Background  Foot ulcer is a common complication of poorly controlled diabetes and peripheral vascular disease (PVD). The current standard of treatment for diabetic foot ulcers includes the management of underlying risk factors, wound debridement, use of antibiotics for infection, off-loading with cast, and revascularisation surgery. The glyceryl trinitrate (GTN) patch is currently off-licence in treating PVD or diabetic foot ulcers. This study aims to evaluate the effectiveness of the GTN patch in preventing amputation, improving pain control, and reducing the size of tissue loss (ulcer/gangrene) or localised ischaemic area. Method This is a pilot study of 30 patients who were started on the GTN patch from February 2020 to October 2021. Inclusion criteria were patients who have critical limb-threatening ischaemia (CLTI) and with no viable options or are at high risk for revascularisation, both endovascular and open surgery. Patients who were on a GTN patch for less than six weeks at the time of data collection or had unclear outcomes were excluded. The outcomes were retrospectively collected on prevention of amputation, improvement in pain control, and reduction in tissue loss (the size of ulcer/gangrene) or localised ischaemic area with the use of a GTN patch. The binomial test was used to compare the observed outcome of the GTN patch and the expected outcome, which was assumed to be 50% in this study. Results  Ninety-three per cent (93%) of the patients who had GTN patches successfully avoided amputation (p<0.0001). Eighty-four per cent (84%) of patients reported better pain control (p=0.0022) and improvement in the size of ulcer/gangrene/localised ischaemic areas (p=0.0005). Conclusion The GTN patch is effective in preventing amputation, improving pain control, and reducing the size of ulcer/gangrene/localised ischaemic areas in patients who have end-stage CLTI and no viable options or who are at high risk for revascularisation surgery.

Background: Peripheral artery disease (PAD) is on the rise worldwide, ranking as the third leading cause of atherosclerosis-related morbidity; much less is known about its trends in hospitalizations among methamphetamine and cocaine users. Objectives: We aim to evaluate the overall trend in the prevalence of hospital admission for PAD with or without the use of stimulant abuse (methamphetamine and cocaine) across the United States. Additionally, we evaluated the PAD-related hospitalizations trend stratified by age, race, sex, and geographic location. Methods: We used the National Inpatient Sample (NIS) database from 2008 to 2020. The Cochran Armitage trend test was used to compare the trend between groups. Multivariate logistic regression was used to examine adjusted odds for PAD and CLI hospitalizations among methamphetamine and cocaine users. Results: Between 2008 and 2020, PAD-related hospitalizations showed an increasing trend in Hispanics, African Americans, and western states, while a decreasing trend in southern and Midwestern states (p-trend <0.05). Among methamphetamine users, an overall increasing trend was observed in men, women, western, southern, and midwestern states (p-trend <0.05). However, among cocaine users, PAD-related hospitalization increased significantly for White, African American, age group >64 years, southern and western states (p-trend <0.05). Overall, CLI-related hospitalizations showed an encouraging decreasing trend in men and women, age group >64 years, and CLI-related amputations declined for women, White patient population, age group >40, and all regions (p-trend <0.05). However, among methamphetamine users, a significantly increasing trend in CLI-related hospitalization was seen in men, women, White & Hispanic population, age group 26-45, western, southern, and midwestern regions. Conclusions: There was an increasing trend in PAD-related hospitalizations among methamphetamine and cocaine users for both males and females. Although an overall decreasing trend in CLI-related hospitalization was observed for both genders, an up-trend in CLI was seen among methamphetamine users. The upward trends were more prominent for White, Hispanic & African Americans, and southern and western states, highlighting racial and geographic variations over the study period.

