RESUMO

Brown-Vialetto-Van Laere syndrome (BVVLS) is a rare neurodegenerative disorder of childhood. According to the previous reports, it has various primary signs and symptoms. Because of the simple treatment with riboflavin supplementation, it is important to have suspicious to this disease and begin treatment even before genetic test confirm. We report a five-year-old girl with BVVLS that manifest with hearing problems, first. There was obvious improvement in her disease clinical signs with riboflavin supplementation treatment.

Segmentation of organs and lesions could be employed for the express purpose of dosimetry in nuclear medicine, assisted image interpretations, and mass image processing studies. Deep leaning created liver and liver lesion segmentation on clinical 3D MRI data has not been fully addressed in previous experiments. To this end, the required data were collected from 128 patients, including their T1w and T2w MRI images, and ground truth labels of the liver and liver lesions were generated. The collection of 110 T1w-T2w MRI image sets was divided, with 94 designated for training and 16 for validation. Furthermore, 18 more datasets were separately allocated for use as hold-out test datasets. The T1w and T2w MRI images were preprocessed into a two-channel format so that they were used as inputs to the deep learning model based on the Isensee 2017 network. To calculate the final Dice coefficient of the network performance on test datasets, the binary average of T1w and T2w predicted images was used. The deep learning model could segment all 18 test cases, with an average Dice coefficient of 88% for the liver and 53% for the liver tumor. Liver segmentation was carried out with rather a high accuracy; this could be achieved for liver dosimetry during systemic or selective radiation therapies as well as for attenuation correction in PET/MRI scanners. Nevertheless, the delineation of liver lesions was not optimal; therefore, tumor detection was not practical by the proposed method on clinical data.

BACKGROUND: Following rubella virus control, the most important cause of congenital infections is human cytomegalovirus (HCMV). Congenital CMV (cCMV) may happen both in primary and non-primary maternal infections. The present study aimed to screen cCMV in symptomatic newborns suspected of congenital rubella syndrome (CRS) in Iran. METHODS: Out of 1629 collected infants' serum samples suspected of CRS but negative for rubella IgM, 524 samples were selected regarding cCMV complications. These samples were divided into two age groups: 1- one month and younger, 2- older than 1 month up to one year. Anti-HCMV IgM detection was performed on these serums. Then HCMV IgG avidity assay and HCMV DNA detection were carried out on all samples with positive and borderline results in IgM detection. RESULTS: Herein, 3.67% of symptomatic infants aged one month and younger had positive and borderline HCMV IgM, 12.5% of which had a low avidity index (AI). HCMV IgM detection rate among symptomatic infants older than one month to one year was 14.5%. Identified genotypes in this study were gB-1(63.63%), gB2 (18.18%), and gB3 (18.18%), respectively. CONCLUSIONS: This comprehensive study was performed on serum samples of symptomatic infants clinically suspected of cCMV from all over Iran. There was a good correlation between serology findings and PCR.

Gout, characterized by elevated uric acid levels, is a common inflammatory joint disease associated with pain, joint swelling, and bone erosion. Existing treatments for gout often result in undesirable side effects, highlighting the need for new, safe, and cost-effective anti-gout drugs. Natural products, including medicinal plants and phytochemicals, have gained attention as potential sources of anti-gout compounds. In this review, we examined articles from 2000 to 2020 using PubMed and Google Scholar, focusing on the effectiveness of medicinal plants and phytochemicals in managing gout. Our findings identified 14 plants and nine phytochemicals with antigout properties. Notably, Teucrium polium, Prunus avium, Smilax riparia, Rhus coriaria, Foeniculum vulgare, Allium cepa, Camellia japonica, and Helianthus annuus exhibited the highest xanthine oxidase inhibitory activity, attributed to their unique natural bioactive compounds such as phenolics, tannins, coumarins, terpenoids, and alkaloids. Herbal plants and their phytochemicals have demonstrated promising effects in reducing serum urate and inhibiting xanthine. This review aims to report recent studies on plants/phytochemicals derived from herbs beneficial in gout and their different mechanisms.

