The aims of this study were to evaluate the exposure to permethrin and cypermethrin of young children aged between 2 and 5 years in Nakhon Pathom and Sing Buri provinces, Thailand. A questionnaire that included general demographic information, household characteristics, insecticide usage and exposure-related behavior in children was used to interview parents or family caregivers. Permethrin and cypermethrin concentrations on floor surfaces and children's hands, as well as their urinary metabolites, were analyzed by gas chromatography coupled with mass spectrometry. The results showed that permethrin and cypermethrin were detected in 62% and 83% of the children's hand wipe samples, with geometric mean (GM) levels of 0.02 µg and 0.04 µg, respectively. Permethrin and cypermethrin were detected in 79% and 93% of floor surface wipe samples, with GM levels of 0.90 µg/m2, and 1.49 µg/m2, respectively. For children's urine, the GM concentrations of cis- and trans-DCCA, 3-PBA, and total pyrethroid metabolites were 0.84, 0.31 and 1.23 nmol/g creatinine, respectively. This study found that household insecticide product usage and having a tile floor were associated with increased permethrin concentrations on the children's hands and floor surfaces. However, cypermethrin concentrations on floor surfaces were significantly higher in families using aerosol insecticide sprays and insecticide products in the living room and bedroom. The predictors of the total pyrethroid, DCCA and 3-PBA metabolites are permethrin on children's hands or floor surfaces and cypermethrin on floor surfaces.

Acetochlor is a chloroacetanilide selective pre-emergent herbicide used for controlling grass and broadleaf weeds in crops. This study compared the acetochlor exposures of backpack and tractor sprayers and assessed whether dermal or air exposures were more important contributors to the overall body burden as measured by urinary metabolites. Sixty sugarcane farmers in Nakhonsawan province, Thailand participated in the study, and breathing zone air and dermal patch samples were collected during spraying. Urine samples were collected before spraying, at the end of the spraying task, and on the day after spraying. For backpack and tractor sprayers, there was no significant difference in their breathing zone air concentrations, total body dermal samples, or urinary 2-methy-6-methyaniline (EMA) concentrations on the day after spraying. In addition, although most backpack and tractor sprayers wore long pants and long sleeve shirts, they were still exposed to acetochlor, as evidenced by a significant increase in the urinary EMA from before spraying (GM = 11.5 µg/g creatinine) to after spraying (GM = 88.5 µg/g creatinine) to the next day (GM = 111.0 µg/g creatinine). Breathing zone air samples were significantly correlated with those of total body dermal patch samples and with urinary EMA concentrations after spraying. This suggests that both air and dermal exposure contribute to urinary EMA levels.

Cypermethrin, a pyrethroid insecticide, is frequently spread on agricultural farmlands and is also used in households in Thailand. Conventional pesticide-using farmers (n = 209) were recruited from the Phitsanulok and Nakornsawan provinces. Certified organic farmers (n = 224) were also recruited in the Yasothorn province. The farmers were interviewed via a questionnaire and the urine from their first morning void was collected. The urine samples were analyzed for 3-phenoxybenzoic acid (3-PBA), cis-3-(2,2-dichlorovinyl)-2,2-dimethylcyclopropane carboxylic acid (cis-DCCA), and trans-3-(2,2-dichlorovinyl)-2,2-dimethylcyclopropane carboxylic acid (trans-DCCA). The results showed no significant difference in the urinary cypermethrin metabolites between the conventional farmers and the organic farmers, for whom the usage of cypermethrin was not accounted for. However, when conventional farmers who used cypermethrin on the farm and in the home were compared with conventional farmers who did not use any cypermethrin or with organic farmers, a significant difference was noted for all metabolites except for trans-DCCA. These findings show that the most significant exposures to cypermethrin are among conventional farmers who use the insecticide on their farms or in their homes. However, measurable levels of all metabolites were found among both conventional and organic farmers who only used cypermethrin in the home or not at all, suggesting that the at-home use of pyrethroids and other possible exposures from pyrethroid residues on market-bought food may contribute to urinary levels of pyrethroids that exceed those of the general population in the US and Canada.

