layered veneers.
