Introduction: Physical exercise can generate reactive oxygen species that damage essential cellular components, such as fatigue. Flavonoids, due to their low toxicity and antioxidant activity, have been studied as potential dietary supplements to improve sports performance. Objective: To describe the scientific evidence that has been obtained on the efficacy of the use of flavonoids as supplements to improve sports physical performance. Methods: A search was carried out, in PubMed and Google Scholar databases, on the efficacy of the use of flavonoids as supplements for physical and sports performance. The keywords were used flavonoid, supplementation, sport, exercise, fatigue, muscle, fats, physical activity. Articles published from 2018 to 2022, in English, that met the selection criteria were taken into account. Results: Flavonoids are capable of reducing fatigue due to physical exertion in a sports context, mainly due to their ability to regulate the activity of endogenous antioxidant agents such as superoxide dismutase, catalase and glutathione peroxidase enzymes, which are responsible for the neutralization of reactive oxygen species. In addition, they are capable of favoring the neutralization of toxic metabolites that are generated during exercise and of increasing energy reserves (glycogen in the liver and muscles) and muscular resistance. Conclusions: There is scientific evidence that flavonoids are capable of improving sports physical performance, mainly by reducing fatigue and increasing muscular resistance.

RESUMO

Background: Psychological stress is a reaction to an unexpected situation that favours adaptation and response to the event. However, when psychological stress is chronic or very intense, it can induce changes in various systems and tissues, causing diseases or aggravating existing ones. Objective: To briefly analyse the pathophysiological conditions caused by psychological stress. Method: A narrative review of the scientific literature on pathophysiological conditions as a consequence of psychological stress was performed. Results: Psychological stress can induce various conditions at the gastrointestinal, immune and cardiovascular levels. This is mainly due to the neurobiological and endocrine response because when faced with a stressful stimulus, a deregulated release of glucocorticoids and catecholamines is generated, altering the normal physiology of the organism. Gastrointestinal disorders are mainly due to goblet cell dysfunction, resulting in intestinal hyperpermeability, inflammation and infection. Changes at the immune level lead to an increase in inflammatory responses but a decrease in the protective activities of the immune system. Finally, cardiovascular conditions include atherosclerosis, increased blood pressure and stroke. Conclusion: Psychological stress can induce real physiological pathologies and, in some cases, fatal ones. Some of the molecular mechanisms involved in these pathologies have already been studied and identified. Knowledge of these molecular mechanisms can help clinicians and therapists to improve the treatment and therapy of patients.

Significant advances documenting the costs associated with insomnia have been achieved. However, those related to insomnia associated with mood disorders remain understudied, even though insomnia is more severe in the presence of comorbid conditions such as depression and anxiety. The aim of this study was to determine the direct and indirect costs of insomnia associated with depression and anxiety disorders (DAD) from the perspective of the patient in a private healthcare system. This was an observational study of chronic insomnia associated with DAD at a private Sleep Disorders Clinic in Mexico City between 2019 and 2020. Patients were followed for up to one year. Healthcare resource utilization data were collected through clinical records. Direct and indirect costs associated with insomnia treatment were estimated through micro-costing. The estimated economic burden was projected to 5 years adjusting for inflation and discounting future costs. A deterministic sensitivity analysis was performed. The median cost of the first year of insomnia treatment associated with DAD was US$3537.57 per patient. The work productivity loss represented the highest economic burden (63.84%) followed by direct medical costs (28.32%), and the direct non-medical costs (7.85%). The estimated annual economic burden for patients treated in the private healthcare system in Mexico was US$293 million. The costs of insomnia associated with DAD at a private clinic in México were found to be high. The burden of the costs faced by these patients is substantial relative to the median income of the population. The economic costs at an individual and societal levels are substantial. RESUMO