This study measured pesticide levels on children's hands, home indoor surfaces, and drinking water among Thai conventional and organic farm families in three provinces. Farm families in Nakhonsawan and Phitsanulok provinces were the conventional farmers and those in Yasothon province were the organic farmers. Samples were collected in the dry and wet seasons over 2 years (2017 and 2018). All samples were analyzed for organophosphates (OPs), carbamate, pyrethroid, triazines, glyphosate, and aminomethylphosphonic acid (AMPA). On children's hands, the highest concentrations for the 2 palmar surfaces were found for cypermethrin (7.46 µg) and fipronil (2.88 µg). On home surfaces of approximately 1000 cm2, the highest concentrations were found for cypermethrin (27.94 µg) and fipronil 49.76 µg)/1000 cm2. For the conventional farmers, the most common pesticides on children's hand wipes were cypermethrin and fipronil, which are used as in-home pesticides as well as agricultural pesticides. However, home surface wipes showed other pesticides associated with agriculture were presented in the home during the seasons when they were used, suggesting spray drift or carry home sources for these pesticides. During the wet season, pesticides were found in the drinking water of all provinces. Most common were fenitrothion, profenofos, fenthion, atrazine, and AMPA. Profenofos was found at levels above the health guidelines. During 2017, there was extensive flooding in Thailand which may have contributed to the drinking water contamination, even though most farmers report using tap water or bottled drinking water. Future work is needed to assess the risk of all sources of pesticide exposures in children and to repeat the drinking water pesticide contamination analysis.

Organophosphate (OP) pesticides are used by most farmers to remove insects and to increase productivity; however, questions remain on the long-term health impacts of their use. This study assessed the relationship between OP biomarker levels and metabolic biomarker parameters. Conventional farmers (n = 213) and organic farmers (n = 225) were recruited, interviewed, and had physical health examinations. Serum glucose and lipid profiles, triglycerides, total cholesterol, high-density lipoproteins (HDL), and low-density lipoproteins (LDL) were measured. The average age, gender, education, and self-reported agricultural work time, work in second jobs, smoking status, alcohol consumption, insecticide use at home, home location near farmlands and years of pesticide use were significantly different between the conventional and organic farmers. The urinary OP metabolite levels were also significantly different between the two groups. With an increase in urinary diethyl phosphate, dimethyl phosphate and dialkyl phosphate metabolites, the total cholesterol, LDL and HDL, were significantly increased for all farmers after controlling for age, gender, alcohol consumption, years of pesticide use, and home location near farmlands. The results are consistent with our previous studies which suggests that pesticide usage, especially organophosphates, may increase the risk of cardiovascular disease and stroke among Thai farmers.

This cross-sectional study aimed to evaluate occupational hazards, health conditions and personal protective equipment used among healthcare workers. Information from the sample size of 1,128 healthcare workers were collected using questionnaires. The healthcare workers participated in this study were from five departments including inpatient, outpatient, surgery and anesthesia, nutrition service and hospital support services departments in five hospitals in Thailand. The results indicated that the majority of healthcare workers were female; these healthcare workers work 9.0 to 11.1 hours/day on average and were exposed to several chemical, biological and physical hazards. The healthcare workers in the nutrition service department reported the highest percentage of musculoskeletal disorder and respiratory problems. The highest percentage of skin problems were reported by healthcare workers in surgery and anesthesia department. The results showed musculoskeletal disorder, respiratory and skin problem significantly differed among healthcare workers in the five departments including the wrists/hands (p = 0.024), upper back (p = 0.009), chest pain symptoms (p = 0.004), and dry/wound symptoms (p = 0.013). Healthcare workers did not have adequate protection from work-related hazards in their workplace. Health education programs, control measures and organizational policies should be implemented to mitigate the hazards for healthcare workers in hospitals.