Background: Despite decreasing the global burden of measles disease after the introduction of vaccination, measles remains one of the most devastating childhood diseases. Since genotype B3 is reported as a predominant Measles Virus (MeV) genotype recently, the current study aimed to better understand MeV genetic variation by analyzing the complete sequence of Hemagglutinin (H) gene associated with outbreaks of circulated genotypes in Iran. Methods: Nine positive measles specimens were selected from three circulated different genotypes H1, B3, and D4. Two different regions of MeV RNA were detected by RT-PCR assay. Sequence data and phylogenetic trees were analyzed and constructed by MEGA X software program. Moreover, missense and silent mutations in critical positions of the MeV-H protein were investigated. Results: The result of phylogenetic analysis from the C-terminus of the Nucleoprotein gene (NP-450) and the complete H gene revealed that the mean sequence diversity was 0.06%-0.08% and 0.04%, respectively. Genotype H1 had the highest mutation in this study; however, the substitutions in genotype B3 fundamentally occurred in critical epitopes. Moreover, genotype D4 was more stable than genotypes B3 and H1. Conclusion: Mutations were investigated in the whole sequence of H protein. Moreover, the mutations that occur in the critical sites of the protein have an important effect on the pathogenicity of the virus. In this way, we were able to illustrate why genotype B3 is more transmissible than other measles genotypes and is the most important circulating genotype around the world.

Ganoderma lucidum is a promising medicine with a high amount of antioxidants and calcium. The selection of appropriate drying process methods in food science has a chief role to reach the best final characteristics. This study aimed to investigate the effects of air velocity and temperature in the convective dryer, sample distance, and infrared power in infrared dryers on the drying kinetics and quality of Ganoderma lucidum slices. In addition, Response Surface Methodology based on central composition design was used to optimize and analyze drying conditions. The ranges of temperature and air velocity were 40-60 °C and 0.5-1.5 m/s, respectively in the convective drying process while the range of distance and infrared power was 4-16 cm and 500-1500 W, respectively in the infrared drying process. It is worth mentioning that antioxidant and calcium contents were greatly enhanced during the drying procedures. Moreover, the values of the total color difference ranged between 8.21 and 19.66 for the convective dryer and 8.14 and 28.85 for the infrared dryer. A kinetic study indicated that dried samples by the infrared dryer could rapidly reach equilibrium moisture content due to exposure to IR radiation. Consequently, the results indicated that the infrared dryer has better performance than the convective dryer regarding drying time, energy consumption, and amount of calcium and antioxidant.

RESUMO

Fatigue is a complication of hemodialysis (HD). We examined the effect of aromatherapy on fatigue in 62 HD patients. Data were collected using a questionnaire and the Piper Fatigue Scale. It was found that fatigue decreased in the intervention group (P < .05). Lavender aromatherapy is useful in reducing fatigue.

Abstract There is no biodistribution or imaging data on 99mtechnetium (Tc)-hexamethyl propylamine oxime (HMPAO)-labeled platelets in the literature. The current study aimed to present updated information about the clinical procedures for preparation and use of labeled platelets. Following two-step centrifugation at 1500 and 2500 rpm, the platelets were extracted from whole blood into platelet-rich plasma (PRP) above the buffy coat and then from PRP into a platelet pellet at the bottom of the tube. The 99mTc-HMPAO-labeled platelets were inspected for purity, viability, release of 99mTc from platelets, and sterility. Also, microscopic examination and thin layer chromatography (TLC) were performed. Biodistribution was assessed following necropsy in BALB/c mice and through imaging of New Zealand rabbits. The separation ratio was estimated at 98%, and radiochemical purity was measured to be 80%. The labeling efficiency was above 30% in more than half of the assays (range: 17-43%). The release of 99mTc from platelets was 9% per hour at 37ºC. After 24 hours, stability was estimated at 54% in the human serum. The target organs of mice included the spleen and liver. In rabbits, the imaging results indicated liver as the target organ. Thyroid uptake was negligible up to 90 minutes. Based on the findings, extraction of platelets and labeling them with 99mTc-HMPAO is a feasible and safe approach in routine practice.

OBJECTIVE: Epstein-Barr virus (EBV) and Human Herpes virus 6 (HHV-6) are believed to involve in multiple sclerosis (MS) pathogenesis. Natural killer (NK) and CD8+ T cells have essential roles in handling viral infections and their phenotypic and functional properties may be influenced following exposure to viral infections. Here, we investigated the association of NK and CD8+ T cells subpopulations frequency with EBV and HHV-6 viral load in MS patients. MATERIALS AND METHODS: In this case-control study, EBV and HHV-6 viral load were evaluated in plasma of newly diagnosed relapsing-remitting MS (RRMS) patients at relapse phase (n=23), who were not on disease-modifying therapy (DMT), and sex- and age-matched healthy controls (n=19) using real-time polymerase chain reaction (PCR). The frequency of NK and CD8+ T cells subsets were assessed by CD27, CD28, CD45RO, CD56, and CD57 markers using flow cytometry. RESULTS: Despite the increased level of EBV viral load in RRMS patients compared to the control group, there was no statistically significant difference in EBV and HHV-6 copy numbers between the studied groups. In addition, a significant decrease was observed in the percentages of CD56bright CD57- and CD56dim CD57+ CD8low CD45RO- NK cells in RRMS patients in comparison to healthy controls. Analysis of CD8+ T cell subsets showed a substantially high proportion of CD27+ CD28+ CD45RO+ CD57- CD8hi T cells in patients at relapse phase compared to controls. The frequency of NK and T cells subtypes was not associated with EBV and HHV6 plasma viral loads. CONCLUSION: These findings further highlight the variation of NK and CD8+ T cells subsets frequency in clinically active RRMS patients. Since the composition of cells was not associated with EBV and HHV-6 viral load, perhaps other viral infections may be involved in altered NK and CD8+ T cells subpopulation. Larger cohort studies are needed to confirm these results.

