RESUMO

In paper influence of a cytoprotective drug "Mildrocard" on morfo-functional condition of cardiorespiratory system at patients with chronic heart failure with concomitant chronic obstructive pulmonary disease is estimated. It is established, that joining "Mildrocard" to complex therapy associated to pathology promotes reduction clinical display of heart failure, shows cardioprotective and pulmoprotective effects.

RESUMO

There is formation of free radicals in mesangial cells in patients with chronic glomerulonephritis which increases destruction of renal tissue and enable autoimmune inflammation. The unbalance between activity of oxidizing and antioxidazing starts developing. It accelerates the progression of the disease. The article presents the assessment of influence of enzyme medication Wobenzym on main indices of oxidizing and antioxidazing systems. It was established the presence of antioxidant effect in Wobenzym medication. The use of this medication in combination with other drugs and without them enables restoration of the disturbed balance.

RESUMO

Analysis of clinical-roentgen-laboratory features of the course of hereditarily determined gout in 74 patients is represented. It is exhibited that hereditarily induced gout begins at the young age and is accompanied by the acute disturbance of purine exchange resulting in serious relapsing course of the articular (with availability of radiological attributes of osteal destruction) and the renal syndromes (urate nephrolithiasis, immune complex glomerulonephritis, tubulointerstitional nephritis) as well as in the early occurrence and rapidly progressing chronic renal insufficiency. The disease is accompanied by high frequency of the cardiovascular pathologies negatively influencing the mean lifetime of patients.

RESUMO

Thyroid gland function was investigated in rats with experimental hyperuricemia. Data obtained indicate that dyshormonal changes are secondary ones and their development depends on the state of purine metabolism. Administration of allopyrinol, an uric acid depressant drug, improved thyroid gland function.

Gout belongs to often-occurring rheumatic pathologies and its incidence among the male population of Europe has been rapidly increasing during the recent years. The pathogenesis of the disease is insufficiently studied and the existing methods of pathogenetic therapy are low effective. The disease pathogenesis with regard for dishormonal disorders has been recently under discussion. 107 patients with primary gout and 70 white outbred male rats were examined. Disorders in the hypophysis-gonad system (i.e. hyperproduction of progesterone, and suppression of the synthesis of testosterone and estradiol, which are to a great extent typical for patients with chronic arthritis and with nephropathy of the proteinuric type) developed in men with gout. Similar changes were provoked experimentally by purine-exchange violations, which is indicative of their secondary nature. An exogenous administration of androgens at experimental hyperuricemy led to the normalization of purine exchange and of blood hormones, it also ensured an alleviation of pronounced morphological signs of arthritis. The application of testosterone and its analogues for people exceeds, by their effectiveness, the known treatment methods.

RESUMO

A total of 107 patients with primary gout were examined. The pituitary-gonadal system is imbalanced in male patients with gout, which manifests by hyperproduction of progesterone and suppressed production of testosterone and estradiol. These changes are more pronounced in patients with chronic arthritis and proteinuric nephropathy. Similar dyshormonal changes are experimentally simulated in rats by induction of purine metabolism disorders. Exogenous injection of androgens in experimental hyperuricemia led to normalization of purine metabolism and hormonal homeostasis.

Patients with chronic glomerulonephritis (CG) develop disturbances of lipid blood spectrum leading to additional damage to renal structure. The existent methods of pathogenetic therapy have no effect on lipid imbalance. Recently, many autoimmune diseases have been treated with systemic enzyme therapy (SET). The authors studied SET effect in disturbed lipid metabolism in experimental glomerulonephritis. Experimental animals showed morphological and biochemical changes similar to those in CG of man. SET reduced renal tissue damage and symptoms of dyslipoproteinemia.

Renal fibronectin synthesis is impaired in patients with chronic glomerulonephritis. We measured urinary fibronectin for evaluating the efficiency of various methods of treatment. Traditional therapy of patients with the nephrotic syndrome at the stage of renal failure leads to decrease of fibronectinuria, which can be indicative of the progress of nephrosclerotic process in the renal parenchyma; monotherapy with wobenzyme during the azothemic stage of disease in patients with the urinary and nephrotic syndrome does not cause statistically significant changes in the level of urinary fibronectin, which can be regarded as inhibition of nephrosclerosis process. Hence, wobenzyme is the drug of choice, decreasing the velocity of nephrosclerotic processes, when pathogenetic therapy is largely limited or precluded. Combination of wobenzyme with pathogenetic drugs in patients with the nephrotic syndrome and intact renal function suppresses fibronectinuria due to mutual potentiation of the antiinflammatory effect. Decrease of fibronectin concentration in the urine after wobenzyme monotherapy in patients with the urinary syndrome without signs of chronic renal insufficiency confirms the antiinflammatory effect of the drug.

AIM: To try polyenzyme drug vobenzim in chronic glomerulonephritis (CGN). MATERIALS AND METHODS: 174 CGN patients were randomized into 2 groups. The study group of 39 patients received conventional pathogenetic treatment plus vobenzim (initial dose 18-24, maintenance 12-15 dragees a day). Control group of 135 patients were given only pathogenetic therapy. 67 patients appeared ineligible. RESULTS: 3-6 months after the treatment the response was seen in 61.5% patients of the study and 17.8% patients of the control group. Side effects of vobenzim in CGN treatment were absent. CONCLUSION: Enzyme therapy with vobenzim in CGN is safe and effective.

RESUMO

Gonadotropic function of hypophysis was studied in 109 patients with gout. Raised level of luteinizing hormone (LH) was found in men while women had decreased content of LH and follicle-stimulating hormone (FSH). Severe lesions of joins and kidneys were associated with suppressed FSH secretion. The authors consider correction of hormonal dysfunction in gout to be advisable. Certain correlative relations were established to exist between levels of gonadotropins, sex hormones and indices of purine metabolism.

RESUMO

The examination of 241 gout patients (207 males and 34 females) revealed a marked sexual dimorphism of the disease related to the articular and renal syndromes. Male gout is characterized by higher blood levels of uric acid and oxypurinole in reduced clearance of the latter, while in female gout there were hyperxanthinoxidasemia and hypo-cAMPemia. Unbalance of sex and gonadotropic hormones, which appeared multidirectional and dependent on the sex, was observed in males in the form of lower concentrations of blood testosterone and estradiol against elevated levels of progesterone and lutropin, in females manifested with hypoprogesteronemia and reduced gonadotropin.