In order to determine whether a particle is plasmonic, dielectric, or chiral, different complex processes and chemicals are applied in lab setups and pharmaceutical industries. Sorting or categorizing a particle based on distinct optical forces can be a novel technique. When a beam of light interacts with a particle, it usually pushes the particle in the direction of the light's propagation. Counterintuitively, it can also pull the particle toward the light beam or move it toward a lateral direction. As far as we know, to date, no comprehensive report exists regarding a single optical arrangement capable of inducing entirely distinct behaviors of force for three disparate types of independently placed single Rayleigh particle. This study introduces an all-optical technique aimed at effectively sorting nanoscale Rayleigh-sized objects employing a plasmonic substrate, when each distinct type of single particle is placed over the substrate independently. Unfortunately, this proposed technique does not work for the cluster or mixture of distinct particles. In our proposed configuration, a simple linearly polarized plane wave is incident onto the plasmonic substrate, thereby engendering completely different responses from three different types of nanoparticles: Gold (plasmonic), SiO2 (dielectric), and Chiral particles. We conducted individual tests for our setup using linearly polarized plane waves at angles of 30-degree, 45-degree, and 60-degree individually. Consistent results were obtained across all angles. In each of the three distinct setups involving the aforementioned particle, a dielectric Rayleigh particle experiences an optical pulling force, a plasmonic Rayleigh particle experiences an optical pushing force, and a chiral Rayleigh particle encounters an optical lateral force. These distinctive force behaviors manifest as a result of the intricate interplay between the material properties of the nanoparticles and the characteristics of the plane-polarized beam, encompassing aspects such as plasmonic response, chirality, and refractive index. Moreover, this technique presents an environmentally sustainable and economically viable alternative to the utilization of expensive and potentially hazardous chemicals in nanoparticle sorting processes within industrial domains.

Introduction Failure of infrainguinal bypass grafts remains a major problem tackled by vascular surgeons despite a meticulous surgical technique. All infrainguinal bypasses should go under routine surveillance to pick the grafts at risk for the prevention of graft failure. Objectives The aim was to find out if we were adhering to the European Society of Vascular Surgery (ESVS) guidelines in the management of chronic limb-threatening ischaemia (CLTI) patients, including postoperative follow-up and to monitor whether the patients were having postoperative duplex surveillance scans to pick any graft at risk. Methods All patients who underwent infra-inguinal bypass procedures for CLTI during the last eight months (from mid-January to mid-September 2023) in our vascular unit were included. Retrospective data were collected. Results A total of 38 patients had lower limb bypass procedures over the last eight months (from 15 January till 14 September 2023). However, two femoral-femoral (fem-fem) crossovers, one Ilio-popliteal, and one pedal bypass were excluded. Thus, a total of 36 patients were included in the study (n=34). The vast majority (n=27, 79.4%) had femoro popliteal bypass anastomosing distally to above knee (AK) or below knee (BK) popliteal artery, and the rest (n=7, 20.5%) had distal bypass (fem-distal or pop-distal bypass). Moreover, 18% of patients had amputation, 15% of patients died, and 61% of the remaining patients were on surveillance. Of those, who were not on surveillance, 44% of them had graft occlusion. Conclusion Surveillance can predict graft at risk, and the graft occlusion can be prevented by appropriate intervention. Every vascular unit should have its own post-procedural follow-up strategies.

A family of bis[(R or S)-N-1-(Ar)ethylsalicylaldiminato-κ2 N,O]-Δ/Λ-zinc(II) {Ar=C6 H5 (ZnRL1 or ZnSL1 ), p-CH3 OC6 H4 (ZnRL2 or ZnSL2 ) and p-ClC6 H4 (ZnRL3 or ZnSL3 )} compounds was synthesized and investigated by multiple methods. They feature Λ/Δ-chirality-at-metal induction along the pseudo-C2 axis of the molecules. The chirality induction is quantitative in the solid state, explored by X-ray crystallography and powder X-ray diffraction (PXRD), where R or S-ligated complexes diastereoselectively yield Λ or Δ-configuration at the metal. On the other hand, Λ and Δ-diastereomers co-exist in solution. The Λâ‡†Δ equilibrium is solvent- and temperature-dependent. Electronic circular dichroism (ECD) spectra confirm the existence of a diastereomeric excess of Λ-ZnRL1-3 or Δ-ZnSL1-3 in solution. DSC analysis reveals thermally induced irreversible phase transformation from a crystalline solid to an isotropic liquid phase. ECD spectra were reproduced by DFT geometry optimizations and time-dependent DFT (TD-DFT) calculations, providing ultimate proof of the dominant chirality atmetal in solution.