The current nucleic acid signal amplification methods for SARS-CoV-2 RNA detection heavily rely on the functions of biological enzymes which imposes stringent transportation and storage conditions, high cost and global supply shortages. Here, a non-enzymatic whole genome detection method based on a simple isothermal signal amplification approach is developed for rapid detection of SARS-CoV-2 RNA and potentially any types of nucleic acids regardless of their size. The assay, termed non-enzymatic isothermal strand displacement and amplification (NISDA), is able to quantify 10 RNA copies.µL-1. In 164 clinical oropharyngeal RNA samples, NISDA assay is 100 % specific, and it is 96.77% and 100% sensitive when setting up in the laboratory and hospital, respectively. The NISDA assay does not require RNA reverse-transcription step and is fast (<30 min), affordable, highly robust at room temperature (>1 month), isothermal (42 °C) and user-friendly, making it an excellent assay for broad-based testing.

BACKGROUND: Scleroderma is a complex disease involving autoimmune, vascular, and connective tissues, with unknown etiology that can progress through any organ systems. OBJECTIVE: Yet, no cure is available; the thorough treatment of scleroderma and current treatments are based on controlling inflammation. Nowadays, medicinal plants/natural-based formulations are emerging as important regulators of many diseases, including autoimmune diseases. Here, we provided an overview of scleroderma, also focused on recent studies on medicinal plants/natural-based formulations that are beneficial in scleroderma treatment/prevention. METHODS: This study is the result of a search in PubMed, Scopus, and Cochrane Library with "scleroderma", "systemic sclerosis", "plant", "herb", and "phytochemical" keywords. Finally, 22 articles were selected from a total of 1513 results entered in this study. RESULTS: Natural products can modulate the inflammatory and/or oxidative mediators, regulate the production or function of the immune cells, and control the collagen synthesis, thereby attenuating the experimental and clinical manifestation of the disease. CONCLUSION: Natural compounds can be considered an adjunct treatment for scleroderma to improve the quality of life of patients suffering from this disease.

Since its inception, the coronavirus disease 2019 (COVID-19) pandemic has infected millions of people around the world. Therefore, it is necessary to find effective treatments against Severe Acute Respiratory Syndrome Coronavirus-2 (SARS-CoV-2), as it is the viral source of COVID-19. Alkaloids are one of the most widespread plant-derived natural compounds with prominent antiviral effects. Accordingly, these phytochemicals have been promising candidates towards discovering effective treatments for COVID-19. Alkaloids have shown potential anti-SARS-CoV activities via inhibiting pathogenesis-associated targets of the Coronaviridae family that are required for the virus life cycle. In the current study, the chemistry, plant sources, and antiviral effects of alkaloids, as well as their anti-SARS-CoV-2 effect with related mechanisms, are reviewed towards discovering an effective treatment against COVID-19.

Brain tumor localization and segmentation from magnetic resonance imaging (MRI) are hard and important tasks for several applications in the field of medical analysis. As each brain imaging modality gives unique and key details related to each part of the tumor, many recent approaches used four modalities T1, T1c, T2, and FLAIR. Although many of them obtained a promising segmentation result on the BRATS 2018 dataset, they suffer from a complex structure that needs more time to train and test. So, in this paper, to obtain a flexible and effective brain tumor segmentation system, first, we propose a preprocessing approach to work only on a small part of the image rather than the whole part of the image. This method leads to a decrease in computing time and overcomes the overfitting problems in a Cascade Deep Learning model. In the second step, as we are dealing with a smaller part of brain images in each slice, a simple and efficient Cascade Convolutional Neural Network (C-ConvNet/C-CNN) is proposed. This C-CNN model mines both local and global features in two different routes. Also, to improve the brain tumor segmentation accuracy compared with the state-of-the-art models, a novel Distance-Wise Attention (DWA) mechanism is introduced. The DWA mechanism considers the effect of the center location of the tumor and the brain inside the model. Comprehensive experiments are conducted on the BRATS 2018 dataset and show that the proposed model obtains competitive results: the proposed method achieves a mean whole tumor, enhancing tumor, and tumor core dice scores of 0.9203, 0.9113 and 0.8726 respectively. Other quantitative and qualitative assessments are presented and discussed.

