RESUMO

The concentration of NT-proBNP in saliva and blood serum was studied. The study included 58 individuals divided in two groups. The main group (group I) included 34 patients with chronic generalized periodontitis. The control group (group II), comparable in age and gender, included 24 examined individuals without signs of inflammatory diseases of oral cavity. The concentration of NT-proBNP in saliva and blood serum was detected by technique of solid-phase enzyme immunoassay using the test-system "NT-proBNP-ELISABEST" (Vector-Best, Russia). The concentration of NT-proBNP in saliva of patients of main group reliably exceeded its concentration in saliva of the examined from control group. The median and interquartile range made up to correspondingly 178.5 (128-253) pg / ml in main group and 105 (72.5 - 144.5) pg, respectively / ml in control group (p = 0.02). The comparison of concentration of NT-proBNP in blood serum samples established no reliable differences (p = 0,27). The evaluation of the relationship between content of NT-proBNP in saliva and blood serum demonstrated absence of close linear correlation dependence between the concentration of this compound in blood serum and saliva both in the main group (R = 0.143, p = 0.419) and the control group (R = 0.178; p = 0.405), and for all the examined as well (R = 0.252, p = 0.056). The higher concentration of NT-proBNP in saliva in comparison with blood serum can be explained by formation of proteolysis products cross-reacting in the used system.

One hundred forty-three patients aged 24-56 with chronic apical periodontitis were included in the study. The patients were divided in two groups according to treatment options: 51 patients underwent conventional root canal treatment (control group) and in 92 treatment was accomplished by hydrodynamic irrigation (main group). The main group was also subdivided in two subgroups, one of which received hydrodynamic irrigation with 3% sodium hypochlorite (47 patients) and the other additionally received laser root canal therapy. The patients were followed-up 7 days, 6 months and 2 years after procedure. The results showed that bone restoration occurred 1,3 times more rapidly in hydrodynamic irrigation and laser therapy group than by conventional treatment protocol.

Thirty-six patients with light and moderate chronic periodontal disease (CPD) were included in the study. The patients were divided in two groups according to treatment options: first group (18 patients) received conventional periodontal treatment, second (18 patients) - in addition to conventional treatment were treated by phototherapy (infrared laser light). Control group included 10 persons with no signs of periodontal disease. Treatment effectiveness was assessed by local cytokine profile (IL-1ß, IL-1ra, IL-6, IL-10, TNF-α, OPG and RANKL). Increased level of inflammatory cytokines TNF-α, IL-1ß, IL-6 confirmed pronounced inflammation in periodontal tissues in CPD patients. OPG level was lower than in control group while RANKL level increased significantly. After treatment in both CРD groups TNF-α and IL-6 decrease was observed, IL-1ß (had not changed. In the second group RANKL level also decreased dramatically - 6.7-fold compared to its values before treatment.

The aim of this work was the determination of the state of local immunity in periodontal complex in patients with chronic generalized periodontitis (CGP). 96 individuals were examined (mean age 43.6+/-1.2 years). All the patients were divided into 2 groups: basic group with CGP patients (76 persons) and comparative group - individuals with intact periodontium (20 persons). To evaluate local immunity in dentogingival fluids the determination of concentrations of IgG, IgM, and IgA immunoglobulins has been used, as well as TNF-alpha, IL-1, IL-6, IL-8, INF-gamma, IL-1ra, IL-10, and IL-4 cytokines, and also factors controlling the state of bone tissue, namely, osteoprotegerine (OPG), and RANK-ligand. In gingival fluid of CGP patients the increase in both pro-, and anti-inflammatory mediators with indication to Th2-deviation (decrease of INF-gamma level and elevation of IL-4 level) was observed. CGP patients exhibited in their periodontal complex marked increase of IgG, IgM, and IgA concentrations that apparently evidenced to the consequence of local polyclonal activation of B-lymphocytes. Gingival fluid of CGP patients showed the elevation of RANKL, TNF-alpha, and IL-1 levels, and the decrease in OPG concentration that could be the reason for osteoclast activation and subsequent destruction of bone tissue. In case of CGP in the zone of periodontium developed <> inflammation that is characterized by elevated level of IL-8 and predominance of neutrophil number over the quantity of other types of leukocytes.

RESUMO

Alternative methods for prevention and treatment of dental caries are presented, based on the use of laser and magnetic laser exposure (patent No. 2053818, in Russia) with a new generation laser device with Optodan microprocessor monitoring (patent No. 2014107, Russia). The methods are intended for wide clinical application in children and adolescents in pedodontic departments and outpatient centers and particularly for group and individual use in dental rooms at school.

RESUMO

Periodontal diseases, one of which is chronic catarrhal gingivitis (CCG), represent a medicosocial problem. Standard therapy often fails to bring about stable results. Addition of low-energy laser exposure at wavelengths of 0.63 micron and 0.85-0.89 micron to therapeutic complexes helps attain stable results, which is confirmed by clinical, functional, immunological, and x-ray data and densitometry. Laser therapy exerts a multifactorial pathogenetic effect, eliminating inflammation in periodontal tissue, normalizing microcirculation in the periodontium, improving mineral saturation of osseous tissue of the alveolar processes of the jaws, and improving local immunity of the oral cavity. All this accelerates treatment and decreases the probability of relapses.