Melanogenesis is the production of melanin from tyrosine by a series of enzyme-catalyzed reactions, in which tyrosinase and DOPA oxidase play key roles. The melanin content in the skin determines skin pigmentation. Abnormalities in skin pigmentation lead to various skin pigmentation disorders. Recent research has shown that the expression of EMP2 is much lower in melanoma than in normal melanocytes, but its role in melanogenesis has not yet been elucidated. Therefore, we investigated the role of EMP2 in the melanogenesis of MNT1 human melanoma cells. We examined TRP-1, TRP-2, and TYR expression levels during melanogenesis in MNT1 melanoma cells by gene silencing of EMP2. Western blot and RT-PCR results confirmed that the expression levels of TYR and TRP-2 were decreased when EMP2 expression was knocked down by EMP2 siRNA in MNT1 cells, and these changes were reversed when EMP2 was overexpressed. We verified the EMP2 gene was knocked out of the cell line (EMP2 CRISPR/Cas9) by using a CRISPR/Cas9 system and found that the expression levels of TRP-2 and TYR were significantly lower in the EMP2 CRISPR/Cas9 cell lines. Loss of EMP2 also reduced migration and invasion of MNT1 melanoma cells. In addition, the melanosome transfer from the melanocytes to keratinocytes in the EMP2 KO cells cocultured with keratinocytes was reduced compared to the cells in the control coculture group. In conclusion, these results suggest that EMP2 is involved in melanogenesis via the regulation of TRP-2 expression.

Advanced or metastatic breast cancer affects multiple organs and is a leading cause of cancer-related death. Cancer metastasis is associated with epithelial-mesenchymal metastasis (EMT). However, the specific signals that induce and regulate EMT in carcinoma cells remain unclear. PRR16/Largen is a cell size regulator that is independent of mTOR and Hippo signalling pathways. However, little is known about the role PRR16 plays in the EMT process. We found that the expression of PRR16 was increased in mesenchymal breast cancer cell lines. PRR16 overexpression induced EMT in MCF7 breast cancer cells and enhances migration and invasion. To determine how PRR16 induces EMT, the binding proteins for PRR16 were screened, revealing that PRR16 binds to Abl interactor 2 (ABI2). We then investigated whether ABI2 is involved in EMT. Gene silencing of ABI2 induces EMT, leading to enhanced migration and invasion. ABI2 is a gene that codes for a protein that interacts with ABL proto-oncogene 1 (ABL1) kinase. Therefore, we investigated whether the change in ABI2 expression affected the activation of ABL1 kinase. The knockdown of ABI2 and PRR16 overexpression increased the phosphorylation of Y412 in ABL1 kinase. Our results suggest that PRR16 may be involved in EMT by binding to ABI2 and interfering with its inhibition of ABL1 kinase. This indicates that ABL1 kinase inhibitors may be potential therapeutic agents for the treatment of PRR16-related breast cancer.

Road crash fatality is a universal problem of the transportation system. A massive death toll caused annually due to road crash incidents, and among them, vulnerable road users (VRU) are endangered with high crash severity. This paper focuses on employing machine learning-based classification approaches for modelling injury severity of vulnerable road users-pedestrian, bicyclist, and motorcyclist. Specifically, this study aims to analyse critical features associated with different VRU groups-for pedestrian, bicyclist, motorcyclist and all VRU groups together. The critical factor of crash severity outcomes for these VRU groups is estimated in identifying the similarities and differences across different important features associated with different VRU groups. The crash data for the study is sourced from the state of Queensland in Australia for the years 2013 through 2019. The supervised machine learning algorithms considered for the empirical analysis includes the K-Nearest Neighbour (KNN), Support Vector Machine (SVM) and Random Forest (RF). In these models, 17 distinct road crash parameters are considered as input features to train models, which originate from road user characteristics, weather and environment, vehicle and driver condition, period, road characteristics and regions, traffic, and speed jurisdiction. These classification models are separately trained and tested for individual and unified VRU to assess crash severity levels. Afterwards, model performances are compared with each other to justify the best classifier where Random Forest classification models for all VRU modes are found to be comparatively robust in test accuracy: (motorcyclist: 72.30%, bicyclist: 64.45%, pedestrian: 67.23%, unified VRU: 68.57%). Based on the Random Forest model, the road crash features are ranked and compared according to their impact on crash severity classification. Furthermore, a model-based partial dependency of each road crash parameters on the severity levels is plotted and compared for each individual and unified VRU. This clarifies the tendency of road crash parameters to vary with different VRU crash severity. Based on the outcome of the comparative analysis, motorcyclists are found to be more likely exposed to higher crash severity, followed by pedestrians and bicyclists.