Prolonged use of mechanical ventilation (MV) can lead to greater complications for a patient. In clinical practice, it is important to identify patients who could fail in the extubation process. However, accurately predicting the outcome of this process remains a challenge. The diaphragm muscle is one of the most active elements in the breathing process. On the other hand, there are several techniques to derive respiratory information from the ECG signal. Signals derived from diaphragmatic activity and from the ECG, such as the envelope of the surface diaphragm electromyographic signal (sEMGi) and the respiratory signal derived from the electrocardiogram (ECG) could contribute to analyze the respiratory response in patients assisted by MV. This work proposes the analysis of the coherence between sEMGi and EDR signals to determine possible differences in the respiratory pattern between successful and failed patients undergoing weaning. 40 patients with MV, candidates for weaning trial process and underwent a spontaneous breathing test were analyzed, classified into: a successful group (SG: 19 patients) that maintained spontaneous breathing after the test, and a failed group (FG: 21 patients) that required reconnection to the MV. The cross correlation, power spectral density and magnitude squared coherence (MSC) of the sEMGi and the EDR signals were estimated. According to the results, the MSC parameters such as area under the curve and mean coherence value presented statistically significance differences between the two groups of patients (p = 0.024). Our results suggest that both sEMGi and EDR signals could provide information about the behavior of the respiratory system in these patients. Clinical Relevance- This study analyzes the correlation and the coherence between the envelope of the surface electromyographic signal and the respiratory signal derived from the ECG to characterize the respiratory pattern of successful and failed patients on weaning process.

RESUMO

In clinical practice, when a patient is undergoing mechanical ventilation, it is important to identify the optimal moment for extubation, minimizing the risk of failure. However, this prediction remains a challenge in the clinical process. In this work, we propose a new protocol to study the extubation process, including the electromyographic diaphragm signal (diaEMG) recorded through 5-channels with surface electrodes around the diaphragm muscle. First channel corresponds to the electrode on the right. A total of 40 patients in process of withdrawal of mechanical ventilation, undergoing spontaneous breathing tests (SBT), were studied. According to the outcome of the SBT, the patients were classified into two groups: successful (SG: 19 patients) and failure (FG: 21 patients) groups. Parameters extracted from the envelope of each channel of diaEMG in time and frequency domain were studied. After analyzing all channels, the second presented maximum differences when comparing the two groups of patients, with parameters related to root mean square (p = 0.005), moving average (p = 0.001), and upward slope (p = 0.017). The third channel also presented maximum differences in parameters as the time between maximum peak (p = 0.004), and the skewness (p = 0.027). These results suggest that diaphragm EMG signal could contribute to increase the knowledge of the behaviour of respiratory system in these patients and improve the extubation process.Clinical Relevance-This establishes the characterization of success and failure patients in the extubation process.

RESUMO

Infection by SARS-Cov-2 (COVID-19) has affected practically all the world. This joint position statement of Latin American Medical Societies provides an updated guide for the prevention, diagnosis, and treatment of osteoporotic patients in the face of possible clinical scenarios posed by the COVID-19 health crisis. BACKGROUND: Infection by SARS-Cov-2 (COVID-19) has affected practically all the world. Characterized by high contagiousness, significative morbidity, and mortality in a segment of those infected, it has overwhelmed health services and forced to redirect resources to the emergency while impacting the attention of acute non-COVID-19 and many chronic conditions. OBJECTIVE: The objective of this study is to provide an updated guide for the prevention, diagnosis, and treatment of osteoporotic patients in the face of possible clinical scenarios posed by the COVID-19 health crisis. METHODS: A task force, of bone specialists with a wide range of disciplines in the field of osteoporosis and fragility fracture, was convened with the representation of several professional associations, namely, the Mexican Association of Bone and Mineral Metabolism (AMMOM), the National College of Geriatric Medicine (CONAMEGER), the Latin American Federation of Endocrinology (FELAEN), the Mexican Federation of Colleges of Obstetrics and Gynecology (FEMECOG), the Mexican Federation of Colleges of Orthopedics and Traumatology (FEMECOT), and the Institute of Applied Sciences for Physical Activity and Sports of the University of Guadalajara (ICAAFYD). Clinical evidence was collated, and an evidence report was rapidly generated and disseminated. After finding the gaps in the available evidence, a consensus opinion of experts was made. The resulting draft was reviewed and modified accordingly, in 4 rounds, by the participants. RESULTS: The task force approved the initial guidance statements, with moderate and high consensus. These were combined, resulting in the final guidance statements on the (1) evaluation of fracture risk; (2) stratification of risk priorities; (3) indications of bone density scans and lab tests; (4) initiation and continuation of pharmacologic therapy; (5) interruptions of therapy; (6) treatment of patients with incident fracture; (7) physical therapy and fall prevention; and (8) nutritional interventions. CONCLUSION: These guidance statements are provided to promote optimal care to patients at risk for osteoporosis and fracture, during the current COVID-19 pandemic. However, given the low level of available evidence and the rapidly evolving literature, this guidance is presented as a "living document" and future updates are anticipated.

