PURPOSE: To evaluate the effectiveness of the texture analysis of axillary high-resolution 3D T2-weighted imaging (T2WI) in distinguishing positive and negative lymph node (LN) metastasis in patients with clinically node-negative breast cancer. METHODS: Between December 2017 and May 2021, 242 consecutive patients underwent high-resolution 3D T2WI and were classified into the training (n = 160) and validation cohorts (n = 82). We performed manual 3D segmentation of all visible LNs in axillary level I to extract the texture features. As the additional parameters, the number of the LNs and the total volume of all LNs for each case were calculated. The least absolute shrinkage and selection operator algorithm and Random Forest were used to construct the models. We constructed the texture model using the features from the LN with the largest least axis length in the training cohort. Furthermore, we constructed the 3 models combining the selected texture features of the LN with the largest least axis length, the number of LNs, and the total volume of all LNs: texture-number model, texture-volume model, and texture-number-volume model. As a conventional method, we manually measured the largest cortical diameter. Moreover, we performed the receiver operating curve analysis in the validation cohort and compared area under the curves (AUCs) of the models. RESULTS: The AUCs of the texture model, texture-number model, texture-volume model, texture-number-volume model, and conventional method in the validation cohort were 0.7677, 0.7403, 0.8129, 0.7448, and 0.6851, respectively. The AUC of the texture-volume model was higher than those of other models and conventional method. The sensitivity, specificity, positive predictive value, and negative predictive value of the texture-volume model were 90%, 69%, 49%, and 96%, respectively. CONCLUSION: The texture-volume model of high-resolution 3D T2WI effectively distinguished positive and negative LN metastasis for patients with clinically node-negative breast cancer.

Introduction: Present study was to investigate hs-CRP concentration, brain structural alterations, and cognitive function in the context of AD [Subjective cognitive decline (SCD), mild cognitive impairment (MCI), and AD]. Methods: We retrospectively included 313 patients (Mean age = 76.40 years, 59 SCD, 101 MCI, 153 AD) in a cross-sectional analysis and 91 patients (Mean age = 75.83 years, 12 SCD, 43 MCI, 36 AD) in a longitudinal analysis. Multivariable linear regression was conducted to investigate the relationship between hs-CRP concentration and brain structural alterations, and cognitive function, respectively. Results: Hs-CRP was positively associated with gray matter volume in the left fusiform (ß = 0.16, pFDR = 0.023) and the left parahippocampal gyrus (ß = 0.16, pFDR = 0.029). Post hoc analysis revealed that these associations were mainly driven by patients with MCI and AD. The interaction of diagnosis and CRP was significantly associated with annual cognitive changes (ß = 0.43, p = 0.008). Among these patients with AD, lower baseline CRP was correlated with greater future cognitive decline (r = -0.41, p = 0.013). Conclusion: Our study suggests that increased hs-CRP level may exert protective effect on brain structure alterations and future cognitive changes among patients already with cognitive impairment.

Both objective and perceived social isolations were associated with future cognitive decline and increase risk of Alzheimer's disease (AD). However, the impacts of perceived social isolation depending on different clinical stages of AD have not been elucidated. The aim of this study was to investigate the influence of perceived social isolation or loneliness on brain structure and future cognitive trajectories in patients who are living with or are at risk for AD. A total of 176 elderly patients (mean age of 78 years) who had complaint of memory problems (39 subjective cognitive decline [SCD], 53 mild cognitive impairment [MCI], 84 AD) underwent structural MRI and neuropsychological testing. Loneliness was measured by one binary item question "Do you often feel lonely?." Voxel-based morphometry was conducted to evaluate regional gray matter volume (rGMV) difference associated with loneliness in each group. To evaluate individual differences in cognitive trajectories based on loneliness, subgroup analysis was performed in 51 patients with AD (n = 23) and pre-dementia status (SCD-MCI, n = 28) using the longitudinal scores of Alzheimer's Disease Assessment Scale-cognitive component-Japanese version (ADAS-Jcog). Whole brain VBM analysis comparing lonely to non-lonely patients revealed loneliness was associated with decreased rGMV in bilateral thalamus in SCD patients and in the left middle occipital gyrus and the cerebellar vermal lobules I - V in MCI patients. Annual change of ADAS-Jcog in patients who reported loneliness was significantly greater comparing to these non-lonely in SCD-MCI group, but not in AD group. Our results indicate that perceived social isolation, or loneliness, might be a comorbid symptom of patients with SCD or MCI, which makes them more vulnerable to the neuropathology of future AD progression.