The study evaluated the nutritional quality and investigated the heavy metals concentration in muscle tissues of five commercially important marine fish species, including brownspotted grouper (Epinephelus chlorostigma), squaretail coralgrouper (Plectropomus areolatus), black pomfret (Parastromateus niger), goldbanded jobfish (Pristipomoides multidens), and blueskin seabream (Polysteganus coeruleopunctatus) from the Red Sea, Jeddah Coast, Saudi Arabia. Significant differences (p < 0.05) were observed in the proximate chemical composition of fish muscles in these species. The highest protein content (17.66 ± 0.58%) was achieved in blueskin seabream while the lowest (15.28 ± 0.46%) was observed in brownspotted grouper. The highest lipid content (2.97 ± 0.45%) was recorded in squaretail coralgrouper while the lowest (1.52 ± 0.26%) was observed in blueskin seabream. Heavy metal concentrations varied significantly within and between fish species under study (p < 0.05). Significant differences in the concentration of heavy metals among fish species were recorded. Results revealed that the bioaccumulation of Cr, Fe, Ni, and Cd in muscles of fish species under study was higher than the standard concentration, but that of Mn, Cu, and Pb were less than the standard concentration recommended in the EU, FAO, and WHO guidelines. In conclusion, these fish species represent a high-quality food source but is unsafe due to the level of certain minerals in their tissues. Results also indicated that the Red Sea environment is contaminated with heavy metals, which was reflected in the tissues of fishes used in this study.

This study evaluates changes in fish species diversity over time in Hakaluki Haor, an ecologically critical wetland in Bangladesh, and the factors affecting this diversity. Fish species diversity data were collected from fishers using participatory rural appraisal tools and the change in the fish species diversity was determined using Shannon-Wiener, Margalef's Richness and Pielou's Evenness indices. Principal component analysis (PCA) was conducted with a dataset of 150 fishers survey to characterize the major factors responsible for the reduction of fish species diversity. Out of 63 fish species, 83% of them were under the available category in 2008 which decreased to 51% in 2018. Fish species diversity indices for all 12 taxonomic orders in 2008 declined remarkably in 2018. The first PCA (climatic change) responsible for the reduced fish species diversity explained 24.05% of the variance and consisted of erratic rainfall (positive correlation coefficient 0.680), heavy rainfall (-0.544), temperature fluctuation (0.561), and beel siltation (0.503). The second PCA was anthropogenic activity, including the use of harmful fishing gear (0.702), application of urea to harvest fish (0.673), drying beels annually (0.531), and overfishing (0.513). Finally, the third PCA was loaded with the fishermen age (0.719), education (-0.767), and fishing experience (0.695) of the fishers. Deepening of beels could enhance dry season water availability and shelter the fish. Imposing fisheries regulations to reduce human activities is inevitable to sustain haor fisheries.

Agricultural sustainability is of foremost importance for maintaining high food production. Irresponsible resource use not only negatively affects agroecology, but also reduces the economic profitability of the production system. Among different resources, soil is one of the most vital resources of agriculture. Soil fertility is the key to achieve high crop productivity. Maintaining soil fertility and soil health requires conscious management effort to avoid excessive nutrient loss, sustain organic carbon content, and minimize soil contamination. Though the use of chemical fertilizers have successfully improved crop production, its integration with organic manures and other bioinoculants helps in improving nutrient use efficiency, improves soil health and to some extent ameliorates some of the constraints associated with excessive fertilizer application. In addition to nutrient supplementation, bioinoculants have other beneficial effects such as plant growth-promoting activity, nutrient mobilization and solubilization, soil decontamination and/or detoxification, etc. During the present time, high energy based chemical inputs also caused havoc to agriculture because of the ill effects of global warming and climate change. Under the consequences of climate change, the use of bioinputs may be considered as a suitable mitigation option. Bioinoculants, as a concept, is not something new to agricultural science, however; it is one of the areas where consistent innovations have been made. Understanding the role of bioinoculants, the scope of their use, and analysing their performance in various environments are key to the successful adaptation of this technology in agriculture.