RESUMO

This work describes a special case of pollution potential assessment applied to an abandoned sulfide tailings impoundment located in the Riotinto mining district (Huelva), near the Tinto River. Three overlapping levels of discharged tailings were recognized in the impoundment, from deeper to upper: pale yellow to white, red, and brownish-yellow. Mineralogical, physical, and chemical characteristics of tailings, water leachates, water, and sulfate efflorescent salts were analyzed. The total toxic element content and the leachate concentration were respectively used to calculate two indices that support potential toxicity assessment: the Index of Contamination (IC) and the Hazard Average Ratio (HAQ). According to the IC values, all tailings samples showed a high potential for contaminating soils and sediments, especially the intermediate tailings with up to As (8.6 g kg-1), Pb (14.8 g kg-1), and Cu (1 g kg-1). Deeper tailings leachate was extremely saline and acidic, with a very high concentration of sulfates and toxic elements, exceeding the values: 2600, 980, 30, and 17 mg L-1 for SO4, Fe, Al, and Cu, respectively. For this reason, these deeper tailings were linked to the saline and acid seepage, and also to the sulfate acid efflorescences formed at the dike toe. In conclusion, the studied abandoned tailings impoundment is related with a high likelihood of polluting the environment, represented by very high IC and HAQ values. However, when the Tinto River is considered the receiving water body, the severity of the potential contamination must be judged as very low.

RESUMO

This research describes the risk of death in elderly after hip fracture according to their strength, measured by hand grip. The result is that the weaker the patient, the greater the risk of death after hip fracture, highlighting the need to assess the force in those patients. For the coming years, most of hip fractures will occur in developing countries. It has been described that low muscular strength, measured by grip strength, increases the risk of mortality in those with hip fracture, in both high-and low- income countries. The objective of this study was to determine the mortality among patients with hip fracture and lower hand grip strength (HGS). MATERIAL AND METHODS: We conducted a cohort and longitudinal study at Hip and Pelvic Surgery Department of a tertiary hospital, in Monterrey, Mexico. The study included patients aged over of 69, admitted for hip fracture surgery from February 1st 2013 to July 31st 2014. HGS measurement was performed by a trained physician at arrival to emergency department prior to surgery; clinimetric variables were asked, and a complete medical history was included. RESULTS: A total of 670 patients were included in the study and grouped in different tertiles according to hand grip strength. During follow-up, there were 112 deaths (17.4%), 61 (27.5%) in tertile 1, 37 (17.1%) in tertile 2, and 14 (6.8%) in tertile 3, p < 0.001. The association remained significant after adjusting for confounding variables. Less than 5% of patients discharged from hospital were identified with osteoporosis. CONCLUSION: Lower hand grip strength in patients with a hip fracture is associated with high mortality after hip fracture.