OBJECTIVES: To evaluate the correlation of ADC and IVIM-diffusion kurtosis (DK) model parameters with quantitative histological parameters of whole-slide imaging (WSI). METHODS: This retrospective study (September 2015-July 2016) included 37 consecutive patients (all females; median age: 64 years, range, 41-82 years), each with a single invasive breast ductal carcinoma lesion with a mass appearance on preoperative MRI. DWI with b-values of 0, 50, 100, 300, 550, 850, and 1000 s/mm2 was performed. ADC maps were generated with b-values of 50 and 850 s/mm2. The IVIM-DK model was analysed using the following formula: [Formula: see text] where S is the signal intensity, b is the b-value, f is the perfusion fraction, D* is the pseudo-diffusion coefficient of the vascular component, D is the diffusion coefficient of the non-vascular component, and k is the diffusion kurtosis. Whole tumour segmentation was performed to obtain the mean ADC, f, D*, D, and k. Quantitative histological parameters were obtained using cytokeratin immunostaining and WSI. The correlation of ADC and IVIM-DK model parameters with quantitative histological parameters was examined by Pearson's correlation. RESULTS: The ADC was significantly correlated with the area ratio of interstitium (r = 0.53, p = 0.00082) and entropy (r = -0.58, p = 0.00019). k was significantly correlated with the area ratio of cancer cell nuclei (r = 0.53, p = 0.00079). CONCLUSIONS: Since the ADC reflected the area ratio of interstitium and entropy, and diffusion kurtosis reflected the area ratio of cancer cell nuclei, these parameters may be effective in distinguishing between benign and malignant breast tumours and in grading breast cancer.

PURPOSE: To compare the image quality between turbo spin-echo (TSE)-diffusion weighted imaging (DWI) and single-shot echo-planar imaging (EPI)-DWI, and to verify the diagnostic performance of the apparent diffusion coefficient (ADC) parameters of the two techniques by using histogram analysis in terms of differentiation between ductal carcinoma in situ (DCIS) and invasive ductal carcinoma (IDC) lesions. METHODS: Ninety-four women with 94 lesions diagnosed as breast cancer by surgery underwent IRB-approved preoperative magnetic resonance imaging, including TSE and EPI-DWI with b-values of 50 and 850 s/mm2. Twenty lesions were identified as DCIS and 74 as IDC. Image quality [signal-to-noise ratio (SNR), contrast-to-noise ratio (CNR), and geometric distortion] was evaluated quantitatively and compared between the TSE and EPI-DWI. A histogram analysis of the entire tumor voxel-based ADC data was performed, and the 10th, 25th, 50th, 75th, and 90th percentile values of each technique were compared between DCIS and IDC lesions. RESULTS: The SNR and CNR of TSE-DWI were significantly higher than those of EPI-DWI (P < 0.0001 and < 0.0001). The geometric distortion of TSE-DWI was significantly lower than that of EPI-DWI (P < 0.0001). In TSE-DWI, the 10th, 25th, 50th, and 75th percentile values were significantly different between the DCIS and IDC lesions (P = 0.0010, 0.0004, 0.0008, and 0.0044, respectively). In EPI-DWI, the 50th and 75th percentile values were significantly different between the two groups (P = 0.0009 and 0.0093). There was no significant difference in the area under the curve of the receiver operating characteristic analysis of the 10th, 25th, 50th, and 75th percentile values of TSE-DWI, and the 50th and 75th percentile values of EPI-DWI (P = 0.29). CONCLUSION: The image quality of TSE-DWI was better than that of EPI-DWI. DCIS lesions were distinguished from IDC lesions with a wider range of percentile values in TSE-DWI than in EPI-DWI, although diagnostic performance was not significantly different between the techniques.