Symbionts contribute nutrients that allow insects to feed on plants. The whitefly Bemisia tabaci Middle East-Asia Minor 1 (MEAM1) is a polyphagous pest that depends on symbionts to provide key nutrients that are deficient in the diet. Here, we established three whitefly populations on eggplants, cucumbers, and tomatoes and observed that they harbored the same symbiont taxa in different quantities. The amount of the primary symbiont, Portiera, decreased with increasing concentrations of host-plant essential amino acids (EAAs). Whitefly populations transferred to different plant species exhibited fluctuations in Portiera amounts in the first three or four generations; the amount of Portiera increased when whitefly populations were transferred to plant species with lower EAAs proportions. As for the secondary symbionts, the whitefly population of eggplants exhibited lower quantities of Hamiltonella and higher quantities of Rickettsia than the other two populations. The changes of both symbionts' abundance in whitefly populations after host-plant-shifting for one generation showed little correlation with the EAAs' proportions of host plants. These findings suggest that host-plant nitrogen nutrition, mainly in the form of EAAs, influences the abundance of symbionts, especially Portiera, to meet the nutritional demands of whiteflies. The results will inform efforts to control pests through manipulating symbionts in insect-symbiont associations.

Low proportion of essential amino acids (EAAs) is one of the barriers for animals to use phloem as a diet. Endosymbionts with EAAs synthesis functions are considered crucial for ameliorating the lack of EAAs in insects' diets. In this study, we transferred the insects from a cabbage-reared Myzus persicae population onto 3 new plant species including eggplant, tobacco and spinach. The performance on these plants was evaluated and the dynamics of endosymbionts in relation to this host alternation were recorded. We found that the EAAs ratio in phloem was largely determined by the concentrations of non-essential amino acids and the higher proportion of EAAs seemed to favor the population establishment on new plant species and the growth of primary endosymbionts inside insects, which indicated that nitrogen quality was an important factor for aphids to infest and spread on new plant hosts.

Degradation of soil and water from discharge of untreated industrial effluent is alarming in Bangladesh. Therefore, buildup of heavy metals in soil from contaminated effluent, their entry into the food chain and effects on rice yield were quantified in a pot experiment. The treatments were comprised of 0, 25%, 50%, 75% and 100% industrial effluents applied as irrigation water. Effluents, initial soil, different parts of rice plants and post-harvest pot soil were analyzed for various elements, including heavy metals. Application of elevated levels of effluent contributed to increased heavy metals in pot soils and rice roots due to translocation effects, which were transferred to rice straw and grain. The results indicated that heavy metal toxicity may develop in soil because of contaminated effluent application. Heavy metals are not biodegradable, rather they accumulate in soils, and transfer of these metals from effluent to soil and plant cells was found to reduce the growth and development of rice plants and thereby contributed to lower yield. Moreover, a higher concentration of effluent caused heavy metal toxicity as well as reduction of growth and yield of rice, and in the long run a more aggravated situation may threaten human lives, which emphasizes the obligatory adoption of effluent treatment before its release to the environment, and regular monitoring by government agencies needs to be ensured.

OBJECTIVES: Acinetobacter spp. has emerged as an important opportunistic pathogen responsible for nosocomial infections in many health-care settings worldwide. The study describes the clinico-epidemiology and antimicrobial susceptibility of Acinetobacter spp. in a tertiary health-care institution. Methodology : Acinetobacter spp. were isolated from 141 specimens of the patients who reported to Universiti Kebangsaan Medical Centre (UKMMC). The sources of specimens were wound, skin and soft tissue, respiratory and urinary tract from patients in various wards. Clinio-epidemiological features of patients infected with Acinetobacter spp. were recorded. Standard bacteriological techniques with API 20NE kits and disk diffusion method were followed for identification and antibiotic sensitivity of the organisms. RESULTS: One hundred and forty one patients with positive culture for Acinetobacter spp. were identified. Soft tissue/wound and respiratory tract were among the commonest sites of Acinetobacter spp. isolation. The isolates were most frequently obtained from ICU. All isolates were multi-drug resistant and had a resistance rate of more than 70% to most antibiotics, except polymyxin B. CONCLUSION: High prevalence of multi-drug resistance Acinetobacter spp. provides essential information on judicious antibiotic selection for empirical therapy in our health-care institution.