In Thailand, glyphosate is popular herbicide to control pests in the agricultural sector. This study aimed to measure glyphosate exposure concentrations through inhalation, dermal contact, and urinary glyphosate concentrations among 43 vegetable farmers spraying glyphosate in Bungphra Subdistrict, Phitsanulok Province. Four types of spraying equipment were used, manual pump backpack (n = 3), motorized spray backpack (n = 22), battery pump backpack (n = 16), and high pressure pump (n = 2). Breathing zone air samples were collected using glass fiber filters; dermal contact samples were collected using 100 cm2 cotton patches attached on 10 body locations and urine samples were collected at 3 time points: morning void urine the day before spraying, the end of spraying event, and the morning void urine the next day of spraying. The results showed that the geometric mean (GM; geometric standard deviation [GSD]) of breathing zone concentrations of glyphosate exposure were 9.37 (10.17) µg/m3. The GM (GSD) of total dermal patches exposure concentrations were 7.57 (0.01) mg/h. The legs, back, and arms were the most exposed body areas. The GM (GSD) of urinary glyphosate was found highest among vegetable farmers using manual backpack 46.90 (1.35) µg/g creatinine. Farmers should wear masks and boots to reduce glyphosate exposure by inhalation and dermal contact.

Sugarcane farmers and cutters have been reported to be at high risk of acute kidney injury. This cross-sectional study aimed to assess acute kidney injury biomarkers, and cross-shift change among 150 sugarcane cutters and 98 sugarcane farmers in Thailand. Physical health examination, environmental measurements, and measured urinary neutrophil gelatinase-associated lipocalin (NGAL) were assessed. Cross-shift change of urine creatinine (uCr), albumin creatinine ratio (ACR), and NGAL between sugarcane farmers and cutters was compared. Factors influencing abnormal post-shift ACR and NGAL were analyzed by binary logistic regression adjusted with covariates. Sugarcane cutters were significantly more likely to take nonsteroidal anti-inflammatory drugs (NSAID) and herbs to relieve pain than sugarcane farmers. Males were more likely to be current smokers and drinkers although 62% of cutters and 56% of farmers were female. Sugarcane farmers and cutters had similar post-shift estimated glomerular filtration rate (eGFR) and ACR levels. Cross-shift measurements showed that both sugarcane cutters and sugarcane farmers had significantly higher post-shift geometric mean (GM) urinary NGAL, but that the cross-shift changes of NGAL were significantly higher for sugarcane cutters (182%) vs. sugarcane farmers (112%). Water intake less than recommendation of subjects was a significant predictor of a higher risk for an abnormal post-shift ACR after controlling for covariates, while being a sugarcane cutter was a predictor of a lower risk of an abnormal risk of a post-shift NGAL after controlling for covariates. Measurements of heat stress (WBGT plus workload) found that sugarcane cutters exceeded recommended exposures while sugarcane farmers were compliance to the TLV. These findings suggest that to protect sugarcane farmers and cutters, preventative measures are needed.

Agriculture in Thailand, which employs over 30 percent of the workforce and contributes significantly to the country's gross domestic product, is a key sector of its economy. Import and use of pesticides has increased over the past decade due to Thailand's major role as a leading exporter of food and agricultural products. The widespread and poorly regulated use of pesticides presents a potential risk to the health of farmers, farm families, the general population including children and the environment. This article is a result of the Southeast Asia GEOHealth Network Meeting of February 2019. It summarizes the current situation on pesticide use and regulation in Thailand and reports research findings on the potential health and environmental impacts of pesticide use, as well as highlighting gaps in research that could play an important and influential role in future policy initiatives on pesticides. Although Thailand has made remarkable progress in improving agricultural health and safety and similarly strong research and policy programs are being developed in other countries in the region, there are still significant gaps in research and policy that need to be filled.

