RESUMO

From a position of urodynamic evaluation, obstructive urination represents a violation of the phase of urinary bladder emptying. The study was aimed to evaluation of urodynamic parameters in women with obstructive urination on a background of conservative treatment. The study included 40 women with obstructive urination aged 26 to 72 years. All patients received alpha-adrenoblocker Omnic Okas (tamsulosin) at a dose 0.4 mg once a day in the morning for 3 months. Initially, a visual assessment of uroflougramm revealed curves tipical for the obstructive type of urination. Quantitative parameters showed a decrease in the maximum urine flow rate and lengthening the time of urination. Treatment with alpha-adrenoblocker contributed to shortening the time of urination, and increased maximum urine flow rate. Before treatment, ultrasound examination revealed the presence of residual urine volume of 60-210 mL (mean 129 ml) after urination in 32 (80%) patients. After treatment, presence of residual urine volume of 50-90 ml (mean 65 ml) was noted only in 9 (22.5%) patients. It is concluded that the selective alpha-adrenoblocker Omnic-Okas neutralizes the effect mediated by alpha-adrenoceptors in the area of the bladder neck and proximal urethra, and contribute to the normal detrusor contraction, and therefore, eliminates the functional obstruction, while significantly improving the quality of life of patients.

This investigation was aimed at the study of age changes of penis connective tissue structures. Tissue fragments of penis were obtained from 20 cadavers of men at the age of 20-38 years in group I, and from 20 cadavers of men at the age of 41-59 years in group II. The criteria for the exclusion of material from the research were arterial hypertension, diabetes mellitus, atherosclerosis of internal iliac arteries, Peyronie's disease, and anomalies of genital organ development. It was shown that in the cavernous body of penis, aging was associated with the increased amount and thickening of collagen and argyrophilic fibers, decreased content and thinning of elastic fibers, and the reduced amount of smooth muscle cells (SMC). The average area of fibroblast and SMC nucleolus was not different in both groups studied. The average area of endotheliocyte nucleolus was equal to 1.9+/-0.9 microm2 in group II, being lower than that one in group I, in which this index was equal to 2.1+/-0.9 microm2. No differences in the content of type III and IV collagen were found between the study groups. Age-associated decrease in the average area of endothelial cell nucleolus in the cavernous bodies may reflect the reduction of the activity of these cells and may indicate the development of endothelial dysfunction, which is one of the most important steps in the morphogenesis of age-related male erectile dysfunction.

The aim of this study was to examine the connective tissue of penis in normal individuals and in patients with erectile dysfunction (ED) and Peyronie's disease (PD) using computer methods of image analysis. Penis tissues were obtained from 20 males aged 20-40 years who died in accidents, penis biopsies were taken from 23 patients with ED and 9 patients with PD (average age: 51 +/- 11.5 years). In both groups of patients, the volumetric fraction of collagen fibers in the tunica albuginea and corpora cavernosa was increased, while that one of elastic fibers was decreased. At the same time, the changes of elastic fibers were noted: the fibers become thinner and formed "rods". The reduction of the amplitude and the wavelength in the collagen fibers of the tunica albuginea in patients with ED and the presence of fibrous plaques in corpora cavernosa in in patients with PD were registered. The methods of computer image analysis may improve the morphologic diagnosis of ED and PD.

AIM: To evaluate the effect of impasa on activity of N0-dependent processes in cardiological patients with erectile dysfunction (ED). MATERIAL AND METHODS: 58 patients with IHD and angina of effort and 48 patients with hypertension participated in the study of efficacy of conventional treatment combined with impasa on the course of the basic disease, peripheral blood count of desquamated endotheliocytes, microcirculation and erectile function. RESULTS: IHD patients given impasa showed increased exercise tolerance, less number of anginal attacks, improved coronary microcirculation, improved metabolism of vascular endothelium. Manifestations of erectile dysfunction regressed. Hypotensive therapy also reduced the above manifestations in hypertensive patients. CONCLUSION: The addition of impasa to conventional cardiotropic and hypotensive therapy promotes ED regression.

A total of 58 patients with ischemic heart disease and angina of effort (FC II) (mean age 55.6 years) participating in the study were diagnosed to have erective dysfunction (ED) of a mild (35%), moderate (57%) and severe degree (8%). All the patients were randomized into two groups. Group 1 (n=21, mean age 56.4 years) received standard cardiotropic therapy (nitrates, beta-blockers, ACE inhibitors on demand, diuretics, antioxidants) and placebo. Group 2 (n=37, mean age 54.3 years) received the same standard cardiotropic therapy plus impase (1 tablet each other day for 3 months). The results of the trial show that impase addition to cardiotropic therapy raised exercise tolerance, diminished the number of anginal attacks in mild and moderate exercise, enhanced coronary microcirculatory blood flow, increased reserve circulation index by 34%, improved metabolism of vascular endothelium in the whole body. Impase acts pathogenetically in endothelial insufficiency. Prevention of endothelial dysfunction by impase allows both to stop progression of cardiovascular disease and to prevent erectile dysfunction.

