Objective: To explore the feasibility of a randomised controlled trial of a multiple sclerosis online course (MSOC) via qualitative analysis of participant semi-structured interviews. Methods: The MSOC was developed in two arms: intervention arm which contained evidence-based lifestyle modification recommendations, and standard-care arm which delivered information from MS websites reflecting standard advice. Participants were recruited via online advertisements, completed a baseline questionnaire, and randomised. Seven modules were delivered over six weeks. Course completers were invited to participate in semi-structured interviews. Within a qualitative paradigm, interviews were analysed using reflexive thematic analysis. Results: Fourteen of the 17 course completers were interviewed: 86% (12/14) female; mean age 50 years; residing in Australia, New Zealand, and the USA, predominantly had relapsing-remitting MS, mean time from diagnosis 9.5 years, and patient-determined disease steps disability assessment distributed evenly across all three categories. Four themes were identified: 1) "Hearing our stories" (hope for the future, feeling represented, exploring the journey of others with MS, learning from diversity, and wanting to connect with others); 2) "Taking the plunge" (not wanting to be first, feelings of nervousness or reluctance and feeling confronted); 3) "The accessibility and safety of being online" (ease of access, going at your own pace, making it work and not letting others down); and 4) "unpacking the course" (ease of navigation, visuals, understandability, and length and timing). Conclusion: Participants felt represented, found the course accessible and content was easy to understand. These experiences provide important insights and considerations for this digital health intervention.

RESUMO

The management of multiple sclerosis (MS) has progressed significantly with the emergence of mHealth technologies. Uptake of mHealth apps amongst people with MS, and clinical and demographic characteristics of mHealth adopters is unknown outside North America. Participants of the HOLISM study were queried about their mHealth apps use. We summarize mHealth app usage, and clinical and demographic characteristics of mHealth app adopters using descriptive statistics. Overall, 3.1% of respondents reported using an mHealth app, most of whom were located in Australasia and North America. This study provides insight regarding mHealth app usage within a large international cohort of people with MS.

RESUMO

Human immunodeficiency virus (HIV) infection in a patient with end-stage renal disease was considered a contraindication for renal transplantation till now despite the advent of highly active antiretroviral therapy with the apprehension that immunosuppression would further jeopardize the already compromised immune status of the patients. Renal transplantation in HIV patients is rare in developing countries including ours. Here we report a series of four cases of renal transplantation in HIV patients.

RESUMO

This report describes a simple chemical free method that was successfully used by a team of European and Indian scientists (www.qub.ac.uk/tipot) to remove arsenic (As) from groundwater in a village in West Bengal, India. Six such plants are now in operation and are being used to supply water to the local population (www.insituarsenic.org). The study was conducted in Kasimpore, a village in North 24 Parganas District, approximately 25 km from Kolkata. In all cases, total As in treated water was less than the WHO guideline value of 10 microg L(-1). The plant produces no sludge and the operation cost is 1.0 US$ per day for producing 2000 L of potable water.

Bilirubin above a threshold level is toxic to human system and is excreted in urinary and through gastrointestinal tract. The role of bilirubin as antioxidant is debatable. This paper aims at elucidating the role of bilirubin as an antioxidant in neonatal jaundice patients. It is observed that bilirubin up to 6 mg/dl in blood acts as an antioxidant and above 12.5 mg/dl is strongly prooxidant. Phototherapy is the accepted therapeutic management of neonatal jaundice and has been shown to enhance the oxidative stress. Approaches have been taken to formulate a herbal medication which will reduce bilirubin level in the neonates without inducing additional damages. The ethanolic extract of sweet lime peel, administered orally at a dose of 72 microg is found to reduce the oxidative stress in erythrocytes of phenylhydrazine-induced jaundiced rats treated with phototherapy.

Over 90% of Rett syndrome (RTT) cases have a mutation in the X-linked gene encoding methyl CpG binding-protein 2 (MeCP2). A mouse model that reprises clinical manifestations of the disease would be valuable for examining disease mechanisms. Here, we characterize physical and behavioral measures, as well as brain region volumes in young adult mice that have mutations in mouse methyl CpG binding-protein 2 gene (Mecp2) to serve as a baseline for other studies. Hemizygous males, which produce no functional protein, exhibit hypoactivity and abnormalities in locomotion, stereotypies, and anxiety reminiscent of the clinical condition. The mutant males also exhibit cognitive deficits in fear conditioning and object recognition relative to wildtypes. Volumetric analyses of male brains revealed a 25% reduction in whole brain volume in mutants relative to wildtypes; regional differences were also apparent. Mutants had decreased volumes in three specific brain regions: the amygdala (39%), hippocampus (21%), and striatum (29%). Heterozygous females, which produce varying amounts of functional protein, displayed a less severe behavioral phenotype. The mutant females exhibit abnormalities in locomotion, anxiety measures, and cognitive deficits in object recognition in an open field. This study provides the first evidence that the abnormal motor and cognitive behavioral phenotype in Mecp2 mice is consistent with specific volume reductions in brain regions associated with these behaviors, and shows how these data parallel the human condition. The Mecp2 mutant mice provide a very good model in which to examine molecular and behavioral mechanisms, as well as potential therapeutic interventions in RTT.

Sixty strains of Escherichia coli, were isolated and identified from 94 samples of infantile diarrhoea and traveller's diarrhoea. Eleven strains were found to be colicinogenic and colicin 'H' was found to be predominant in conjunction with other colicins. Five strains were enteropathogenic of which 055 and one untypable strain were highly enteropathogenic. A positive correlation could be established between haemolytic character, dulcitol fermentation and the virulence of the strain involved.

Existing works on pellet dispersion ballistics are confined to some data-based models derived from statistical analysis of observed patterns on targets but the underlying process causing the dispersion lacks due attention. The present article delves into the relatively unexplored areas of dispersion phenomena, and attempts to develop a theoretical model for general application. The radial velocity distribution of pellets has been worked out by probing into the physical process of dispersion based on transfer of momentum from undispersed shot mass to dispersed pellets. The ratio 2u/v0 (u = root mean square (r.m.s.) radial velocity and v0 = muzzle velocity of the pellets) is found to be fairly constant for a fixed gun-ammunition combination and has been suitably designated as 'Dispersion Index' (DI) characterising its dispersion capability. The present model adequately accounts for pellet distribution on targets and it appears that 'Effective Shot Dispersion' (ESD) as introduced by Mattoo and Nabar [ESD = [(4/N0)sigma Ri2]1/2, where N(0) is the total number of pellets and Ri is the radial distance of the i-th pellet from centre of pattern], gives a faithful numerical measure of overall dispersion at a given distance. A relationship between ESD and firing distance, incorporating the effects of air resistance and gravity has been worked out, which reveals that DI controls the dispersion at a given distance. For small distances (less than 20 m) the relation reduces to a linear one, as already observed empirically and looks like ESD = E0+DI x firing distance, E0 being a parameter dependent on gun and ammunition. The present model, unlike earlier ones, is versatile enough to explain the natures of the dependence of dispersion on firing distance as well as on gun-ammunition parameters, which are essential for a faithful reconstruction of a crime scene. The model has been tested with such experimental data as are available and reasonable agreement is observed.

Existing methods seem inadequate to identify propellants of small arms ammunition. The present article describes that viscosity may be effectively used to differentiate and identify those. The temperature dependence, intra- and inter-batch variation and storage effect of propellants were studied.

A quick and sensitive method has been developed for the chemical detection of nitrous derivatives discharged from a firearm. Detection by fluorescence is especially suitable for blood-stained articles such as clothing, which often give confusing results when the conventional methods are used.