PURPOSE: Musculoskeletal Disorders (MSDs) are one of the most common occupational diseases responsible for work restriction and absenteeism. The purpose of the study was to compare the prevalence of MSDs between ready-made garment (RMG) factories that are compliant versus non-compliant with International Labor Organization (ILO) based Occupational Safety and Health (OSH) standards, and to identify other factors associated with a higher prevalence of MSDs. METHODS: A multicenter cross-sectional study was conducted among 410 randomly selected female workers (age: >18 years; ≥1 year of work experience) from 12 randomly selected RMG factories in the Dhaka district of Bangladesh from February to December 2019. Factories were stratified as OSH compliant or non-compliant. Data were collected using a structured questionnaire consist of socio-demographic, work-related factors, and Nordic Musculoskeletal Questionnaire (NMQ) for MSD assessment. Robust Poisson regression was used to investigate the association of MSDs with OSH compliance, socio-demographic and other work-related factors. RESULTS: Respondents' mean age was 26.9 ± 5.9 years. A total of 235 (57%) of the workers reported musculoskeletal pain in at least one body part during the last month. Among the nine reported MSDs, the prevalence of lower back pain was highest (41%) followed by pain in the knees (33%). Multivariable analysis showed lower prevalence of any MSD among workers in OSH compliant factories [aPR = 0.43, 95% CI: 0.31-0.58], and workers with regular working hours (8 hrs/day) [aPR = 0.79, 95% CI: 0.62-1.00]. The risk of developing any MSD was significantly higher for age group 25-30 years [aPR = 1.16, 95% CI: 1.01-1.33] and age group ≥31 years [aPR = 1.21, 95% CI: 1.04-1.1.39]. CONCLUSION: The study found a high burden of musculoskeletal disorders among female RMG workers, especially those in non-OSH compliant factories that work long hours. Enforcement of minimum ILO-based OSH standards and introduction of low-cost interventions to improve working conditions in non-compliant factories would help sustain the pace of development in this sector in Bangladesh, and ensure the health of the women who work there.

Alteration of the hypothalamic-pituitary-adrenal (HPA) axis hormones has been associated with a range of chronic metabolic and cardiovascular health conditions. This study evaluated whether type of farming (organic versus conventional) or the number of self-reported days of spraying pesticides in the past 8 months was associated with diurnal cortisol levels. Salivary cortisol levels were measured four times a day (waking, 30 min after waking, 6 h after waking and bedtime) longitudinally, 8 months apart during three rounds of data collection. Pesticide using (conventional) and organic farmers were recruited to participate. Pesticide use in the previous 8 months was determined as the number of spray days for each type of pesticide used (herbicide, insecticide, fungicide) from self-reported questionnaires. Estimates of cortisol levels at four time points, the cortisol awakening response (CAR), and the diurnal cortisol slope (DCS) were estimated with a longitudinal mixed model that accounted for the non-linearity of cortisol levels across the day. Conventional farmers had significantly lower cortisol levels at waking than organic farmers (3.39 versus 3.86 ng ml-1), 30 min after waking (5.87 versus 6.96 ng ml-1), 6 h after waking (1.62 versus 1.88 ng ml-1), and lower diurnal cortisol slope (-2.26 versus -2.51 ng ml-1). Farmers who frequently applied herbicides (90th percentile of the number of spray days in the past 8 months) had significantly lower waking, 30 min after waking, 6 h after waking, bedtime and diurnal cortisol slopes compared with those with no spray days of herbicide in the past 8 months (organic and some of the conventional farmers). Those who frequently applied insecticides in the past 8 months had significantly lower bedtime levels and diurnal cortisol slopes, compared with those with no spray days of insecticide in the past 8 months. There were no significant differences in cortisol hormones between those who frequently applied fungicides and those who did not spray fungicides. Repeated pesticide use appears to be disrupting the HPA axis and depressing the normal diurnal cortisol rhythm among conventional Thai farmers.