In this study, a modified mesoporous adsorbent (MSU-S) impregnated by aluminum was used to remove the aromatic sulfur compounds from n-decane as the model fuel. Physical and chemical properties of as-synthesized adsorbent were investigated by XRD (X-Ray Diffraction), SEM (Scanning Electron Microscopy), FTIR (Fourier Transform Infrared spectroscopy) and BET (Brunauer-Emmett-Teller) method. Adsorptive desulfurization of model fuel was studied through batch and continues processes under mild temperature and normal atmospheric pressure. The equilibrium adsorption was modeled by Langmuir, Temkin, and Freundlich and the kinetics of adsorption was studied through first, second and intraparticle diffusion models. It was figured out that Temkin and the pseudo-second-order model were best fitting the adsorption equilibrium and describing the kinetics, respectively.

The present study evaluates the drilling fluid density of oil fields at enhanced temperatures and pressures. The main objective of this work is to introduce a set of modeling and experimental techniques for forecasting the drilling fluid density via various intelligent models. Three models were assessed, including PSO-LSSVM, ICA-LSSVM, and GA-LSSVM. The PSO-LSSVM technique outperformed the other models in light of the smallest deviation factor, reflecting the responses of the largest accuracy. The experimental and modeled regression diagrams of the coefficient of determination (R2) were plotted. In the GA-LSSVM approach, R2 was calculated to be 0.998, 0.996 and 0.996 for the training, testing and validation datasets, respectively. R2 was obtained to be 0.999, 0.999 and 0.998 for the training, testing and validation datasets, respectively, in the ICA-LSSVM approach. Finally, it was found to be 0.999, 0.999 and 0.999 for the training, testing and validation datasets in the PSO-LSSVM method, respectively. In addition, a sensitivity analysis was performed to explore the impacts of several variables. It was observed that the initial density had the largest impact on the drilling fluid density, yielding a 0.98 relevancy factor.

In heat exchange applications, frost formation on the cold surface causes a decrease in the rate of heat transfer and growth in the pressure drop. Thus, the study on the frost thermal conductivity has a significant and vital place for the engineers and researchers dealing with the heat exchangers. In the literature, there is a lack of accurate and applicable methods for determination of frost thermal conductivity. Additionally, the high cost and difficulties of experimental works clarify the importance of computational and mathematical methods. The errors in the determination of frost thermal conductivity on parallel surface channels can cause inaccuracy in estimations of frost density and thickness. The main aim of present work is suggesting Gaussian Process Regression (GPR) models based on four different kernel functions for the estimation of frost thermal conductivity in terms of time, air velocity, relative humidity, air temperature, wall temperature, and frost porosity. To achieve this purpose, a total number of 57 frost thermal conductivity values has been collected. Comparing the suggested GPR models and other available computational methods express the quality of the developed models. The best predictive tool has been selected as a GPR model, including Matern kernel function with R2 values of 0.997 and 0.994 in training and testing phases, respectively. In addition, the effectiveness of discussing variables on frost thermal conductivity has been investigated by sensitivity analysis and showed that air temperature is the most effective parameter. The present work gives engineers an insight into frost thermal conductivity and the effective parameters in its determination.The significant advantage of present work is the accurate prediction of thermal conductivity by a brief knownledge in artificial intelligence.

Wastewater management is a significant challenge, especially for arid and semiarid countries due to the water scarcity crisis. Considering human health, environmental regulations, wastewater treatment plant expenses, and water reusability, this study aims to assist in designing an efficient and economical WWTP. Technical and economic simulation and modeling are important for designing, constructing, and predicting the requirements of wastewater treatment plant designs. Simulation of a project before its implementation, in the first place, reduces additional costs, and from another point of view, a project is examined and researched from different aspects. In this study, three wastewater treatment plants based on conventional activated sludge, contact stabilization, and step aeration were simulated and evaluated technically and economically using the Zargandeh treatment plant data in Tehran. In three scenarios, GPS-X software investigated the effect of raw wastewater variations between their minimum and maximum intervals on effluent quality, and because the range of inlet effluent was different, more precise results were obtained. Then, the economic costs of the project, including implementation, maintenance, and also energy use, were calculated by Capdetwork software, and contact stabilization was found to be more cost-effective.