RESUMO

Recent interest in protein intake per meal is observed in studies that have reported the protein intake patterns in different countries; however, comparisons of these data are lacking. We aimed to compare protein intake patterns and the percentage of inadequate protein intake (IPI) per day and meal in older adults from different countries. We acquired data of protein intake in older adults from four countries (Mexico, United States of America, Germany, and United Kingdom). We compared protein intake (per day and meal), IPI per day and meal, and the number of meals with an adequate protein content among countries. The IPI per day significantly differed among countries for <0.8 and <1.0 (both p < 0.001), but not for <1.2 g/kg/d (p = 0.135). IPI per meal (<30 g/meal) did not differ among countries at breakfast (p = 0.287) and lunch (p = 0.076) but did differ at dinner (p < 0.001). Conversely, IPI per meal (<0.4 g/kg/meal) significantly differed among countries at breakfast, lunch, and dinner (all p < 0.001). The percentage of participants that ate ≥30 g/meal or ≥0.4 g/kg/meal at zero, one, and two or three meals per day significantly differed among countries (all p < 0.05). IPI at breakfast and lunch (<30 g/meal) was a common trait in the analyzed samples and might represent an opportunity for nutritional interventions in older adults in different countries.

RESUMO

This work explores the ability of the Spanish Geochemical Database to identify the existence of polluted stream sediments due to the presence of abandoned mine wastes. The mining district of Sierra de Cartagena-La Unión has been used as test area. A total of 18 abandoned tailings facilities were selected and samples were taken for analysis of the content in potential toxic elements. The results were contrasted with geochemical background values in order to identify which geochemical elements could entail anomalies potentially reflected in the Spanish Geochemical Database. For the study area, As, Cd, Pb, and Zn have been identified as potential toxic elements which can be used as geochemical indicators of sediment pollution processes originated in abandoned mine wastes. Furthermore, the correlation between Cd and Zn allows them to be considered as the best indicators of potential sediment contamination. The study of anomaly thresholds, through the analysis of accumulated frequencies of the contents of these elements in the sediments of the Spanish Geochemical Database, allows identifying sediments that may have been contaminated. A Mann-Whitney test contrasting the data of tailings, polluted sediments, and non-polluted sediments has been performed in order to verify differences among these types of samples. In addition, a geospatial analysis has demonstrated a very close relationship between contaminated sediments and the presence of mining wastes upstream. This methodology can be applied to any other mining region, to recognize the possible influence of pollution processes derived from the presence of abandoned mine wastes taking advantage of existing geochemical databases.

RESUMO

Adequate protein intake per day has been associated with a lower risk of physical disability; however, if adequate protein intake per meal is also associated is unknown. The purpose of this study was to analyze the association between adequate protein intake per meal and physical disability in daily living activities in Mexican adults aged ≥60 years. We assessed the number of meals per day with an adequate protein content (24 h dietary recall), the presence of physical disability in daily living activities (two validated questionnaires), and their association in 187 participants through logistic regression. Consuming two or three meals per day with ≥30 g each was associated with lower risk of physical disability on Transportation (OR [95% CI]: 0.06 [0.01-0.50], p = 0.01), Shopping (0.05 [0.01-0.40], p = 0.004), Feeding (0.06 [0.01-0.74], p = 0.028), and Transfer (0.09 [0.01-0.98], p = 0.048). On the other hand, consuming two or three meals per day with ≥0.4 g/kg each was associated with lower risk of physical disability on Shopping (0.21 [0.05-0.89], p = 0.034) and Transportation (0.12 [0.03-0.48], p = 0.003). The consumption of two or three meals per day with adequate protein content is associated with lower risk of physical disability in Mexican adults aged 60 years and older.

RESUMO

PURPOSE: To describe the proportions of inadequate protein intake (IPI) per day and per meal and their association with functionality in middle to older aged Mexican adults. MATERIALS AND METHODS: In a cross-sectional design, we evaluated the protein intake and functionality of instrumental activities of daily living (IADL) and activities of daily living (ADL) of 190 middle to older aged Mexican adults. IPI was considered as any protein intake: <1.2 g/kg/day, <30 g/meal, or <0.4 g/kg/meal. Functionality was organized into three groups: high, middle, and low scores. The first was set as the reference, and the other was considered as impaired functionality. With a multinomial logistic regression, we analyzed the association between IPI per day and per meal with impaired functionality. RESULTS: A high proportion of participants showed IPI per day. The meal with the highest proportion of IPI was dinner, followed by breakfast and lunch for both criteria. IPI at lunch was a significant risk factor for impaired functionality in ADL when assessed with the 30 g/meal criterion (low scores, OR 3.82 (95% CI, 1.15-12.65); middle scores, OR 2.40 [1.03-5.62]). For the 0.4 g/kg/meal criterion, IPI at dinner was a significant risk factor for IADL middle scores only (OR 7.64, [1.27-45.85]). CONCLUSION: IPI per meal is high in middle to older aged Mexican adults, and at specific meals, it is a significant risk factor for impaired functionality in activities of daily living.