The objective of this study was to investigate the relationship of acute pesticide exposures and acute changes in thyroid hormones among Thai farmers. We recruited 78 farmers, who were scheduled to spray insecticides (chlorpyrifos and/or cypermethrin) or herbicides (paraquat and/or glyphosate). On the day before spraying, farmers collected their first morning void urine and went for blood collection. On the spray day, urine samples were collected at end of the spraying event and they were interviewed with questionnaires. The next morning, the first morning void urine and blood samples were collected. Blood samples were analyzed for thyroid hormones. Urine samples were analyzed for the metabolites of the pesticide sprayed. The results showed that the thyroid hormones, free triiodothyronine (FT3) and total triiodothyronine (T3) were significantly reduced as urinary chlorpyrifos metabolite increased the day after spraying. Total thyroxine (T4) significantly increased as cypermethrin metabolites increased the day after spraying. T4 significantly increased as urinary glyphosate levels increased; however, FT3 and T3 decreased significantly as urinary paraquat levels increased the day after spraying. These findings suggest that acute exposures to the pesticides chlorpyrifos, cypermethrin, paraquat and glyphosate can produce acute effects on the hypothalamic-pituitary-thyroid (HPT) axis, acutely altering thyroid hormone levels.

Many pesticides are endocrine-disrupting chemicals that can interfere with hormone levels. This study aimed to assess the longitudinal impact of exposure to pesticides on thyroid hormone levels, including Thyroid Stimulating Hormone (TSH), Free Triiodothyronine (FT3), Free Thyroxine (FT4), Triiodothyronine (T3), and Thyroxine (T4). Both conventional (i.e., pesticide using) and organic farmers were interviewed using questionnaires, and blood samples were collected at 7-9 a.m. to determine thyroid hormone levels for four rounds, with a duration of eight months between each round. A linear mixed model of the natural log of the individual hormone levels used random intercepts for subjects while controlling gender, baseline age, and body mass index (BMI) was used to compare between conventional and organic farmers or the impact of cumulative days of spraying insecticides, herbicides or fungicides. The estimated marginal means of the thyroid hormone levels (TSH, FT3, T3, and T4) estimated from the linear mixed models were significantly higher among the conventional farmers than organic farmers. As cumulative spray days of insecticide, herbicide or fungicide increased, TSH and FT3 increased significantly. FT4 decreased significantly as cumulative spray days of insecticide or fungicide increased. These findings suggest that the insecticides, herbicides, and fungicides sprayed by conventional farmers exert endocrine-disrupting activities, altering the hypothalamic-pituitary-thyroid (HPT) axis homeostasis.

Organophosphates (OPs) are widely used for pest and weed control in many countries including Thailand. In addition to causing environmental pollution, OPs affect human health by overstimulating neurotoxicants, and OP exposure during pregnancy can lead to adverse health effects of mothers and their fetuses. Using gas chromatography-mass spectrometry with a dedicated extraction protocol to identify OPs in meconium of newborn babies (n = 68) from hospitals in Amnat Charoen, Kanchanaburi and Nakhon Sawan provinces, agricultural regions of Thailand, among ten OP types analyzed, eight were detected in 98% of meconium samples (chlorpyrifos (median ± interquartile range (IQR) 0.08 ± 0.03-0.16 µg/g) in 32% of samples, demeton-s-methyl (0.35 ± 0.26-0.49 µg/g) in 73%, dichlorvos (0.67 ± 0.58-0.71 µg/g) in 38%, dimethoate (0.43 ± 0.09-1.56 µg/g) in 50%, ethion (0.21 ± 0.19-0.26 µg/g) in 12%, malathion (0.28 ± 0.15-0.52 µg/g) in 50%, omethoate (5.63 ± 4.85-8.57 µg/g) in 34%, and tolclofos-methyl (0.08 ± 0.03-0.10 µg/g) in 41%). There are no significant differences in these parameters from babies whose mothers did and did not work in the agricultural or who lived near (within one km) and distant from farmland. The findings should be of benefit in developing programs to protect pregnant women and newborn babies from exposure to OP pesticides.