Current contrast-enhanced magnetic resonance angiography (MRA) and non-contrast-enhanced balanced steady-state free precession (bSSFP) MRA cause susceptibility artifacts from metallic devices in assessing endovascular visceral-artery interventions. The aims of this study are to investigate and compare image quality (IQ) and susceptibility artifacts of three-dimensional (3D) ultrashort echo time (UTE) time-spatial labeling inversion pulse (Time-SLIP) with those of 3D bSSFP Time-SLIP and to assess denoising deep learning reconstruction (dDLR) for the improvement of the signal-to-noise ratio (SNR) in 3D UTE with sparse sampling in phantoms and human subjects. This is a prospective type of study. Pulsatile glycerin-water flow phantom with platinum-tungsten-alloy coil, stainless-steel, nitinol, and cobalt-alloy stents were used. Ten healthy volunteers (seven males) and three patients (two males) were included in this study. 3D UTE Time-SLIP and 3D bSSFP Time-SLIP at 3T were used. The phantom-based study compared the signal-intensity ratio of the device levels (SRdevice ) and distal segments (SRdistal ) to the proximal segments. The volunteer-based study measured SNR, contrast ratio (CR), and IQ. The patient study evaluated local artifacts from metallic devices. Statistical tests included paired t-tests, Wilcoxon-signed rank tests, and Kruskal-Wallis tests. In the phantom-based study, SRdevice was small with UTE Time-SLIP, except the stainless-steel stent. SRdistal was greater (49.1%-90.4%) on bSSFP images than UTE images (-11.1% to 9.6%). Among volunteers, dDLR in UTE images improved SNR (p < 0.05) and IQ (p < 0.05), but CR was unaffected. UTE Time-SLIP showed inferior SNR and IQ than bSSFP Time-SLIP in images with and without dDLR (p < 0.05 for each). However, among patients, UTE Time-SLIP showed reduced metal artifacts compared to bSSFP Time-SLIP. Irrespective of the lower SNR and IQ of 3D UTE Time-SLIP than those of 3D bSSFP Time-SLIP, the former appeared to better depict flow after stenting or coiling. This indicates the potential of 3D UTE Time-SLIP to provide suitable diagnostic images of target vessels. dDLR improved SNR with reducing artifacts related to radial sampling, while maintaining the contrast. Background: Decreased bone mineral density (BMD) was associated with poorer cognitive function and increased risk of Alzheimer's disease (AD). However, objective evidence for the relationship between osteoporosis and AD in humans has not been extensively described. Objectives: We aimed to evaluate the relationships between BMD and the cortical volumes of brain regions vulnerable to AD; hippocampus, parahippocampal gyrus, precuneus, posterior cingulate, and angular gyrus, using voxel-based morphometry (VBM), to investigate the association between bone loss and AD. Methods: A cohort of 149 consecutive elderly participants who complained of memory disturbance underwent high-resolution structural brain magnetic resonance imaging (MRI) and dual-energy X-ray absorptiometry (DXA). We used SPM12 software to conduct a voxel-based multiple regression analysis to examine the association between femoral neck BMD values and regional gray matter volume (rGMV) on structural T1-weighted MRI. Results: After adjusting for subject age, gender, total brain volume (TBV), and mini-mental state examination (MMSE) scores, the multiple regression analysis showed significant correlations between BMD loss and rGMV decline in the left precuneus, which is an important neural network hub vulnerable to AD. Conclusion: These data suggest that the bone and brain communicate with each other, as in "bone-brain crosstalk," and that control of BMD factors could contribute to cognitive function and help prevent AD.