The sweetpotato whitefly, Bemisia tabaci biotype B is extremely polyphagous with >600 species of host plants. We hypothesized that previous experience of the whitefly on a given host plant affects their host selection and performance on the plants without previous experience. We investigated the host selection for feeding and oviposition of adults and development and survival of immatures of three host-plant-experienced populations of B. tabaci, namely Bemisia-eggplant, Bemisia-tomato and Bemisia-cucumber, on their experienced host plant and each of the three other plant species (eggplant, tomato, cucumber and pepper) without previous experience. We found that the influence of previous experience of the whiteflies varied among the populations. All populations refused pepper for feeding and oviposition, whereas the Bemisia-cucumber and the Bemisia-eggplant strongly preferred cucumber. Bemisia-tomato did not show strong preference to any of the three host plants. Development time from egg to adult eclosion varied among the populations, being shortest on eggplant, longest on pepper, and intermediate on tomato and cucumber except for the Bemisia-cucumber developed similarly on tomato and pepper. The survivorship from egg to adult eclosion of all populations was highest on eggplant (80-98%), lowest on pepper (0-20%), and intermediate on tomato and cucumber. In conclusion, the effects of previous experience of whiteflies on host selection for feeding and oviposition, development, and survivorship varied depending on host plants, and host plants play a stronger role than previous experience. Preference of feeding and oviposition by adults may not accurately reflect host suitability of immatures. These results provided important information for understanding whitefly population dynamics and dispersal among different crop systems.

Two Bangladeshi infectious bursal disease virus (IBDV) isolates collected in 2007, termed GB1 and GB3, were subjected to comparative sequencing and phylogenetic analyses. Sequence analysis of a 474-bp hypervariable region in the VP2 gene revealed that among four major amino acid substitutions observed in the strains, two were unique to GB1 and GB3 (Ser217Leu and Ala270Thr) while one substitution was only found in GB1 (Asn299Ser). Among IBDVs from Bangladesh including GB1 and GB3, the rate of identity and homology was around 97~99%. The amino acid sequences of GB1 and GB3 differ from those of previous Bangladeshi IBDV isolates and contain amino acid substitutions Pro222Ala and Asn299Ser (in GB3 only). Phylogenetic analysis revealed that GB1 and GB3 are grouped with other very virulent IBDVs of European and American origin in contrast to two previously isolated Bangladeshi IBDV strains (GenBank accession Nos. AF362776 and AF260317), which belong to the Asian group. It was concluded that GB1 and GB3 belong to a very virulent group of IBDVs. However, amino acid sequences of GB1 and GB3 differ from those of the other Bangladeshi IBDVs by one or two amino acids encoded in the hypervariable region of the VP2 gene.

Tenualosa ilisha is a popular, tasty fish found in the rivers of Bangladesh round the year. The fish is rich in oil which provides mainly 16:0 and 18:1 fatty acids but also a little of w-3 polyunsaturated fatty acids (w-3 PUFAs). The present study was undertaken to evaluate the effect of eating hilsa fish in hypercholesterolemic subjects. The fish was indeed found to have hypocholesterolemic effect in subjects who had hypercholesterolemia. After 10 months of eating 100g hilsa fish per day, serum total cholesterol level fell from 285.1 to 244.6 mg/dl (14.2% decrease) in the hypercholesterolemic subjects. But the difference was not statistically significant (p < 0.05). The fall in total cholesterol was exclusively due to fall in LDL-cholesterol. Serum triglyceride, serum HDL-cholesterol increased in the experimental subjects by 12.5%. On the other hand, serum total cholesterol, HDL-cholesterol, LDL-cholesterol and triglyceride levels were not changed in control subjects. Both in control and experimental subjects there were no changes in body weight and blood pressure during the study period. The results indicate that hilsa fish, although it is fatty and contains cholesterol, but it may reduce blood cholesterol level in hypercholesterolemic subjects.