RESUMO

BACKGROUND: Several anthropometric equations that estimate skeletal muscle mass (SMM) have been published, but their applicability and accuracy among athletes are still uncertain. PURPOSE: To assess the accuracy of different anthropometric equations that estimate SMM in professional male soccer players, as compared to dual-energy X-ray absorptiometry (DXA) as the reference method. METHODS: In this cross-sectional study, we evaluated 179 professional male soccer players aged between 18 and 37 years. Anthropometric measurements (height, body weight, skinfold thicknesses, and girths) and a DXA whole body scan were performed the same day for each participant, and SMM was estimated with nine anthropometric equations (Heymsfield, Martin, Doupe, Kerr, Drinkwater, Lee, De Rose, and two equations published by Kuriyan). To determine differences between SMM estimated with anthropometric equations and SMM evaluated with DXA, a Kruskal-Wallis test was performed using Dunn's test as post hoc. The significance level was set at p < 0.05. We calculated the mean difference and 95% limits of agreement for the analyzed equations (Equation - DXA). RESULTS: Only Heymsfield's and Lee's equations showed no significant differences with DXA. Heymsfield's equation had the smallest mean difference (-0.17 kg), but wider limits of agreement with DXA (-6.61 to 6.94 kg). Lee's equation had a small mean difference (1.10 kg) but narrower limits of agreement with DXA (-1.83 to 4.03 kg). CONCLUSIONS: In this study, the prediction equation published by Lee et al. showed the best agreement with DXA and is able to estimate SMM accurately in professional male soccer players.

RESUMO

BACKGROUND: There are several published anthropometric equations to estimate body fat percentage (BF%), and this may prompt uncertainty about their application. PURPOSE: To analyze the accuracy of several anthropometric equations (developed in athletic [AT] and nonathletic [NAT] populations) that estimate BF% comparing them with DXA. METHODS: We evaluated 131 professional male soccer players (body mass: 73.2 ± 8.0 kg; height: 177.5 ± 5.8 cm; DXA BF% [median, 25th-75th percentile]: 14.0, 11.9-16.4%) aged 18 to 37 years. All subjects were evaluated with anthropometric measurements and a whole body DXA scan. BF% was estimated through 14 AT and 17 NAT anthropometric equations and compared with the measured DXA BF%. Mean differences and 95% limits of agreement were calculated for those anthropometric equations without significant differences with DXA. RESULTS: Five AT and seven NAT anthropometric equations did not differ significantly with DXA. From these, Oliver's and Civar's (AT) and Ball's and Wilmore's (NAT) equations showed the highest agreement with DXA. Their 95% limits of agreement ranged from -3.9 to 2.3%, -4.8 to 1.8%, -3.4 to 3.1%, and -3.9 to 3.0%, respectively. CONCLUSION: Oliver's, Ball's, Civar's, and Wilmore's equations were the best to estimate BF% accurately compared with DXA in professional male soccer players.

RESUMO

A metameric colour matching test was designed to study inter-observer variability. Blue-yellow metameric matching to a white-light continuum was used to define the optimal wavelengths at which each of eight non-colour-defective observers achieved a match. The tests involved chromatic stimuli on a 2° bipartite field, with a white-light continuum presented on the left half, and a mixture of two monochromatic stimuli on the right half. The luminance of these chromatic stimuli was adjusted by the researcher using a staircase method, with the observer providing feedback about the similarity in luminance and chromaticity between the two halves of the field. Two series were performed for each observer, using different fixed yellow wavelengths. Since for each fixed yellow wavelength the match with the target white can be achieved by only one corresponding blue wavelength which is particular for each observer, the initial blue wavelengths were approximations based on the 2° CIE 1931 standard observer. Once the observers had attained an achromatic match, they modified the blue wavelength to achieve a perfect match of both halves. Generally, the observers found this modification of the blue wavelength necessary to achieve the metameric match. Each observer had a particular optimal blue wavelength which differed between the two series. The differences between the deviations from the standard observer for the two series were constant in value among the observers.