BACKGROUND: There is limited evidence of parametric MR mapping to characterize carotid plaques associated with cerebral ischemic events. PURPOSE: To explore the apparent diffusion coefficients (ADCs) and longitudinal relaxation rates (R1 ) of carotid plaques, including areas of hemorrhage, lipid-rich/necrotic core (LR/NC) without hemorrhage, and fibrous tissue (Fbr) STUDY TYPE: Prospective. SUBJECTS: Twelve patients who underwent carotid endarterectomy. FIELD STRENGTH/SEQUENCE: R1 was measured using double angle Look-Locker acquisition on 3T systems. Single-shot spin-echo echo-planar imaging with fat suppression and outer-volume suppression (OVS-DWEPI) with b values of 10 and 500 s/mm2 was used for diffusion-weighted imaging. ASSESSMENT: A phantom study using diluted gadolinium solutions and polyvinyl alcohol solutions was used to validate the two protocols. Regions of interest (ROIs) were manually outlined on MR images for areas of LR/NC, hemorrhage, and Fbr based on histological cross-sections. Pixel-based R1 and ADC values in the ROIs were plotted for each component. The probability density function of the plots determined the optimum contours to separate the three components in the ADC-R1 plane. The LR/NC, hemorrhage, and Fbr regions were mapped on MR images based on the above results and compared to histological results. STATISTICAL TESTS: The R1 values of the phantom measurements were tested using Bland-Altman analysis. The accuracies of the MRI classification were calculated. RESULTS: R1 values <8 s-1 calculated using our method agreed with those calculated using an inversion-recovery fast-spin-echo sequence (error, ≤0.1 s-1 ). ADC values obtained using OVS-DWEPI were 4.1% higher than those obtained using standard echo-planar imaging. LR/NC (R1 , 0.4-1.2 s-1 ; ADC, 0-1.5 µm2 /ms), hemorrhage (R1 ≥ 1.5 s-1 ; ADC, 0.5-1.5 µm2 /ms), and Fbr (R1 , 0.2-0.8 s-1 ; ADC, 1.5-2.9 µm2 /ms) were separated on the plots. The accuracies of MRI classification were LR/NC, 0.86; hemorrhage, 0.79; and Fbr, 0.77. CONCLUSION: The combination of ADC and R1 values measured using our method enabled differentiation among LR/NC, hemorrhage, and Fbr. LEVEL OF EVIDENCE: 1 Technical Efficacy: Stage 2 J. Magn. Reson. With shortening of the gantry of magnetic resonance imaging (MRI) systems, large field-of-view (FOV) imaging has become difficult because static magnetic field nonuniformity and gradient magnetic field nonlinearity exacerbate geometric distortion of MR images. However, results of earlier studies have demonstrated that view angle tilting (VAT) can reduce severe image distortion attributable to local susceptibility effects of metals. Although VAT is usually applied to local magnetic field nonuniformity, in principle VAT is expected to correct distortion also for peripheral images in large-FOV MRI. Results from this phantom experiment using VAT with large-FOV verified the effectiveness of distortion correction. The experiment using VAT showed reduction of maximum distortion from 23.6 to -1.9 mm. Furthermore, results of a volunteer study confirmed the distortion correction capability of VAT: it reduced distortion and improved visibility of the anatomical structure. In conclusion, experimentally obtained results underscore VAT effectiveness for improving distortion in large-FOV MRI.

RESUMO

The noise power spectrum (NPS), an index for noise evaluation, represents the frequency characteristics of image noise. We measured the NPS in PROPELLER (Periodically Rotated Overlapping ParallEL Lines with Enhanced Reconstruction) magnetic resonance (MR) imaging, a nonuniform data sampling technique, as an initial study for practical MR image evaluation using the NPS. The 2-dimensional (2D) NPS reflected the k-space sampling density and showed agreement with the shape of the k-space trajectory as expected theoretically. Additionally, the 2D NPS allowed visualization of a part of the image reconstruction process, such as filtering and motion correction.

BACKGROUND: To determine the viability of using magnetic resonance imaging measurement of optic nerve morphology as an objective analysis of glaucomatous damage. DESIGN: Retrospective study conducted at Tohoku University Hospital. PARTICIPANTS: Thirty-eight eyes of 19 patients with open-angle glaucoma. METHODS: Patients were scanned with T2-weighted and 3-T diffusion tensor magnetic resonance imaging, and parameters of the optic nerve, including fractional anisotropy, apparent diffusion coefficient and cross-sectional area, were determined. Conventional parameters of glaucomatous damage, including circumpapillary and macular retinal nerve fibre layer thickness, and mean deviation and average total deviation of the central 16 test points from the Humphrey Field Analyzer, were then compared with the magnetic resonance imaging-derived parameters. Spearman's coefficient of correlation was calculated to determine the significance of the correlation. MAIN OUTCOME MEASURE: Correlation coefficient between the magnetic resonance imaging parameters and the parameters of glaucomatous damage. RESULTS: Mean deviation was significantly correlated with all magnetic resonance imaging parameters (fractional anisotropy: r = 0.53, apparent diffusion coefficient: r = -0.44, cross-sectional area: r = 0.70). Circumpapillary retinal nerve fibre layer thickness was significantly correlated with fractional anisotropy (r = 0.60) and cross-sectional area (r = 0.47), but not apparent diffusion coefficient (r = -0.29). Central macular function and macular retinal nerve fibre layer thickness were also significantly correlated with magnetic resonance imaging parameters. CONCLUSIONS: Optic nerve magnetic resonance imaging parameters were significantly correlated to glaucomatous damage. Magnetic resonance imaging analysis of the optic nerve may, thus, have value as an objective instrument to assess glaucomatous degeneration, including the function of the macula.