The aim of this longitudinal study is to assess how pesticide use may impact metabolic biomarkers by collecting and comparing data from conventional (n = 13) and organic farmers (n = 225) every eight months for four rounds. Farmers were interviewed about family health history, food consumption behaviors, self-reported health problems, agricultural activities, and history of pesticide use. Systolic and diastolic blood pressure and body mass index (BMI) were measured. Blood samples were collected for total cholesterol, low-density lipoprotein (LDL), high-density lipoprotein (HDL), blood glucose, and triglycerides. A linear mixed model with random intercepts for subjects was used to compare the metabolic biomarkers between conventional and organic farmers and to examine the impact of the number of pesticide spray days for all four rounds after controlling for covariates. The conventional farmers reported using insecticides, herbicides, and fungicides. The marginal means for chemical farmers were significantly higher than organic farmers for total cholesterol, LDL, HDL, glucose, systolic and diastolic blood pressure, BMI, and waist circumference. Increasing the number of days of spraying either insecticides or fungicides was associated with an increase in HDL, LDL, and cholesterol levels. Increasing the number of herbicide spray days was associated with an increase in systolic and diastolic blood pressure and a decrease in BMI. These findings suggest that pesticide-using conventional farmers may be at higher risk of metabolic disease in the future.

Organic farming has been promoted in Thailand by King Rama the ninth. In addition to being healthier for consumers, organic farming is healthier for agricultural workers. The cross-sectional study was conducted to investigate the frequency of chronic disease conditions, accidents, health symptoms, and ergonomic problems among 243 conventional (pesticide using) farmers and 235 organic farmers. Data were collected using questionnaires in face-to-face interviews. The results indicated symptoms that could be related to pesticide exposure (skin rashes, water blisters, headache, dizziness, and loss of appetite) were significantly higher among conventional farmers than organic farmers. The organic farmers reported significantly more health symptoms such as hives, chest pain, mild fever, flatulence, and frequent urination than the conventional farmers. The organic farmers reported significantly more pain, numbness, or weakness in the wrists/hands, fingers, upper back, hips, and ankles/feet than conventional farmers.

Thai agriculturists heavily used paraquat in agricultural areas to control weed and grasses. This study determined paraquat exposure among backpack sprayers in Thailand and identified determinants of occupational exposure. Breathing zone air and dermal samples were collected from 57 backpack sprayers while spraying. Spot urine samples were collected on the day before spraying, end of spraying event and the next day after spraying. The subjects were interviewed about general demographics, agricultural activities, pesticide application and personal protective equipment used while applying paraquat. Paraquat concentrations in urine samples, air samples and dermal samples were determined by HPLC with a fluorescence detector. The median IQR of urinary paraquat concentrations on the day before spraying, end of spraying event, the next day after spraying were 2.51 (0.81-5.59), 8.23 (3.3-13.73) and 3.48 (1.03-8.19) µg/g creatinine, respectively. Concentrations of air samples and total dermal exposures were 5.15 (2.28-10.12) µg/m3and 92.66 (34.37-1647.46) µg/hr, respectively. Use of battery powered backpack sprayer and standing upwind effectively reduced inhalation exposures. Wearing a long sleeve shirt, long pants, boots, latex gloves and balaclava could reduce paraquat concentration on dermal exposure among backpack sprayers.