Terminal deoxynucleotidyl transferase (TdT) enzyme plays an integral part in the V(D)J recombination, allowing for the huge diversity in expression of immunoglobulins and T-cell receptors within lymphocytes, through their unique ability to incorporate single nucleotides into oligonucleotides without the need of a template. The role played by TdT in lymphocytes precursors found in early vertebrates is not known. In this paper, we demonstrated a new screening method that utilises TdT to form libraries of variable sized (vsDNA) libraries of polynucleotides that displayed binding towards protein targets. The extent of binding and size distribution of each vsDNA library towards their respective protein target can be controlled through the alteration of different reaction conditions such as time of reaction, nucleotide ratio and initiator concentration raising the possibility for the rational design of aptamers prior to screening. The new approach, allows for the screening of aptamers based on size as well as sequence in a single round, which minimises PCR bias. We converted the protein bound sequences to dsDNA using rapid amplification of variable ends assays (RAVE) and sequenced them using next generation sequencing. The resultant aptamers demonstrated low nanomolar binding and high selectivity towards their respective targets.

OBJECTIVES: In patients with papillary thyroid cancer (PTC), sentinel lymph node (SLN) radio-guided biopsy is not routinely used for detection of involved neck lymph nodes (NLN); 99mTc- antimony sulfide colloid (99mTc- ASC) has been used for this purpose. In this study, besides 99mTc-ASC another radiotracer, 99mTc-phytate (99mTc-P) with different doses and injection methods were evaluated. METHODS: Twenty-two patients, scheduled to undergo thyroidectomy for PTC, were injected for radio-guided SLN biopsy in the morning of operation in 3 groups: intra tumoral injection of about 1 mCi 99mTc-P (group A; n=5); peritumoral injection of less than 3 mCi 99mTc-ASC (group B; n=6); and peritumoral injection of 3 to 5 mCi 99mTc-ASC with application of massage (group C; n=9). A patient refused to complete the study. A patient with follicular thyroid cancer was also excluded. No NLN was detected in the pre-operative ultra-sonographic examinations of all patients. Central neck dissection was done for all the participants. The presence of radio guided detected NLN and results of pathology were assessed. RESULTS: In group A and B, no SLN was detected. NLNs were resected in 4 patients in group A and B; 2 of them involved by the tumor. In group C, 6 out of 9 patients (66.7%) had between 1 to 6 SLNs; the procedure failed to detect NLN in a patient in group C with surgically resected reactive NLN (failure rate 1 out of 7). CONCLUSION: The results underscored the significance of SLN radio guided biopsy in patients with PTC; the radiotracer, dose and method of injection may affect the detection rate.

OBJECTIVE: For preoperative radionuclide myocardial perfusion imaging, metabolic equivalent is one of the key factors to evaluate the appropriateness. Duke Activity Status Index is a practical method to calculate metabolic equivalents. We intended to validate Duke Activity Status Index in our population for the assessment of preoperative myocardial perfusion imaging appropriateness. METHODS: A total of 542 patients referred for myocardial perfusion imaging were recruited. A questionary compiled from Duke Activity Status Index was filled out based on which metabolic equivalents were calculated. Demographic data and history of cardiac risk factors were also collected. Myocardial perfusion imaging was performed using a 2-day stress-rest protocol either by exercise tolerance test or by pharmacologic stress through injection of Tc-MIBI and imaging by a dual-head gamma camera. RESULTS: Out of 542 patients, 369 (68.1%) were evaluated for preoperative risk assessment. Metabolic equivalents (oxygen consumption/min/kg) were calculated at 9.3 ± 5.1, 10.8 ± 4.8, and 8.7 ± 5.1 in total, preoperative patients and patients evaluated for ischemia due to nonsurgical purposes, respectively (p = 0.001). The myocardial perfusion imaging was rarely appropriate in 291 (79.5%), maybe appropriate in 67 (18.3%), and appropriate in 8 (2.2%) patients. The prevalence of abnormal myocardial perfusion imaging was 22.5%, 28.4%, and 12.5% in "rarely appropriate," "maybe appropriate," and "appropriate" scenarios, respectively. Metabolic equivalents were similar between patients with normal and abnormal myocardial perfusion imaging (8.7 ± 5.0 vs. 8.5 ± 5.4). CONCLUSION: Either Duke Activity Status Index is not a proper tool for calculation of metabolic equivalents or the appropriate use criteria is not operational in the population of Iranian preoperative patients in which cultural factors may contribute.