PURPOSE: Accurate longitudinal relaxation time (T1) of arterial blood is important in evaluating blood flow in tissue by arterial spin labeling magnetic resonance (MR) imaging. Few studies have reported the T1 of human arterial blood in vivo, especially using 3-tesla MR imaging. T1 values of human venous blood in vivo have been reported, but they differ from those measured in vitro. We aimed to evaluate the accurate T1 of human arterial blood in vivo. METHODS: We measured T1 values of blood in 10 healthy volunteers in vivo using an inversion-recovery fast gradient-echo sequence and 3-tesla MR imaging unit. We also measured hematocrit (Hct) values of venous blood samples. After nonselective application of the inversion pulse using a body coil, we obtained MR imaging signals of arterial blood in the abdominal aorta. Similarly, we measured the signals of venous blood in the internal jugular vein. Inversion times varied between 200 and 5000 ms for imaging of the abdominal aorta and 200 and 2500 ms for imaging of the jugular vein. We also acquired signals without the inversion pulse. We estimated T1 values from the data by nonlinear least squares fitting of a 3-parameter model. RESULTS: The T value (mean±standard deviation) of arterial blood was 1779±80 ms and of venous blood, 1694±77 ms. The average Hct value was 0.47. The R1 (=1/T1) of arterial blood was related to the Hct value as: R1=(0.59±0.16)Hct+(0.29±0.07) (mean±standard error) s⁻¹. For the venous blood, R1=(0.70±0.11)Hct+(0.27±0.05) s⁻¹. CONCLUSION: We observed a T1 of human arterial blood in vivo of 1779±80 ms at a mean hematocrit value of 0.47 as determined by 3T MR imaging; an even longer T1 value is expected with a hematocrit value less than 0.47.

RESUMO

We studied the feasibility of high spatial and temporally resolved imaging by a combination of keyhole imaging, which is a fast imaging method, and a 3.0T MRI system which inherently has a high signal-to-noise ratio (SNR). In keyhole imaging, full k-space reference data are collected, followed by repeated dynamic acquisition of the central low-frequency data alone, and this makes dynamic acquisition faster. The purpose of our article is to describe the effects of the reference data on the reconstructed dynamic images with the assumption of vessel visualization. For efficient use of keyhole imaging, it is important to optimize the keyhole percentage and timing of reference data acquisition for the object of interest.

We used functional magnetic resonance imaging (fMRI) to explore the change in brain activation associated with the learning of Korean words written in Han-gul characters (K-words) by young Japanese at two stages. Subjects were 12 right-handed native Japanese without previous knowledge of Korean words and characters. On the first day they were taught the pronunciation and meaning of 20 K-words. Then, after the first fMRI session (on day 2), they were given a set of 20 cards with the words and corresponding photographs. They also received a tape and were instructed to memorize the 20 K-words by studying them every day until the day of the second fMRI session (day 16). During the fMRI sessions, 20 Japanese words written in kana syllabograms (J-words) and the 20 previously presented K-words, as well as 20 new K-words (Kn-words) were presented visually for silent reading. The first J-word reading, relative to the first K-word reading, showed activation in the left angular gyrus. K-word reading relative to J-word reading during both sessions showed activation in occipital regions. Within these activated areas, session by condition interaction was found only in the left angular gyrus. The interaction between session and condition resulted from the fact that the differences in blood oxygenation-level-dependent signals between K-words and J-words and between Kn-words and J-words were significantly greater in the first session than in the second session. From the results, we concluded that patterns of brain activation changed as the memory of the 20 K-words became fixed through daily practice and that reading of both Korean words and Japanese syllabograms engaged the left angular gyrus.

