RESUMO

Giant cell arteritis (GCA) is an inflammatory cranial and/or extracranial vasculitis. Although cranial GCA is widely recognized, extracranial GCA is underdiagnosed because of its nonspecific and atypical presentations. We report a case of asymptomatic extracranial GCA with ascending thoracic aortopathy discovered incidentally during surgical mitral valve repair. (Level of Difficulty: Intermediate.).

OBJECTIVES: To evaluate and compare perceived pain and jaw function impairment during the first 4 weeks with slow maxillary expansion (SME) using quadhelix and rapid maxillary expansion (RME) using conventional banded hyrax. MATERIALS AND METHODS: Sixty patients aged 10.2 to 15 years were enrolled and consecutively recruited to either the quadhelix group (QG) or hyrax group (HG). A questionnaire was used to evaluate pain, jaw function impairment, and analgesic consumption in the first 7 days, at 2 weeks, and at 4 weeks. RESULTS: Fifty-five patients (43 girls and 12 boys) completed the questionnaire at all time points (27 in the QG and 28 in the HG). Except at 4 hours, there were no significant differences between the groups regarding pain from teeth, tongue, and palate. Patients started to adapt after day 3. Patients in the HG group reported significantly higher scores for difficulty in swallowing (moderate to severe) during the first 6 days. In both groups, minimal effects were found on speech and the majority of patients did not experience difficulty in yawning or laughing. There was no significant difference in analgesic consumption between the groups. No correlations were found between age, gender, or malocclusion type and any of the investigated outcomes. CONCLUSIONS: Quadhelix for SME and conventional banded hyrax for RME were well tolerated by patients after 1 week. The decision to use either appliance could be based on factors not related to patient experiences.

Global concerns have been observed due to the outbreak and lockdown causal-based COVID-19, and hence, a global pandemic was announced by the World Health Organization (WHO) in January 2020. The Movement Control Order (MCO) in Malaysia acts to moderate the spread of COVID-19 through the enacted measures. Furthermore, massive industrial, agricultural activities and human encroachment were significantly reduced following the MCO guidelines. In this study, first, a reconnaissance survey was carried out on the effects of MCO on the health conditions of two urban rivers (i.e., Rivers of Klang and Penang) in Malaysia. Secondly, the effect of MCO lockdown on the water quality index (WQI) of a lake (Putrajaya Lake) in Malaysia is considered in this study. Finally, four machine learning algorithms have been investigated to predict WQI and the class in Putrajaya Lake. The main observations based on the analysis showed that noticeable enhancements of varying degrees in the WQI had occurred in the two investigated rivers. With regard to Putrajaya Lake, there is a significant increase in the WQI Class I, from 24% in February 2020 to 94% during the MCO month of March 2020. For WQI prediction, Multi-layer Perceptron (MLP) outperformed other models in predicting the changes in the index with a high level of accuracy. For sensitivity analysis results, it is shown that NH3-N and COD play vital rule and contributing significantly to predicting the class of WQI, followed by BOD, while the remaining three parameters (i.e. pH, DO, and TSS) exhibit a low level of importance.

BACKGROUND: Achalasia is an uncommon disease treated by decreasing the lower esophageal sphincter resting pressure. This study compared the safety and efficacy of esophago-gastric myotomy via laparoscopic, robotic, and per-oral endoscopic approaches. METHODS: A retrospective review of data on patients with achalasia or other esophageal dysmotility disorder undergoing laparoscopic, robotically assisted, or per-oral endoscopic myotomy (POEM) procedures between 2013 and 2017 was performed. Patient demographics, comorbidities, procedure details, length of stay, 30-day readmission rate, and combined technical complication (full-thickness injury, conversion to open, and delayed perforation) were compared. Multiple logistic regression analysis was performed to determine which factors contributed to combined technical complication. RESULTS: There were 171 patients who underwent esophago-gastric myotomy with 161 (94.2%) having achalasia. There were 40 laparoscopic Heller myotomies with partial fundoplication, 44 robotic Heller myotomies with partial fundoplication, and 87 POEM procedures performed during the study period. Baseline statistical differences were found among the groups in regard to gastroesophageal reflux symptoms, arrhythmia, hypertension, and congestive heart failure. Laparoscopic Heller myotomy had significantly higher combined technical complications (7, 17.5%) compared to robotically assisted Heller myotomy (0, 0%) and POEM (1, 1.1%). Multivariate analysis showed that laparoscopic Heller myotomy (OR 32.22; 95% CI 2.66, 389.83; p = 0.01), myocardial infarction (OR 27.94; 95% CI 1.66, 471.10; p = 0.02), and history of smoking (OR 8.87; 95% CI 1.29, 61.15; p = 0.03) were risks for developing combined technical complications. CONCLUSION: Robotically assisted Heller myotomy and POEM are safe and efficacious treatments for achalasia with lower rates of technical complications compared to laparoscopic Heller myotomy. With the advancements in endoscopic instruments and robotic surgery, POEM and robotically assisted Heller myotomy should be considered in the treatment of achalasia and esophageal dysmotility disorders.

We discuss the accuracy and performance of the adaptive neuro-fuzzy inference system (ANFIS) in training and prediction of dissolved oxygen (DO) concentrations. The model was used to analyze historical data generated through continuous monitoring of water quality parameters at several stations on the Johor River to predict DO concentrations. Four water quality parameters were selected for ANFIS modeling, including temperature, pH, nitrate (NO3) concentration, and ammoniacal nitrogen concentration (NH3-NL). Sensitivity analysis was performed to evaluate the effects of the input parameters. The inputs with the greatest effect were those related to oxygen content (NO3) or oxygen demand (NH3-NL). Temperature was the parameter with the least effect, whereas pH provided the lowest contribution to the proposed model. To evaluate the performance of the model, three statistical indices were used: the coefficient of determination (R (2)), the mean absolute prediction error, and the correlation coefficient. The performance of the ANFIS model was compared with an artificial neural network model. The ANFIS model was capable of providing greater accuracy, particularly in the case of extreme events.

Uncontrolled releases of Tigris and Euphrates River waters after the 2003 war have partially restored some former marsh areas in southern Iraq, but restoration is failing in others because of high soil and water salinities. Nearly 20% of the original 15,000-square-kilometer marsh area was reflooded by March 2004, but the extent of marsh restoration is unknown. High-quality water, nonsaline soils, and the densest native vegetation were found in the only remaining natural marsh, the Al-Hawizeh, located on the Iranian border. Although substantially reduced in area and under current threat of an Iranian dike, it has the potential to be a native repopulation center for the region. Rapid reestablishment, high productivity, and reproduction of native flora and fauna in reflooded former marsh areas indicate a high probability for successful restoration, provided the restored wetlands are hydraulically designed to allow sufficient flow of noncontaminated water and flushing of salts through the ecosystem.
