RESUMO

Phytohaemagglutinin (PHA) is a lectin obtained from Phaseolus vulgaris (red kidney beans), that acts as a mitogen in human leucocyte culture and is commercially available from Gibco. This PHA (Gibco) was found to be very expensive, hence other inexpensive sources that can be used in all kinds of cytogenetics labs (rich and poor), were attempted. One such successful attempt was PHA extract from seeds of P.vulgaris. This paper details the methodology of extraction and application of PHA from seeds of P.vulgaris. Attempts has been made to identify the chemical and physical properties of the products in the extract, analyzed by various spectroscopic and analytical techniques. The analysis clearly indicates that the product from Phaseolus seeds extract was found to be similar to the commercially available PHA (Gibco) in the cytogenetic study of human leucocyte cultures. The present study enforces the possible utility of the plant extract directly for human leucocyte cultures.

A prospective study was carried out to assess the value of bronchoscopic cryotherapy for palliation of inoperable bronchial carcinoma with bronchial obstruction. Symptoms, lung function, and chest radiographic and bronchoscopic findings were recorded serially before and after 81 cryotherapy sessions in 33 consecutive patients. Most patients improved in terms of overall symptoms, stridor, and haemoptysis and they had an overall improvement in dyspnoea. Objective improvement in lung function was seen in 58% of patients and the changes in lung function correlated with symptoms. Bronchoscopic evidence of relief of bronchial obstruction was seen in 77% of patients and 24% showed improvement in degree of collapse on the radiograph. There were no important complications. These results compare favourably with the results in published series of patients having laser therapy. It is concluded that bronchoscopic cryotherapy is valuable for the palliation of inoperable bronchial carcinoma.

RESUMO

A 50 year old male presented with massive lymphadenopathy and was found to be in extramedullary blast crisis is reported. Lymphadenopathy regressed on busulphan therapy and the overt blast crisis took 15 months to occur after initial presentation.

RESUMO

A 16 year old male was admitted with a clinical picture suggesting an intracranial pyogenic infection. He also has focal convulsions and left abducens palsy. CT scan showed a subdural empyema with interhemispheric extension. Since the patient was improving clinically, neurosurgical intervention was deferred. Spontaneous interhemispheric empyema is an uncommon condition. In our case conservative management was curative in what is considered a neurosurgical emergency.

RESUMO

The inspiratory timing of lung crackles in patients with bronchiectasis was compared with the inspiratory timing of the lung crackles in chronic bronchitis and alveolitis. In severe obstructive chronic bronchitis the lung crackles are typically confined to early inspiration while in alveolitis the lung crackles continue to the end of inspiration but may begin in the early or the mid phase of inspiration. In uncomplicated bronchiectasis on the other hand, the lung crackles typically occur in the early and mid phase of inspiration, are more profuse, and usually fade by the end of inspiration. In addition in bronchiectasis, crackles are also usually present in expiration, they are gravity independent and become less profuse after coughing.