PURPOSE: To assess the biochemical properties of band heterotopia in comparison with other cortical developmental malformations (CDMs) by using proton magnetic resonance spectroscopy (1H-MRS). METHODS: We performed localized single-voxel 1H-MRS studies on 13 patients [five band heterotopia (BH), two focal cortical dysplasia (CD), two unilateral CD, one bilateral perisylvian dysplasia, three hemimegalencephaly]. CDMs other than BH were categorized as CD. Spectra were acquired from volumes of interest (VOIs) localized in the CD and in normal-appearing cortex on the contralateral side. In BH patients, the VOIs were the external cortex and the laminar heterotopia. For the BH study, spectra also were obtained from the cortex of age-matched normal volunteers. RESULTS: The spectra of CD lesions were characterized by significantly lower ratios of N-acetyl aspartate to creatine (NAA/Cr) and by higher choline to Cr (Cho/Cr) ratios than in the contralateral remote cortex (p = 0.01 and 0.01, respectively). The NAA/Cr and Cho/Cr ratios of the external cortex of BH were not significantly different from those of normal volunteers. The NAA/Cr ratio of the laminar heterotopia was not significantly different from that of the external cortex (p = 0.12) or normal volunteers (p = 0.60), whereas Cho/Cr was significantly higher in laminar heterotopias than in the external cortex (p = 0.04) or controls (p = 0.03). CONCLUSIONS: 1H-MRS can distinguish between the metabolic properties of BH and CD.

Temporal context information is crucial to understanding human episodic memory. Human lesion and neuroimaging data indicate that prefrontal regions are important for retrieving temporal context memory, although the exact nature of their involvement is still unclear. We employed functional magnetic resonance imaging (fMRI) to elucidate the neural basis of two kinds of temporal context memory: the temporal order of items between lists and within a list. On the day of the fMRI experiment, subjects memorized a list of 30 pictures in the morning and another list of 30 pictures in the afternoon. During the scanning session, the subjects performed three tasks. In a between-lists task, they were asked to judge the temporal order between two items that had been presented in different lists. In a within-list task, they were asked to judge the temporal order between two items that had been presented in a single list. We found bilateral prefrontal activities during these two temporal context memory tasks compared with a simple item-recognition task. Furthermore, in direct comparison between these two tasks, we found differential prefrontal activities. Thus, right prefrontal activity was associated with temporal order judgment of items between lists, whereas left prefrontal activity was related to temporal order judgment of items within a list. These results indicate that retrieval processes of two kinds of temporal context memory are supported by different, but overlapping, sets of cerebral regions. We speculate that this reflects different cognitive processes for retrieving temporal context memory between separate episodes and within a single episode.

The aim of this study was to identify the neuroanatomical basis of the retrieval of people's names. Lesion data showed that patients with language-dominant temporal lobectomy had impairments in their ability to retrieve familiar and newly learned people's names, whereas patients with language-nondominant temporal lobectomy had difficulty retrieving newly learned people's names. Functional magnetic resonance imaging experiments revealed activations in the left temporal polar region during the retrieval of familiar and newly learned people's names, and in the right superior temporal and bilateral prefrontal cortices during the retrieval of newly learned information from face cues. These data provide new evidence that the left anterior temporal region is crucial for the retrieval of people's names irrespective of their familiarity and that the right superior temporal and bilateral prefrontal areas are crucial for the process of associating newly learned people's faces and names.

[reaction: see text] Novel synthesis of cephalotaxine 1 based on tertiary N-acyliminium ion chemistry starting from alkynylamide 2 was achieved. The key steps include the preparation of pyrroloisoquinoline 4 from alkynylamide 2, the ring expansion of pyrroloisoquinoline 4 to pyrrolobenzazepine 12, and the construction of cyclopentapyrrolobenzazepine ring system 6, all of which are derived from N-acyliminium ion intermediates.

Tilivalline 1, a metabolite from Klebsiella pneumoniae var. ocytoca, was easily synthesized in five steps from (S)-proline and 3-(benzyloxy)isatoic anhydride 4g. This synthesis is based on modified Curtius reactions of 3-substituted phthalic anhydrides 11 to 3-substituted isatoic anhydrides 4, conversion of lactams 6 to the acyliminium precursors 7 and stereoselective introduction of indole from the less hindered side of 7.
