RESUMO

PIP: The extent of the HIV epidemic in the Caribbean is described as related to the subregional coordinating project CAREC, or Caribbean epidemiology Center, an agency under Pan American Health Organization, for 19 English speaking countries and Suriname. By July 1990, 1702 AIDS cases had been reported to CAREC, out of 5726 cases in 27 Caribbean countries excluding Puerto Rico. 90% of the cases occurred in the 5 largest, Bahamas, Barbados, Bermuda, Jamaica and Trinidad and Tobago. Initially the transmission pattern was predominantly among male homo- and bisexuals, but not it is mostly heterosexual with a growing mother-to-child transmission. All countries are now screening blood or blood donors. CAREC is coordinating epidemiologic surveillance, helping national laboratories to screen by providing confirmatory tests, providing culture-relevant health education materials and AIDS information, assisting with surveys, holding training workshops for health care workers, and assisting member countries in designing education and counseling programs for high risk women.^ieng

A review of surveillance data on AIDS and HIV infection in the 18 English-speaking Caribbean countries and Suriname suggests that the epidemiologic pattern of AIDS in the Caribbean is evolving from an epidemic that began in 1983 among homosexual and bisexual males to one in which cases are increasingly resulting from heterosexual contact, with different countries at various stages of transition. Overall, there has been a decline in the male to female case ratio. Perinatal transmission is already a major problem in many countries--19% of cases in the Bahamas are among children under 15 years of age. Serosurveys conducted in Trinidad and Tobago, Jamaica, Antigua, St. Vincent and the Grenadines, and other countries show high HIV seroprevalence among homosexuals (15-40%), prisoners (4-10%), prostitutes (up to 13%), and cocaine users (2%); at present, prevalence in the general population continues to be low.

RESUMO

In 1981, a measles outbreak in an Arkansas university involved 16 students and four persons who were contacts of these students. The first two cases were in students who recently returned from Honduras. Of the 16 students with measles, 12 had significant gastrointestinal symptoms; five had elevated liver transaminase, and eight were hospitalized. Only two of the students were considered adequately immunized. A voluntary immunization clinic held on the university campus resulted in 67 per cent of 3,076 students being vaccinated.
