RESUMO

This article presents a case of a multimorbid 63-year-old woman with chronic kidney disease and heparin-induced thrombocytopenia (HIT). Following the insertion of a central venous catheter, she developed phlegmasia cerulea dolens (PCD) in her left arm, a rare and severe complication of deep vein thrombosis (DVT). Given the severity of the case, adapting to anticoagulant contraindications or unavailability, management with catheter-directed thrombolysis and mechanical thrombectomy was made. It is concluded that catheter-directed thrombolysis and mechanical thrombectomy are valuable therapeutic alternatives in critical situations where treatment options are limited.

RESUMO

Cocaine abuse is a public health concern with well-documented cardiovascular complications. However, acute limb ischemia remains a rare and underreported consequence. We present a case of a 36-year-old man with acute right lower limb ischemia following heavy cocaine use, successfully managed with systemic heparin and intra-arterial nitroglycerin. The case highlights considering cocaine as a potential cause of acute limb ischemia and the efficacy of endovascular therapy. Further case reports with this diagnosis and their management are crucial for establishing the best strategies and improving outcomes in these scenarios.

Gastroepiploic artery aneurysms and pseudoaneurysms pose diagnostic challenges due to their rarity and overlapping radiological features. This case report presents an 82-year-old woman with sudden-onset severe abdominal pain with computed tomography revealing hemoperitoneum and saccular dilations adjacent to the stomach's greater curvature, suggestive of vascular pathology. Selective abdominal arteriography confirmed three saccular dilatations in the gastroepiploic artery, which were managed successfully with coil embolization. The discussion emphasizes the importance of accurate diagnosis, distinguishing between aneurysms and pseudoaneurysms, and prompt intervention to mitigate the risk of hemorrhagic complications of either of them. The case underscores the significance of endovascular management in such rare and critical scenarios.

The endovascular treatment of acute lower limb ischemia associated with cocaine use is an underexplored topic. This scoping review aims to map existing information and point out potential knowledge gaps for future research. We searched databases with a strategy of terms and keywords (Cocaine, Acute, Lower Limb, and Ischemia) for articles related to acute ischemia in the lower limbs and cocaine use. We established eligibility and exclusion criteria and searched without restrictions on language or date of publication. We obtained five case reports published between 2004 and 2015. Most of the patients were men with an average age of 38 years. Treatments were heterogeneous. Most patients showed improvement after surgical thrombectomy. The sample size and variety of interventions limit the generalizability of the results, so it is necessary to do more studies with robust methodologies to standardize treatments and improve the understanding of the condition.

Resumen La hemosiderosis pulmonar idiopática es una entidad rara caracterizada por hemorragia alveolar capilar. Su tríada clásica es hemoptisis crónica o recurrente, anemia por deficiencia de hierro y opacidades en las imágenes pulmonares. El objetivo de esta revisión fue determinar la frecuencia en radiografía y tomografía de tórax, de los hallazgos de hemosiderosis pulmonar idiopática en adultos, reportados en la literatura durante los últimos 20 años, de acuerdo con los hallazgos semiológicos en imagen, localización y distribución. Se hizo una búsqueda de publicaciones en bases de datos. Se seleccionaron 42 estudios, se estratificaron variables y se recopilaron los hallazgos. La consolidación y las opacidades reticulonodulares fueron el hallazgo más frecuente en radiografía. En tomografía el hallazgo principal fue el vidrio deslustrado en la región basal. No hubo diferencias en los hallazgos semiológicos de imagen en cuanto al compromiso de acuerdo con el género, aunque las mujeres tendieron a presentar una distribución difusa. Abstract Idiopathic pulmonary hemosiderosis is a rare entity characterized by capillary alveolar hemorrhage. Its classic triad is chronic or recurrent hemoptysis, iron deficiency anemia, and opacities in lung images. The objective of this review was to determine the frequency in chest radiography and chest tomography of the findings of idiopathic pulmonary hemosiderosis in adults, reported in the literature during the last 20 years, according to the semiological imaging findings, location and distribution. A search of publications in databases was made. Forty-two studies were collected, variables were stratified in different categories and the findings were compiled. The most frequent finding in chest radiography were consolidation and reticulonodular opacities. On the other hand, the main finding in tomography was ground glass opacities in the basal region. There were no differences in the pattern of compromise by gender, although women tended to have a more diffuse distribution. MÉD.UIS.2020;33(2):55-64.

El síndrome del ligamento arcuato medio y el síndrome de cascanueces son entidades raras, con baja prevalencia, a menudo subdiagnosticadas y asintomáticas en la mayoría de las ocasiones. Se presenta el caso de una paciente con ambas entidades, lo cual probablemente le genera dolor abdominal como síntoma común.


The Arcuate ligament syndrome and the Nutcracker syndrome are rare disorders, with low prevalence, commonly underdiagnosed and asymptomatic in most cases. We present the case of a female patient with both syndromes, which probably causes her abdominal pain as a common symptom.