Pesticides can act as endocrine disruptors by different mechanisms including inhibition of iodine absorption, increases in thyroid hormone clearance, decreased cellular uptake of thyroid hormones, or changes in expression of thyroid hormone regulated genes. This study examined how exposure to pesticides impacts thyroid hormone levels, thyroid stimulating hormone (TSH), triiodothyronine (T3), thyroxine (T4), free T3 (FT3), and free T4 (FT4) by comparing conventional (n = 195) and organic farmers (n = 222), and by evaluating which types of pesticides might be associated with changes in thyroid hormone levels. Questionnaires were used to collect information about farmer characteristics, self-reported stress, agricultural activities, and history of pesticide use. Conventional farmers were asked to report the type and quantity of pesticides used each day. The TSH, FT3, T3, and T4 levels of conventional farmers were 1.6, 1.2, 1.3, and 1.1 times higher than those of organic farmers, respectively, after adjusting for covariates. Several specific herbicides had a significant relationship between the amount applied and an increase in thyroid hormone levels, after covariate adjustment. They included: paraquat (TSH, FT3 and T3); acetochlor (FT4); atrazine (TSH, FT3 and T3); glyphosate (T4); diuron (TSH) and the "other" herbicides including alachlor, propanil, and butachlor (FT4 and T3). The most commonly used herbicide among conventional farmers was glyphosate, followed by paraquat, 2,4-dichlorophenoxyacetic acid (2,4-D). These findings suggest that exposure to pesticides could impact the development of metabolic diseases and other health outcomes by altering the endocrine system (the thyroid hormone levels) through the hypothalamic-pituitary-thyroid (HPT) axis. This work is a part of a longitudinal study which will evaluate the sub-chronic effects of repeated exposure to different types of pesticides on thyroid hormone levels.

Agricultural workers who have concurrent exposure to pesticides and noise are at increased risk of hearing loss. We recruited 163 Thai conventional and 172 organic farmers to answer our questionnaires about personal demographics, agricultural activities, and pesticide and agricultural machinery use. This information was used to calculate the years of conventional (pesticide use) farming and the years of agricultural noise exposure, and to estimate semiquantitative metrics for pesticide exposure (cumulative intensity score-years) and cumulative noise exposure (dB(A)-years) for each conventional farmer. All participants underwent pure tone audiometric testing. The mean hearing threshold in the low-frequency band (0.5-2 kHz) and high-frequency band (3-6 kHz) were used for analysis. Years involved in conventional farming and years using agricultural machinery were associated with an increase in the average hearing threshold for the high-frequency band after controlling for age, ever exposed to industrial noise and cigarette smoking. The highest category of cumulative insecticide exposure (score-years), cumulative organophosphates exposure (score-years) and cumulative noise exposure (dB(A)-years) were also associated with an increased high-frequency band hearing threshold among conventional farmers. Results from the full cohort and the subcohort of conventional farmers support each other and the hypothesis that pesticide and noise have an additive effect on hearing, since no model interactions were significant.

Pesticide exposure has been implicated as a risk factor for developing a wide range of adverse health issues. Some examples are metabolic syndromes, including diabetes. This study investigated the relationship between current occupational use of pesticides and metabolic and cardiovascular biomarker levels among organic and conventional farmers in Thailand. In total, 436 recruited farmers were divided into two groups: conventional farmers (n = 214) and organic farmers (n = 222). Participants, free of diabetes, were interviewed and submitted to a physical examination. Serum samples were collected for clinical laboratory analyses, i.e., serum glucose and lipid profiles (triglycerides, total cholesterol, high-density lipoproteins, and low-density lipoproteins). Potential risk factors such as smoking, alcohol consumption, and heavy exercise were significantly different between the two groups. There were significant differences in terms of the years of pesticide use, pesticide use at home, sources of drinking water, and distance between the farmers' homes and farms between the groups. After adjusting for confounders, current conventional farmers had significantly higher abnormal body mass index (BMI), waist circumference, body fat percentage (% body fat), triglyceride, total cholesterol, and low-density lipoprotein values as compared to organic farmers. Conventional farmers had higher risk of many metabolic and cardiovascular risk factors as compared to organic farmers, putting them at higher risk of metabolic diseases in the future.