RESUMO

A use of computed express analysis of variations in RR-cardiointerval durations for patients in acute period of hemorrhagic stroke permitted to evaluate the condition severity and disease outcome prognosis before the objective changes of clinical symptomatology. A recovery was reliably observed with a positive and stable trend to U-potential and S-ULF, So, Ik indices normalization. A decrease of the indices towards a critical level was detected in cases with lethal outcome.

The authors analyse the results of preoperative preoperation of 143 patients with benign prostatic hyperplasia (BPH). Conventional preoperative preparation (antibiotics, uroantiseptics and phytotherapy) was used in combination with intravascular laser irradiation of blood (group 1, 30 patients), local laser therapy (transurethral and transrectal) (group 2, 27 patients), variation of laser methods (group 3, 28 patients) and alone (61 patients, group 4). Such preoperative preparation was aimed at prophylaxis of thrombohemorrhagic complications after adenomectomy. The efficiency of the treatment was assessed by hemostasis parameters. It was found that conventional antiinflammatory therapy had a weak effect on hemostasis, the greatest positive effects being achieved with combined laser preoperative preparation. Hyperfibrinogenemia, thrombinemia and activation of XII + a-dependent finrinolysis were stopped in patients of group 3. Because of less number of postoperative hemorrhages, hemotransfusions and transfusions of blood preparations were used less frequently, red cell parameters improved, exacerbations of chronic pyelonephritis occurred less often.

RESUMO

The use of computed express analysis of RR-cardiointerval duration variability in patients with acute stage of ischemic stroke allowed to obtain the data on state dynamics and predict the disease outcome before the objective changes in clinical symptomatology. The recovery is observed in normalization of sigma, beta 1, beta 2, U, S-ULF, S-LF and S-HF indices. An opposite trend is observed in cases of lethal outcome.

Low-intensity laser therapy administered in the form of intravenous blood irradiation, transrectal and transurethral prostatic irradiation and their combination as preoperative preparation and correction of immunity disturbances in patients with benign prostatic hyperplasia (BPH) were studied. The response to the treatment was evaluated by positive changes in the immune status and bacterial contamination of the urine and prostatic tissue. Conventional preoperative preparation (uroantiseptics, antibiotics and phytotherapy) fails to correct signs of T-cell immunodeficiency, depression of phagocytic activity of neutrophils, significantly reduce bacteriurea. Laser therapy as intravenous laser blood radiation acts immunomodulatorily on cellular immunity and normalized the proportion of T-helpers of the first and second order (T-suppressors) and neutrophil phagocytosis. The antibacterial effect of this technique on urinary microflora and prostatic tissue is not very high. Local laser therapy is a potent immunostimulator of T- and B-lymphocytes, increased the index of immunoregulatory cells' proportion, activated phagocytosis of neutrophils. It has pronounced antibacterial effect against gram-negative urinary microflora and tissue of the prostate. Combined laser therapy produced the highest immunomodulating action on T-lymphocytes and immunostimulating one on B-lymphocytes, potentiated phagocytic ability of neutrophils, elevated index of the immunoregulatory cells, but was unable to correct their imbalance completely. Antibacterial effects of combined laser therapy were the highest, including the bacterial group Proteus-Providencia. Preoperative low-intensity laser therapy of BPH reduced the number of postoperative pyoinflammatory complications, hospital stay, severity of postoperative period.

RESUMO

A new method for analyzing the chaotic component of EEG is proposed. The method is based on estimating the fractal dimension of fluctuations of alpha-rhythm power (the square of amplitude). It is shown that the dimensions of the background EEG fragments for epilepsy patients is significantly higher than that in norm, indicating a disbalance of cerebral mechanisms that control the alpha-activity in this disease. A tendency toward the disturbance of the normal fractal structure of EEG in a group of patients with initial signs of epilepsy was revealed. This suggests that the method is of considerable promise for setting the individual long-term prognosis of the development of the epileptic syndrome.

RESUMO

Antiblastomic effect of low strength low frequency fluctuating magnetic field with 1/f power spectrum (1/f MF) analogous to spectra of normal biological fluctuations is studied experimentally. For the first time it is demonstrated that stabilization of cell-cell surface interactions as well as suppression of mitotic activity of malignant cells exposed to 1/f MF may result in considerable inhibition of cancer growth in animals. These effects were shown to be absent in weak low frequency magnetic fields with spectra other than 1/f.

The existence of universal non-specific mechanism of action of external factors upon the living tissue based on the DEL-reaction is suggested.