RESUMO

Several studies have indicated that dyslexics show a deficit in speech perception (SP). The main purpose of this research is to determine the development of SP in dyslexics and normal readers paired by grades from 2nd to 6th grade of primary school and to know whether the phonetic contrasts that are relevant for SP change during development, taking into account the individual differences. The achievement of both groups was compared in the phonetic tasks: voicing contrast, place of articulation contrast and manner of articulation contrast. The results showed that the dyslexic performed poorer than the normal readers in SP. In place of articulation contrast, the developmental pattern is similar in both groups but not in voicing and manner of articulation. Manner of articulation has more influence on SP, and its development is higher than the other contrast tasks in both groups.

RESUMO

This study focused on spelling development in Spanish children from elementary grades. A sample of 1045 was selected from 2nd to 6th grade belonging to four schools in Tenerife Island with an age range between 7 and 12 years old (M = 113.8, SD = 17.6). We administered a standardized writing test that includes diverse subtests to assess spelling, ruled and not ruled, and various written composition tasks (i.e., writing a story based on vignettes, describing a character and writing a story). We calculated the average of correct spellings in each variable and school level, and we also analyzed the type of misspellings that children made across different writing tasks. We found that spelling is acquired by 4th-grade children when it is not ruled, whereas the spelling of ruled words is acquired by 5th-grade children. When we analyzed the misspellings in a dictation task, we found that the children confused spelling of the graphemes c/s/z/x. Across different writing tasks, we found that students committed more misspellings with the graphemes b/v, h y c/s/z/x before they finished the 4th elementary grade.

Manner of articulation has more influence on SP, and its development is higher than the other contrast tasks in both groups

This study focused on spelling development in Spanish children from elementary grades. A sample of 1045 was selected from 2nd to 6th grade belonging to four schools in Tenerife Island with an age range between 7 and 12 years old (M= 113.8, SD= 17.6). Across different writing tasks, we found that students committed more misspellings with the graphemes b/v, h y c/s/z/x before they finished the 4th elementary grade

RESUMO

BACKGROUND: In a previous study we reported that 4,4'diaminodiphenylsulfone (dapsone) has anticonvulsant activity using kainic acid (KA) model. This work shows the behavioral and electrophysiologic changes caused by systemic application of several dapsone derivatives. These derivatives include disodium salt of 4,4'-diaminodifenylsulfone N,N'-diformaldehyde sulfoxylate (I), 4,4'-diaminodiphenylsulfone N,N'-didextrose sulfonate (II), sodium dibisulfite 4,4'-biscinamilidenamindiphenyl sulfone (III), and N,N'-dimethyl-4,4'-dimethylphenylsulfone (IV), which were synthesized and purified in our laboratory. METHODS: A KA model was used to provoke limbic seizures. Limbic seizures were provoked by injection, KA, and electrophysiologic recorder at the following concentrations: 6.25 and 12.50 mg/kg of III and 6.25 and 12.50 mg/kg of IV. RESULTS: Compounds III and IV caused decrease of postdischarges; we found percentage of protection of 55.60 and 70.78%, respectively. This showed possible anticonvulsant activity of these compounds (III and IV), while I and II showed no significant changes. We also studied whether there was a dose-dependence relationship, and different doses of compound IV were evaluated (25.00, 12.50, 6.25, 3.12, and 1.62 mg/kg). We found that greatest anticonvulsant effect occurred using doses of 3.12 and 6.25 mg/kg (two of the three lowest doses). CONCLUSIONS: We concluded that IV at doses of 3.25 and 6.25 mg/kg has anticonvulsant effect because it diminished duration of the first limbic seizure induced by KA; latency of first limbic seizure crisis was also increased. Both facts demonstrated possible therapeutic application of compound IV as anticonvulsant.
